Septiembre 2011</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300002&lng=es&nrm=iso&tlng=es <![CDATA[<b>Determinación de puntos de corte para la circunferencia de cintura a través de curvas ROC en población pediátrica de tres regiones de Venezuela evaluada en el SENACREDH</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300003&lng=es&nrm=iso&tlng=es Introducción: En el contexto del Segundo Estudio Nacional de Crecimiento y Desarrollo Humano de la República Bolivariana de Venezuela (SENACREDH), se establecen por primera vez dentro de valores oficiales y de representatividad nacional, curvas ROC para determinar puntos de corte para la circunferencia de cintura en población pediátrica. Objetivos: Determinar puntos de corte para la circunferencia de cintura, a través de curvas ROC, en población pediátrica. Métodos: Se evaluaron factores de riesgo para síndrome metabólico en 4.387 individuos de 5 a 19,99 años en las regiones: Centro Norte Costera, Orinoco y Andina (11 estados), los cuales representaron para la ponderación de los datos 1.066.864 habitantes. Con dichos datos se validaron en un modelo de regresión lineal múltiple las variables de mayor importancia que explican la circunferencia de cintura. Luego usándolas en conjunto como definitorias de riesgo para síndrome metabólico al tener más de una condición, se procedió a emplearlas como variable dependiente para generar curvas ROC y sus coordenadas con la circunferencia de cintura (CCI). Resultados: Del total de niños, niñas y adolescentes, 54,80% correspondió a varones y 45,20% a hembras. El promedio de la circunferencia de cintura fue 66,896 cm. (±10,49 cm., rango 44,4 a 127,2 cm., p25=59,5 cm., p50=65,7 cm., p75=72,8 cm.). Los valores de la media (±DE) de CCI se incrementaron con la edad, en ambos sexos, y se asociaron significativamente utilizando el modelo de regresión lineal múltiple, en los coeficientes, área grasa (ag) (p<0,001), HDL colesterol (hdl) (p=0,005), peso (p<0,001), triglicéridos (trig) (p=0,004), suma de pliegues (sdp) (p<0,001) e IMC (imc) (p<0,001). Se encontró con un solo factor el 35,6%, y el resto con más de un factor (39,7%). Se generaron 6 curvas ROC por edad y sexo, observando que los puntos de corte son consistentes con mayores valores en varones que en hembras y valores más altos a mayores edades. Conclusión: Estas primeras curvas ROC permiten establecer la base para la definición estandarizada de valores de circunferencia de cintura deseables y no deseables en la población de estudio y en forma preliminar a lo que se definirá para el país a la conclusión del SENACREDH (Diciembre 2011) y su publicación en 2012.<hr/>Background: In the context of the Second National Study on Human Growth and Development of the Bolivarian Republic of Venezuela (SENACREDH), for the first time and within official and nationally representative data, ROC curves have been established for waist circumference in a pediatric population. Objectives: Determinate cut-off levels for waist circumference using ROC curves, in a pediatric population. Methods: Risk factors for metabolic syndrome were evaluated in 4,387 individuals, 5 through 19.99 y-old of the regions: North Central Coastal, Orinoco and Andean (11 states). They represented in the weight of data 1,066,864 pop. With those data a multiple linear regression model for those variables of higher importance explaining waist circumference, was validated. Then, using them jointly as defining for metabolic syndrome risk when more than one of them was present, we used them as dependent variable in order to generate ROC curves and its coordinates for the waist circumference (WC). Results: From the total of children and adolescents, 54.80% were males and 45.20% females. Mean waist circumference was 66.896 cm. (±10.49 cm., range 44.4 through 127.2 cm., p25=59.5 cm., p50=65.7 cm., p75=72.8 cm.). Mean values of WC (±DE) increased with age, in both genders, and were significantly associated at the multiple linear regression model, regard the coefficients, with fat area (ag) (p<0.001), HDL cholesterol (hdl) (p=0.005), weight (p<0.001), triglycerides (trig) (p=0.004), sum of skin-folds (sdp) (p<0.001) and BMI (imc) (p<0.001). With one factor of the only was found 35.6%, and the rest with more than one (39.7%). Six ROC curves were generated by age and sex, observing that the cut-off points were consistent with higher values in males than in females and at higher ages. Conclusions: These first ROC curves allow to establish the basement for the standardized definition of desirable and non-desirable waist circumference values in the study population and in a preliminary form, what will be defined for the country when the SENACREDH is concluded (December 2011) and published in 2012. <![CDATA[<b>Seroprevalencia contra toxocara canis en niños de 1 a 6 años con y sin síntomas respiratorios de Barquisimeto, Venezuela</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300004&lng=es&nrm=iso&tlng=es Introducción: sabiendo la existencia de casos de Síndrome de Larva Migrans Visceral en Venezuela, es necesario conocer la seroprevalencia contra uno de sus agentes causales: Toxocara canis. Objetivo: determinar la presencia de anticuerpos séricos IgG anti- Toxocara canis y su relación con síntomas respiratorios y factores de riesgo para Síndrome de Larva Migrans Visceral en niños de 1 a 6 años, que acudieron a dos ambulatorios urbanos de Barquisimeto-Venezuela. Métodos: se evaluaron 215 niños y niñas de 1 a 6 años de edad con y sin síntomas respiratorios. Previa información y solicitud del consentimiento informado de sus representantes, se les entrevistó y tomó muestra de sangre para determinar anticuerpos IgG séricos anti-Toxocara canis por ELISA, con el estuche DRG®. Análisis estadístico: se calcularon porcentajes y proporciones y se usaron Prevalencia Relativa (PR) y Chi Cuadrado con intervalo de confianza de 95%. Resultados: se detectaron anticuerpos en 34,4%. El mayor porcentaje de seroreactivos estuvo representado por los niños con síntomas respiratorios (83,8%), los del grupo de edad de 3 a 4 años (55,4%), el sexo masculino (54,1%), quienes refirieron contacto con tierra y/o geofagia (95,9%) y contacto con cachorros caninos menores de 3 meses de edad (90,5%), quienes consumían agua de calidad inadecuada (85,1%) y quienes disponían inadecuadamente las excretas caninas (60,8%). Conclusión: La elevada seroprevalencia crea la necesidad de nuevos estudios para evaluar con precisión las características epidemiológicas de la infección por T. canis en Lara y Venezuela a fin de establecer planes adecuados de control.<hr/>Introduction: In view of the presence of Visceral Larva Migrans Syndrome in Venezuela, it is necessary to know the seroprevalence against one of its causal agents: Toxocara canis. Objective: a transversal study was performed to determine the presence of serum IgG anti-Toxocara canis antibodies and their relation with respiratory symptoms and risk factors for Visceral Larva Migrans Syndrome in patients from 1 to 6 years, who assisted to two urban outpatient clinics of Barquisimeto-Venezuela. Methods: 215 children 1 to 6 years old with and without respiratory symptoms were evaluated. Previous information on the investigation and request of the consent in writing of their care givers, an interview was withheld and a blood sample was drawn to determine serum IgG anti-Toxocara canis antibodies by the ELISA method, with DRG® commercial kit. Statistic analysis: percentage and proportions were calculated and Relative Prevalence (RP) and χ Squared used, with an interval of statistical confidence of 95%. Results: antibodies were detected in 34.4% of the children. The greater percentage of seroreactives was represented by children with respiratory symptoms (83.8%), those in the age group of 3 to 4 years (55.4%), boys (54.1%), those who referred soil contact and/or geophagy (95.9%) and contact with puppies under three months of age (90.5%), that consumed water of inadequate quality (85.1%) and that had inadequate disposal of canine excretes (60.8%). Conclusion: the high seroprevalence creates the necessity of new studies to evaluate accurately the epidemiologic characteristics of the infection by T. canis in Lara and Venezuela in order to establish suitable control plans. <![CDATA[<b>Beneficios del tratamiento antirretroviral de alta eficacia en pacientes pediátricos con infección por VIH</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300005&lng=es&nrm=iso&tlng=es Introducción. El tratamiento antirretroviral de alta eficacia en pacientes con infección por VIH mejora la condición inmunológica y en consecuencia disminuye morbilidad y mortalidad. Objetivo. Determinar la frecuencia de hospitalización y mortalidad en pacientes pediátricos con infección VIH según tipo de tratamiento antirretroviral. Método. Se realizó un estudio clínico, retrospectivo y comparativo. Se incluyeron pacientes con infección VIH del Hospital de Niños “J.M. de los Ríos” (Caracas, Venezuela), entre 1993-2009. Los datos registrados fueron: año de primera evaluación, mecanismo de transmisión viral, condición inmunológica y virológica, tratamiento antirretroviral, necesidad de hospitalización y muerte. El análisis estadístico incluyó Chi cuadrado. Resultados. Se evaluaron 234 pacientes: 73,1%(n=171) de transmisión vertical y 26,9%(n=63) horizontal. La hospitalización y mortalidad disminuyó desde el año 2000, cuando hubo acceso universal a tratamiento antirretroviral de alta eficacia. De los pacientes, 50,9% requirió hospitalización y 30,3% falleció. En la primera evaluación había inmunosupresión en 67,6% de los pacientes que requirieron hospitalización y 43,3% de los que no (p<0,05) y en el 75,8% de los fallecidos y 47% de los sobrevivientes al final del estudio (p<0,05). En los hospitalizados, la mayoría no recibía tratamiento de alta eficacia (74,6%) y tenía inmunosupresión (82,2%) con carga viral detectable (96,4%). Hallazgos similares se encontró en la mayoría de los fallecidos: ausencia de tratamiento de alta eficacia (92,9%), inmunosupresión (95,1%) y carga viral detectable (100%). Conclusiones. El tratamiento antirretroviral de alta eficacia disminuye la mortalidad y la necesidad de hospitalización. Los pacientes con inmunosupresión en la primera evaluación tuvieron mayor frecuencia de complicaciones. Al momento de la hospitalización o del fallecimiento la mayoría no recibía tratamiento de alta eficacia, presentaba inmunosupresión y tenía carga viral detectable.<hr/>Background. Highly active antiretroviral therapy in HIV-infected patients minimizes viral replication, restores immune status and thus decreases morbidity and mortality. Objective. Determine hospitalization and mortality frequency in pediatric patients with HIV infection according to the type of antiretroviral treatment. Method. It was performed a clinical and retrospective study which included pediatric patients with HIV infection, attended at Hospital de Niños "J.M. de los Rios (Caracas, Venezuela) between 1993-2009. The recorded data were: year at first assessment, mechanism of viral transmission, immunological and virological status, antiretroviral treatment, need for hospitalization and death. Statistical analysis included Chi square. Results. 234 patients were evaluated: 73.1% (n=171) of vertical transmission and 26.9% (n=63) horizontal. Hospitalization and mortality decreased since 2000, when there was greater access to highly active antiretroviral therapy. Of all patients, 50.9% required hospitalization and 30.3% died. At first evaluation, there was immunosupression in 67.6% of those patients who required hospitalization and 43.3% who did not (p <0.05). Equally there was immunosupression in 75,8% of patients who died and 47% of the survivors at the end of the study (p<0,05). In hospitalized patients, most received no highly active antiretroviral therapy (74.6%) and had immunosupression (82.2%) with detectable viral load (96.4%). These findings were also seen in most patients who died: no highly active antiretroviral therapy (92.9%), immunosupression (95.1%) and detectable viral load (100%). Conclusions. Availability of highly active antiretroviral therapy decreases hospitalization and mortality. At first assessment, patients with immunosupression had higher complication rates. At the time of hospitalization or death, most patients did not receive highly effective antiretroviral therapy and had immunosupression with detectable viral load. <![CDATA[<b>Epilepsia en niños con esclerosis tuberosa experiencia en el Instituto Autónomo Hospital Universitario de los Andes, Merida 2005-2011</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300006&lng=es&nrm=iso&tlng=es Introducción: La esclerosis tuberosa (ET) es una anomalía genética, multisistémica, susceptible de originar tumores del sistema nervioso central. Las crisis epilépticas son manifestaciones comunes y constituyen el principal problema terapéutico. Objetivo: Describir las características epilépticas de los pacientes pediátricos con diagnóstico de complejo de esclerosis tuberosa (CET), controlados en el Servicio de Neurología Pediátrica del Instituto Autónomo Hospital Universitario de Los Andes. Métodos: Se realizó un estudio observacional, retrospectivo, tipo serie de casos con ET y Epilepsia. Se describió sexo, edad de diagnóstico e inicio de crisis, motivo de consulta, tipo de crisis epiléptica, hallazgos electroencefalográficos y de imagen, asociación a trastornos conductuales, severidad de compromiso intelectual y manifestaciones dermatológicas. Resultados: Doce pacientes cumplieron criterios diagnósticos de CET, 10 (83%) fueron epilépticos, de éstos el 50% cursó con epilepsia de difícil control, 60% tuvo crisis parciales, 40% generalizadas. El 100% mostró alteraciones electroencefalográficas, 30% con patrón hipsarrítmico. 50% tenían alteraciones estructurales, tipo túber cortical en 80%. En 70% se controlaron las crisis con Acido Valpróico y en un caso se requirió dieta cetogénica estricta. El signo extraneurológico más constante fueron máculas hipocrómicas (100%). Conclusión: Aunque las convulsiones no forman parte de los criterios diagnósticos, son el motivo más frecuente de consulta, que en asociación con máculas hipocrómicas, hace sospechar diagnóstico de ET. La variedad, refractariedad e inicio temprano de crisis requieren en muchos casos politerapia para el control, lo cual favorece el pronóstico del paciente.<hr/>Introduction Tuberous sclerosis (TS) is a genetic, multisystemic, likely to cause central nervous system tumors. Seizures are common manifestations are the main therapeutic problem. Objetive: To describe the epileptic characteristics, of pediatric patients with diagnosed with tuberous sclerosis complex (TSC), controlled in the Pediatric Neurology Department University Hospital Institute of Los Andes. Methods: was performed an observational, retrospective, case series, with ET and Epilepsy. Described: sex, age of the diagnosis and initiation of crisis, reason for visit, seizure type, electroencephalographic findings and images, behavioral disorders, severity of intellectual engagement and dermatologic manifestations. Results: Twelve patients met the criteria diagnostic CET, 10 (83%) were epileptic, of these 50% passed with epilepsy of difficult control. 60% had partial seizures (40%) generalized. The 100% showed EEG abnormalities, hypsarrhythmic pattern 30% . The 50% of cases had structural abnormalities, 80% cortical tuber type. In 70% was achieved crisis control with valproic acid and in one case was required strict ketogenic diet .The extraneurological sign more constant were the hypochromic macules (100%). Conclusion: Although seizures are not part of the diagnostic criteria, are the most frequent reason for consultation in partnership with hypochromic macules to suspect a diagnosis of ET. The variety, refractoriness and early onset of crisis, often require polytherapy to control, which favors the patient's prognosis. <![CDATA[<b>Difteria-tétano-pertusis</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300007&lng=es&nrm=iso&tlng=es Los componentes de la vacuna contra la Difteria, Tétanos y Tosferina a células completas (DTP) fueron integrados a mediados de la década de los años 40 e igualmente, fue administrada en forma masiva, llevando virtualmente a la eliminación de la difteria. Posteriormente, a partir de 1991, el Toxoide Diftérico y el Tetánico se combinan con la vacuna acelular contra la tosferina, ya que no contiene células completas de B. pertussis, sino solo unas cuantas proteínas antigénicas (DTPa). En los últimos 10 años se ha observado aumento de los casos de tosferina en niños mayores, adolescentes y adultos jóvenes. La presentación de tosferina a estas edades es atípica, de difícil diagnóstico, no son adecuadamente tratadas y conforman grupos que actúan como reservorios de la enfermedad, que favorecen la trasmisión de la B. pertussis a los niños de menor edad. Con la aprobación en 2005 de la vacuna dTpa, se puede completar el esquema de vacunación o administrar como dosis de refuerzo desde los 7 años de edad, a las embarazadas a partir de las 20 semanas de gestación y en el puerperio. Con esta estrategia se logrará la disminución en la aparición de nuevos casos de tosferina. Es bueno recordar que, ni la infección natural, ni la vacunación con un esquema completo, confieren inmunidad permanente.<hr/>The components of whole cell vaccine against Diphtheria, Tetanus and Pertussis (DTP) were integrated during the mid years 40. The vaccine was also massively administered leading to the virtually elimination of diphteria. Later, after 1991 diphteric and tetanic toxoids were combined with the acellular vaccine against pertussis, since it does not contain whole cells of B pertussis, but only some antigenic proteins (DTPa). During the last 10 years an increase in Pertussis cases has occured in elder children, adolescents and young adults. The clinical presentation of Pertussis at these ages is atypical, the disease is difficult to diagnose, patients are not treated adequately and therefore constitute groups that act as reservoirs of the disease, and favor the transmission of B pertussis to small children. With the approval in 2005 of the vaccine dTpa, the vaccination scheme may be completed, or a booster dose may be administered from 7 years of age, or during pregnancy after 20 weeks of gestation or during puerperium. The appearance of new cases of pertussis may be avoided with this strategy. It should be remembered that, neither natural infection, nor the vaccination with a complete scheme, confers permanent immunity. <![CDATA[<b>Vacunas anti Neumocócicas</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300008&lng=es&nrm=iso&tlng=es Existen vacunas contra el Streptococcus pneumoniae. Una de polisacáridos capsulares con 23 antígenos y las vacunas conjugadas, denominadas así, porque los polisacáridos están conjugados a una mutante no tóxica de la toxina diftérica o a la proteína D del Haemophilus influenzae no tipificable, a una proteína transportadora del toxoide tetánico y otra de toxoide diftérico. La vacuna de 23 antígenos se recomienda en niños con edad igual y mayor de 24 meses, con alto riesgo para enfermedad neumocócica invasiva y complicaciones, debidas a enfermedades subyacentes y se administra en esquema mixto con vacunas anti neumocócica conjugadas. En 2002, la Organización Mundial de la Salud determinó que la vacuna anti neumocócica heptavalente (Prevenar® 7) es el estándar de oro, con la cual debían ser comparada toda nueva vacuna conjugada, por lo que definieron, necesarios para su aprobación, dos criterios de no inferioridad. Primer criterio: Una vez administrada la vacuna se registren títulos de anticuerpos medidos por ELISA iguales o mayores de 0,35 μg/mL para los serotipos comunes a ambas vacunas y los adicionales. Segundo criterio: Los anticuerpos deben tener capacidad funcional, lo cual se establece mediante la cuantificación de la actividad opsonofagocítica que debe ser igual o mayor de 1:8. Las vacunas conjugadas 10valente (Synflorix®) y 13 valente (Prevenar® 13) cumplen ambos criterios. De no ser posible completar el esquema o administrar refuerzo, con el mismo tipo de vacuna conjugada inicial, puede hacerse con cualquier otra vacuna.<hr/>Several vaccines against Streptococcus pneumoniae are available. One made of capsular polysaccharides with 23 antigens and the conjugated vaccines, thus denominated, because the polysaccharides are conjugated to a nontoxic mutant of the diphteric toxin or to the D protein of the nontipificable Haemophilus influenzae, to a transporting protein of tetanic and diphteric toxoids. The 23 antigens vaccine is recommended for children 24 months of age and older, with high risk for invasive pneumococcal disease and complications due to underlying diseases. It is administered in a mixed scheme with anti pneumococcal conjugated vaccines. In 2002, the World Health Organization (WHO) determined that the heptavalent vaccine (Prevenar® 7) is the gold standard, with which all new conjugated vaccine should be compared. For this reason, they defined two non inferiority criteria as necessary for the approval. First criterion: Once administered the vaccine and measured the amount of antibodies by ELISA, these must be equal or above 0.35 μg/mL for the common serotypes to both vaccines and to the additional serotypes. Second criterion: The antibodies must have functional capacity, which is determined by the opsonophagocitic activity that must be equal or greater than 1:8. The 10 valent (Synflorix®) and the 13 valent (Prevenar® 13) conjugated vaccines fulfill both criteria. If it is not possible to complete the vaccine scheme or to administer a booster dose, with the same type of conjugated vaccine, this may be done with any other vaccine. <![CDATA[<b>Nuevas vacunas de BCG</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300009&lng=es&nrm=iso&tlng=es La tuberculosis continúa con alta prevalencia en los países más pobres del mundo. Aún cuando se cuenta con procedimientos diagnósticos, medicamentos y una vacuna de eficacia limitada que previene las formas graves de la enfermedad, como es la vacuna de Bacilos de Calmette y Guérin; dos factores han generado nuevo interés por la obtención de nuevas vacunas contra el bacilo de la tuberculosis: La coinfección del Virus de Inmunodeficiencia Humana y tuberculosis, y el surgimiento de Mycobacterium tuberculosis multi-resistente a antibióticos. Los objetivos principales de las nuevas vacunas son: prevención de la infección de personas no expuestas previamente, prevención de la reactivación de la infección latente y acción terapéutica para prevenir recaídas de los enfermos de tuberculosis. Actualmente, las líneas de investigación, en más de 12 proyectos, que más apoyo reciben son las vacunas de Bacilos de Calmette y Guérin modificadas por recombinación genética, las cepas atenuadas de M. tuberculosis, las vacunas de subunidades y las vacunas de ADN.<hr/>Tuberculosis continues to be highly prevalent in poor countries. Even though the availability of diagnostic procedures, drugs and a vaccine of limited effectiveness that prevents the serious forms with the disease, namely the vaccine of Calmette and Guérin bacilli, two factors have generated new interest for the production of new vaccines against tuberculosis bacilli: the coinfection of the Virus of Human Immunodeficiency and tuberculosis, and the appearance of multi-resistant tuberculosis Mycobacterium. The primary targets of new vaccines are: prevention of the infection in people not previously exposed, prevention of reactivation of latent infection and therapeutic action to prevent relapses in tuberculosis patients. At the moment, the lines of investigation in more than 12 projects, that receive more support are the vaccines of Calmette and Guérin Bacilli modified by genetic recombination, the attenuated stocks of M. tuberculosis, the subunits vaccines and the DNA vaccines. <![CDATA[<b>Polio</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000300010&lng=es&nrm=iso&tlng=es Polio continúa endémica en: Nigeria, Afganistán Pakistán e India. La iniciativa global de erradicación de polio de la OMS estableció que para 2013 no debe haber ningún niño paralítico en el mundo por el virus salvaje o por el virus derivado de la vacuna. En esta revisión se describen ambas vacunas contra el polio, la oral y la inactivada, su inmunogenicidad, seguridad y las condiciones a cumplir por un país para que cambie su esquema de vacunación de polio oral a inactivada. La vacuna polio oral ha permitido la erradicación de la enfermedad en varios continentes incluyendo América; sin embargo conlleva riesgos, tales como polio paralítica asociada a vacuna (VAP-siglas en inglés-) y parálisis producida por polio virus derivado de la vacuna (VDP-siglas en inglés-). La Vacuna Polio Inactivada (VPI) es segura e inmunogénica, puede ser administrada en combinaciones vacunales. Para que un país cambie a VPI debe tener cobertura y esquema óptimo de esta vacuna, 90% de, cobertura de DTP3, vigilancia adecuada de parálisis flácida, no estar próximo en la actualidad o recientemente a un país con polio endémico. Strategic plan of Global Poliomyelitis Eradication Initiative (GPEI) of the WHO is that by 2013 no child will be paralyzed by a wild or vaccine derived poliovirus. This paper describes both oral and inactivated vaccine, safety concerns with the use of OPV, immunogenicity of IPV and the conditions to be full filled in order for a country to deliver IPV as a regular vaccine schedule. Oral polio vaccine has successfully contributed to global polio eradication in several continents including America. However, it carries risks, such as Vaccine Derived Poliovirus (VDP) and Vaccine Associated Paralytic Polio (VAPP). Inactivated Poliovirus Vaccine (IPV) is safe and immunogenic; it may be administered as monovalent or in a combined shot. Countries opting to switch from OPV to IPV should have: optimal IPV coverage and schedule, 90% of DTP 3 coverage, good surveillance of flaccid paralysis cases, and should not be near a country with endemic polio recently or at the present time. Are neither currently or were not recently polio endemic nor has close contacts with such areas. High immunization coverage is essential to ensure adequate population immunity.
