OBJECTIVE: Breast cancer in young women has different biological and clinical characteristics when is compared with breast cancer in third age women. METHOD: A prospective study of risk factors for development breast cancer was done comparing both age groups. Thirty six patients < 40 years and 100 patients > 64 years from a total of 515 breast cancer patients were included between april 2006 and april 2007. Family history of breast and ovary cancer, hormonal risk factors (endogenous and exogenous) previous history of breast disease were studied. RESULTS: The significant results were as follows: Family history of breast cancer (31 % < 40 years vs. 14 % > 64 years, P = 0.015), family history of ovary cancer (25 % < 40 years vs. 15 % > 64 years, P = 0.04), mean age of first terminal pregnancy (21.11 years > 64 years vs. 23.0 years < 40 years, P = 0.05), mean number of terminal pregnancies (5.1 > 64 years vs. 2.2 < 40 years, P = 0.00014), and oral contraceptives ingestion (67 % < 40 years vs. 13 % > 64 years, P = 0.0000001). CONCLUSION: We concluded that family history of breast and ovary cancer, age of first terminal pregnancy, number of terminal pregnancies, and oral contraceptives ingestion were the most important risk factors in young women with breast cancer, being the first time that family history of ovary cancer and number of terminal pregnancies are reported.
The patient with malignancy disease has a high risk to develop respiratory infections for Pneumocystis jirovecii pneumonia. Investigations about pneumocystosis in oncological patients in Venezuela are scarce. The objective of this work was to detect Pneumocystis jirovecii in oncological patients by the method of direct immunofluorescence technique. Thirty one respiratory specimens (bronchoalveolar lavage, spontaneous and induced sputum, and tracheal aspirates) received in 10 months, 8 specimens of them (25.5) were positive the distribution by malignancy disease was the following: 18 solid tumors, and 13 leukemias, and lymphomas. No statistically significant differences were found between the studied groups and positive results (P>0.05). The complementary laboratory tests, related to the presence of Pneumocystis, were not statistically significant either P>0.05). Is necessary to include this diagnosis in the microbiological differential study of low respiratory tract infections in oncological patients, since these patients show unspecific symptoms, and have a high possibility to develop pneumocystosis.
The miofibroblastoma is a benign tumor of the breast of very infrequent presentation that mainly affects the male breast. The main histological characteristic is the mesenchymal ancestry characterized by the proliferation of plump and spindle cells surrounded by collagen and that derive from fibroblasts. The surgery is the main treatment with preservation of the breast in female patients. We presented a case of a female patient of 64 years old, which presents a solitary tumor with progressive growth by 5 years, until she decides to ask for medical aid, considering the preoperating diagnosis of an tumor of mesenchymal type; which subsequent to the surgical treatment and confirmed by immunohistochemistry the presence of miofibroblastoma of the breast, being this finding very unusual in the clinical practice, and to be considered at the time of making differential diagnosis.
OBJECTIVE: The purpose of these work is evaluate the surgical management of non-palpable breast cancer diagnosed as a mammographic visible mass on screening, non visible ultrasonographic. METHODS: Analyzed retrospectively of tree cases with the diagnosis of non palpable breast lesion visible at the screening mammography and non visible at the ultrasound, they were excised after preoperative tumor localization with a wire. The performance of the sentinel lymph node biopsy was evaluated, if it was in the second surgery after the definitive histopathology confirmation. RESULTS: All the three cases underwent wire localization prior to the tumorectomy, in 2 cases the tumorectomy had wide margins, and in the other one they were close. In one patient we did tumorectomy and sentinel lymph node biopsy in the same surgical procedure. Two cases had invasive lobulillar carcinoma, one of them multifocal and in the other case invasive ductal. The average ages were 63 years (62 to 64 years). The averages of the lesion were 0.9 cm (range in four lesions 0.7 to 1.2 cm). In all the patient were performed the sentinel lymph node biopsy, being positive in just one of the cases. CONCLUSIONS: In patients the wire radio-localization represents a simple and efficient method to obtain the biopsy and could be performed in any center that counts with a mammographic. We suggest performing the sentinel lymph node biopsy with patent blue during the initial surgery if there is a strong confirmation of malignant.
