This issue is especially concerning in the setting of immunocompromised, hospitalized patients, in particular when surgery, invasive procedures, or prosthetic implants are carried out, patients are admitted in intensive care units, or underlying chronic disorders and immunodeficiency are of concern, and broad-spectrum antibiotics are widely used in the environment; moreover, a community spread of resistant Gram-positive cocci has been recognized during recent years. The spectrum of antimicrobials available for an effective management of these relevant infections is significantly threatened by the emerging of methicillin-resistant and more recently glycopeptide-resistant strains. The streptogramine association represented by quinupristin/dalfopristin, the oxazolidinone derivative linezolid, and the recently licensed daptomycin and tigecycline, together with a number of glycopeptides, fluoroquinolones, cephalosporins, and other experimental compounds, represent an effective response. It is due to the innovative mechanisms of action of these compounds, their maintained or enhanced activity against multiresistant pathogens, their effective pharmacokinetic/pharmacodynamic properties, their frequent possibility of synergistic activity with other compounds effective against Gram-positive pathogens, and a diffuse potential for a safe and easy administration, also to compromised patients. The main problems related to the epidemiology of multiresistant gram-positive infection, the potential clinical indications of all recently available compounds compared with the standard of care of treatment of resistant Gram-positive infections, and updated data on efficacy and tolerability of all these compounds, are updated and outlined on the ground of a review of recent literature evidences. Effects of Strength Training and Jatamansi on reducing Hand Tremor amongst archers http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642008000200003&lng=en&nrm=iso&tlng=en Objectives: To investigate and compare the effects of strength training (Shoulder girdle and respiratory muscles) and drug (Jatamansi) on reducing hand tremor in archers. Design: Randomized experimental, different subject design. Setting: Institutional study done at Manipur Archery Association. Interventions: Thera-band (Hygienic Corporation) and Nardostachys jatamansi (GMP Certified) were used as interventions. Pre & Post Hand Steadiness Score (Arm-Hand-Steadiness Tester on 4 mm hole) and Performance Test Scores (AAPHER Archery Test) was evaluated. 45 National Level Archers within age 16-28yrs were assigned into three groups: Group I (Medicine), Group II (Strength Training) and Group III (Control). Medicine was consumed for 4 weeks continuously 3 gm TDS and Thera-band strength training was given for 3 times weekly for 4 weeks. Main Outcome Measures: It was hypothesized that strength training is more effective in reducing hand tremors than medicinal or control groups. Results: Arm-Hand-Steadiness: Numbers of Contact (NOC): On inter Group Comparison of the three Groups using One Way ANOVA, left and right arm showed significant differences (F = 5.64, p<0.05; F = 8.97, p<0.001) respectively and on Post Hoc Scheffe’s analysis, Group II showed maximum improvement. Total Contact Time (TCT): On inter Group comparison of the three Groups using One Way ANOVA, left and right arm showed significant differences (F = 12.50, p<0.001; F = 9.23, p<0.001) respectively and On Post Hoc Scheffe’s analysis, Group II showed maximum improvement. Archery Performance: On inter Group comparison of the three Groups using One Way ANOVA showed significant difference (F = 6.53, p<0.005) and on Post Hoc Scheffe’s analysis, Group II showed maximum improvement. Conclusion: Intervention of strength training and jatamansi may be effective for an archer however, Strength training with Thera-band and Respiratory Muscles Conditioning intervention in combination increases steadiness and archery performance.
Folic acid has been used as a coadjuvant of antidepressant treatment, and low levels have been reported in depressed patients. Due to the role of folic acid and the relevance of serotonergic system in lymphocytes during depression, the aim of this study is to explore the capacity for serotonin production and the presence of tryptophan hydroxylase in lymphocytes of patients treated with fluoxetine and folic acid. Patients were diagnosed by the criteria of Diagnostic and Statistical Manual of the American Psychiatric Association, the severity of the depressive episode was evaluated by Hamilton Scale for Depression. Twenty seven patients (21-58 years) selected did not present other disorder neither risk of suicide. They were randomly distributed, some (14) received 20 mg/d of fluoxetine plus 10 mg/d folic acid, and others (13) fluoxetine and placebo. Control group was composed by 15 apparently healthy subjects (26-49 years). Ten patients from each experimental group finished the study. Blood samples were taken prior and six weeks after treatment. Plasma homocysteine decreased with folic acid. Lymphocytes were isolated by density gradients with Ficoll/Hypaque and differential adhesion to plastic. Serotonin concentration was not different between the two groups and neither respecting controls, but it was low in those that received treatment. Serotonin synthesis from tryptophan was lower in patients than in controls and decreased in patients after the treatments. The number of lymphocytes with the enzyme was lower in patients and decreased after folic acid. The results could be in agreement with the monoaminergic hypothesis of depression, in which modifications of central and peripheral serotonin systems take place. Decreases of serotonin concentration after treatments and of its synthesis before treatment might indicate modulations in relation to lymphocyte function in depressed.
Favored by the appearance of the human immunodeficiency virus (HIV), tuberculosis has significantly increased. It has been stated that TB and HIV make a lethal partnership: one accelerating the advance of the other and viceversa; that iswhy early diagnosis of TB has become so important. This paper is aimed at analysing the positivity of the bacteriological diagnosis of Mycobacterium tuberculosis in our laboratory, from January 2000 to December 2006, in symptomatic patients, including HIV+ patients receiving treatment in our institution. All samples studied were processed by conventionaldiagnostic techniques, direct examination as well as by culture and identification tests. A total of 8151 samples were analysed, but Mycobacterium tuberculosis was isolated in only 205 of those samples for a 2,5% of positivity. Susceptibility test, through Canetti’s method, was applied to the strains isolated; no resistant strains to any of the antitubercular drugs used were observed. This fact makes us state that these types of studies are extremely important for the epidemiological surveillance of this disease. Results obtained show that the HIV/AIDS/TB association in our country has evolved slowly and that its increase is not comparable to that reported in the international literature, this show the efficient of the two National Control Programs for this diseases.
This paper describes the advances in the development and implementation of the ontology to represent the domain concept of a Generator System of Constructivist Teaching-Learning Environment Based on Learning Objects under development named AMBAR. The ontology was developed using the open source software Protégé, with the OWL plugin. The domain and scope of the ontology was determined, and after reviewing the ontology work available, the class’s names and the class hierarchy were defined. The main classes were: Services, Metadata, Rol, Method, Learning Objects, Activities, Competencies, Framework and Person. These classes were described and their relationship established. The ontology development allowed to share and to find consensus view inside AMBAR´s multidisciplinary team, and will let AMBAR to have intelligent search mechanisms based in the semantic web vision.
The Bauhinia megalandra leaves aqueous extract, has been commonly used in Venezuela for the empiric treatment of diabetes mellitus. In this work, the effect of B. megalandra aqueous extract on hepatic epinephrine or dibutyril cAMP stimulated glycogenolysis, was studied. The oral administration of the plant extract to feed rats decreased the epinephrineraised glycemia in a statystical significant way. Furthermore, liver slices from feed rats, incubated in the presence of B. megalandra extract, released less glucose in response to epinephrine or dibutiryl cAMP than controls. This results shows a decrease in the hepatic glycogenolysis due to the effect of B. megalandra leaves aqueous extract probably as result of the inhibition of the glucose-6-phosphatase enzyme.
