En este trabajo se analiza la respuesta de anticuerpos IgA específicos frente a extractos de huevos de A. lumbricoides en el suero de niños de comunidades rurales del estado Miranda. Se evaluaron 100 niños entre 3 y 14 años de edad y se tomaron muestras de suero y el diagnóstico coproparasitológico. Según lo reportado fueron clasificados en: infectados solo por A. lumbricoides (Asc+), no infectados (NSOP), infectados con A. lumbricoides y otros parásitos (Asc+/OP+) e individuos no infectados por A. lumbricoides pero con otros parásitos intestinales (Asc-/OP+). El extracto fue probado por ELISA, Western Blot e inmunocitoquímica. El perfil electroforético del extracto mostró 7 bandas proteicas de 102, 92, 85, 65, 50, 45 y 30kDa. Por la ELISA, el 26% de los pacientes infectados por A. lumbricoides fueron seropositivos, mientras que el 56% en el grupo NSOP fueron seronegativos. El ensayo de inhibición con extracto de A. lumbricoides permitió identificar 3 bandas proteicas especificas reconocidas por la inmunoglobulina IgA de 92, 85 y 65 kDa diferentes a las reconocidas por el extracto de verme adulto. El nivel de anticuerpos fue significativamente mayor en los infectados que en los no infectados. La superficie de los huevos fue reconocida por la IgA en el suero de los individuos infectados. Estos resultados, indican que el huevo de Ascaris induce anticuerpos específicos contra proteínas que podrían ser utilizados como antígenos en pruebas serológicas que complementan el diagnóstico de Ascariasis.<hr/>Ascariasis is a disease caused by the nematode A. lumbricoides affecting 1 billion people worldwide. In this study the specific response for IgA antibodies against extracts of A. lumbricoides eggs in the serum of children in rural areas of Miranda state. One hundred children were evaluated at ages 3 and 14 and were taken serum samples and diagnosis of stools. According to the reported, were classified into four groups: infected by A. lumbricoides (Asc +), uninfected (NSOP), infected A. lumbricoides and other parasite (Asc +/OP +); and individuals with other parasitic infections without A. lumbricoides (ASC/ OP+). The extract was tested by ELISA, Western blotting and immunocytochemestry. The electrophoretic profile showed 7 bands of 102, 92, 85, 65, 50, 45 and 30kDa. By ELISA, the 26% of patients in group A. lumbricoides were positive, while the 56% in the NSOP group were seronegative. Immune assay by inhibition with A. lumbricoides extract allowed identify three bands for the IgA antibodies of 92, 85 and 65 kDa different to the bands recognized by adult extracts of A. lumbricoiedes. The levels of antibodies were significantly higher in infected than non-infected children. In addition, the protein component on the surface of eggs was recognized by sera of infected individuals. These results indicate that eggs of Ascaris induce specific antibodies against proteins that could be used as antigens in serological diagnosis for complementing the diagnosis for Ascariasis. <![CDATA[<b>IgG antibody response against fractions of <i>Giardia duodenalis </i>in infected individuals</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200004&lng=en&nrm=iso&tlng=en La Giardiasis es una infección causada por Giardia duodenalis, transmitida por el agua o alimentos contaminados con quistes del parásito, principalmente en escolares que habitan zonas rurales. Se conoce poco sobre la respuesta de anticuerpos específicos, la identificación de antígenos permitirá el diseño de pruebas para evaluar seroprevalencia. En este estudio fueron identificadas proteínas para la respuesta IgG anti-Giardia por inmunodetección. Fueron evaluados ochenta individuos entre 4-80 años, de ambos sexos, de comunidades rurales, fueron clasificados según la presencia o ausencia de quistes del parásito en las heces. Las proteínas fueron obtenidas a partir de un extracto soluble y fracciones por centrifugación diferencial de la cepa WB, analizadas por electroforesis en geles de poliacrilamida, transferidas a membranas de nitrocelulosa y probadas por Western Blot para la respuesta IgG anti-Giardia. Fue observada una prevalencia general entre 9 y 12% de parasitosis intestinales, con 22% de individuos positivos para Giardia duodenalis, 46% negativos en heces y 32% positivos para Giardia duodenalis y otras parasitosis. El perfil de proteínas reveló bandas entre 190-15kDa, donde fueron reconocidas para la respuesta específica de anticuerpos IgG, proteínas en la fracción 2 de 190, 176 y 51kDa, en la fracción 3 una proteína de 23kDa. La identificación de proteínas reactivas para la respuesta IgG en las fracciones 2 y 3 permitirá el diseño de pruebas serológicas más específica para discriminar seropositivos de seronegativos.<hr/>Giardiasis is an infection caused by Giardia duodenalis, transmitted by consuming water or food contaminated with Giardia cysts, mainly in children living in rural areas. Little is known about proteins associated to the specific antibody response, the identification of specific antigens is necessary for studies of seroprevalence. The aim of this study was to select antigenic proteins from soluble extract or fractions of the parasite for the design of specific diagnostic tests. Were evaluated eighty individuals between 4-80 years, from rural communities were classified according to the presence or absence of cysts in the stool. The soluble extract and the fractions were obtained from WB (ATCC) strain of Giardia sp by differential centrifugation, analyzed by polyacrylamide gel electrophoresis and transferred to nitrocellulose membranes and finally tested by Western Blots for the IgG antibody response. The general prevalence was between 9-12%, and 22% of them were positive for G. duodenalis, 46% of individuals were negative in feces; and 32% were positive for G. duodenalis and other parasites. The protein profile revealed several bands from 190-15kDa; in which were recognized mainly proteins of 190, 176 and 51kDa in the fraction 2, and a protein of 23kDa in the fraction 3. The infection by G. duodenalis represents a human public health, and the identification of reactive proteins for the IgG response in the fractions 2 and 3, would allow us the design of serological test more specific, in order to discriminate the seropositive individuals from those seronegative. <![CDATA[<b>Assessment of risk factors that predispose to hypertension through a logistic regression model</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200005&lng=en&nrm=iso&tlng=en La hipertensión arterial (HTA) representa uno de los factores de riesgo que más contribuye a la enfermedad cardiovascular y actualmente se desarrollan modelos de predicción de riesgo a padecerla. El objetivo de esta investigación fue evaluar la reproducibilidad de un modelo de regresión logística en una población del estado Carabobo, Venezuela, así como la introducción de nuevas variables que mejoren dicho modelo. A 202 pacientes se les evaluó distintos factores de riesgo de HTA con los cuales se evaluó la reproducibilidad de un modelo ya planteado. Posteriormente se evaluó la introducción de nuevas variables al modelo que pudieran mejorar el mismo, utilizando el método del paso a paso de regresión logística. El modelo de predicción de riesgo que sirve como base a este estudio incorpora 3 variables: Presión arterial sistólica (PAS), edad e índice de masa corporal (IMC), de los cuales en este trabajo sólo edad e IMC resultaron significativas (p <0,000 y p <0,012 respectivamente). Una segunda regresión logística adicional, evaluó la introducción de nuevas variables al estudio, donde solo el fenotipo de cintura hipertrigliceridemia (CHT) contribuye a mejorar el modelo predictivo de la HTA. Por tanto, se encontró reproducibilidad parcial del modelo de predicción de riesgo de HTA, además de mejorar el mismo, al añadir la variable fenotipo de CHT. Se recomienda realizar nuevas investigaciones en otras poblaciones venezolanas así como estudios que involucren otras covariables clínicas.<hr/>High blood pressure (hypertension) is one of the risk factors that contribute most to cardiovascular disease. Adequate risk prediction models are needed to address prevention. The objective of this research was to evaluate the reproducibility of a logistic regression model in a population of Carabobo, Venezuela, and the introduction of new variables to improve the model. A total of 202 patients were assessed various risk factors of hypertension with which the reproducibility of an already proposed model was evaluated. Later, the introduction of new variables to the model that could improve it by using the stepwise logistic regression method was evaluated. The model risk prediction that serves as the basis of this study incorporates three variables: systolic blood pressure (SBP), age and body mass index (BMI), from which, in this work, only age and BMI were significant (p-value <0.000 and p <0.012 respectively). A second additional logistic regression assessed the introduction of new variables to the study, where only hypertriglyceridemic waist (HTW) phenotype helps to improve the predictive model of hypertension. Therefore, partial reproducibility of risk prediction model of hypertension was found, in addition to improving it by adding the variable HTW phenotype. We recommend further research in other Venezuelan populations and studies involving other clinical covariates. <![CDATA[<b>Colorations assessment techniques for diagnosis of cutaneous Leishmaniasis</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200006&lng=en&nrm=iso&tlng=en La Leishmaniasis es una protozoosis causada por parásitos del género Leishmania. Desde el punto de vista clínico se clasifican en formas cutánea, cutáneo-mucosa y visceral. El diagnóstico clásico de Leishmaniasis está basado en la verificación de amastigotas mediante la observación al microscópico de frotis, tomados por aspirados o biopsia de la lesión y teñidos con coloraciones tipo Romanowsky. En el presente estudio se planteó evaluar la utilidad de diferentes técnicas de coloración citológicas para el diagnóstico parasitológico de Leishmaniasis. Se elaboró un instrumento de medición usado con escala de estimación. Se utilizaron hámster (Mesocricetus auratus) inoculados experimentalmente con un aislado del parásito causante de Leishmaniasis cutánea, mantenidos en el bioterio experimental del Instituto de investigaciones Biomédicas de la Universidad de Carabobo sede Aragua (BIOMED-UC). Se seleccionaron aquellos hámsteres con seis meses hasta un año de inoculación que presentaban la formación de un nódulo en el lugar de inoculación. Se realizaron 60 frotis por aposición de los fragmentos de los nódulos y se clasificaron en tres grupos de 20 láminas cada uno, para colorearlas con 3 técnicas de coloración: Giemsa, Gaffney y Pap-mart®, luego fueron observadas al microscopio óptico con objetivo de 100X por 3 expertos en el diagnóstico de Leishmania y se evaluó cada coloración mediante una lista de cotejo debidamente validada. Los resultados de la investigación permitieron determinar que a pesar de que la tinción de Giemsa es la óptima y adecuada para demostrar las formas amastigotas en los frotis citológicos por observarse las estructuras con mayor nitidez y precisión, la coloración con Pap-Mart también permite hacer el diagnóstico de esta parasitosis, la cual pudiera ser utilizada como una alternativa en el momento que no se cuente con el reactivo de Giemsa en el mercado.<hr/>Leishmaniasis is a protozoal infections caused by parasites of the genus Leishmania. Clinicalyis are classified in cutaneous, muco-cutaneous, difuse and visceral forms. The classic diagnosis of leishmaniasis is based on the verification of amastigotes by observing microscopic smears, aspirates or taken by biopsy of the lesion and stained with Romanowsky type stains. In the present study it was proposed to evaluate the usefulness of different staining techniques for cytological diagnosis parasitologicof Leishmaniasis. A measuring instrument used to estimate scale was developed. Hamster (Mesocricetus auratus) experimentally inoculated with an isolate of the parasite that causes cutaneous leishmaniasis, kept in the experimental animal facility of the Institute of Biomedical Research at the University of Carabobo Aragua headquarters (BIOMED-UC) were used. Hamsters were selected those six months to a year inoculation presenting a nodule formation at the site of inoculation. 60 smears were performed by apposition of the fragments nodules were classified into three groups of 20 sheets each, to color with 3 coloring techniques: Giemsa, Gaffney and Pap-Mart®, then were observed under an optical microscope with objective 100X by 3 experts in diagnosing Leishmania, and each color was evaluated by a list of matching properly validated. The research results allowed determining that although Giemsa staining is optimal and adequate to demonstrate the amastigote forms in cytological smears observed structures more clearly and accurately, staining with Pap-Mart also allows the diagnosis of this parasitosis, which could be used as an alternative at the time is not counted with Giemsa reagent on the market. <![CDATA[<b>Histological factors associated with discordant results between biopsy and DNA testing in the diagnosis of human papillomavirus (HPV)</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200007&lng=en&nrm=iso&tlng=en El diagnóstico de la infección cervical ocasionada por el virus del papiloma humano (VPH), se realiza básicamente a través de la biopsia y/o de métodos moleculares como la reacción en cadena de la polimerasa (PCR). Los resultados de estos estudios en ocasiones no coinciden. En este estudio se evalúa la posible influencia de diferentes factores asociados a resultados falsos positivos y a falsos negativos de ambas pruebas. Se analizaron 271 muestras de cuello uterino de pacientes que acudieron a la consulta ginecológica durante el período 1999-2009, a las cuales se les tomó biopsia por presentar hallazgos colposcópicos y/o citológicos sospechosos de infección por VPH. Tomando en consideración al resultado de PCR como prueba estándar de oro, la biopsia presentó 14% de falsos positivos y 16,6% de falsos negativos, el diagnóstico de VPH en biopsia y PCR presentó mayor concordancia cuando las muestras abarcan la zona de transformación endo-exocervical, así como la presencia de metaplasia escamosa en la muestra y el tamaño del fragmento mayor (>0,5 cm); mientras que el tiempo de fijación, el tipo e intensidad de la inflamación, la hemorragia y la erosión del epitelio no fueron significativos. En conclusión, a fin de disminuir la discrepancia de los resultados entre la biopsia y prueba de ADN, sería importante que la muestra de biopsia incluya la zona de transformación del cuello uterino y al menos un fragmento mayor de 0,5 cm.<hr/>The diagnosis of cervical infection caused by human papilloma virus (HPV) is made basically through the biopsy and/or molecular methods such as polymerase chain reaction (PCR). The results of these studies sometimes do not match. In this work is evaluated the possible influence of different factors associated with results false positive and false negative of both tests. Were analyzed 271 samples from uterin cervix of patients presenting to the gynecologist during the period 1999-2009, to which were taken biopsies due present findings colposcopics and/or cytological suspicious of HPV infection. Considering the result of PCR as gold standard test, biopsy showed 14% false positive and false negative 16.6%, the diagnostic of VPH in biopsy and PCR showed greater concordance when samples include endo-exocervical transformation zone, and the presence of squamous metaplasia in the sample and size of the largest fragment (>0.5 cm); while the fixation time, the type and intensity of inflammation, bleeding and erosion of the epithelium were not significant. In conclusion, in order to reduce the discrepancy between the results of the biopsy and test DNA, it is important that the biopsy sample includes the transformation zone of the uterin cervix and at least one fragment greater than 0.5 cm. <![CDATA[<b>Stillbirth</b>: <b>epidemiological characterization</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200008&lng=en&nrm=iso&tlng=en La muerte fetal es la defunción del feto antes de su expulsión o separación completa del cuerpo de la madre, independientemente de la duración del embarazo. En un alto porcentaje se desconocen sus causas, aunque la literatura ha establecido como factores de riesgo la hipoxia intrauterina y las malformaciones congénitas. Se realizó un estudio retrospectivo, descriptivo, con el objetivo de determinar, a través de revisión de las historias clínicas, las características epidemiológicas de las muertes fetales en la Maternidad Dr. José Luis Facchin de Boni, en el período enero 2007 marzo 2013. La población fue 1236 gestantes con diagnóstico de muerte fetal, determinando una tasa de 56,28 muertes fetales por cada 1000 nacimientos y abortos. La mayoría de dichas muertes fetales (85,03%) ocurrieron por debajo de las 37 semanas, con predominio del sexo masculino. Además los factores médicos como el síndrome anémico, los obstétricos como las malformaciones fetales y las complicaciones hemorrágicas fueron los más frecuentemente encontrados en los casos de muerte fetal. Otro hallazgo importante de la investigación, fue que el peso de los fetos muertos fue menor que el peso de los nacidos vivos de la misma edad gestacional.<hr/>Stillbirth is the death of the fetus before their expulsion or complete separation of the body of the mother, regardless of the length of pregnancy. A high percentage of its causes are unknown, although the literature has established as risk factors intrauterine hypoxia and birth defects. A retrospective, descriptive study was conducted in order to determine, through review of medical records, the epidemiological characteristics of stillbirths in the Maternidad Dr. José Luis Facchin Boni, between January 2007 and March 2013. The population was 1236 pregnant women diagnosed with stillbirth, determining a rate of 56.28 stillbirths per 1000 births and abortions. Most of these stillbirths (85.03%) were below 37 weeks, with predominance of males. Medical factors such as anemic syndrome, the obstetric factors such as fetal malformations and hemorrhagic complications were most frequently found in cases of stillbirth. Another important finding of the research was that the weight of dead fetuses was less than the weight of live births of the same gestational age. <![CDATA[<b>Dysraphia-exencephaly-anencephaly sequence associated with amniotic band, Klinefelter syndrome and polyhydramnios</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1316-71382016000200009&lng=en&nrm=iso&tlng=en Los defectos del tubo neural son un grupo heterogéneo y complejo de anomalías del sistema nervioso central que se encuentran entre las anormalidades congénitas humanas más frecuentes, sólo superados por los defectos cardiovasculares, excluyendo las alteraciones cromosómicas. Se caracteriza por ser el resultado final de una anomalía progresiva que se inicia con un disrafismo del neuroporo rostral por un defecto mesenquimal, seguido de una segunda fase de exposición de hemisferios cerebrales bien diferenciados desarrollado fuera del cráneo embrionario concluyendo con una fase de desintegración. Puede presentarse como evento único o asociado a otras malformaciones. Reportamos tres casos de secuencia disrafia-exencefalia-anencefalia, uno de ellos en presencia de polihidramnios, otro asociado a síndrome de brida amniótica y otro cuyo estudio cromosómico reporta síndrome de Klinefelter, todos con un resultado común, mal pronóstico perinatal.<hr/>Neural tube defects are a complex of central nervous system abnormalities that are among the most common human congenital abnormalities, second only to cardiovascular defects, excluding chromosomal abnormalities and heterogeneous group. It is characterized as the end result of a progressive anomaly that starts with a dysraphism the rostral neuropore by a mesenchymal defect, followed by a second phase exhibit distinct cerebral hemispheres developed beyond the embryonic skull concluding with a phase of disintegration. It can occur as a single event or associated with other malformations. We report three cases of anencephaly -exencephaly-dysraphism sequence, one of them in the presence of polyhydramnios, other associated amniotic band syndrome and other chromosomal study reports Klinefelter syndrome, all with a common result, poor perinatal outcome.
