Congenital hypothyroidism and phenylketonuria are metabolic disorders which may have clinical consequences and cause severe mental retardation, and the appearance of physical and neurological sequelae that affect child development. One of the tools that strengthen early diagnosis is neonatal screening, The purpose of this research is to determine the incidence early cases of congenital hypothyroidism and phenylketonuria ( PKU) neonatal in Cojedes state during the period January 2008 - December 2014. The research was part of a pattern of non-experimental, retrospective and descriptive research, led to a population consisting of live births who were held in dried blood on filter paper quantitation of thyroid stimulating hormone (TSH) ELISA method and fluorescent probe for quantification of the concentration of phenylalanine (PHE). The data collection technique used was the rating scale. Results: Of 54,152 live births in this period metabolic screening was performed at 35,988 newborns which represent 66.46 % of the population subjected to it. 4 positive cases were recorded for TSH constituting 0.01 %, representing 50% males and 50 % females, with an incidence of 1: 2276 and no case for PKU. Recommendation: Provide information on the importance of Neonatal Metabolic Research or screening of these diseases to pregnant women.
Gastroschisis and Omphalocele are the most frequent and important birth defects of the abdominal wall in the newborn period. In our country there are few hospitals with the necessary infrastructure to care for these infants. Many of them have to wait valuable time to be operated while the institution where they will be transfered is located, increasing the risk of Abdominal Compartment Syndrome and sepsis. Objective: To compare the surgical outcome between the use of the Umbilical Cord Patch versus the Conventional Primary Closure of the abdominal wall in newborns. Methods: Twenty patients were equally divided into 2 groups. Group A patients underwent abdominal closure using the Umbilical Cord Patch and Group B patients underwent a Conventional Primary Closure of the abdominal wall (10). Results: No statistically significant difference was seen in hospitalization time and complications. Where as significant statistical difference was seen in intra-abdominal pressure (IAP), onset of oral intake and survival between the two procedures. Conclusion: The Umbilical Cord Patch seems to offer advantages to those infants with delayed closure of the abdominal wall because the technique creates a mesothelial surface in contact with the intestine, it is an autologous tissue, available for use at birth and easly applied. The study showed greater benefits in terms of lower increase of IAP, early onset of oral fedding, and higher survival rate compared with Conventional Primary Closure. Fanconi-Bickel syndrome.
We present a case of Fanconi- Bickel Syndrome, a rare type of carbohydrate metabolism disorder. Clinical presentation begins during the first months of life with failure to thrive, hepatomegaly, fasting hypoglycemia, rickets and renal proximal tubular dysfunction. Diagnosis is established on the basis of clinical manifestations, radiological findings of rickets, and laboratory investigations showing proximal tubular dysfunction, characterized by glucosuria with fasting hypoglycemia, metabolic acidosis, hypophosphatemia, phosphaturia and aminoaciduria. It was described in 1949 by Fanconi and Bick.
We report a case of a 24 months old child, diagnosed with corticodependent thrombocytopenia in which active cytomegalovirus infection was confirmed sustituir resaltado por: by means of serology and viris isolation in urine.. Due to the patient's worsening, we suspected a link between both processes and started oral valganciclovir, given for a total of 5 months. We documented excellent simultaneous response in both pathologies, and without relevant side effects.
The therapeutic potential of probiotics, as agents capable of altering the intestinal microbiota and provide beneficial health effects have led to its use in common diseases in children, but the results have been mixed. It seems to be more effective in preventing clinical manifestations of lactose intolerance and prevention of colic and necrotizing enterocolitis, as well as maintenance of remission in ulcerative colitis. Although its effectiveness is lower, it is recommended the use in the management of colic. At present its use for treating Crohn's disease or necrotizing enterocolitis is not recommended. Although there are successful results, there are still no firm recommendations H. pylori infection. All recommendations must be for specific strains of probiotics in each disease.
Probiotics may act in the host in a multifactorial way. In the intestinal lumen, probiotics work through interaction with the intestinal microbiota or by direct metabolic effects in the intestinal mucosa and epithelia. Probiotics affect digestive processes and are associated to the mucosal immune system and other organs such as the immune system, brain and urogenital epitellium. In the lumen, probiotics produce fermentation of low or non-absorbable substrates, modulate the production of gas, deconjugate bile salts, act to acidify short chain fatty acids and increase the intestinal transit. Probiotics act in the mucosa as a barrier and as bactericides; they inactivate toxins, reduce inflammation and stimulate immunologically complements and IgA. Although there are multiple probiotics available, there are few which have demonstrated a clear benefit in humans. Not one, however, has been found to produce all the beneficial effects. The colonization of the human gut by microbiota plays an important role in the development of obesity and other metabolic and immunological diseases. Microbiota are considered as inmuno-modulators capable of acting on innate and acquired or specific immunity. The influence of intestinal microbiota extends beyond the gastrointestinal tract and has an important role in the two-way communication between the latter and the central nervous system.
