Se caracteriza por presencia de alodinia, disestesia, parestesias y paroxismos de dolor agudo. MÉTODOS: Se realizó un estudio prospectivo, aleatorizado, fase III para determinar la eficacia, en términos de respuesta clínica, del tratamiento con gabapentina comparada con carbamazepina en mujeres con cáncer de mama estadios I, II, III y IV, con dolor neuropático. Las pacientes eran aleatorizadas a recibir (brazo A) gabapentina dosis de 300 mg de inicio y cada tres días se aumentaba 300 mg (25 pacientes) el (brazo B) carbamazepina 200 mg de inicio y cada tres días se aumentaba 200 mg (25 pacientes) hasta obtener respuesta completa o la mayor dosis establecida. RESULTADOS: Las tasas de respuesta global fue 63 % en el brazo A y 25 % en el brazo B (P = 0,032). Respuesta completa 19 % vs 6 % en el brazo A y B respectivamente. Respuesta parcial 44 % (brazo A) vs 19 % (brazo B). Enfermedad estable 31 % vs 38 % en el brazo A y brazo B respectivamente, progresión de enfermedad 6 % (brazo A) y 38 % (brazo B). Con toxicidades similares en ambos grupos. CONCLUSIONES: El esquema de tratamiento con gabapentina es más eficaz y bien tolerado que el de carbazepina, en pacientes con dolor neuropático y cáncer de mama.<hr/>OBJECTIVES: The pain neuropatic in breast cancer is for effect of the lesions and the dysfunction in the nervous system it is characterized by alodinia presence, disestesia, parestesias and paroxysms of sharp pain. METHODS: Realized a prospective study, randomized, phase III. to determine the effectiveness, in terms of clinical answer, of the treatment comparative gabapentina with carbamazepina in women with breast cancer stage I, II, III and IV, with sintomatology of pain neuropatic. Which were randomized to receive (arm A) gabapentina doses of 300 mg beginning and every three days you increased 300 mg (25 patients) the (arm B) carbamazepina 200 mg beginning and every three days you increased 200 mg (25 patients) until obtaining complete response or found the biggest established dose. RESULTS: The rates of global response were 63 % in arm A and 25 % in the arm B (P = 0.032) the complete response was 19 % vs 6 % in the arm A and B respectively. The partial response was 44 % (arm A) vs 19 % (arm B). Stable disease 31 % vs 38 % in the arm A and arm B respectively, progression of the disease 6 % in (arm A) and 38 % in (arm B) with similar toxities in both groups. CONCLUSIONS: The treatment outline with gabapentina is more effective and very more tolerated versus carbazepina, in the patient with pain neuropatic and breast cancer disease. <![CDATA[<b> Evaluación del estado nutricional en niños con cáncer</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300003&lng=en&nrm=iso&tlng=en OBJETIVOS: La desnutrición constituye el diagnóstico secundario más común en pacientes con cáncer; esto conlleva a que en el momento de diagnosticar la enfermedad, se realice la evaluación nutricional, que permitirá establecer el soporte nutricional adecuado y el seguimiento del paciente. El objetivo principal de este trabajo es evaluar el estado nutricional de niños que asisten a la consulta de oncología pediátrica del Instituto Oncológico “Dr. Luis Razetti”. MÉTODOS: Se realizó estudio descriptivo, transversal y prospectivo de 57 pacientes con patología oncológica que asistieron a la consulta del servicio de oncología pediátrica del Instituto Oncológico “Dr. Luis Razetti”, del Distrito Capital durante los meses enero- mayo año 2006, a los cuales se les practicó evaluación integral del crecimiento y estado nutricional. RESULTADOS: Se evidenció malnutrición por déficit en 23 (40,4 %) de los niños, nutrición normal en 28 (49,1 %) y malnutrición por exceso en 6 (10,5 %). En cuanto a la relación entre tratamiento y malnutrición, encontramos una diferencia estadísticamente no significativa. El 100 % de los pacientes con enfermedad avanzada presentaban algún grado de malnutrición por déficit, encontrando una diferencia estadísticamente significativa. CONCLUSIÓN: Los resultados obtenidos nos permite hacer las siguientes recomendaciones: 1. Estandarizar criterios de riesgo nutricional para pacientes pediátricos oncológicos, adaptados a la realidad nacional. 2. Evaluación nutricional integral a todos los pacientes pediátricos oncológicos al momento del diagnóstico. 3. Intervención nutricional individualizada. 4. Soporte nutricional y apoyo psico-social través de un equipo multidisciplinario e interdisciplinario.<hr/>OBJECTIVES: The malnutrition is the more frequent second diagnosis in patients with cancer; these due to be at the diagnosis moment of the disease it is necessary to make the nutritional evaluation, which will allow us to establish the best suitable nutritional support and the follow up of the same patients. The purpose of this study is to evaluate the nutritional state of the children whom assist to consult of Pediatric Oncology Department “Dr. Luis Razetti” Oncology Institute. METHODS: Realized descriptive, cross-sectional and prospective study of 57 patients with oncology pathology who attended in the consultation of the Service of Pediatric Oncology, Dr. Luis Razetti of Capital District between the months January and May of year 2006. To whom was practice integral evaluation of the growth and the nutritional state. RESULTS: There evidenced of malnourished for deficit in 23 patients (40.4 %), normal well-nourished in 28 (49.1 %) and malnourished for overweight in 6 (10.5 %). The relationship between malnutrition and oncology therapy is not statistically significant. Nevertheless, 100 % of the patients with advanced cancer were some grade of undernourished found a statistically a difference. CONCLUSION: The results allow us to make the following recommendations: 1. To standardize criteria of nutritional risk for oncology pediatric patients, adapted to the national reality. 2. Nutritional Evaluation to all the oncology pediatrics patients at the time of diagnosis. 3. Individualized nutritional intervention. 4. Nutritional and psycho-social support thought a multidisciplinary and an interdisciplinary staff. <![CDATA[<b> Tipificación del virus del papiloma humano  en carcinoma de células escamosas de cabeza y cuello</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300004&lng=en&nrm=iso&tlng=en OBJETIVOS: El cáncer de cabeza y cuello es atribuido al consumo de tabaco, alcohol, el virus del papiloma humano representa un riesgo potencial para estos tumores. MÉTODOS: Se evaluaron 29 muestras de pacientes con carcinoma de células escamosas histológicamente confirmados en el servicio de cabeza y cuello del Hospital Oncológico “Padre Machado”. Se les realizó una toma de muestra del tumor la cual fue inicialmente congelada a 80º C luego procesada a través de reacción en cadena de polimerasa, se determinó el tipo de virus que infectaba al tejido muestreado. Se documentó sobre datos demográficos: edad, sexo, uso de tabaco, alcohol, localización, estadio y grado tumoral, se realizó análisis estadístico con “t” de Student. RESULTADOS: El 34,48 % resultó positivo para ADN del virus, siendo los de bajo riesgo los implicados. Factor asociado a presencia del virus papiloma fue significativo el hábito tabáquico, si era en la actualidad, y la localización del tumor en cavidad oral. Sexo, edad, consumo de alcohol, grado tumoral, tamaño, estado ganglionar y estadio clínico no fueron estadísticamente significativos. CONCLUSIÓN: La mayoría de trabajos de cáncer de cabeza y cuello han enfocado el estudio de virus de alto grado como 16 y 18 en etiología de tumores. Tipos como l6 y 11 no han sido identificados en muchas lesiones malignas. Nuestro principal hallazgo sugiere que los virus 6 y 11 pueden estar involucrados en la génesis del carcinoma de células escamosas de cabeza y cuello.<hr/>OBJECTIVES: Head and neck squamous cell carcinoma is a disease attributed to tobacco and alcohol abuse; however, human papillomavirus also represents a potential risk factor for this malignancy. METHODS: We examined tumor specimens from 29 patients with histological confirmed, newly diagnosed at the head and neck department of the Oncology Hospital “Padre Machado”. Fresh tumor specimens were frozen and stored at -80º C until processing. We determined the presence or absence of high-risk or low-risk DNA in all tumor tissues by using polymerase chain reaction demographic data, including age at diagnosis; sex, alcohol, tobacco exposures, tumor site, stage, and grade were documented. The t Student-test was used to examine the association of virus with these factors. RESULTS: 34.48 % were positive to AND of virus. The low risk virus was the implicated. Current smokers in virus positive were most frequent than in negative if it was recent and the tumors mainly located in oral cavity. Gender, age, alcohol consumption, grade, size, lymph nodes status, clinical stages were not significant stadistic associated with presence of virus. CONCLUSIONS: The majority of studies of head and neck squamous cell carcinoma have focused on high grades virus how 16 and 18 in the etiology of this tumors Other types-such as 6 and 11 -have not been identified in many head and neck malignancies. Our findings suggest that virus 6 and 11 may play a role in the etiology of head and neck squamous carcinoma. <![CDATA[<b> Neoplasias infiltrantes de vulva</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300005&lng=en&nrm=iso&tlng=en OBJETIVOS: El cáncer de vulva es una enfermedad de mujeres en edad avanzada, también ha sido observado en mujeres premenopáusicas. Generalmente es del tipo carcinoma de células escamosas, aunque se presentan otros tipos histológicos. La cirugía es la terapéutica de elección por ser más eficaz, incluye exéresis amplias, vulvectomías parciales, simples, radicales (esta última con vaciamiento inguino femoral) y exenteraciones pélvicas. La radioterapia y poliquimioterapia son terapéuticas generalmente adyuvantes al tratamiento quirúrgico. MÉTODOS: Trabajo de investigación de tipo retrospectivo, descriptivo con estudios de distribución de frecuencias. El universo correspondió a 36 pacientes ingresados al servicio de ginecología oncológica del Instituto de Oncología “Dr. Miguel Pérez Carreño” de la ciudad de Valencia, Venezuela, entre los años de 1995 y 2006, y con diagnóstico de neoplasia infiltrante de vulva. RESULTADOS: De la totalidad de las pacientes evaluadas se obtuvo que la mayor incidencia de esta patología se encontraba entre las pacientes mayores de 71 años con 36,11 %. La histología más frecuente fue el carcinoma epidermoide. Más de un 75 % de las pacientes ingresó en estadios avanzados. El tratamiento más realizado fue la vulvectomía radical con un 63,8 %. CONCLUSIÓN: En base a los datos obtenidos se sugiere lograr mejoras en los métodos de pesquisa para así lograr diagnósticos en etapas más tempranas, de esta manera evitar cirugías más agresivas que repercuten en un aumento de la morbi-mortalidad en este grupo de pacientes, y así lograr una mejor calidad de vida.<hr/>OBJECTIVES: The cancer of vulva is fundamentally a disease of women in age outpost, but it has been observed in premenopausic women. It is generally the escamosus type carcinoma the most common, although other types can be appearing. The surgery is the gold standar and includes big exegesis, partial and simple and radical vulvectomy, (these last with femoral inguinal evacuating), and pelvic exenterating surgery. The radiotherapy and the polichemotherapy are generally adjuvant treatment. METHODS: This is a retrospective, descriptive type of investigation with studies of distribution of frequency. The universe in study is 36 patients entered to the service of gynecology oncology of the Institute of Oncology. “Dr. Miguel Perez Carreño” of the city of Valencia, Venezuela, between the ages of 1995 and 2006, and with diagnostic of infiltrated neoplasia of vulva. RESULTS: From the totality of the evaluated patients it was obtained that the greater incidence of this pathology was found between 71 age and greater patients with 36.11 % the most common histology was escamosus carcinoma. More of 75 % of the patients were in advanced stages. The most common treatment was radical vulvectomy with a 63.8 %. CONCLUSION: On the basis of the collected data it is suggested to manage improvements in the of search of early stages, this way let us avoid an aggressive surgery which they repel in an increase of the morbimortality in this group of patients, and to find one better quality of life. <![CDATA[<b> Resección colorrectal laparoscópica </b>: <b> Experiencia inicial</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300006&lng=en&nrm=iso&tlng=en OBJETIVOS: Evaluar factibilidad resultados a corto plazo de la resección colorrectal laparoscópica en pacientes con patologías benignas y malignas de colon y recto. MÉTODOS: Estudio retrospectivo, descriptivo incluyó pacientes programados para resección colorrectal laparoscópica durante 6 años, entre noviembre 2000 marzo de 2 006. Se analizaron: edad, sexo, diagnóstico de ingreso, distribución anatómica, procedimiento quirúrgico, tiempo quirúrgico, complicaciones intraoperatorias, tasa de conversión, morbilidad posoperatoria y estancia hospitalaria. En caso de patología oncológica, se evaluó: estadiaje patológico, número de ganglios disecados, tamaño, negatividad de márgenes quirúrgicos, recurrencia local a distancia. RESULTADOS: Se incluyeron 26 pacientes con patología colorrectal diversa programados para resección laparoscópica. Edad promedio fue 56,3 años. 20 pacientes tenían diagnóstico de adenocarcinoma colorrectal (76,92 %). El 46,15 % de los adenocarcinomas estaban localizados en recto. Se realizaron 10 hemicolectomías radicales derechas (38,46 %), 8 resecciones anteriores bajas de recto (30,76 %), 4 resecciones abdomino-perineales (15,38 %), 3 sigmoidectomías (11,58 %) una colectomía total (3,84 %). La tasa de conversión fue 38,46 %. Tiempo quirúrgico promedio 324 minutos. La morbilidad a los 30 días de posoperatorio fue 23,27 %. No hubo mortalidad. Tiempo promedio de hospitalización fue 6,7 días. En los casos oncológicos, el promedio de ganglios disecados fue 12. Los márgenes fueron negativos en todos los casos. CONCLUSIONES: La resección colorrectal laparoscópica es factible. Ofrece buenos resultados a corto plazo, aunque a expensas de un tiempo quirúrgico mayor. En patología maligna, los criterios oncológicos de radicalidad no se ven comprometidos.<hr/>OBJECTIVES: Evaluate feasibility and short term results of laparoscopic colorectal resection in patients with benign and malignant colorectal disease. METHODS: Retrospective descriptive series included patients scheduled for colorectal laparoscopic resection during 6 year period: November 2000, March 2006. Clinical aspects in medical charts were analyzed: age, sex, and preoperative diagnosis, anatomical location of disease, surgical procedure, operative time, intraoperative complications, conversion rate, postoperative morbidity, mortality hospital stay. Cases with diagnosis of colorectal carcinoma we analyzed pathologic staging, number retrieved lymph nodes, size negativity margins local or distant recurrence. RESULTS: 26 patients with colorectal disease scheduled for laparoscopic colorectal resection were included. Mean age was 56.3 years. 20 patients had diagnosis of colorectal adenocarcinoma (76.92 %). 46.15 % of these were located in rectum. 10 radical right colectomies were performed (38.46 %), 8 low anterior resections (30.76 %), 4 abdominoperineal resections (15.38 %), 3 sigmoidectomies (11.58 %) and one total colectomy (3.84 %). Conversion rate was 38.46 %. Mean operative time was 324 minutes. Morbidity at first 30 days of postoperative period was 23. 37 %. No postoperative deaths were registered. Mean hospital stay was 6. 7 days. In those cases with malignant colorectal disease, the mean number of retrieved lymph nodes was 12. Surgical margins were negative in all cases. CONCLUSIONS: Laparoscopic colorectal resection is feasible. It offers good short term results in spite of a prolonged operative time. Oncological criteria are not violated when managing malignant colorectal disease. <![CDATA[<b> Sarcomas de tejidos blandos  no rabdomiosarcoma en niños</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300007&lng=en&nrm=iso&tlng=en OBJETIVOS: Determinar incidencia, epidemiología factores pronósticos en niños con sarcomas de tejidos blandos no rabdomiosarcoma). MÉTODOS: Se revisaron las historias de 38 pacientes obteniéndose datos relacionados con: edad, sexo, localización, volumen tumoral, histología, inmunohistoquímica, metástasis, tratamiento, status actual. RESULTADOS: Entre 19952005 hubo 38 casos. Veintiuno masculinos, edad media 8,5 años (rango 4m-16 años). Un 65,78 % requirieron inmunohistoquímica para diagnóstico definitivo. Biopsia por aspiración con aguja fina realizada en 39,47 % sin obtenerse diagnóstico histológico. Tumores malignos de la vaina de los nervios periféricos representaron 31,57 % fibrosarcomas (18,4 %). Las extremidades fueron las más afectadas (39,47 %) seguido por la región torácica (26,31 %). Ventiseis pacientes (68,42 %) alcanzaron remisión completa con cirugía y 4 con cirugía, quimioterapia y/o radioterapia. No hubo pacientes metastáticos. Un 65,78 % fueron malignos. Tumor < 5 cm 52,63 %. Hubo 2 recaídas locales en pacientes con cirugías incompletas. 28 estadios I, 9 II y 1 ST III. Cinco murieron por progresión y 3 están vivos con enfermedad. Un 78,24 están vivos sin enfermedad con una media de seguimiento de 5,16 años. CONCLUSIONES: En nuestro centro se diagnostican 3,8 casos/año, con una incidencia de 4,2 %. Los varones y la localización en extremidades son los más afectados. La inmunohistoquímica es fundamental para el diagnóstico y la BAAF no fue útil y retardo el diagnóstico. Tumores intraabdominales, histología alto grado, tumores ≥5 cm fueron factores adversos. El tratamiento de elección es la cirugía, quimioterapia y radioterapia permiten rescatar algunos pacientes.<hr/>OBJECTIVE: Determine the incidence, epidemiology and the prognostic factors in children with non-rhabdomyosarcoma soft-tissue sarcoma. METHODS: 38 patients were reviewed obtaining itself data related: Age, sex, anatomic site, tumor size, immunohistochemistry, metastases, treatment and present status. RESULTS: 1995-2005 were 38 cases. 21 males average 8.5 years (rank 4m-16 years). 65.78 % required immunohistochemical for diagnosis definitive. Biopsy by aspiration with fine needle was made in 39.47 % without obtaining histological diagnosis. Malignant peripheral nerve sheath tumor represented 31.57 % fibrosarcomas (18.42 %). The most common anatomic site affected were extremities (39.47 %) followed by chest wall (26.31 %). 26 patients (68.42 %) reached complete remission with surgery and 4 with surgery plus chemotherapy and/or x-ray. There was no metastatic disease. 65.78 % were malignant. 18 patients (47.27 %) had tumors ≥ 5cm. There were 2 local relapses in patients with incomplete surgeries. 28 stages I, 9 II 1 ST III. 5 died by progression 3 are alive with disease. 78.24 % are alive without disease at a median follow-up of 5.16 years. CONCLUSIONS: The incidence of non-rhabdomyosarcoma soft-tissue sarcoma in our center was 4.2 %. There is a slight male predominance. The extremities were the primary site more affected. Immuno-histochemestry is fundamental for diagnosis. BAAF was not useful and delayed the diagnosis. Intraabdominal tumors, high grade tumors and tumors ≥ 5 cm were adverse factors. The main treatment is the surgery and chemotherapy, radiotherapy allowed rescuing some patients. <![CDATA[Tumores neuroendocrinos: Presentación de dos casos de tumores carcinoides del pulmón]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300008&lng=en&nrm=iso&tlng=en Los tumores carcinoides son un grupo de neoplasias poco frecuentes, y representan una variedad de tumores derivados de la cresta neural, en el pulmón son de aparición rara. Los carcinoides son de localización endobronquial y clínicamente se presentan como atelectasias, neumonía recurrente, absceso pulmonar y bronquiectasias. Se reportan los casos de dos pacientes quienes consultaron por tos, dificultad respiratoria y hemoptisis, observándose en la radiografía de tórax de ambos presencia de atelectasias en el campo pulmonar derecho. Se realiza fibrobroncoscopia flexible y se evidencian en ambos casos una lesión endobronquial localizada en un paciente en el bronquio lobar inferior derecho y en el otro paciente en el bronquio principal derecho. En uno de los pacientes se realizó lobectomía inferior derecha y en otro paciente una neumonectomía derecha, reportando el estudio histopatológico en ambos casos la presencia de un tumor carcinoide.<hr/>The carcinoid tumors are a group of neoplasm that represents a rare variety of tumors derived from the neural crest. In the lung are rare. The carcinoid are neoplasm of endobronquial location and the clinical manifestations includes in both: atelectasy, recurrent pneumonia, pulmonary abscess and bronquiectasy. We presented the cases of two patients whom consult by cough, respiratory difficulty and hemoptysis, there are being observed in the two cases the presence of right lung actelectasy in the thorax x-ray. The flexible fibrobronchoscopy realized reported an endobronchial neoplasm localized in one patient in the inferior right lobe and in the other patient a lesion in the entrance of the principal right bronchus. The surgical procedure development in one patient included: an inferior right lobectomy and in the other a right neumonectomy. The hispatological study in both cases reported: carcinoid tumor. <![CDATA[<b> Leiomiosarcoma hepático </b>: <b> Reporte de un caso y revisión de la literatura</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822007000300009&lng=en&nrm=iso&tlng=en OBJETIVO: Se presenta el caso de paciente masculino de 20 años, quien en el mes de febrero de 2005, presenta debilidad general y pérdida de peso progresiva cuantificada en 8 kg. MÉTODOS: Desde inicio de marzo de 2005 evidencia palidez cutánea, hipertermia precedida de escalofríos, de predominio vespertino, dolor en epigastrio, opresivo, no irradiado, motivo por el cual consulta a facultativo en su localidad, donde es hospitalizado. Permanece 30 días, recibiendo tratamiento con antibióticos sin mejoría, ameritando transfundir por presentar hemoglobina de 6 g/dL, sin sitio evidente de sangrado. Ecosonograma abdominal: hepatoesplenomegalia y LOE lóbulo hepático izquierdo, gastroscopia y colonoscopia normal. Resonancia magnética abdominal: evidencia de hepatomegalia asociado a la presencia de lesión de apariencia tumoral en el lóbulo hepático izquierdo, con realce heterogéneo. La evolución no fue la de un absceso hepático y la imagen ecosonográfica es de comportamiento sólido, por lo que se solicita a cirugía laparoscopia diagnóstica. Se descarta la posibilidad de toma percútanea guiada por eco en vista de alta posibilidad de sangrado. Se practica hepatectomía izquierda, biopsia: reporta: leiomiosarcoma hepático. Luego de la cirugía el paciente evoluciona satisfactoriamente. CONCLUSIÓN: A pesar de su poca frecuencia, los sarcomas hepáticos deben tomarse en cuenta en el diagnóstico diferencial de tumores hepáticos sólidos en el adulto, en ausencia de cirrosis. El único tratamiento que permite expectativas de supervivencia es quirúrgico, mediante hepatectomías regladas o “atípicas” con márgenes de seguridad e incluso en algunos casos trasplante hepático.<hr/>OBJECTIVE: Present a clinical case of 20 years old male, it initiates in the month of February of 2005, when presents a displays general weakness and progressive loss of quantified weight as 8 kg. METHODS: From beginning of March of 2005 demonstrates cutaneous pallor, chills, in the evening, epigastric pain, not radiated, reason by which he consults to facultative in its locality, where he is hospitalized. He remains 30 days, receiving treatment with antibiotics without improvement, needing a transfusion to present/display hemoglobine of 6 g dL, without no evident site of bled. Abdominal ecosonogram: hepatoesplenomegaly LOE hepatics lobe left. Gastroscopy and normal colonoscopy. El ameloblastoma multiquístico es un tumor odontogénico benigno, localmente agresivo, que constituye 1 % de los tumores maxilares. Su alta tasa de recidiva local, la invasión más allá del límite radiológico exigen su resección en bloque con importante impacto estético para el paciente. MÉTODOS: Estudio descriptivo de caso clínico y revisión de la literatura de paciente del servicio de cirugía I “Hospital Vargas” de Caracas. RESULTADOS: Paciente de 43 años portadora de ameloblastoma multiquístico recidivado en antro maxilar derecho tratado con resección craneofacial con márgenes oncológicos y reconstruidos con rotación de colgajo microquirúrgico de recto abdominal. CONCLUSIÓN: La resección craneofacial es el tratamiento de elección para lesiones tumorales localizadas por encima de la línea de Ohngren´s. Una resección adecuada debe incluir piso de la fosa craneal anterior, paredes medial, superior de la órbita y usualmente contenido orbitario. En la mayoría de las instancias el ojo debe ser sacrificado. Los ameloblastomas son tumores odontogénicos benignos localmente agresivos en un 15 % 20 % se localizan en el maxilar superior, teniendo esta localización peor pronóstico por un mayor índice de recidivas posquirúrgicas, invasión tejidos vecinos y porcentaje de malignización, requiriendo resección en bloque, con un impacto estético importante para el paciente. La rotación de colgajo microquirúrgico de recto abdominal es una opción versátil para la reconstrucción de estos pacientes.<hr/>OBJECTIVES: The craniofacial surgery is therapy of choice for tumors lesions which compromise the upper structure bones, orbital fosse and anterior cranial fosse. The multicyst ameloblastoma is a benign odontogenic tumor, locally aggressive, and accounts for 1 % of all maxilla tumors. The high rate of local recurrence and extension beyond radiological limits requires in bloc surgery, with a major esthetic impact for the patient. METHODS: A case report descriptive study and literature review, of patient to viewer in the Service of Surgery I of Hospital Vargas of Caracas. RESULTS: Female patient of 43 years old with diagnoses of multirecurrent multicyst ameloblastoma who undergo to craniofacial surgery with oncological margins, and she was reconstructed with a micro free flap of rectum abdominal. CONCLUSION: The craniofacial surgery is the therapy of choice for tumors located upper Ohngren´s line. One adequate resection must include anterior cranial fosse floor and orbital medial and upper walls and the orbital content. Orbital exenteration is sometimes necessary. In the mayority of cases the eye was sacrificed. Ovario derecho, otro de 6 cm, ovario izquierdo, útero fibromatoso, biopsia intra-operatoria: teratoma quístico con zona de tejido tiroideo, se difiere para biopsia posoperatoria. La biopsia definitiva: tejido tiroideo con carcinoma papilar en teratoma quístico maduro de ovario derecho, ovario izquierdo: teratoma quístico maduro. Se presenta caso ya que se encuentran zonas de tejido tiroideo en menos del 15 % de teratomas quísticos maduros de ovario, sólo ocasionalmente pueden tener cualquier tipo de tumor diferenciado papilar o folicular de tiroides.<hr/>A clinical case of 42 years old woman, with increase abdominal size, 6 month of evolution; physical examination: a pelvic tumor up to umbilical scar, movable, firm, not painful. The ultrasound and CT scan report: tumor in right ovary, of 15.2 x 10.3 cm, with calcium and grease, uterus of 12x9x4 cm, suggestive of dermoid cyst and fibrous uterus. We found at surgery tumor in right ovary, of 15 cm, with large stem; another of 6 cm, in left ovary, fibrous uterus; intraoperatory biopsy: dermoid cysts of right ovary with thyroid gland tissue, the pathologist defer the intraoperatory biopsy. Definitive biopsy: thyroid gland tissue with papillary cancer in right ovarian dermoid cyst, dermoid cyst of left ovary. En este reporte se presenta un paciente con un tumor de células gigantes en el esternón y se revisa la bibliografía. Se trata de paciente masculino de 37 años de edad que inicia su enfermedad actual hace aproximadamente un año, se caracterizaba por un dolor progresivo y aumento de volumen a nivel de la región esternal, por lo cual consulta a facultativo quien solicita radiología convencional y tomografía del tórax, y gammagrama óseo, estos reportaron hallazgos sugestivos de tumor en esternón. Al paciente se le practica biopsia incisional que reporta tumor de células gigantes. Se realiza resección oncológica con esternectomía parcial y reconstrucción inmediata colocando malla protésica y cemento óseo. En Venezuela no han sido reportados casos de tumor de células gigantes localizados en el esternón. En nuestra institución este es el primer caso descrito.<hr/>The giant cell tumor is an unusual tumor, being the sternum and in exceptional site of presentation. In these report we present a patient with giant cell tumor in sternum, and brief review the literature. Is a male patient of 37 years, with current illness began one year before, characterized for given by progressive increase of pain and volume in sternum, he consult to physician and was evaluated, who petition for conventional radiographic and tomography of thorax, and an osseous bony scan, with suspicious of sternum’s tumor. The patient underwent incision biopsy of sternum that histological diagnostic report a giant cell tumor. We realized him a total oncological resection of the tumor and the sternum and immediately reconstruction defect is filled with osseous cement and prosthesis. In Venezuela there have not been reported any cases of giant cell tumor in sternum.
