Aunque la vacuna BCG puede disminuir la severidad de la tuberculosis, su aplicación en pacientes con infección VIH, conlleva un riesgo potencial de enfermedad por Mycobacterium bovis. Objetivo. Determinar las complicaciones inducidas por vacuna BCG en pacientes con infección vertical por VIH. Método. Se realizó un estudio clínico y transversal. Se incluyeron pacientes con infección VIH de transmisión perinatal del Hospital de Niños “J.M. de los Ríos”, (Caracas-Venezuela), entre 1999 y 2010 que fueron vacunados con BCG. La información registrada incluyó presencia de complicaciones vacunales, edad para ese momento y cuando se realizó el diagnostico VIH, condición inmunológica, tratamiento y evolución. El análisis estadístico se realizó mediante pruebas descriptivas. Resultados. Se evaluaron 96 pacientes, de los cuales 16,7%(n=16) tuvo complicación: con enfermedad local o regional 14,6%(14/96) específicamente adenitis 100%(14/14), absceso o fístula 28,6%(4/14) y úlcera 7,1%(1/14); con enfermedad distante o diseminada 2,1%(2/96). La media de la edad al momento de la complicación fue 0,7±0,4 años y para ese momento 81,3%(13/16) tenía inmunosupresión. En 7,3%(7/96) de los pacientes, el diagnóstico VIH se realizó por la complicación. Con tratamiento antirretroviral de alta eficacia y terapia médica o quirúrgica para la complicación, el 87,5%(14/16) de los pacientes tuvo evolución satisfactoria. La mortalidad asociada a la vacuna se registró en 1,1%(1/96) y ocurrió en un paciente con enfermedad diseminada e inmunosupresión grave. Conclusiones. Las complicaciones ocurrieron en 16,7% de los pacientes: hubo enfermedad local/regional en 14,6% y enfermedad distante/diseminada en 2,1%. La mayoría de los pacientes complicados tenía inmunosupresión. El tratamiento médico apropiado para el virus y para el Mycobacterium bovis y en algunos casos también resección quirúrgica condujo a evolución satisfactoria en 87.5% (14/16).La mortalidad por enfermedad diseminada ocurrió en 1,1%.<hr/>Background. Although the BCG vaccine can reduce the severity of tuberculosis, its application in patients with HIV infection involves a potential risk of disease due to Mycobacterium bovis. Objective. To determine BCG vaccine-induced complications in patients with vertical HIV infection. Method. A clinical cross-sectional study was performed. Patients with perinatal-acquired HIV infection attended at Hospital de Niños "J. M. De los Ríos "(Caracas - Venezuela), between 1999-2010 and who were vaccinated with BCG were included. Recorded data included: presence of vaccine complications and age at that time, age at time of HIV diagnosis, immune status, treatment and outcome. Statistical analysis was done using descriptive tests. Results. 96 patients were included, of whom 16.7% (n=16) had complications: 14.6% (14/96) with local or regional findings, specifically 100% (14/14) adenitis, 28.6% abscess or fistula and 7.1% ulcer; 2.1 (2/96) had distant or diseminated disease. Average age at time of complication was 0.7 ± 0.4 years. At that time 81.3% (14/16) had immunosuppresion. HIV diagnosis was made because of the complication in 7.3% (7/96) With highly active antiretroviral therapy and medical or surgical therapy for the complication, 87.5% (14/16) of patients had satisfactory outcome. The vaccine-associated mortality was 1.1% (1/96) and occurred in a patient with disseminated disease and severe immunosuppression. Conclusions. Complications occurred in 16.7% of recipients: disease was local or regional in 14.6%, mainly axillary adenitis and distant disease or disseminated in 2.1%. Appropriate medical treatment for the virus and Mycobacterium bovis and in some cases, surgical resection resulted in satisfactory outcome in 87.5% (14/16) of patients. Mortality due to disseminated disease occurred in 1.1%. <![CDATA[<b>Deficiencias visuales en 13 niños</b>: <b>diagnósticos, causas y asesoramiento genético</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400003&lng=es&nrm=iso&tlng=es La alteración de algunas de las estructuras del ojo es capaz de producir deficiencias visuales y éstas pueden ser leves, moderadas o severas, de origen genético o de origen ambiental. El déficit visual severo es discapacitante para un niño y las patologías asociadas aumentan sus limitaciones y la dependencia. El asesoramiento genético y las medidas preventivas están orientados a ayudar a los padres a tener un hijo sano. OBJETIVOS: Determinar los diagnósticos y las causas genéticas o ambientales de las deficiencias visuales en 13 niños, así como el asesoramiento genético dado a las madres o parejas. MÉTODOS: Estudio retrospectivo y descriptivo. Revisión de las historias médicas de todos los 13 niños con deficiencias visuales y las madres o parejas atendidos en la Consulta de Diagnóstico y Asesoramiento Genético, Medicina Física y Rehabilitación, Hospital Universitario “Dr. Antonio M. Pineda”, Barquisimeto, marzo 2007 a noviembre 2008. RESULTADOS: 8 varones y 5 hembras. En nuestra primera consulta, la edad promedio de los pacientes fue 4 años 10 meses. La edad promedio de las madres, 30 años. Primigestas: 7/13. La edad promedio de los padres, 37 años. Cinco pacientes presentaron deficiencias visuales de causa genética: glaucoma congénito bilateral (2), retinoblastoma bilateral por neomutación (2) y retinoblastoma bilateral de origen paterno. las de causa ambiental se diagnosticaron en 8 pacientes, principalmente: retinopatía del prematuro, atrofia óptica, déficit visual cortical, corioretinitis y microftalmos. CONCLUSIONES: Muchas deficiencias visuales pueden prevenirse con el asesoramiento genético, medidas preventivas, buen control prenatal y óptima atención obstétrica y neonatal.<hr/>The alteration of some of the structures of the eye may produce visual deficiencies and these can be benign, moderate or severe, of genetic or environmental origin. The severe visual deficit is incapacitating for a child and the associated pathologies increase his limitations and dependency. Genetic counseling and preventive measures are intended to help the parents in having a healthy child. OBJECTIVES: To determine the diagnoses and the genetic or environmental causes of visual deficiencies in 13 children and the genetic counseling given to the mothers or couples. METHODS: This is a retrospective and descriptive study. We reviewed the medical records of all the 13 children with visual deficiencies and the mothers or couples who attended the Genetic Diagnosis and Counseling Clinic, Physical Medicine and Rehabilitation Service, University Hospital “Dr. Antonio M. Pineda”, in Barquisimeto, from March 2007 to November 2008. RESULTS: 8 males and 5 females. In our first consultation the average age of the patients was 4 years 10 months. The average age of the mothers was 30 years. First pregnancy: 7/13. The average age of the fathers was 37 years. Five patients had visual deficiencies of genetic causes: congenital bilateral glaucoma (2), bilateral retinoblastoma due to new mutation (2) and bilateral retinoblastoma of paternal origin. Those of environmental causes were diagnosed in 8 patients, mainly: retinopathy of the premature, optic atrophy, cortical visual deficit, chorioretinitis and microphtalmos. CONCLUSIONS: Many visual deficiencies can be prevented with genetic counseling, preventive measures, good prenatal control and optimum obstetric and neonatal care. <![CDATA[<b>Taquicardia supraventricular en pediatría</b>: <b>Presentación clínica y manejo. Experiencia del servicio de cardiología hospital de Niños JM de los Ríos. </b><b>Caracas</b><b>, Venezuela</b><b>. 2001-2011</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400004&lng=es&nrm=iso&tlng=es Introducción: La taquicardia paroxística supraventricular (TPSV) representa la segunda causa más frecuente de arritmias en pediatría (1). Objetivos 1. Determinar las características clínicas y el tratamiento en la emergencia. 2. Evaluar la respuesta al tratamiento y los fármacos utilizados. Metodología: Estudio retrospectivo de 69 casos con ingreso en el Servicio de Cardiología del Hospital de niños JM de los Ríos de 2001-2011. Criterio de Ingreso: pacientes con Diagnóstico de TPSV que ingresaron a la emergencia. Resultados: Se seleccionaron 69 pacientes. Edad promedio: 5,9 años (DE ± 3,5). Dos con antecedentes de Cardiopatía Congénita (2,9 %). La frecuencia cardíaca media fue de 284 lat./min (DE ± 30,66). El diagnóstico más frecuente fue TPSV por reentrada, en 35 pacientes (51%); el resto de los pacientes presentaron Wolff Parkinson White (WPW). Cinco pacientes respondieron a maniobras vagales. El tratamiento de elección en la crisis aguda fue Adenosina, y en casos de fallar el tratamiento inicial y de inestabilidad hemodinámica se utilizó la cardioversión eléctrica (16 pacientes). Para mantenimiento se indicó: Betabloqueantes y Amiodarona siendo efectivos (60,8%). Cuatro pacientes necesitaron ablación por radiofrecuencia (1 paciente presentó recaída posterior al tratamiento). Conclusiones: La presentación clínica de la TPSV varía con la edad. La mayoría de pacientes respondieron al tratamiento con adenosina. El tratamiento de mantenimiento con Betabloqueantes y Amiodarona es efectivo.<hr/>Paroxysmal supraventricular tachycardia (PSVT) is the most common pediatric arrhythmia. The objectives of this study were to determine the clinical features and treatment of PSVT in the emergency room and to evaluate the response to treatment and drugs employed. Methods: This is a retrospective study of 69 children admitted to the cardiology department of the Children's Hospital JM de los Rios from 2001 to 2011. Inclusion criteria: patients diagnosed with PSVT who were admitted to the emergency room. Results: 69 patients. Mean age: 5.9 years (SD ± 3.5). Two had a history of congenital heart disease (2.9%). The average heart rate was 284 beats / min (SD ± 30.66). The most frequent diagnosis was reentrant SVT, 35 patients (51%); the rest corresponded to Wolf Parkinson White syndrome. Five patients responded to vagal maneuvers. The treatment of choice in the acute crisis was adenosine. Electrical cardioversion was utilized in cases of initial treatment failure and hemodynamic inestability (16 patients). Maintenance treatment with beta-blockers and amiodarone was indicated in most cases (60.8%) with an effective response. Four patients required ablation, one of them relapsed. Conclusions: 1. clinical presentation of PSVT varies with age. 2. most patients responded to treatment with adenosine. 3. maintenance therapy with beta blockers and amiodarone is effective. <![CDATA[<b>Vasculitis gingival como complicación de sepsis de punto de partida enteral</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400005&lng=es&nrm=iso&tlng=es Vasculitis es la inflamación de vasos sanguíneos, con isquemia, necrosis y compromiso sistémico. Se describe el manejo multidisciplinario de niña de 4 meses quien ingresó al Hospital Universitario de Maracaibo con fiebre, diarrea, deshidratación, consciente, con períodos de somnolencia, abdomen distendido, ausencia de ruidos hidroaéreos. Posteriormente presenta convulsión, deterioro del estado general, lesiones eritematosas múltiples y en rodetes gingivales, de color blanco parduzco, necróticas, manifestaciones de shock séptico. Antecedente de ingestión de N-butilbromuro de hioscina. Laboratorio: anemia, leucocitosis, proteína C reactiva elevada, cultivos negativos. Biopsia del rodete gingival: vasos sanguíneos con paredes infiltradas por células inflamatorias mononucleares y polimorfonucleares dispersas y necróticas, destrucción y oclusión del endotelio, confirmando la vasculitis gingival. Recibió antibioticoterapia, soporte hemodinámico, cirugía de zonas necrosadas. Egresó a los 15 días con diagnóstico de vasculitis gingival por sepsis de origen enteral. El manejo interdisciplinario médico-odontológico facilita la resolución de situaciones generales con compromiso bucodental.<hr/>Vasculitis is the inflammation of blood vessels, with ischemia, necrosis and systemic involvement. This paper describes the multidisciplinary management of a 4 month old female infant that was admitted to the Hospital Universitario de Maracaibo with fever, diarrhea, dehydration, periods of somnolence, distended abdomen, and absence of bowel sounds. Later, the patient presented seizures, deterioration of her general condition, multiple erythematous and brownish white, necrotic lesions of the gingival border mucosa, and septic shock manifestations. The patient had a history of ingestion of N-butylbromid hyoscine. Laboratory tests: anemia, leukocytosis, elevated C-reactive protein, negative cultures, biopsy of the gingival border: blood vessel walls infiltrated by scattered and necrotic polymorphonuclear and mononuclear inflammatory cells, destruction and occlusion of the endothelium confirming gingival vasculitis. Treatment included antibiotics, surgery of necrotic areas and hemodynamic support. She was discharged after 15 days with the diagnosis of gingival vasculitis secondary to enteral sepsis. Medical and dentistry interdisciplinary management facilitates the resolution of general situations with oral involvement. <![CDATA[<b>VPH</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400006&lng=es&nrm=iso&tlng=es Actualmente la Administración de Drogas y Alimentos (FDA) de Estados Unidos aprobó dos vacunas para prevenir la infección por VPH (Virus de Papiloma Humano): Gardasil® (vacuna tetravalente) y Cervarix ® (vacuna bivalente). Ambas vacunas son muy efectivas en la prevención de infecciones persistentes por los tipos 16 y 18 de VPH, dos de los VPH de “alto riesgo” que causan la mayoría (70%) de los cánceres de cuello uterino y en menor porcentaje de cáncer de ano y pene. Gardasil® impide también la infección por los tipos 6 y 11 de VPH, los cuales causan prácticamente todas (90%) las verrugas genitales. Se presenta un resumen de la inmunogenicidad, eficacia, indicaciones, modo de empleo y presentaciones comerciales de estas dos vacunas.<hr/>The Food and Drug Administration (FDA) of USA licensed two vaccines for the prevention of VPH (Human Papillomavirus) infection: Gardasil® (quadrivalent vaccine) and Cervarix® (bivalent vaccine). Both vaccines are very effective in the prevention of persistent infection by serotypes 16 and 18 of VPH, two of the “High Risk” VPH, which cause 70% of cervical cancers and in low percentages anal and penile cancers. Gardasil® prevents infection by serotytpes 6 and 11, which cause almost all (90%) of genital warts. This review presents the immunogenicity, efficacy, recommendations, doses, administration and commercial presentation of both vaccines. <![CDATA[<b>Rotavirus</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400007&lng=es&nrm=iso&tlng=es La infección por rotavirus es responsable de 125 millones de casos, de más 500.000 defunciones anuales y de 40% de la hospitalización por diarrea en menores de 5 años de edad. En países en desarrollo la tasa de infección es más alta en el grupo de edad de 3 a 11 meses, quienes presentan mayor letalidad producto de la desnutrición y de la dificultad para acceder oportunamente a los servicios de salud; se observa que al año de vida, 65-80 % de los niños han desarrollado anticuerpos contra el rotavirus y 95% a los 2 años. Actualmente se utilizan dos vacunas contra el rotavirus, las cuales han demostrado ser seguras, eficaces y poco relacionadas con invaginación intestinal. En Venezuela, la vacuna monovalente-humana se introdujo en el Programa Ampliado de Inmunizaciones en abril de 2006. Un estudio previo mostró que la administración masiva de dos dosis de esta vacuna contra el rotavirus es altamente costo-efectiva. Cuatro años después en un estudio nacional se evaluó el impacto y se evidenció reducción de 50% de la tasa de mortalidad en los menores de 5 años, siendo mayor en el grupo de menores de 1 año con 55% y en el grupo de 1-4 años de 44%. Sin embargo el seguimiento de este programa nos indica que las coberturas de inmunización contra rotavirus en Venezuela siguen siendo bajas.<hr/>Infection by rotavirus is responsible for 125 million cases, 500,000 annual deaths and 40% of hospitalizations for diarrhea in children under 5 years of age. In developing countries the rate of infection is higher in the group of 3 to 11 months of age, which present a higher lethality, product of undernourishment and difficulties to accede opportunely to health services. During the first year of life, 65 to 80% of children have developed antibodies against rotavirus and 95% will achieve this by the age of two. At the moment two vaccines against rotavirus are available, and have demonstrated to be safe, effective and with very low association with intestinal invagination. In Venezuela, the monovalent-human vaccine was introduced in the Extended Program of Immunizations in April of 2006. A previous study showed that the massive administration of two doses of this vaccine against rotavirus is highly cost-effective. Four years later, a national study showed a reduction of mortality rate of 50% in children under 5 years of age, 55% reduction in those less than one year and 44% reduction in the group of 1-4 years of age. Nevertheless the follow up of this program indicates that immunization coverage against rotavirus in Venezuela continues to be low. <![CDATA[<b>Vacunas meningocócicas</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0004-06492011000400008&lng=es&nrm=iso&tlng=es La enfermedad meningocócica, aunque poco frecuente, es severa y puede causar la muerte en 10% de quienes la contraen, de allí la importancia de la inmunización para prevenirla. Existen varias clases de vacunas, como las polisacáridas que aun cuando pueden inducir protección, no son inmunogénicas en niños menores de 2 años ni inducen inmunidad de rebaño. La administración de dosis de refuerzo produce hiporespuesta. Las vacunas conjugadas pueden ser monovalentes como la Serogrupo C que demostró una reducción en un 93% de la enfermedad en poblaciones con altas coberturas vacunales, y las tetravalentes AC W135, Y/D (conjugada al toxoide diftérico y A; C, W135, Y/CRM139 conjugada a una mutante no tóxica de toxina diftérica Ambas son inmunogénicas y seguras. Estudios epidemiológicos con A; C; W135, Y/D descartan aumento de riesgo al Síndrome de Guillain Barre (SGB) posterior a su administración. Se recomienda administrar dosis única a adolescentes más un refuerzo. El personal de alto riesgo a la enfermedad (Asplenia anatómica o funcional, alteración del complemento, déficit de Properdina, VIH) deben recibir dos dosis más refuerzos cada cinco años.<hr/>Meningococcal disease is a rare but serious infection, up to 10% of persons who contract disease die, so it is very important to immunized for Meningococcal disease protection and prevention. There are two types of vaccine: Polysaccharides that even though induces protection ,is not immunogenic in children younger of 2 years, don’t induce herd immunity and produce hypo responsiveness to a booster dose. Conjugate vaccines can be monovalent serogroup: C which demonstrated 93% reduction of serogroups C disease in population with high vaccine coverage.Also there are two quadrivalent serogroups A;C;W135;Y vaccines one conjugate to D (difteric toxoid) and other to /CRM 139(mutant no toxic of dfsteric toxin. Studies showed their immunogenicity up to 55 years and boths are safes. Epidemiological study with A;C;W135,Y/D disproves any evidence of increased risk to SGB after its administration Recommended schedule is to immunized all adolescent, with a dose plus a booster. High risk people of invasive meningococcal disease (anatomic or functional asplenia,terminal complement or prperdin deficiencies,HIV) should rereceived, 2 doses, plus boosters every 5 years.
