Obesity is a result of the combined effects of genes, environment and lifestyle. The Leptin (LEP) and leptin receptor (LEPR) are genes that have been extensively evaluated in search for variants that may be associated with obesity and cardiometabolic complications. Objective: To evaluate the possible association between polymorphisms G2548A of the LEP gene and Gln223Arg of the LEPR gene with the development of obesity and insulin resistance (IR) in prepubertal children and adolescents. Methods: We studied 314 children 2-11 years, grouped by anthropometric and biochemical parameters: a) overweight /obese without IR (n = 133), b) overweight /obese with IR (n = 75) and c) healthy controls (n = 70). Genotyping was performed by Polymerase Chain Reaction-Restriction Fragment Length Polymorphisms (PCR-RFLP) method. Genotype-risk associations were studied. We then compared the average values for anthropometric and biochemical parameters. Results: The genotypic frequency for polymorphism G2548A of LEP gene was 51% for the G/A genotype, 33% G/G and 16% A/A genotype; for polymorphism Gln223Arg of LEPR gene was of Gln/Arg 49%, Gln/Gln 31% and Arg/Arg 20%. Significant difference was found in the distribution of different genotypes of the LEPR gene in children with overweight/obesity with IR compared to the control group (OR = 2.6; 95% CI = 1.17 to 5.75; p < 0.05). Conclusion: We observed an association between the presence of Gln/Gln genotype of the LEPR gene with insulin resistance (cardiometabolic risk factor) children, rendering these children with overweight/obese with IR 2,6 times more likely to be with insulin.
It has been shown that metabolic acidosis accelerates the progression of Chronic Kidney Disease (CKD) and that modern diet is characterized by a high content of acid -forming elements that could contribute to this metabolic disorder. The aim of this study was to estimate the Potential Renal Acid Load (PRAL) in CKD patients attended at the Department of Nephrology at the Children's Hospital J.M. de los Ríos from April 2014 through February 2016. Method: A descriptive cross-sectional study was conducted in 28 children (10 months-17 years) with the diagnosis of CKD in stages 1 , 2, 3 and 4. Nutritional status was evaluated by weight, height and mid-arm circumference, and the diet composition by food frequency questionnaire and 24 hours intake reminder. PRAL was calculated by the method of Manz and Remer. Results: Height and nutritional status were under normal values in 15 and 9 patients respectively. Meat and cereal intake were high, whereas vegetables and fruit intake were low. Protein intake was high in 22 patients. PRAL was 16 ±10.7 mEq/day and a positive correlation was observed with protein intake (p=0,001), daily rations of meat (p=0,010), fat (p=0,006), and cereals (p=0,022), and a negative correlation with vegetable intake (p=0,032). Conclusions: The dietary pattern of children with CKD may contribute to the progression of the disease by promoting metabolic acidosis. PRAL must be addressed as part of the nutritional treatment of children with CKD.
In central precocious puberty (cPP) there is a premature activation of the hypothalamic-pituitary-gonadal (hPg) axis that increases the production of sex steroids, which in turn stimulates growth rate, bone age advancement and the emergence of secondary sexual characteristics in both sexes. In the presence of cPP intracranial pathology must be ruled out, although with significant differences between boys and girls. The concept that cerebral palsy is associated with abnormal sexual maturation has been raised but has not been established. The intimate mechanism by which an intracranial activation insult occurs in early puberty is unknown, although it is speculated that on one hand there may be a mechanical factor that alters the inhibition of gonadotropin releasing hormone (gnrh) and secondly, depending on the type of injury there may be a predominance of one or other cell type, capable of secreting substances that trigger the secretion of gonadotropins.
Congenital malaria is a relatively rare disease in the context of neonatal pathologies. We present a case of a 24 day old newborn male with congenital malaria by Plasmodium vivax and falciparum whose diagnosis was performed through the study of thick film. The newborn comes from a non- endemic area were as the mother because of worktravelled to an endemic area of our country in Bolivar state. The mother had a maternal history of seasonal malaria during the I trimester of gestation with irregular treatment. The clinical manifestation of the newborn was fever 40 °c and the antimalarial treatment was performed with Artesunate at 60 mg with adequate clinical response, confirmed by the thick film negativization at the end of the treatment. It is important to consider the differential diagnosis with neonatal sepsis especially in patients from endemic areas and risk. Congenital malaria is real and therefore it is recommended that newborns of mothers with malaria should be examined immediately. Risk factors for Infecction by Respiratory Sincitial Virus in Venezuela: Experts meeting.
Respiratory syncytial virus (RSV) is one of the most common causative agents of respiratory infections in children under 2 years of age. The most common clinical form is bronchiolitis. Within this vulnerable population, infants born before 32 weeks gestation and those with bronchopulmonary dysplasia or with cyanotic congenital heart disease, heart failure or pulmonary hypertension are more susceptible; infants with lung anomalies, neuromuscular diseases, cystic fibrosis or severe immunosuppression are also at risk. RSV causes chronic inflammatory changes that lead to short, medium and long term sequelae. Prevention has proven to be the best measure to reduce complications and costs and palivizumab prophylaxis has been effective in infants at risk. The following article aims to review the risk factors involved in infection by respiratory syncytial virus and establish the criteria for prophylaxis with palivizumab.
