La infección se adquiere por contacto de la piel erosionada o reblandecida, mucosas y conjuntiva, con la orina de animales y/o humanos infectados. El objetivo del presente estudio fue caracterizar los aspectos epidemiológicos y clínicos de la leptospirosis en el estado Bolívar y comparar la sensibilidad, especificidad del LEPTO-Dipstick y el antígeno termorresistente de Leptospira (TR). Durante doce meses consecutivos se tomaron muestras séricas de 31 pacientes con síndrome febril prolongado y clínica compatible de leptospirosis, admitidos en el Complejo Hospitalario Universitario "Ruiz y Páez" de Ciudad Bolívar, Venezuela. La determinación de anticuerpos IgM anti-Leptospira se llevó a cabo utilizando un kit comercial LEPTO-Dipstick y la prueba en placa con el antígeno termorresistente de leptospira (TR) en todas las muestras. Además, se realizó la prueba de aglutinación microscópica (MAT). La prevalencia general de leptospirosis en los pacientes febriles que consultaron al hospital, según el TR, fue del 80,6% (25/31). Se demostró que los serovares más frecuentes en el Estado Bolívar fueron: icterohaemorrhagiae, copenhageni (21,3%), autummalis y australis (12,8%). Se demostró que el antígeno TR y el LEPTO Dipstick tienen una sensibilidad buena (> 80%) y especificidad baja (25%). No hubo concordancia entre ambos métodos (Kappa: -0,2). En el estado Bolívar existe una elevada prevalencia de leptospirosis en pacientes febriles por lo que se debe descartar esta infección en aquellos pacientes que presenten fiebre prolongada y factores de riesgos para la enfermedad<hr/>Leptospirosis is caused by numerous serovars of Leptospira interrogans. The infection is acquired through the contact of softened skin, mucous and conjunctive with the urine of animals and humans infected by animal carriers. The aim of this study was to determine the clinical and epidemiological features of leptospirosis in Bolívar state Venezuela and to compare the sensibility and specificity of the LEPTO-Dipstick, with the test of macroscopic agglutination in plate with the termorresistent antigen (TR). Thirty one sample of serums were processed of patients with leptospirosis clinic, admitted in the Hospital Universitario "Ruiz y Páez" of Ciudad Bolívar during 12 months. Detection IgM antibodies of anti-Leptospiras was carried out by means of LEPTO-Dipstick and antigen TR. The Microscopic Agglutination Test (MAT) was also, carried out. The general prevalence of leptospirosis in patients who attended the "Ruiz y Paez" Hospital was of 80.6% (n = 25) by means of the determination of the termorresistent antigen. The most frequent serovars identified in Bolívar State, were the serovars: icterohaemorrhagiae, copenhageni (21.3%), autummalis and australis (12.8%). TR antigen and LEPTO Dipstick had a sensibility of 80% and a specificity of 25%. The agreement between both methods was null (Kappa: -0.2). Bolívar state has a high leptospirosis prevalence and the infection should be discarded in those patients with long-term fever and risk factors for the illness <![CDATA[<B>Phenotypic characterization of species of <I>Malassezia</I> in healthy skin of an university student population.</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400003&lng=en&nrm=iso&tlng=en Las levaduras del género Malassezia forman parte de la piel normal del hombre y otros vertebrados. La reciente descripción de nuevas especies para el género ha incentivado interés en el estudio de las mismas en diversos países, por lo tanto, es importante realizar investigaciones tendentes a obtener datos epidemiológicos de las especies de Malassezia en países tropicales como Venezuela, en el cual este tipo de trabajo no se ha reportado. El estudio se realizó en piel sana de población estudiantil universitaria. Las muestras fueron tomadas de diferentes áreas del cuerpo e inoculadas en el medio modificado de Dixon y Sabouraud Dextrosa Agar a 32°C. La identificación se realizó siguiendo las claves para la identificación de especies descrita por Gueho y colaboradores y el test de difusión en tween propuesto por Guillot y colaboradores. En esta investigación la especie aislada fue M. furfur, correspondiendo el mayor porcentaje de positividad al grupo etáreo de 16 a 20 años (66,7%). Las localizaciones anatómicas predominantes fueron espalda y pecho. (33,3 y 26,6%) Según las pruebas estadísticas aplicadas, Test exacto de Fisher’s y Chi cuadrado, no se encontró diferencias significativas entre los grupos evaluados. Cabe destacar la importancia de continuar las investigaciones en otros grupos etarios, para establecer cuales son las especies prevalentes en nuestra región y evaluar su potencial patógeno<hr/>The yeasts of the Malassezia genus are part of the normal skin of man and other vertebrates. The description of new species for this genus has induced on their study in several countries. For this reason, is important to do research in order to get epidemiologic data about Malassezia species in tropical countries like Venezuela, where the new Malassezia species have not been reported This study was made on healthy skin of a university student population. The samples were taken from different body areas and inoculated in Dixon modified medium and Sabouraud dextrose agar medium e incubated at 32°C. The identification was achieved following the key of species described by Gueho et al. and the tween diffusion test proposed by Guillot et al. In this investigation the isolated specie was M. furfur, corresponding the major positive percentage to the age group from 16 to 20 years old (66.7%). The presence of Malassezia was predominant on shoulder and chest (33.3% and 26.6%). There were not found significant differences between the evaluated groups, as was demonstrated by the applied statistical tests, exact Fisher’s test and Chi square test. It is important to continue the investigations on other age groups, in order to establish the prevalent species in our region and evaluate their pathogenic potential <![CDATA[<B>Evaluation of K-<I>ras</I> 12 oncogene mutations in Venezuelan patients with <I>Helicobacter pylori</I> infection.</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400004&lng=en&nrm=iso&tlng=en En el presente trabajo se evaluó la presencia de mutaciones del gen K-ras 12 en pacientes infectados con H. pylori y su asociación con distintas patologías de las vías gastroduodenales superiores. Se estudiaron 62 pacientes a quienes se les practicó endoscopia digestiva superior, tomando 3 biopsias utilizadas para el estudio histológico, pruebas de biología molecular y la determinación de mutaciones puntuales del gen K-ras 12. Los resultados mostraron una alta incidencia de la infección por H. pylori en pacientes con gastritis crónica activa (GCAc) (90%), gastritis crónica atrófica (GCAt) (70%), metaplasia intestinal (MI) (67%), cambios displásicos (CD) (83%), disminuyendo en cáncer gástrico (CG) (33%). En la evaluación de las mutaciones del oncogen K-ras 12 se observó que 68% de los pacientes presentaron mutaciones en los diferentes aminoácidos analizados. En el codon 12 del gen K-ras, se detectaron mutaciones puntuales simples y combinadas en la misma muestra proveniente de diferentes patologías gástricas, registrándose en los casos de gastritis crónica activa (GCAc) el mayor número de mutaciones, disminuyendo a medida que progresan las etapas de diferenciación histológica hacia cáncer gástrico. Los resultados indicarían que existen mutaciones específicas que pudieran emplearse como marcadores moleculares de malignidad y el eventual desarrollo de cáncer gástrico<hr/>This work evaluated the infection of H. pylori in the different gastric pathologies and its association with the oncogen K-ras 12. Endoscopy was performed in 62 patients and 3 biopsies from the antral region were taked and used for the histological diagnostic, PCR, and point mutations determination. The results showed a high incidence of H. pylori infection in patients with active chronic gastritis (AcCG) 90%, chronic atrophic gastritis (AtCG) 70%, intestinal metaplasia (IM) 67%, dysplasia (D) 83%, and decrease in in gastric cancer (GC) 33%. Evaluation of the oncogen K-ras 12 showed that 68% of the patients presented mutations in the different analyzed amino acids. In the 12 codon of the K-ras gene, we observed simple point mutations and combination in the same sample in different gastric pathologies. In AcCG samples were detected the greater number of mutations. A decrease of the point mutations were observed in the progression stages to gastric cancer. The presence of these specific mutations would be tumor markers and it determine the possible development of gastric tumors <![CDATA[<B>Homocysteinemia and its relationship with the methylentetrahydrofolate reductase polymorphism in various ethnic groups from western Venezuela</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400005&lng=en&nrm=iso&tlng=en Con el propósito de establecer la prevalencia de la hiperhomocisteinemia y su relación con el polimorfismo de la metilentetrahidrofolato reductasa, se estudiaron tres grupos étnicos del Occidente de Venezuela, localizados en la región del Estado Zulia (60 individuos Wayuú en la alta Goajira venezolana, 42 inmigrantes italianos y 77 venezolanos mestizos). La homocisteína fue determinada por ensayo inmunoenzimático utilizando lámpara de flourescencia polarizada, el folato sérico fue medido por radioinmunoanálisis y el polimorfismo de la MTHFR se realizó mediante reacción en cadena de la polimerasa y análisis de restricción. Se encontró una alta proporción en las variantes de la MTHFR 677TT y C677TT en los indígenas wayuú (78%), italianos (76%) y mestizos venezolanos (63,6%), con asociación significativa entre los niveles altos de homocisteína y concentraciones menores de folato en los wayuú (p < 0,0001) y en los venezolanos mestizos (p < 0,001). Las variantes polimórficas de la MTHFR se asociaron con hiperhomocisteinemia en el 23,3% de los indígenas wayuú (OR: 6,17; IC 95: 0,74-51.36), el 9,5% de los italianos (OR: 0,93; IC 95: 0,085-10,10) y en el 20,7% de los mestizos venezolanos (OR: 5,2; IC 95: 1,08-24,90; p < 0,03). No se encontró asociación entre hiperhomocisteinemia y deficiencia de folato en ninguna de las poblaciones estudiadas. Se concluye que a pesar de una alta prevalencia de variantes para la MTHFR en los grupos étnicos estudiados, el factor nutricional juega un papel importante en el control de la hiperhomocisteinemia y sus posibles consecuencias cardiovasculares<hr/>The prevalence of hyperhomocysteinemia and C677T MTHFR polymorphism was studied in various ethnic groups from Western Venezuela (60 Wayuu Indians, 42 italian immigrants and 77 Venezuelan mestizos) in relation with the prevalence of hyperhomocysteinemia and the C677T MTHFR polymorphism. Homocysteinemia was determined by polarized fluorescence immunoassay in an IMX system, serum folate was measured by radioimmunoanalysis and the MTHFR genotype was determined by PCR and restriction analysis. Hyperhomocysteinemia was defined as a value over 2 SD above the mean value for normal MTHFR (CC677) in each group. The prevalence of MTHFR variants (C677T and 677TT) was elevated in all ethnic groups (78% among the wayuu, 76% among Italians and 63% among mestizos) with a significant association between the concentrations of homocysteine and the levels of serum folate among the wayuu (p<0,0001) and the mestizos (p < 0,001) only. Hyperhomocysteinemia was associated with MTHFR variants in 23% of the wayuu (OR: 6.17, CI 95: 0.74-51.36), 9.5% of the Italians (OR: 0.93, CI 95: 0.085-10.10) and 20.7 of the Venezuelans mestizos (OR: 5.2, CI 95: 1.08-24.90, p > 0.03). There was no relationship between hyperhomocysteinemia and folate deficiency in any of the groups studied. In conclusion, despite a high prevalence of C677T MTHFR variants in these ethnic groups of western Venezuela, the lack of no evidence of hyperhomocysteinemia combined with folate deficiency may imply that the nutritional status of these groups plays an important role in the control of hyperhomocysteinemia as a risk factor for cardiovascular disease <![CDATA[<B>Importance of IgG anti-CagA antibodies of <I>Helicobacter pylori</I> in Venezuelan patients with gastric diseases.</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400006&lng=en&nrm=iso&tlng=en La infección por Helicobacter pylori está asociada actualmente con diferentes formas de enfermedades digestivas, incluyendo gastritis, úlceras duodenales, gástricas y linfomas del tipo Maltoma.El microorganismo es considerado como un importante factor de riesgo para el desarrollo de cáncer gástrico. El objetivo de este estudio fue caracterizar la respuesta inmune sérica de Inmunoglobulina G (IgG), contra los antígenos CagA, ureasa y extracto total de Helicobacter pylori, en un grupo de pacientes con diferentes formas de enfermedad de las vías digestivas superiores. Fueron evaluados 66 pacientes sintomáticos referidos para examen endoscópico, provenientes del Servicio de Gastroenterología del Hospital Universitario de Caracas, Venezuela. La respuesta inmune fue evaluada mediante ensayo inmunoabsorbente (ELISA) usando antígenos recombinantes para Ureasa y CagA. La infección por Helicobacter pylori fue determinada por el cultivo de las biopsias gástricas y prueba de ureasa. Los resultados mostraron la presencia de H. pylori en 48/66 (72,7%) pacientes mediante cultivo microbiológico y prueba de ureasa. De los 66 pacientes evaluados en este estudio, 45 (68%) fueron positivos para el extracto total de H. pylori, 34/66 (51%) presentaron anticuerpos contra CagA y 18/66 (27%), tuvieron anticuerpos anti-ureasa. La tasa de positividad de anticuerpos anti-CagA en pacientes con úlcera gástrica, cáncer gástrico, y gastritis crónica fue de 87,8%, 77,7% y 40,8% respectivamente. Del presente estudio se puede concluir que los niveles de anticuerpos anti-Cag A y anti extracto total fueron similares para pacientes con enfermedad gastroduodenal severa, incluyendo, úlcera gástrica y adenocarcinoma<hr/>Helicobacter pylori is one of the most common bacterial infections worldwide. It is associated with chronic gastritis, peptic ulcer disease and constitutes a major risk factor for gastric adenocarcinoma and lymphoma. The aim of this study was to evaluate the specific serologic immunoglobulin G (IgG) response to whole cells proteins, CagA and urease antigens of Helicobacter pylori in a Venezuelan population. We evaluated 66 patients from the Hospital Universitario de Caracas, attending in the gastroscopy service. H. pylori infection was detected by culture and rapid urease test. IgG antibodies against CagA and ureases were tested by enzyme-linked immunosorbent assay method using highly purified recombinant antigens. We demonstrated the presence of H. pylori in 48/66 (72,7%), by culture and rapid urease test. We found a seroprevalence of 45 (68%) to whole cells, 34/66 (51%) to CagA and 18/66 (27%) to urease. The positive rates of CagA antibodies in patients with gastric ulcer, gastric cancer and chronic gastritis were 87,8%, 77,7% y 40,8% respectively. The serum antibodies anti-CagA were similar between peptic ulcer disease and gastric cancer patients <![CDATA[<B>Sistema de detección y tipificación de Papilomavirus del tracto genital inferior</B>: <B>Hibridización <I>in situ</I> y confirmación con reacción en cadena de la polimerasa</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400007&lng=en&nrm=iso&tlng=en We developed a simplified and non-isotopic in situ hybridization procedure for the detection of Human Papillomavirus (HPV) in routine Papanicolaou cervical smears. The assay involves one oligonucleotide (malignant probe) which recognizes high risk HPV 16 and 18, and another which detects HPV 6 and 11 (benign probe). We adapted the system to fulfill the requirements of gynecologists and cytologists, assimilating their protocols and simplifying the in situ hybridization assay. When we compared the detection levels reached by the in situ hybridization versus a ladder PCR assay in 156 clinical samples, original designed for this work, the kappa coefficient between both methods is 0.945, indicating a strong agreement between the ISH and the PCR assays<hr/>Se desarrollo un procedimiento de Hibridación in situ no-isotópico y simplificado para la detección de papilomavirus humano (HPV) en citologías de rutina de Papanicolaou. El ensayo involucra un oligonucleotido (sonda maligna) el cual reconoce a los VPH de alto riesgo: 16 y 18, y otra que detecta VPH 6 y 11 (sonda benigna). Se adaptó completamente el sistema a los requerimientos de ginecólogos y citotecnólogos, asimilando sus protocolos y simplificando el ensayo de hibridación in situ. Se compararon 156 muestras clínicas, los niveles de detección tanto en hibridación in situ como en el ensayo de PCR (diseño original para este trabajo), se obtuvieron valores de co-positividad y co-negatividad cercanos a la unidad (> 0,98) <![CDATA[<B>Some immunological aspects in the reaccional states of Hansen disease.</B>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332005000400008&lng=en&nrm=iso&tlng=en El problema clínico crucial en la lepra son los episodios de inflamación aguda que conllevan a daño de los nervios. Aun cuando el Mycobacterium leprae ha sido eliminado mediante el uso de antibióticos, la muerte de la bacteria no es una solución completa al daño ocasionado en los nervios. Dos de los fenómenos inmunopatológicos más frecuentes son: Reacciones tipo I conocidas como reacciones de reversión (RR) y las Reacciones tipo II, como eritema nodoso leproso (ENL). Estas reacciones tipo II se han definido como: una complicación inmunológica en los pacientes multibacilares. Ambos tipos de reacciones están acompañadas por el incremento de citocinas pro-inflamatorias principalmente TNF-alfa, IL-1, IL-2, IL-4, IL-6, IL-8, INF-gamma, IL-10, IL-12, entre otras. En un análisis retrospectivo realizado en una población de 150 pacientes del Servicio Central de Dermatología del Instituto de Biomedicina sometidos a terapia multidroga (PQT) y PQT + inmunoterapia, ambos grupos presentaron fenómenos reaccionales tipo II, sólo el grupo que adicionalmente recibió inmunoterapia presentó además reacciones del tipo I. El grupo de pacientes que presentaron ENL estuvo asociado con presencia de gran cantidad de bacilos en las lesiones<hr/>The crucial clinical problem in leprosy is represented by episodes of intense inflammation that produce nerve damage. When Mycobacterium leprae has been eliminated by means of antibiotics, the death of the bacteria is not a complete solution to the damage caused in nerves. Two of the more frequent immunopathological phenomena in Hansen’s disease are Type I, reactions, known as Reversal Reactions (RR), and Type II reactions, of which the most frequent and well known are those called Erythema Nodosum Leprosum (ENL). Type II reactions have been defined as an immunologic complication in multibacillary patients. Both types of reactions are accompanied mainly by the increase of pro-inflammatory cytokines TNF-alpha, IL-1, IL-2, IL-4, IL-6, IL-8, INF-gamma, IL-10, IL-12, among others. In a retrospective analysis of a population of 150 patients of the Central Service of Dermatology, Institute of Biomedicine, where these patients were subjected to Multidrug Therapy (MDT) and MDT + inmunotherapy (IMT), both groups presented type II reactions, but the group that additionally received inmunotherapy also presented type I reactions. Se conoce que una minoría de las personas que son infectadas por M. tuberculosis son capaces de progresar a enfermedad clínica. Se puede decir, en términos generales, que el 90% de las personas tendrán controlados los bacilos en estado latente para toda la vida, por medio de su sistema inmunológico. Un 5% presentará TBC primaria progresiva y el otro 5% presentará la enfermedad en estados tardíos de la vida, lo que se denomina TBC de reactivación o post-primaria. En los individuos resistentes, el control de la infección o de los bacilos tuberculosos que se encuentran en la región alveolar requiere principalmente del desarrollo de una respuesta de inmunidad celular (RIC) del tipo Th1. Este tipo de respuesta incluye la participación de los macrófagos alveolares, los linfocitos T CD4+ y CD8+, principalmente los linfocitos T <FONT FACE=Symbol>gd</FONT> y la producción de citocinas como: IL-2, IFN-gamma, IL-12, IL-18 y TNF-alfa. Además, es primordial el papel de las células "natural killer" (NK), y de las células epiteliales como parte de la respuesta de inmunidad innata<hr/>Human pulmonary tuberculosis (TB) is a worldwide public health problem, which is caused by Mycobacterium tuberculosis. It is a fact that one third of the world´s population is infected with this mycobacteria, however, only a minority of people infected by M. tuberculosis may develop a clinical disease. In general, about 90% have their bacilli under control in a latent state throughout their lives by means of their immune responses. About 5% will develop primary progressive TB and the remaining 5% will develop the disease in the later stages of their lives, which is known as reactivation or post-primary TB. In resistant individuals, control of the infection mainly requires development of a Th1 cell immunity response.
