The main objective of this study was to examine different results obtained from diverse researches about type 2 diabetes mellitus treatment in the psychology health area. An thorough evaluation of recent investigations in this area was developed, with results that show that as today psychiatric disorders and psychological alterations have become a public health problem that has increased with time and that affects symptomatic populations as well as the over all general population. Even though there’s an association between psychiatric disorders and personality types with chronic complications in patients with type 2 diabetes mellitus and the obvious implication of these disorders in the effects of treatment, the adapting process in these patients to new life styles is hard. There are few studies in the psyconeuroendocrinology area and psychological treatment has been rare, which is why there’s an evident need to expand this research area so we can be able to offer patients the care they need from a multidisciplinary point of view.
Background: The bony metastasis is a common complication of many cancer types. The corrected DPD (cross-linked: Piridoline y Deoxipiridoline colagen), diminished not significantly in the groups. In the subscale of physical well-being clinical improvement was observed in the group of 90 mg (p 0,05). Conclusions: The monthly infusions of Pamidronate, improve the bony pain in patient with bony metastasis.
The lipids peroxidation is a complex process, where unsaturated fatty acids react with molecular oxygen by a mechanism of chain reaction via free radicals, form hidroperoxides which are degraded to a variety of products like alkenals, hidroxialquenals, ketones, alkenos, and others, which can be quantified by different methodologies, like malondialdehyde (MDA), a final product of the degradation of fatty acids. In the present work we determined the susceptibility of low density lipoproteins to be oxidized, in two populations one normal (n=30) between 25-35 years and other with diabetes type 2 (n=25) between 25-50 years, without previous treatment of antioxidant. The extraction of the LDL by ultracentrifugation, oxidized in vitro, and then MDA was determined by the thiobarbituric acid method. The results of malondialdehyde in the normal population was between 0,77 to 1,17mumol/L and in the diabetics population from 1,47 to 2,11 mumol/L with stadistic difference between both population. In conclusion, the normal values of MDA are from 0,77 to 1,17mumol/L, greater values could be indicative of additional risk in these patients.
Neuropathic pain is defined as that which is caused by either a primary injury or dysfunction in the nervous system. Methods: We conducted a prospective, non-randomized, parallel group, comparative clinical trial, where adult patients (n=40) with a clinical diagnosis of neuropathic pain were screened and selected for admission in the trial. Patients were divided into two groups: Group A patients, n=20, were administered lidocaine 1%, 70 mg and levobupivacaine 5 mg, both locally; Group B patients, n= 20, were locally administered lidocaine 1%, 70 mg, levobupivacaine 5 mg, and dexmedetomidine at 3 mg/kg/dose. Sympathetic and peripheral blocks were made to each patient, according to the specific clinical picture and neuropathic pain localization. Pre-block-assessed parameters were again determined 1 and 5 minutes after finalizing the anesthetic procedure. Adverse events, either spontaneously reported or obtained through open questions, were also determined. Results: Tolerability assessment resulted in 42.5% of overall patients not reporting treatment-related adverse events, and again when comparing both groups, those receiving dexmedetomidine reported adverse events less frequently (50%) than Group A patients (35%). Conclusions: We conclude that, besides its sedative properties, dexmedetomidine together with local anesthetics showed excellent analgesia-enhancing effects, with the additional benefit of a favorable side-effect profile, lacking respiratory-depressant actions and improving overall patient management. In addition, patients receiving dexmedetomidine showed changes in heart rate and blood pressure that were both predictable and stable during and after the anesthetic procedures, while the patient achieved anxiolysis, improving his tolerance to the procedure itself.
Third ventricle anteroventral area (AV3V) is sensitive to changes in NaCl concentration and able to modify blood pressure and renal function in response to NaCl concentration. Hypertonic NaCl microinjections in AV3V induce a sympathetic cardiovascular response which increases blood pressure and heart rate; these changes can be modulated by central administration of 5HT2 and AT1 receptor blockers, and also by peripheral administration of alpha and beta adrenergic receptor antagonists. Influence of endothelin receptors (ETR) and atrial natriuretic factor (ANF) on sensitivity to NaCl has not been explored. Hypertonic 1,5 M NaCl (2 muL volume) microinjections in AV3V of normotensive anesthetized rats increased mean arterial blood pressure by 17,9 ± 2.2 mm Hg (p<0.01). ANF microinjections (5mug/2muL) in AV3V neither changed mean arterial blood pressure nor altered the hypertensive response to NaCl 1,5 M microinjection. Microinjection of PD 142893 into AV3V a non selective endothelin antagonist (3mug/2muL) did not modify basal blood pressure but reduced by 47,6% (p<0,01) the hypertensive response to 1,5 M NaCl injection. These results indicates a possible modulator effect of endothelin receptors in the NaCl centrally- evocated cardiovascular response to hypertonic NaCl injections in AV3V. Acute microinjection of ANF in this brain area seems not to be involved in the NaCl-evoked sympathetic cardiovascular response.
A descriptive study was made in 84 patients to determined adverse drugs reactions (DAR) during the IV treatment with some of the following adrenergic agonists: norepinephrine (0,5mu/kg/min), epinephrine (0.1mu/kg/min), dopamine (3mu/kg/min.) and dobutamine (5mu/kg/min). The Yellow Card of CEVIFARE was used as the instrument for registry of the DAR. DAR were analyzed by its type, severity and causality. 83.33% of the patients had DAR, being most frequent: tachycardia (47,61%), pallor (23,80%), hypertension (11,90%) and arrhythmias (7,14%). All the DAR were type A, mild 50%, moderate 40% and serious 10%. 100% of DAR analyzed were proved. Although the adrenergic agonists turned out to be safe, the epinephrine has the risk of producing distal necrosis, thus the Nursing role in practicing pharmacovigilance is to prevent or to diminish the possibility of appearance of DAR in the patients under her care.
Experiments were conducted to investigate the role of brain the alpha1- alpha2 and beta-adrenergic receptors on the renal effects elicited by central injection of ETs. Cerebroventricular administration of endothelins (ETs) to conscious male hydrated rats resulted in an increase in urinary sodium excretion at 1, 3 and 6 hr period of urine collection. Central administration of prazosin (an alpha1-adrenergic receptor antagonist) inhibited the increase in sodium and urine excretion induced by ICV-ETs. Yohimbine (an alpha2-adrenergic receptor antagonist) or atenolol (an beta-adrenergic receptor antagonist) did not altered the urinary response to ET-IVT. Our results demonstrate a role for brain endothelin in the regulation of electrolyte balance and indicate that the sympathetic nervous system, through the alpha1-adrenergic receptor subtype, is involved in the natriuretic action of ETs.
The antineoplastic agents cyclophosphamide (CFM) and radiation X (RAD) can produce neurogenic inflammation in the urinary bladder and in the gastrointestinal tract, respectively. NK1 receptor- antagonists reduce plasma protein extravasation (EPP) caused by diverse antineoplastic drugs; and glucocorticoids increase the efficacy of agents used to treat adverse effects of anti-cancer treatments. In this study we determined whether dexametasone (DEX) modifies the efficacy of NK1 antagonists to inhibit the EPP induced by CFM or RAD. Male rats received one of the following treatments: saline (1 ml/kg), GR203040 (0.3 mg/kg), GR205171 (0.3 mg/kg), DEX (1mg/kg) or the combination of antagonists NK1+DEX, before the administration of CFM or RAD. Cytotoxicity in different tissues (bladder, duodenum, jejune and kidney) was determined by quantification of EPP by the Evan’s blue method and by histological evaluation. The individual pre-treatments with a GR or DEX significantly inhibited the EPP induced by CFM in the bladder and by RAD in duodenum and jejune; and that this inhibition was increased when the combination of a GR+DEX was used. The results suggest that both NK1 receptors and steroid-sensitive mechanisms are involved in CFM or RAD-induced inflammation.
The magnitude, velocity and antihypertensive effectiveness of the nifedipine liberation programmed in microgranules (NLPM) was evaluated in patients with moderate to severe hypertension. A total of one hundred and thirty five patients were evaluated during six weeks in a prospective, open, comparative and crossed study. Of that group, 25.9% (n = 35) were normal patients, the remainder of them were hypertensive patients, 40% (n = 54 patients) received 30 mgrs (8am or 8pm) of NLPM and 20.7% (n = 28 patients) received 60 mgrs (8am or 8pm) of NLPM, 13.3% of the admitted patients (n = 18) were considered as failures of the medication. Both doses at different schedules of the taking diminished the arterial pressures significantly (p <0.05). There were patients that reached the end point a 30 mgrs dose, this achievement was obtained from the 3rd week of medication keeping on these pressure levels until the end of the study, being the highest magnitude of reduction were: 8am PAS 15.5 mmHg (12.2%), PAD 12 mmHg (12.6%), PAM 12.2 mmHg (10.9%) and 8pm of: PAS 14.1 mmHg (9.7%), PAD 9.5 mmHg (10.6%) and PAM 11.2 mmHg (10.05%), on the other hand, in those who achieved the end point with the administration of 60 mgrs, reductions were observed from the 3rd week obtaining the maximum effect at the 6th week being the highest magnitude of reduction of: 8am PAS 26.3 mmHg (16.34%), PAD 18.5 mmHg (17.69%) and PAM 20.9 mmHg (17.0%) and 8pm PAS 22.5 mmHg (15.15%), PAD 18 mmHg (18.1%) and PAM 21.8 mmHg (18.5%). The magnitude of the antihypertensive response of the NLPM was numerically bigger with 60 mgrs than with 30 mgrs, but the response velocity was bigger with 30 mgrs maybe because the blood pressure at the beginning of the study were bigger in that group that required 60 mgrs, (p <0.05) arriving at the end of the study (6th week) to normal pressure figure, similar in both groups (p> 0.05). The heart rate in the group of 30 mgrs (8am and 8pm) showed a significant decrease after the treatment (p <0.05), on the other hand in the group of 60 mgrs these stayed unaffected (p> 0.05). In conclusion NLPM demonstrated to be effective for the managing of patients with light to moderate hypertension, obtaining a bigger magnitude in the blood pressure effect with a dose of 60 mgrs than with a dose 30 mgrs, however, with this last dose, the antihypertensive response was more fast.
Adults with Diabetes are twice more likely to suffer depression, and for this reason is important to know the relationship between depression, diabetes and how this affects an individuals quality of life. The aim of this study was to determine in adult type 2 diabetic patients, depression levels and its main symptoms. 40 type 2 diabetic patients, between 30 and 65 years of age, who assisted for the first time to the Endocrine - Metabolic Research Center "Dr. Félix Gómez", were studied. Depression levels were quantified using the Beck Depression Inventory. Results show that 55% of these patients had some level of depression. Female patients had higher levels of depressión than male patients (female 1,4 ± 0,2 vs. 0,4 ± male 0,1; p < 0,02) and more severe depressive symptoms than men. This study confirms the comorbility between depression and type 2 diabetes mellitus and shows the necesity of psycological treatment.
