Introduction: The typical pain of duodenal ulcer is epigastric burning, which relieved with meals, to 60 or 90 minutes exacerbated after. In patients with consultation atypical pain there was slow clinical diagnosis.
Introduction: In the literature, it is reported an underuse and limited knowledge of hydrogen breathe tests by the medical community. This work aim to provide quantitative data about the knowledge and use hydrogen breath test in the medical community of Monterrey, Nuevo León, México. Materials and methods: A survey was developed and validated thought DELPHI methodology. The survey evaluated knowledge and use of hydrogen breath tests. For the data analysis, descriptive statistics was used. Results: A total of 158 surveys was obtained. Most of the survey physicians were pediatricians (34%). Most of the survey physicians referred not knowing hydrogen breath tests (69%), have not indicated the breath tests (93%), and not knowing the cost (95,6%). 67,1% of the survey physicians described ignorance has the limiting factor for not indicating this kind of test in their practice. Physicians considered hydrogen breath test useful for the diagnosis of carbohydrates intolerance (38,6%), intestinal overgrowth bacterial syndrome (31,6%), and not useful (32,9%). Discussions : There is a considerable ignorance in the medical community about hydrogen breath test. Even further, this diagnostic resource is underutilized in our context. The results present the necessity to give more diffusion and to educate medical community about hydrogen breath tests.
The “Gastric Bypass” is one of the most performed bariatric operatio ns worldwide, and in recent years the number of former bypass patients accrues. The gastric bypass excludes the stomach and duodenum, and it makes very difficult, by oral route, the access to the biliopancreatic tract, by means the endoscopic retrograde cholangiopancreatography (ERCP). Due to the increased evidence of gallstones after bariatric operations, the ERCP could be more often required. In recent years, it has been demostrated that gastric bypass patients can be successfully evaluated endoscopically by laparoscopic transgastric ERCP, as a safe and reliable method. We describe a patient with residual billiary impacted stone in the papilla, who underwent laparoscopic transgastric ERCP and a precut “fistulotomy” technique with needle - knife, was highly successful, without add significant morbidity to the procedure. Esteatosis Pancreática secundaria a tratamiento prolongado con esteroide. Radiologic bone age 11 years. Abd ominal ultrasound: periportal hyperechogenicity, slightly enlarged pancreas with diffuse increase of its echogenicity, head: 30mm, body: 13mm and tail: 30mm. Endoscopic ultrasound: Uncinate process and head with homogeneous parenchyma, preserved echogenici ty. In the ismo, body and parenchyma heterogeneous, with tendency to hyperechogenicity. Wirsung: normal caliber. Clinical diagnosis: Pancreatic steatosis, Exogenous Cushing's Syndrome and Insulin Resistance with vitamin D deficit. Discussion: It is considered that t here is a positive relationship between prolonged steroid use for fatty replacement of the pancreas and associated pathologies as evidenced in this case.
Introduction: pseudo - obstruction of the colon is characterized by marked dilatation in the absence of mechanical obstruction, being uncommon in children. It indicates an imbalance of the autonomic nervous system triggered by drugs, intestinal ischemia, systemic or local inflammation of th e peritoneum among others. Clinical case : male schoolboy of 8 years old, with abdominal pain of strong intensity, colic type, associated with fever, and fifth day without evacuation. Background of Testicular Embryonal Rhabdomyosarcoma, recent chemotherapy with Vincristine, Ifosfamide, Dacinomycin and Adriamycin. Physical examination: painful facies, oral mucosa ulcers and cheilitis. Tachycardia 120xmin.
