Introduction: Exclusive breastfeeding is ideal for the newborn baby because of its nutritional, immunological and emotional contribution, key elements for adequate growth and development. It is recognized that return to work is the most influential factor for the abandonment of breastfeeding. It is necessary to assess the impact of the home lactary in the reduction of the abandonment of exclusive breastfeeding. Methods: An applicative study of experimental, prospective, cross case and analytical type was conducted in the Maternity Service of the San Cristobal Central Hospital. Working mothers willing to breast-feed were selected and a program for the creation and development of a home lactary was implemented; continuity of the home program was verified during the two subsequent months. Results were compared with mothers who did not receive this training. Results: Working mothers who created the home lactary presented a lower percentage of breast feeding abandonment (28) % in comparison with those who did not (60%). (p<0, 001). Among mothers who gave up breastfeeding, returning to work was the main cause in the group with no lactary (57%) and milk insufficiency was the main cause in the group with a lactary (22%). Returning to work as cause for abandonment dropped significantly with the creation of a lactary (p< 0.001) Discussion: The creation of a home lactary decreases significantly the rate of abandonment of breastfeeding in working mothers. The promotion of this type of strategies is an alternative to strengthen the prolongation of exclusive breastfeeding.
Availability of specific information about the human immunodeficiency virus (HIV) in adolescents may favor right decisions to avoid infection risk. OBJECTIVE: Determine knowledge about HIV in adolescents before and after an educational intervention. METHODS: A quasi-experimental, prospective, longitudinal, and comparative study was performed, conducted from the HIV Unit at the Hospital de Niños “J.M. de los Rios" in May 2016. Adolescents over 14 years who attended four private schools in Caracas (Venezuela) were included. A survey with closed questions about the virus was applied in two moments: before and after a specific educational presentation. Statistical analysis included McNemar test. RESULTS: The study included 923 adolescents. Mean age was 15.8 ± 1.1 years. Before the intervention 90.7% indicated that transmission occurs by blood and only 85.2% by semen and 76.2% by vaginal discharge. Regarding sexual practice without preservative, 96.6% indicated that transmission occurs with vaginal penetration, however, only 72.4% with anal penetration and 62.2% with oral relation. More than 50% didn´t know about the possibility of asymptomatic infection. After the intervention an increase in correct answers was found in all evaluated items, with statistically significant differences. CONCLUSIONS: Knowledge acquisition about HIV through formal education requires greater emphasis on relevant aspects. Timely implementation of specific educational strategies in adolescents can change the perception of the virus that may contribute to avoid risky behaviors. Clinical case: An 11 year old boy, who presents with pain in the left inguinal region, increased volume of the ipsilateral thigh, fever and functional limitation. The patient usually practiced baseball every other day. Physical examination: Antalgic position of the left thigh, phlogosis in inguinal and gluteal ipsilateral regions. Paraclinics: Leukocytosis, neutrophilia, elevated C-reactive protein. Nuclear magnetic resonance demonstrates collection in pelvitrocanteric muscles. Antibiotic therapy with Clindamycin and Cefotaxime were prescribed with a satisfactory evolution.
Tuberous sclerosis complex (OMIM #191100 and #613254) is a multisystem disorder that presents an autosomal dominant pattern of inheritance and is characterized by the presence of hamartomas in brain, eyes, skin, kidneys, heart and lungs.
