The integrity of the vascular endothelium is of primary importance for the maintenance of blood fluidity as well as the substances expressed and released by endothelial cells and membranes. Venous and arterial thrombosis are common diseases potentially fatal affecting equally to all races. These ailments are more frequent with increasing age and there are common in hospitalized patients. Heparin at low dosage is being used as preventive for Deep Vein Thrombosis (DVT); higher doses are given in pathologies with a higher risk of venous or arterial thrombosis; Low Molecular Weight Heparins (LMWH) are ideal for prevention of DVT. Oral anticoagulants are a very important therapeutic group against events of thrombosis regardless of ethiology. Aspirin has been the most studied and used molecule to prevent platelet aggregation; actually is recommended at low doses and as been compared with other products used at present for the prevention of platelet aggregation such as ticlopidin, clopidrogrel and slow liberation of aspirin combined with dipiridamol. To overcome the heparin-induced thrombocytopenia there are being tryed thrombin inhibitors with great hope in good results. As for the thrombolytic treatment itself, it should be initiated during the first hours of diagnosis keeping in mind indications and counterindications, being very careful in the choice of the agent, used by experts, in those cases where necessary and in patients in hospitals with facilities for adequate control. It should also be kept in mind the risk of hemorrhage as side effect in the therapeutic with these agents.
Low molecular weight heparin (LMHW) is isolated from unfractionated heparin (UFH). LMWH mechanism of action is exerted on Factor Xa through antithrombin. The most used LMWH are enoxaparin (Clexane), nadroparin (Fraxiparin) and deltaparin (Fragmin). A significant number of clinical assays have compared the effects of LMWH and UFH on deep venous thrombosis and pulmonary thromboembolism, showing LMWH better results than UFH. Based on these studies, dalteparin was used at a dose of 5000 IU/ day during 10 days; indobuphen 400 mg/day and trimetazidine 60 mg/day during 3 months in patients with ischemic cardiopathy. Our result show a significant improvement in these patients, through heart rate stabilization and absence of chest pain as well as tissue damage and necrosis in EKG. According to these results we suggest the use of this non invasive and ambulatory therapy in patients with ischemic cardiopathy.
Today the science of medicine is going through a change of paradigm, due to the development of molecular biology, eco- and molecular epidemiology and evidence-based medicine. The latter’s basic science is clinical epidemiology, understood as an investigative methodology, which has surged as a consequence of the widening gap between other investigative disciplines and the realities of our patients. Medical investigation becomes a necessity in each of these levels brought together by the knowledge of disease causing mechanisms, resulting in a holístically-integrated paradigm, inter and transdisciplinary. Dopamine and Hypertension http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642000000200005&lng=en&nrm=iso&tlng=en Dopamine, a neurotransmitter precursor of norepinephrine, produces cardiovascular and renal effects including increase of myocardial contractility and cardiac output, without changing the heart rate, active and passive vasodilatation, diuresis and natriuresis. These cardiovascular and renal effects are mediated by the interaction of dopaminergic receptors named D1, D2, D3, D4, D5, D6 y D7; the letters of recent description. Actions are attributed to Dopamine that controls blood pressure through central and peripheral nervous systems and target organs such as kidney and adrenal glands in some types of hypertension. Renal dopaminergic deficiency has been related with the capacity to excrete salt in some forms of hypertension. Although it has been proven that dopamine and its derivatives have antihypertensive actions, research continues and for this reason it is important to clarify some physiological and pharmacological aspects of dopamine, its receptors and the clinical uses that could be important in the management of hypertension. Influence of Age and Arterial Blood Pressure on the Anti-hypertensive Action of Dihydro-pyridine Compounds in Hypertensive Patients http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642000000200006&lng=en&nrm=iso&tlng=en ABSTRACT To study the antihypertensive effect of dihydropyridines and correlate with the patient’s age, the degree of hypertension and mean arterial pressure, in three groups of eight hypertensive patients with mild, moderate and severe hypertension selected from the Clinical Pharmacology Unit of Vargas Hospital in Caracas. Mean age, weight and height of the selected patients were 55.66 ± 3.4, years, 67 ± 3.05 Kg and 1.63 ± 0.01 m, respectively. Three acute experimental designs compared with placebo were followed and each group of patients received amlodipine, nitrendipine or isradipine at different times, in single doses of 10, 20 and 5 mg, respectively. The cardiovascular parameters evaluated were: mean arterial pressure, systolic blood pressure, diastolic blood pressure and cardiac frequency at regular intervals during 48, 6 and 24 hours for amlodipine, nitrendipine and isradipine, respectively. Acute administration of amlodipine (10 mg) to hypertensive patients, significantly reduced blood pressure form 185 ± 11.21/100 ± 7.07 to 150.5 ± 10.6/88.6 ± 7.8 mmHg at 8 hours and to 162 ± 10.9/91 ± 5.6 mmHg at 48 hours. Cardiac frequency also increased significantly at 8 and 48 hours in 10 and 9 beats/min, respectively. There was a positive correlation between patient’s age and mean arterial pressure decrease and also between pretreatment and decreased arterial pressure. Two hours after acute administration of isradipine (5 mg) to hypertensive patients, blood pressure decreased significantly from 179.50 ± 7.6/100.26 ± 3.28 mmHg to 143.75 ± 4.7/82 ± 5.18 mmHg. Cardiac frequency increased in 5 beats/min at the same time of measure. There was a positive correlation between patient’s age and arterial pressure decrease and a slight but not significant correlation between pretreatment and mean arterial blood pressure decrease. Acute dose of nitrendipine (20 mg) to hypertensive patients induced a significant fall of arterial blood pressure from 172.25 ± 7.8/107.13 ± 3.58 to 134.5 ± 5.08/81 ± 5.84 mmHg two hours after the administration of the drug, and the cardiac frequency increased significantly in 11 beats/min during this time. There was no significant correlation between patient’s age and the mean arterial pressure fall, neither between pretreatment and the decrease of arterial blood pressure.
In order to determine the incidence of gross vascular complications in adults with Diabetes Mellitus Type 2, was designed a descriptive transversal study. We selected 40 patients with diagnosis of Diabetes Mellitus type 2, attending the Outpatient Diabetic Clinic at the Victorino Santaella Ruiz Hospital in Los Teques, Venezuela and 30 healthy subjects with similar characteristics as group control, from February to November 1998. The patient’s age ranged from 30 to 75 years, all with more than five years since their first diagnosis of Diabetes Type 2 and complying with the inclusion criteria. The patient’s medical records included the presence of cardiovascular risk factors such arterial hypertension, blood lipids and lipoproteins disturbances, smoking and drinking habits and dietetic or pharmacological treatment. We recorded weight and height measurements, lipid profile, basal blood glucose and glycosylated hemoglobin. Results show significant differen-ces between experimental control groups as regards to body mass index (BMI), hip/waist relation, fasting blood glucose and glycosylated hemoglobin. In the experimental group we recorded: age 48±8.05 years (x±SD) corresponding 45% to males and 55% to females. Time since first diagnosis of diabetes 10,4 ± 8,3 years; 15% of the group were alcohol consumers and 10% smokers. The mean values for basal blood glucose and glycosylated hemoglobin (HbA1c) were 160,40±40, 6 mg/dL and 7,4±1, 2% respectively. Arterial hypertension was present in 60% of the experimental group and 70% had dyslipemias according to the Venezuelan Association of Atherosclerosis (AVA) criteria. The BMI was high in 78% of the patients with values of 31,4±4,2. Blood fibrinogen was in the range of 380±50mg/dL. Macrovascular alterations were present in 35.4% of patients at the beginning of the study and in 25% of the cases were alterations of the peripheral vascular type. These findings point out the need for an integral approach in the follow-up of the diabetic patient, establishing additional controls as well as the usual fasting blood glucose, aiming at treating and modifying the risk factors associated with macrovascular complications.
Aluminum is a metal present in most of commercial deodorants as a soluble salt. Apparently, this element is able to produce a wide range of cardiovascular effects like high blood pressure and premature ventricular beats. The aim of this study was to examine 1.500 consecutive patients with clinical and EKG diagnosis of idiopathic premature ventricular beats and the effect of the abolition of such abnormalities with the medical intervention that consisted in: a. omission of the aluminum containing deodorant. B. An antiarrhytmic drug and deodorant omission and c. two antiarrhytmic drugs and deodorant omission. All patient were users of aluminum contained deodorants and did not have any other pathology that could explain this cardiovascular manifestation. Only the cessation the use of deodorants containing aluminum caused a dramatic disappearance of premature ventricular beats in 1.140 patients (76 %). Three hundred patients (20 %) received an antiarrhytmic drug plus deodorant omission and only sixty patients (4%) need two antiarrhytmic drug plus deodorant cessation in order problem. After one month of treatren persist six runths after (end uf the follow -up). The mechanism of action of this effect remains unknown. Further studies must be done in order to dilucidate the role of aluminum and related elements in high blood pressure genesis and oxidative stress related disorders.
Hypertension is a very common condition and the most important risk factor for the occurrence of cardiovascular events. The hyperactivity of the renin-angiotensin-aldosterone system (RAAS) has been attributed as cardiovascular risk factor in subjects with essential hypertension; intrinsic vascular abnormalities in which the RAAS is clearly the milieu for the development of the pathological changes in the blood vessel’s wall, has been found to be the cause of the establishment of hypertension. Many drugs with different therapeutic mechanisms have been used for the treatment of the patient with hypertension and the vascular complications. Nevertheless, the majority of products used, sometimes their utility is limited for the occurrence of adverse effects. Permanent research for new pharmacological agents for the treatment of hypertension has lead to the development of angiotensin II AT1 receptor antagonists. The most important functions mediated by receptor AT1 include vasoconstriction, induction of the production and release of aldosterone, renal reabsortion of sodium, cardiac growth, proliferation of vascular smooth muscle, elevation of peripheral noradrenal action and central activity of sympathetic nervous system, stimulates vasopressin release and inhibits renin from the kidney. Angiotensin II AT1 receptor blockers, oppose the effect of angiotensin II lowering blood pressure without producing cough as side effect as they do not affect levels of bradykinin or P substance, making these products suitable for the treatment of those patients with hypertension that require treatment with a drug blocking the effect of the angiotensin converting enzyme (ACE) but these cannot be used due to cough as a side effect. Semipersonalized Psychological Evaluation of Quality of Life of Hypertensive Patients in a trial with two Calcium Antagonists: a Multicenter Latinamerican Study (Lastlhy) http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642000000200010&lng=en&nrm=iso&tlng=en The main objective of this study was to evaluate well being and physical activity of two hundred and forty eight hypertensive patients, of which, one hundred and seventy seven were female, who had previously finished the Latin Amerícan Study on Lacidipine in Hypertension (LASTLHY). This was an open study carried out in twelve clinical centers situated in Argentina, Brazil, Colombia, Mexico and Venezuela, to compare, over a period of sixteen weeks, the antihypertensive actions of fixed-dose once daily oral monopharmacotherapy of 4 rng of lacidipine (n = 120) patients VS. 30 mg of nifedipine (n = 128) patients aged between 40-65 years old, with mild to moderate hypertension beginning at the end of a four weeks placebo run-in (end of week -1). Well being and physical activity were assessed through an experimental single questionnaire, which was administered taking into consíderation the physical and cultural diversities amongst the clinical centers and patients. The questionnaire included thirteen multiple-choice and eight contingent open questions. The score to each question was multiplied by a coefficient according to the importance of each question for each patient (semipersonal ization); the coefficient was evaluated from cultural and socioeconomic information collected at the time of enrollment. The semipersonalization was carried out by a blind psychological study with respect to the medication and had a high repeatability in the assignment of personalized coefficients to the score of each question. The scores of each question were added to obtain an overall weil being and activity scoring. The possible theoretical range for the overall scoring in this study was 10- 124.
Resistance to ß-lactam antibiotics is mainly due to production of ß-lactamases by gramnegative bacilli.
Resistance to ß-lactam antibiotics is mainly due to production of ß-lactamases by gramnegative bacilli. Over 200 ß-lactamases have been identified to date, therefore we have had increased resistance to ß-lactams effective against gramnegative bacilli, which have been in use since 1963. Combating the surge in antibiotic-resistant (AR) bacteria requires tracking of local resistance. The information must come from ongoing detailed analysis of the results of antimicrobial susceptibility tests that are done routinely to guide the treatment of infections in individual patients. This is the importance of AR surveillance. Materials and Methods: Since 1988 Venezuelan group of Bacterial Resistance (GVRB), in 25-health institutions from seven states are in charge of analysing and publishing AR data of bacterial isolates, proceeding from patients with infection acquired in the community and hospitals. Methods of disc diffusion according to NCCI; WHONET software program (Word Health Organisation NET); statistical analysis by c². Results and Conclusions: A.-The increase of resistance during the decade to: 1) cefotaxime is: E.coli 34%, E.aerogenes 13%, E.cloacae 10%, K.pneumoniae 57%, 2) ceftazidime: E.coli 16%, K.pneumoniae 30%, P.aeruginosa 20%; 3) ceftriaxona: E.coli 17%, E.aerógenes 5%, K.pneumoniae 43%. 4) cefoperazone-sulbactam: no significance in the increase. B.-Resistance to: 5) cefepime:P. aeruginosa 10-14%, Acinetobacter sp.35 %, others around 5 %. 6) piperaciline-tazobactam: resistance is 5 to10 %, except Acinetobacter sp. (40 %); 7) the results of GVRB to imipenem and meropenem is low except P.aeruginosa (10-15 %) and Acinetobacter sp. (25-35%). During the decade there was an increase in GVRB to most ß-lactams. The second half of the 20th century has been a golden age of antibiotics but the outlook is uncertain.
