Diverticular Disease (DD), or Diverticulitis, is one of the most common gastrointestinal diseases affecting the general population in the western world; approximately 20% of patients are symptomatic and 75% suffer from painful DD: acute diverticulitis, 25-33% of those patients may suffer from recurrent episodes. Changes in colon microbiota cause chronic inflammation and epithelial cell proliferation developed in colon mucosa and around the diverticula. Prebiotics restore colon’s microenvironment, from where the treatment of patients with asymptomatic DD is proposed, in order to avoid the Acute Diverticulitis. The study aimed to investigate the proportion of patients who maintained the referral after a previous episode of uncomplicated Acute Diverticulitis, when treated with Bacillus clausii: two billions every eight hour during one year. Materials and Methods: Prospective and retrospective study, with non-probabilistic, intentional-type sampling and control arm. Arm A: Fifty patients diagnosed with uncomplicated Acute Diverticulitis, were administered with a single treatment of Bacillus clausii, two billions, every eight hours during one year. Clinical controls every three months. Arm B (control): 50 untreated patients diagnosed with uncomplicated Acute Diverticulitis. Outcomes: The necessary information to carry out the R variance analysis was introduced; a significant difference was found between the study’s arm averages; a F(1,48)=5.259, p<0.05 was found. Conclusion: because of its biological characteristics, Bacillus clausii prevents Diverticular Disease’s inflammatory complications such as the Recurrent Acute Diverticulitis.
Introduction: Nitazoxanide is a new drug that has shown to be helpful against various intestinal protozoa including Giardia lamblia. However, there are few studies on the subject at national and regional scopes. A study was conducted to test the therapeutic utility of nitazoxanide in children infected with G. lamblia, citizens of Ciudad Bolívar, Bolívar state. Patients and methods: Were diagnosed and selected 21 cases of children parasitized G. lamblia and they were treated with nitazoxanide, then 3 post-treatment controls at 7, 15 and 21 days using the methods of direct parasitological examination, Kato and spontaneous sedimentation were performed. Results: The overall percentage of parasitological cure was 37.5 % (6/16) consisting of 6 children in whom posttreatment parasite was eradicated. From this analysis, 5 of the 21 treated children were excluded because they did not attended one or more post- treatment controls. None of the children who received treatment with nitazoxanide presented adverse effects. Conclusion: In the group studied, and due to their low percentage of parasitological cure, nitazoxanide not appear to be the drug of choice and should be reserved for use in cases of therapeutic failure or when there is intolerance to metronidazole. DOUBLE ACQUIRED PYLORUS: AN UNUSUAL COMPLICATION OF PEPTIC ULCER DISEASE.
The Double Pylorus (DP) is an unusual finding in the gastroscopy where the gastric antrum and the duodenal bulb is connected to by two holes separated by a septum or bridge. Its etiology may be congenital or acquired the latter being the most common form as a complication of peptic ulcer disease (EUP). Most fistulas are located on the lesser curvature of the gastric antrum. There have been described three theories in the formation of acquired DP. It may present with epigastric pain, dyspepsia or gastrointestinal bleeding but sometimes it is an endoscopic or radiological incidental finding. We present a patient who presents with upper gastrointestinal bleedingsymptoms in whom a gastroscopy where performed with this unusual finding which was confirmed by radiological study. In general the prognosis is favorable. Coagulopatia en enfermedad hepática severa.
