Summary A prospective study is done to assess the value of preoperative C reactive protein serum levels in the diagnosis and prognosis of patients with clinical feature of acute appendicitis. Fifty-two patients were included and divided in uncomplicated appendicitis (edematous and flegmonous phases) or complicated appendicitis (gangrene, perforation and/or purulent peritonitis), depending on the clinical and anatomopatological findings of the surgical specimen. This findings were correlated with the preoperative values of C reactive protein, and also with other clinical and paraclinical parameters (time of evolution, fever, abdominal guarding, leukocytosis, neutrophylia and postoperative morbidity). Twenty-five percent (25 %) of the patients with histologic confirmation of acute appendicitis had normal values of C reactive protein (< 1 mg/dL). The patients with complicated appendicitis had superior C reactive protein levels than the patients with uncomplicated disease, being this difference between both groups statistically significant (P < 0.01). However, inside the group of complicated disease, the C reactive protein values weren’t an independent parameter for postoperative morbidity (P > 0.05). Among the others parameters evaluated, only the fever had enough frequency to predict a complicated stage of the disease. We concluded that the C reactive protein is not a trustworthy test to exclude an acute appendicitis. However elevated levels of this test (> 6 mg/dL) are more frequently associated with complicated stages of the disease. In these patients the start of a combined antibiotic therapy in the same preoperative period as well as the total indication for a laparoscopic approach could allow to minimize the postoperative infectious complications that are more frequent when the appendicitis evolved to a complicated phase Resultante materno-perinatal en primigestas de 40 años o más http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0367-47622006000100004&lng=en&nrm=iso&tlng=en El objetivo de la investigación fue determinar las complicaciones obstétricas y perinatales de las primigestas con 40 años o más.
Summary The objective of the investigation was to determine obstetrical and perinatal complications of primigravid aged 40 and older. A prospective study comparing 98 primigravid aged 40 and older with a control group of 98 primigravid pregnant women aged between 20 and 29 years old assisted between January 2001 and March 2004 was performed. Prenatal data, gestational age, delivery, type of delivery, birth weight, preterm labor, perinatal deaths, congenital malformations and small or large for gestational age, Apgar scores at 1 and 5 minutes and presence of meconium in any moment during labor. History of infertility and obesity was significantly more common in the study group. Complications like pregnancy-induced hypertension, malpresentation, second half of pregnancy hemorrhage and labor alterations occurred more frequently in primigravid aged 40 and older. The study group presented a higher significant rate of induced labor, use of oxytocin, malpresentation and cesarean section, preterm labor, low birth weight and small for gestational age newborns. It is concluded that although there was an increased in ante and during labor complications, perinatal evolution was affected by maternal age.
Summary Material of a placenta from pregnancy with diagnostic of placental mesenchymal dysplasia at 27 gestation’s week and fetal dolicocephaly was obtained to be processed by scanning electron microscopy previous macroscopic and microscopic analysis. The macroscopic analysis showed placentomegaly, hydropic stem villi between 2 and 35mm of size, chorionic plate compressed retroplacental hematoma, infarcts, thrombosed fetal vessels and marginal insertion of umbilical cord. The microscopic analysis exhibited invaginations of hypotrophic trophoblast ocassionally hyperplasic.
Summary Our concern arises from the progressive deterioration of the relation ship between the treatment and the morbidity- mortality produced by lung cancer. The research aims to generate effective strategies to contribute to a better prevention of cigarette smoke-produced lung cancer. It’s justified because the research study provides theoretical and referential elements in order to reduce the critical situation in the way medical care is carried out, especially in the prevention treatment. A revision of this phenomenon is made, including field work, emphasizing on the traditional epistemic approach, which reconstructs the base of the link between phenotype and genotype, of an interdisciplinary approach with the idea of presenting prevention strategies guided by the bio-social-reflexive model to the scientific communitie, as it is the case of transformational learning. The methodology used in the investigation was oriented by the complexity paradigm; quantitative moments (deductive), qualitative (inductive) and dialectic (critical theory), emerging the strategy as a method during the experience Inicio de la cardiología en Venezuela http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0367-47622006000100007&lng=en&nrm=iso&tlng=en La cardiología, previamente establecidas sus bases entre los siglos XVI y XX, se inicia en 1920, cuando por primera vez aparece el vocablo "cardiología" como título de la revista Archivos de Cardiología y Hematología, fundada por Pitaluga y Galandre, en España.
