La pérdida de peso del paciente se asocia con el tipo de tumor canceroso, ubicación, estadio, entre otras cosas, y de forma importante con los efectos psicógenos, anorexígenos, de malabsorción, mecánico y tóxicos que suelen ser colaterales al tratamiento antineoplásico. De allí la importancia de que el experto en nutrición debe realizar una adecuada evaluación nutricional integral que permita el diagnóstico nutricional, estudiar el patrón alimentario, determinar los efectos tóxicos del tratamiento antineoplásico para manejar los tiempos de excelencia del tratamiento, síntomas, signos y de esta manera intervenir de forma eficaz optimizando la calidad de vida del paciente y con ello la sobrevida. Existen controversias sobre qué factores alimentarios específicos están relacionados con la etiología del cáncer y los resultados de estudios sobre los factores metabólicos, de allí que la relación cáncer nutrición es compleja.<hr/>Since nutrition can influence the process of carcinogenesis, this study’s objectives are to review the relationship between nutrition and cancer from the point of view of the role of micronutrients in the treatment of cancer patients, and to get to know the deficit relationship and the excess of micronutrients, with the etiology and cancer treatment. At the same time the patient's weight loss relates, among other things, to the type of cancerous tumor, its location, stage thereof, reason for which it may be associated with the deficiency of macro and micronutrients as from psychogenic, anorectics and mal-absorption effects or with mechanical effects as obstruction, among other toxic effects that are common in the treatment of cancer. Hence, the importance that the nutrition expert must have in making an adequate overall nutritional evaluation that allows the nutritional diagnosis, in studying the dietary patterns, to determine the toxic effects of the antineoplastic treatment in order to handle the treatment’s timing excellence, symptoms and signs, and thus act effectively optimizing the patient’s life quality, and therewith his/her survival. There are controversies as to which specific dietary factors are related to cancer etiology and the results of studies on metabolic factors, and therefore, the relationship Cancer-Nutrition is quiet complex. <![CDATA[<b>La transición alimentaria y la doble carga de malnutrición</b>: <b>cambios en los patrones alimentarios de 1961 a 2009 en el contexto socioeconómico mexicano</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400002&lng=es&nrm=iso&tlng=es El propósito de este trabajo fue identificar si existieron cambios en la composición de los patrones alimentarios de 1961 a 2009, si el gasto en algunos alimentos por nivel de ingresos y el aumento en el precio de ciertos alimentos básicos de la dieta en el contexto socioeconómico mexicano, podrían explicar la transición alimentaria y la doble carga de malnutrición. Se llevó a cabo un estudio ecológico con la información de las Hojas de Balance de la FAO. Para conformar los patrones alimentarios se realizó análisis de conglomerados. Además, con los datos de la Encuesta Nacional de Ingresos y Gastos en los Hogares 2012 del Instituto Nacional de Estadística y Geografía se construyeron curvas de Engel para calcular la evolución del precio relativo de algunos alimentos. La dieta se definió en tres patrones alimentarios. La disponibilidad total de energía aumentó de 2316 kcal/persona/ día en 1961 a 3146 en 2009. Las modificaciones del patrón alimentario están en consonancia con la transición alimentaria y nutricional y con la doble carga de malnutrición. Se observó que la energía derivada de los cereales y la proveniente de las leguminosa (frijol) se redujo considerablemente, y de manera simultánea la energía procedente de azúcares, alimentos de origen animal y grasas vegetales tuvieron un aumento notable. El gasto en alimentos fue diferencial de acuerdo al nivel de ingresos. La malnutrición está mediada por la inequitativa distribución del ingreso, el bajo costo relativo de los alimentos ricos en energía, el encarecimiento de alimentos nutritivos y el escaso apoyo a la agricultura.<hr/>The nutrition transition and the double burden of malnutrition: changes in dietary patterns 1961-2009 in the Mexican socioeconomic context.. The purpose of this study was to identify whether there were changes in the composition of dietary patterns from 1961 to 2009, if food patterns by income level and the increase in the price of certainbasic foods of the diet in the socioeconomic Mexican context, could explain the nutrition transition and the double burden of malnutrition. We conducted an ecological study with data from FAO balance sheets. To construct eating patterns cluster analysis was performed. Engel curves were developed with data from the 2012 INEGI ENGH survey and evolution of therelative price of some foods was calculated. The diet was defined in three dietary patterns. The increase in the total availability of energy increased from 2316 kcal/person/day in 1961 to 3146 in 2009. Dietary pattern modifications are in line with the nutrition transition and the double burden of malnutrition. It was observed that the energy derived from cereals andfrom legume (common beans) was significantly reduced, and simultaneously, the energy from sugars, animal foods and vegetable fats had a dramatic increase. Spending on food was differential according to income level.Malnutritionis mediated by theunequal distribution of income, the relatively lowcost ofenergy-dense foods, the increased cost of nutritious foods, and limited support to agriculture. <![CDATA[<b>Calidad de hidratos de carbono de la dieta y su efecto sobre el control metabólico de la Diabetes Tipo 2</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400003&lng=es&nrm=iso&tlng=es El objetivo de este estudio fue determinar la relación entre los parámetros de control metabólico y la calidad de los hidratos de carbono (H de C) de la dieta, en sujetos diabéticos tipo 2 (DM 2), controlados con dieta y/o metformina. A 108 hombres y mujeres con edades comprendidas entre 18 y 60 años, hemoglobinas glicosiladas (HbA1c) entre 6% y 10%, sin sulfonilureas ni insulinoterapia; se les aplicó dos encuestas de recordatorio de 24 horas. Se determinó la ingesta de H de C, el índice glicémico (IG) y la carga glicémica (CG) de la dieta. Desde las fichas clínicas se obtuvieron valores recientes de HbA1c. Los datos fueron tabulados en el programa SPSS versión 17. Se utilizó el test de correlación de Pearson para analizar el grado de asociación entre las variables, considerándose significativo un p < 0,05. El promedio de HbA1c fue de 7,3 ± 1,3 %, el consumo de H de C fue de 219,8 ± 27,0 g/día; el IG fue de 74,9 ± 11,3 % y la CG fue de 164,0 ± 22,04 g. Se encontró correlación positiva y significativa entre el consumo de H de C (r=0,290; p<0,05), IG (r=0,70; p<0,001) y CG (r=0,225; p<0,05) de la dieta con los niveles de HbA1c de los sujetos estudiados. En conclusión la calidad de los H de C, especialmente el IG, de la dieta de los sujetos estudiados se asocia fuertemente al control metabólico de la DM 2.<hr/>The Objective of this study was to determine the relationship between the parameters of metabolic control and quality of carbohydrates (CHO) of the diet in individuals with type 2 diabetes, controlled with diet and / or Metformin. In 108 men and women aged between 18 and 60 years, glycosylated hemoglobin A (HbA1c) between 6 % and 10 %, without sulfonylureas or insulintheraphy; were examined through two separate surveys of 24-hour recall. The CHO intake, GI, GL of diet was analyzed. Values of HbA1c were collected from medical records. Data was tabulated in SPSS version 17 software. The Pearson correlation test was used to analyze the degree of association between variables, considering significant at p < 0.05. The mean HbA1c was 7.3 ± 1.3%, CHO consumption was 219.8 ± 27.0 g / day; GI was 74.9 ± 11.3% and GL was 164.0 ± 22.04 g. A significant positive correlation was found out between the CHO intake (r = 0.290, P < 0.05), GI (r = 0.70, p < 0.001), GL (r = 0.225, p < 0.05) of diet and HbA1c levels in the individuals. In conclusion the study showed that the quality of CHO, mainly GI, are strongly associated with metabolic control of DM 2. <![CDATA[<b>Perfil de ácidos grasos libres (AGL) en suero de jóvenes colombianos con obesidad y síndrome metabólico</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400004&lng=es&nrm=iso&tlng=es La obesidad se relaciona con mayor concentración de ácidos grasos libres (AGL) circulantes. En adultos obesos el perfil de AGL difiere comparado con individuos de peso adecuado, en adolescentes los resultados son contradictorios. El objetivo fue comparar el perfil de AGL de jóvenes obesos con y sin Síndrome Metabólico (SM) y explorar la asociación entre estos AGL con las alteraciones metabólicas propias de la obesidad y el SM. Estudio transversal, con 96 jóvenes entre 10 y 18 años divididos en 3 grupos: 1) obesos con SM, 2) obesos sin SM y 3) peso adecuado (PA), pareados uno a uno por edad, sexo, maduración puberal y estrato socioeconómico. El estado nutricional se clasificó según el índice de masa corporal (IMC) en obesos (IMC>p98) y Peso Adecuado (PA) (IMC p15-p85) según OMS/2007, se evaluó circunferencia de cintura, adiposidad, perfil lipídico, proteína C reactiva ultra sensible (PCRhs), glucemia, insulina y resistencia a la insulina (RI) según homeostatic model assessment (HOMA). La concentración sérica de AGL se determinó mediante cromatografía gaseosa. Ambos grupos de obesos presentaron mayor adiposidad, inflamación, AGL totales y frecuencia de palmitoléico-16:1n7 comparados con PA. Los obesos con SM presentaron más alteraciones metabólicas, mayor cantidad de dihomo-γ-linolénico (DHGL- 20:3n6) y relación 20:3n6/18:2n6 indicativa de mayor actividad de Δ6 desaturasa (D6D). Los AGL totales, palmitoléico- 16:1n7, DHGL-20:3n6, actividad D6D y PCRhs correlacionaron significativamente con variables de adiposidad, RI y triglicéridos. Los resultados en obesos con SM permiten asociar la obesidad central con inflamación, lipolisis aumentada en el tejido adiposo visceral y alteraciones metabólicas.<hr/>Obesity produces greater circulation of free fatty acids (FFA). In adults, the FFA composition changes in states of obesity; in adolescents, the results are contradictory. This study compare the FFA profile of obese youth with and without Metabolic Syndrome (MetS) and explore the association between FFA and metabolic alterations of obesity and MetS. A cross-sectional study with 96 young people between 10 and 18 years old was divided into three groups: 1) obese youth with MetS, 2) obese youth without MetS; and 3) adequate weight (AW), matched according to age, gender, pubertal maturation and socioeconomic stratum. The nutritional status was classified according to the body-mass index (BMI), according to the World Health Organization 2007 (WHO, 2007); the waist circumference (WC), adiposity, lipid profile, highly-sensitive reactive C protein (hsRCP), glucose, insulin and insulin resistance (IR), according to the homeostatic model assessment (HOMA Calculator Version 2.2.2). The FFA serum concentration was determined by gas chromatography. Both obese groups had higher adiposity, inflamation (hsRCP), FFA totals and frequency palmitoleic-16:In7, compared to AW. The obese with MetS presented more metabolic alterations, a greater amount of dihomo-γ-linolenic (DHGL-20:3n6) and a 20:3n6/18:2n6 relation, indicative of increased activity of Δ6 desaturase (D6D). The FFA totals, palmitoleic-16:1n7, DHGL-20:3n6, D6D activity and hsRCP significantly correlated with variables of adiposity, IR and triglicerides. The results in obese with MetS corroborate the association among central obesity, inflammation and increased lipolysis in visceral adipose tissue and metabolic alterations. <![CDATA[<b>Percepciones y conocimientos acerca de la sal, el sodio y la salud en adultos de clase media de la provincia de San José, Costa Rica</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400005&lng=es&nrm=iso&tlng=es En el ámbito mundial se realizan acciones para desarrollar e implementar estrategias que permitan la reducción del consumo de sal/sodio y de esta manera disminuir la prevalencia de hipertensión arterial (HTA) y de enfermedades cardiovasculares (ECV). El objetivo de la investigación fue explorar las percepciones y los conocimientos acerca de la sal, el sodio y la salud de los adultos de clase media de la zona urbana de la provincia de San José; con el fin de proporcionar información para el desarrollo de estrategias educativas. Se realizaron entrevistas (formulario precodificado en el Anexo 1) a 400 personas entre 40 y 55 años de edad de clase media, subdividida en tres subestratos (alto, medio y bajo). Se analizaron frecuencias y se aplicó la prueba de Chi cuadrada. Se encontró que las personas no se sienten en riesgo de desarrollar HTA independientemente del subestrato socioeconómico (p>0,05) y 68% de éstas no consideró un exceso en el consumo de sal/sodio. Además, 70% no tenían conocimiento sobre la relación entre sal y sodio y hubo asociación (p<0,05) entre la subdivisión del estrato socioeconómico y el conocimiento de esta relación. Además, poseían un conocimiento calificado como medio acerca del riesgo del consumo excesivo de sal/sodio y de los beneficios de la disminución de su ingesta. Se concluyó que existen vacíos de conocimientos en la población de Costa-Rica y que es necesario desarrollar estrategias educativas basadas en la situación actual, acompañadas por intervenciones que impacten a las personas en riesgo y cambien sus hábitos de alimentación.<hr/>Organizations and governments globally are making great efforts to develop strategies to reduce population salt intake, and thus reduce the prevalence of hypertension and CVD. The objective of this study was to explore the perceptions and knowledge about salt, sodium and their effects on health of adults of middle income in the urban area of San José; in order to provide information for the development of educational strategies. Four hundred interviews to subjects between 40 to 55 years old, belonging to middle income socioeconomics strata, sub-divided into three levels (high, medium and low) were performed. Frequencies analyses were performed, and the Chi-square test was applied. We found that people felt that they are not at risk of developing hypertension regardless of the sub-middle socioeconomic strata (p> 0.05) and 68% of individuals did not consider that their consumption of salt and/or sodium was excessive. In addition, 70% of subjects had no knowledge about the relationship between salt and sodium, and an association (p <0.05) between the sub-division of the socioeconomic status and knowledge about this relationship was found. Individuals had an average knowledge about the risk of excessive salt/sodium consumption and the benefits of reduced intake. It was concluded that there is a knowledge gap in subjects and there is a need for developing initiatives to reduce salt/sodium intake, shocking enough to make people be more aware of their risk and change their eating habits. <![CDATA[<b>Marcadores de estrés oxidativo y su relación con el estado nutricional en adultos, Ecuador</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400006&lng=es&nrm=iso&tlng=es Se evaluó la relación entre biomarcadores de estrés oxidativo (ácido úrico, bilirrubina y proteína C-reactiva) con el estado nutricional en 321 adultos de Ecuador, pertenecientes al personal administrativo de la Universidad Técnica del Norte, con edades de 43 ± 10 años, (46,30% femenino y 53,61% masculino). Se obtuvo información sociodemográfica, de estilo de vida y epidemiológica; se calculó el índice de Masa Corporal (IMC), porcentaje de grasa y agua corporal, se midió la circunferencia de la cintura (CC) y la presión arterial y se determinó las concentraciones de ácido úrico, bilirrubina y Proteína C reactiva (PCR). Se encontró que 17,9% de la población presentó obesidad y 51,72% sobrepeso. Los valores más elevados de ácido úrico se encontraron en obesos, hipertensos y en quienes realizan actividad física. La bilirrubina total, directa e indirecta se encontró en límites superiores en los grupos con obesidad abdominal, y con actividad física. Las variables % de grasa y de agua influyen en la PCR en el grupo con baja grasa corporal y en las mujeres. En hombres, la PCR se asocia con IMC y CC. En los grupos sobrepeso, alta grasa corporal y PHT, el ácido úrico mostró relación con el % de grasa y la CC En los obesos con el % de agua e IMC y en los grupos obesidad abdominal e HT se asoció con % de agua y CC Se concluye que el porcentaje de agua corporal es un importante indicador nutricional para el desarrollo de estrés oxidativo en esta población.<hr/>In this work it was evaluated the relationship between oxidative stress biomarkers (uric acid, bilirubin and C-reactive protein) with nutritional status in 321 adults of Ecuador, belonging to administrative staff of of the Universidad Tècnica del Norte , aged 43 ± 10 years old (46 30% female and 53.61% male). Socio demographic and epidemiological information and lifestyle were obtained through a survey; The Body Mass Index (BMI) and body fat and body water percentages were calculated; waist circumference (WC) and blood pressure was measured. Determinations of uric acid, bilirubin, and serum C-reactive protein (PCR) were performed. 17.9% of the populations were obese and 51.72% overweight. The highest values of uric acid were found in obese, hypertensive and physical activity groups. The total direct and indirect bilirubin were found in upper limits in abdominal obesity and physical activity groups. The CRP level was influenced by % fat and % water in the low body fat group and in females. In male, BMI and WC were associated with CRP. Uric acid showed relationship with % fat and WC in overweight, high body fat and PHT groups, uric acid was associated with the % water and BMI in obese. Finally, uric acid was associated with % water and the WC in the abdominal obesity, and HT groups’ .The body water percentage is an important indicator to development of oxidative stress in this population. <![CDATA[<b>Replacing of residue from production of palm Palm Royal Australian (Archontophoenix alexandrae) in silage of sugar cane in diets of sheep</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06222014000400007&lng=es&nrm=iso&tlng=es Objetivou-se avaliar a composição química, consumo voluntário e digestibilidade aparente de dietas contendo resíduo oriundo da produção de palmito da palmeira Real Australiana (Archontophoenix alexandrae) em substituição à cana-de-açúcarem ovinos. Foram utilizados 12 ovinos com peso vivo médio de 23,3 ± 2,8 kg, mantidos em gaiolas metabólicas distribuídos em seis quadrados latinos 2x2 implementado em esquema fatorial 3 x 2 (três tipos de resíduo - folha, bainha e composta - e dois níveis de substituição do resíduo, 5% e 15%). Verificou-se maior consumo de matéria seca (MS), matéria orgânica (MO), carboidratos não fibrosos (CNF) na substituição pelo resíduo composta. Os valores médios de digestibilidade aparente da MS, MO, Proteína bruta (PB), fibra insolúvel em detergente neutro corrigido para cinzas e proteína(FDNcp) e nível dietético de NDT foram maiores para o resíduo folha. Constatou- se efeito de interação entre resíduo e o nível de substituição sobre as excreções urinárias de nitrogênio total (EUN), balanço nitrogenado aparente (BNA) e compostos nitrogenados microbianos (NMIC). Resíduos da produção de palmito oriundos da palmeira Real Australiana podem ser utilizados como alimentos volumosos na dieta de animais ruminantes, sendo destes, o resíduo folha e composta os que apresentaram melhores respostas nas características avaliadas.<hr/>The aim of this study was to evaluate the chemical composition, voluntary intake and apparent digestibility of the diets containing residue from palm heart of Australian Royal Palm (Archontophoenix alexandrae) to replace sugar cane on sheep. Twelve sheep were used with average live weight of 23,3 ± 2,8 Kg and they placed in metabolism cages and distributed in six latin square 2 x 2 in a factorial design 3 x 2 (three types of residue -sheet, bark and composed - and two levels of residue’s replacement, 5% and 15%). It was observed higher intake of dry matter (DM), organic matter (OM), nonfiber carbohydrates (NFC) by substitution of composed residue. The average values of apparent digestibility of DM, OM, crude protein (CP), neutral detergent fibre correct for ash and protein (NDFap) and total digestible nutrients (TDN) were higher for sheet residue. There was interaction between type of residue and level of residue’s replacement on the urinary excretion of total nitrogen (NUE), apparent nitrogen balance (BNA) and microbial nitrogen compost (NMIC).
