Burkholderia cepacia is a non fermentative grannegative bacilli (NFGNB) known as an opportunists pathogen for human beings and associated with plant diseases. Its identification is a difficult task, being usually confused with related genera. In this work a collection of 74 NFGNB from environmental and hospital sources were examined by biochemical and PCR methods in order to evaluate the conditions for hospital identification of B. cepacia. The use of conventional biochemical methods, and a selection of complementary tests allowed a satisfactory identification of 28 (37,8%) as B. cepacia, 15 (20,3%) S. maltophilia, 9 (12.2 %) A. xylosoxidans, 19 (25,6%) other Pseudomonas, 1 Ralstonia (1,4%) and 2 non identified ((2.7%). By PCR B. cepacia identification was confirmed and the genomovars or species of the B. cepacia complex (BcC) were differenciated. The majority (12) belongs to the genomovar I (B. cepacia), 5 to II (B. multivorans), 3 to III (B. cenocepacia), 1 to IV (B. stabilis), and 7 to the B. vietnamiensis group. The use of biochemical and molecular methods allowed us the identification of BcC members from environmental and hospital sources.
We used 62 strains initially identified as non fermenting gram-negative bacilli (NFGNB) isolated from patients with a nosocomial infection diagnosis with the purpose of identifying their genus and species. Forty five strains were identified as Acinetobacter baumannii, 10 as Pseudomonas aeruginosa, 3 as Stenotrophomonas maltophilia, 3 as Comamonas acidovorans and 1 as Achromobacter xylosoxidans subspecies xylosoxidans through the API 20NE. Regarding the Acinetobacter strains, the ARDRA allowed to identify 20 strains as A. baumannii and 23 as Acinetobacter genospecies 13TU, but 2 strains were not identified with this method. The ADNr 16S sequence of all the strains included in this study allowed to identify 20 strains as A. baumannii, 23 strains as Acinetobacter RUH1139, 10 as P. aeruginosa, 4 as A. xylosoxidans subsp. xylosoxidans (2 of these four strains strains had been identified as A. baumannii through API20NE), 3 as S. maltophilia, and 1 as C. acidovorans and 1 as β-Proteubacterium. The discrepancies between the biochemical identification by API 20NE and by ARDRA to differentiate the Acinetobacter genospecies was resolved by the ADNr16S sequencing, indicating that the identification of the Acinetobacter isolates, among other NFGNB, through API 20NE, should be confirmed through genetic techniques.
Infections caused by atypical mycobacteria at present constitute a serious health problem, especially in immunocompromised patients. These mycobacteria present particular susceptibility patterns, different from M. tuberculosis, due to which the administration of an adequate treatment requires a fast, simple and sensitive identification method. The PRA technique (PCR Restriction), based on the enzymatic digestion of the amplification product of the hsp65 gene has shown to be an adequate method for the identification of mycobacteria. In this study we compared the PRA technique with the standard mycobacterial identification method, represented by biochemical tests, in 30 isolates from the Tuberculosis Laboratory of the Instituto de Biomedicina. The PRA technique allowed the identification of 96% of the strains analyzed, as compared with 92% of strains identified through biochemical methods. The results obtained were identical in 18 of 22 strains, corresponding to 82% of the results. It is concluded that the PRA technique is a fast, simple and economical method that produces results in concord with traditional techniques, with a lesser degree of error. Based in these results, the use of PRA as routine identification technique for mycobacteria is recommended for clinical laboratories.
Integrons are genetic elements able to capture resistance genes, usually located in plasmids and responsible for dissemination of resistance in Gram negative bacilli. The presence of class 1 integrons and its association with plasmids in 23 multiresistant Klebsiella pneumoniae strains, isolated from different services of hospital “Antonio Patricio de AlcalᔠSucre-Venezuela was studied. By bacterial conjugation and polymerase chain reaction the transference of resistance determinants and the presence of integrons class 1 were evaluated. All isolates were resistant to ceftazidime and aztreonam. Conjugation assays demonstrated the presence of conjugative plasmids in all K. pneumoniae strains analyzed. Integrons were detected in 10 (43.47%) isolates. One integron element was present in nine strains and two elements in one single strain. Integrons identified were associated a plasmids. The presence of integrons class 1 carried by plamids must play an important role in the epidemiology of antibiotic resistance in clinical strains.
Serotype, antimicrobial susceptibility, and severity of the infection according to serotype, were determined in a group of 50 Shigella flexneri strains isolated from children less than 5 years old with diarrhea: 25 children dehydrated and 25 non dehydrated. The highest resistance percent was found for tetracycline (96%), followed by ampicylline (94%), chloramphenicol (90%), amoxicylline clavulonic acid (84%) and trimetoprim-sulfametoxazol (72%). Malnourishment (32%, P<0.05) and serotype 2a (56%, P<0.05) were seen more frequently in dehydrated children, while serotype 2b predominated in non dehydrated children (32%, P<0.05). Prevalence of serotype 2a and its association with the severity of the episode and high antibiotic resistance alert regarding the problem of Shigella infections in our country and reinforce the need of further studies to adjust treatment norms.
We carried out a descriptive prospective study in a series of cases of HIV positive patients to determine the morbid-mortality rates due to human immunodeficiency virus (HIV) at the medical services of the Hospital “Ruiz y Paez” in Ciudad Bolivar, Bolivar State, Venezuela, during the year 2005. We studied 53 adult patients. Data were collected through a registration form. We registered the entrance of an average of 4.4 cases of HIV infections per month; the productive age group (22-31 years) and the male sex were the most frequent. The use of non parenteral drugs predominated as indirect risk factor associated to the acquisition of the HIV infection because it conditions conducts that culminate in unsafe sexual practices in 26.4% (n=14) cases. Fever and weight loss appeared as the most frequent symptoms; the digestive tube and the central nervous system appeared as the most affected organs, Urinary infections, oropharingeal candidiasis and intestinal parasitosis were the most frequent associated infections. The proportion of cases studied for the series was 0.087 and the annual mortality rate was 13.2% (n=7 deaths).
The purpose of this work was to study Epstein Barr infection in HIV+ children less than 12 years old of both sexes, with TARAE and its relationship with the HIV viral load and T CD4+ lymphocyte count. The detection of anti CVA and anti EBNA1 IgM/IgG was done by ELISA, the EBV quantification in peripheral blood by PCR, and the immunophenotype by flow cytometry. The statistical study was done through Spearman’s correlation analysis and Student’s t test. The evaluation included 21 children with ages between 3 and 12 years, 18 of which (85.7%) presented a detectable EBV viral load. Two of them had an acute early infection (IgM CVA and IgM EBNA1 +), 5 an acute late infection (IgM CVA - IgM EBNA+) with a EBV viral load equal to 573 copies/ml, and 14 with past infections ( IgG CVA and IgG EBNA positive), 11 of which had an EBV viral load equal to 646 copies/ml (active chronics). There were no statistical significant differences when comparing the mean EBV viral load in acute and past infections, nor between those loads and the HIV loads, but there were differences in the T CD4+ loads; lower numbers were correlated with high EBV viral loads (p<0.05). Detection of anti CVA and EBNA1 antibodies, as well as EBV quantification through molecular tests, should be done to all HIV positive children, since a lower T CD4+ lymphocyte count means a higher EBV viral load.
The condylomata acuminata is one of the common sexually transmitted disease around the world. These lesions are associated with the Human Papillomavirus (HPV) types 6 and 11 of oncogenic low-risk. In our country, the prevalence for this virus infection is high (65 %). The aim of this study was the detection and typing of HPV by polimerase chain reaction (PCR) in a group of patients that presented warts in the genital area. Ninety biopsies were evaluated coming from patients with suggestive infection for the HPV: 72 (80 %) were histological confirmed as condylomata acuminata and 20% have other pathologies. The HPV was detected in all specimens of condylomata acuminate. HPV type 6 was observed with more frequency (54.17 %), followed by type 11 (36.11 %); three cases (4.17%) showed co-infection with types 6 and 11 and four cases (5.56 %) were non tipificable. These results show a clear association of low oncogenic risk HPV types 6 and 11 and the development of the genital warts unlike to the cervical intraepithelial neoplasia where exists a great HPV genotypes spectrum.
During the course of HIV-1 infection immunephenotyping of infected individuals is important to determine the basal immunological status, as well for initiating and monitoring therapy, for the basal immunephenotyping of recentlyHIV-1 diagnosed individuals we carried out a descriptive and retrospective study at the regional immunology unit (URIC) in Maracay, Venezuela, between January 2004 and December 2006. In the 238 patients included (70.6% men and 29.4% women) we studied CD4/CD8 T lymphocyte numbers through flow cytometry. Age range varied between 2-74 years, and the largest number of cases corresponded to the 25-34 age group. We found CD+ counts under 200 Cells/mm³ in 39.7% individuals; 35.0% had values between 200-499 cells/mm³; and only 25.2% had values over 500 cells/mm³. CD8+ counts in 52.6% of patients were higher than 900 cells/mm³ and 15.8% had counts under 500 cells/mm³. The CD4/CD8 index was lower than 1.0 in 94.9% of the individuals, demonstrating a growing tendency to diagnose HIV infection at a late stage, as shown by the decreased CD4+ counts found. Nevertheless, an increase in the diagnosis of new cases in patients under 35 years and women was also determined. Tissue dissemination and histopathology effects produced by Leishmania mexicana amazonensis in rodents experimentally infected http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562008000200012&lng=en&nrm=iso&tlng=en Al género Leishmania pertenecen diferentes especies responsables de diversas formas clínicas de leishmaniasis.
Listeria monocytogenes (Lm) is a bacterium widely distributed in nature which has been isolated from several animal and is capable of producing disease in human beings, especially when there are predisposing conditions. Pregnant women, their fetuses, aged persons and immunosuppressed individuals are susceptible. Listeriosis during pregnancy can produce intrauterine infection resulting in severe complications such as spontaneous abortion, preterminal deliveries and neonatal infections, resulting in high morbility and mortality. We present the case of a 39 years old patient with a 13-week pregnancy who was submitted to a cerclage technique. She later presented bacterial meningitis. Culture of spinal fluid, blood and urine, among others, were done and Lm was isolated from haemocultures. Finally she was moved to another institution for ventriculo-peritoneal drainage, where she later died. This is the first case of a severe Lm infection in a pregnant woman at the Concepción Palacios Maternity Hospital in Caracas, Venezuela, reported in the literature.
