Hyaluronic acid has important functions in inflammatory and tissue reparation processes. Owing to the varied strategies of the parasites to evade the host´s immune response, as well as the multiple functions and physiological importance of hyaluronic acid, the aim was to study the hyaluronan binding capacity by Ascaris lumbricoides larval stages. Larval concentrates were prepared by hatching A. lumbricoides eggs. The larvae were collected by the Baermann method. The test of serum soluble CD44 detection by Agregation Inhibition was modified. All the larval concentrates presented hyaluronan binding capacity. The obtained results allow to suppose the existence of an hyaluronic acid specific receptor in A. lumbricoides. This receptor eventually might compete with the usual receptors of the host. The parasite might use this mechanism to evade the immune response.
Cryptosporidiosis in food handlers from Venezuela is unknown, being this an important public health problem in immunosuppressed patients. To determine the prevalence of Cryptosporidium sp and other intestinal parasites in food handlers from Zulia State, one hundred nineteen fecal samples were evaluated by wet mount, concentrated according to Ritchie and modified Ziehl-Neelsen staining. Fourteen (11.8%) were positive for Cryptosporidium sp and associated with other protozoosis (P < 0.05), being most frequent Endolimax nana (42.9%). The general prevalence of the intestinal parasitism was 48.7%, emphasizing E. nana (41.2%), followed by Blastocystis hominis (38.7%) and Entamoeba coli (17.6%). The most frequent pathogenic protozoa was Giardia lamblia (13.4%), followed by the complex Entamoeba histolytica/E. dispar (9.2%). 4.1% were positive for intestinal helminthes. The infection by Cryptosporidium sp is frequent in food handlers from Zulia State. Given to the results of this investigation and the nonexistence of studies in this population, is necessary to deepen in the impact of this parasitism in food handlers and the consumers of their products. Ratas macho Sprague-Dawley recibieron ACR (5mg/Kg, i.p), más uno de los siguientes tratamientos: Grupo 1: Salina, 0,10mL/100g i.p.; Grupo 2: Win-51.708(WIN), 25mg/Kg i.p.; Grupo 3: S-metilisotiourea(MITU), 35 mg/Kg i.p.; Grupo 4: Rofecoxib(ROF), 20mg/Kg v.o.; Grupo 5: Meloxicam(MEL), 25mg/Kg i.p.; Grupo 6: combinación MITU+ MEL.
To investigate the role of substance P(sP), nitric oxide (ON) and prostaglandins (PGs) in acrolein(ACR)-induced cystitis, we studied the changes induced by ACR on bladder inducible nitric oxide synthase(iNOS) and mieloperoxidase(MPO) activities, along with PGs and NO metabolites levels. Sprague-Dawley male rats received i.p. ACR (5mg/Kg) plus one of the following treatments: Group 1: saline 0.10 mL/100g i.p.; Group 2: Win-51.708 (WIN) 25mg/Kg i.p.; Group 3: S-metilisothiourea(MITU) 35mg/Kg i.p.; Group 4: Rofecoxib(ROF) 20mg/Kg o.p.; Group 5: Meloxicam(MEL) 25mg/Kg i.p.; Group 6: combination MITU+MEL. ACR-induced mortality was partially prevented by WIN (NK1 antagonist) and MITU (iNOS inhibitor). Animals that survived after 24h of ACR exposure, had histological inflammatory changes in bladder along with increased MPO activity. There was augmentation of nitrates+nitrites and of PGs. WIN didn't prevent any of these effects. ROF and MEL (COX-2 inhibitors) partially protected against bladder inflammation; MITU pre-treatment was able to prevent these changes and those of NO metabolites levels. The MITU+MEL combination produced the highest protection against ACR-induced damage. These results suggest that NO produced via iNOS and PGs produced by COX-1/COX-2, have an important role in the pathogenesis of cystitis induced by ACR. ACR could stimulate iNOS and COX-1/COX-2, producing lymphocyte migration and increases of NO and PGs.
Magnetic resonance imaging (MRI) is useful in the diagnosis of meniscal lesions of the knee. The purpose of this study was to relate the findings of MRI and arthroscopy and to evaluate the morphology of the menisci with tears. 39 patients of both genders were included, whose age range was 13 to 74 years old (mean: 42.6 years), with and without a history of trauma, who underwent MRI and arthroscopy of the knee, due to symptoms of articular lesion. The images of magnetic resonances were analyzed independently by two specialists prior to the arthroscopy. The measurements of the medial and lateral menisci were made in each meniscal horn with sagital images in protonic density and fat-suppression. MRI detected 8 cases of tear of the lateral meniscus of the 11 catalogued by arthroscopy, and 11 cases of tears of the medial meniscus of the 13 catalogued by arthroscopy. The sensibility and specificity of MRI for the lateral meniscal tears were 72% and 100%, and for the medial tears were 85% and 89%. The meniscal tears were localized mainly in the posterior horn. The dimensions of the posterior horn of the lateral meniscus were larger in disrupted menisci (height, 7.1±1.3 mm vs. 6.1 ± 0.7 mm, p <.05; wide, 10.2 ± 1.6 mm vs. 8.8 ± 1.3 mm, p < 0.05). A meniscal tear produces morphological changes, particularly in the posterior horn of the lateral meniscus. Magnetic resonance constitutes the imaging technique of choice for the diagnosis of the meniscal tears. Incremento de la transcripción de sialiltransferasas en muestras de cérvix con neoplasia intraepitelial cervical http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332009000100006&lng=es&nrm=iso&tlng=es Altered sialylation observed during oncogenic transformation, tumor metastases and invasion, has been associated with enhanced sialyltransferases (STs) transcription. Increased mRNA expression of STs (ST6Gal I, ST3Gal III) has been detected in invasive cervical squamous cell carcinoma. A study of the sialic acid concentration in local tissue of cervix and in serum showed a slight elevation in benign inflammatory lesions and a moderate elevation in severe neoplasia, but to date, altered expression of STs in cervical intraepithelial neoplasia has not yet been evaluated. This study investigates the changes in mRNA expression of three STs (ST6Gal I, ST3Gal III, and ST3Gal IV) in cervical intraepithelial lesions (CIN). Alterations of these STs mRNA expression were examined in 35 cervix specimens classified as normal, CIN 1, CIN 2 and CIN 3, by semiquantitative reverse transcription-polymerase chain reaction. mRNA expression of the three STs was enhanced in CIN 1, CIN 2 and CIN 3 with respect to normal tissue, with a significant difference of p < 0.001 (Mann-Whitney U test) for all the enzymes. Our results suggest that altered expression of ST3Gal III, ST3Gal IV and ST6Gal I in CIN could play an important role during malignant transformation and could be related with the enhanced sialic acid expression detected in neoplasic tissues.
Mutations in the K-ras oncogene are common in colo-rectal cancer, which affect the biological behaviour and may influence the susceptibility to therapy in these tumors. The objective of this work was to identify the types of K-ras mutations observed in referred patients with colo-rectal cancer and to relate them to their degree of histological differentiation and clinical stage. Histopathological and clinical data were obtained from medical records. DNA was obtained from both, fresh tissue and tumor tissue embedded in paraffin. The K-ras gene was amplified through the polymerase chain reaction (PCR) and the amplified fragments were digested with restriction enzymes. We found mutations in codons 12 and 13 of the K-ras oncogene in 23.33% of patients. Of these, 28.57% were located at codon 12, 57.14% were at codon 13 and 14.29% at both codons. They were more frequent in tumors located in the left hemicolon and, according to their histological type, were more frequent in well differentiated adenocarcinomas (58.70%) and in mucinous (28.57%). The identified mutations were more frequent in advanced stages (C2) of Dukes' classification. The molecular analysis of the K-ras oncogene made mutations evident, which could be useful in the diagnosis and prognosis of colorectal tumors. The frequency of mutations found in this work is similar to some of those reported worldwide; however, they differ in the more frequent type of mutation, which, in our study, was located at codon 13 in more than 50% of the cases.
In Yucatán, 52% of patients with type 2 diabetes (DT2) present secondary failure to treatment associated with sulphonylurea and metformin. A possible explanation may be due to polymorphisms in the genes IRS1, CAPN10, PPARG2, which are involved in pancreatic β cell dysfunction and a poor response to the action of insulin. The association of the polymorphisms Gly972Arg, SNP43, and Pro12Ala, of the genes IRS1, CAPN10, PPARG2, with the risk of failure to sulphonylurea and metformin therapies was determinated in patients with DT2 in Yucatán, México. One hundred and thirty and two subjects with DT2 were classified in groups of responders (HbA1c< 8%) and non-responders (HbA1c> 8%) to the treatment, according to the control of hyperglucemia with sulphonylurea and metformin. Demographic, anthropometric and metabolic data were obtained from each subject. The polymorphisms were identified by means of DNA analysis by PCR/RFLP and PCR/OAL. Genotypic and allelic frequencies and the Hardy-Weinberg equilibrium were determined. Statistical analyses consisted of X² and multiple logistic regression tests (Epi-Info 2000 and SPSS version 12). Obese subjects carrying the genotype AA SNP43 showed 4.69 times more risk of failure to respond to treatment (p=0.027), when compared with subjects sharing GA genotype: X² (OR= 4.69, IC: 1.15-20.59) and multiple logistic regression, p= 0.048, (OR= 3.72, IC: 1.009-13.718). The interaction between genotype AA and the BMI> 27 showed also a significant difference (p=0.009). The findings suggest the fact that polymorphism SNP43 may influence the response to treatment with sulphonylurea and metformin, the expression being dependent on obesity. Proliferación y supresión circunstancial inducida por antígenos de flagelo y membrana de Trypanosoma cruzi http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332009000100009&lng=es&nrm=iso&tlng=es We have studied, in vitro, proliferation induced by flagella (FE) and membrane (ME) antigenic fractions of T. cruzi epimastigotes, as well as their regulatory effect on the proliferative response to PPD (Protein Purified Derivative). Crude flagella as well as bands from Western blots of flagella and membrane of epimastigotes were tested. Crude flagella elicited higher proliferation in mononuclear cells from patients with Chagasic cardiomyopathy (CDM) than in patients with no evidence of cardiac pathology (INF). Fractionated antigens induced a lower proliferative response, in intensity as well as in frequency, than the crude extracts. With FE, bands between 150 and 24.3 kDa (B3 to B18 with the exception of B4 and B13) induced higher CPM (Counts Per Minute) in CDM. In INF only bands B7 (87.3 to 80.1 kDa), 9 (69.8 to 64.6 kDa) and 13 (45.4 to 41.5 kDa) had high CPM. ME bands also elicited higher proliferation in CDM. However, only 5 out of 14 bands gave CPM higher than 1000 in CDM and none in INF. The mean down regulation (DR) of most bands was similar in both groups. But the frequency of relevant DR elicited by FE was significantly higher in CDM. In contrast the frequency of up regulation (UR) was higher in INF. Bands 13 and 14 of ME did not induce DR in most INF. The discordance between the frequency of relevant DR in CDM and INF was more evident with ME than with FE. The frequency of (UR) was 50% or higher with all ME bands in INF, but, lower than 12% in CDM. The higher UR in INF and of DR in CDM, suggest the presence of some balance or interaction in INF that is lost in CDM. In ME there might be antigens that could be relevant for the immunoprofilaxis of Chagas' disease. The difference in the clinical status of the two groups seems to be associated with the recognition of different groups of antigens together with variations in the nature of the regulation of the response of mononuclear cells to these antigens.
The clinical case of a 6 years old boy, who presented severe right renal trauma with rupture of the pyelocaliceal system and renal injury (IV grade), is reported. Upon admittance to emergency he presented paleness, tachycardia, rapid pulse, intense pain in the right lumbar region and macroscopic hematuria. Using abdominal ultrasound scan, an injury of the right renal parenchyma with perirenal collection was shown. Hematology studies showed a decrease in the hemoglobin count of 3,1 g% during the first posttraumatic 48 hours. A non-surgical conservative treatment was decided, since the patient’s hemodynamic status held stable despite the hemoglobin alterations. To treat renal injury, a percutaneous catheter drainage was placed in the right perirenal space on day 16th from the accident. The patient was released after this treatment and was controlled as an outpatient, until the rupture of the excretory urinary system was sealed (3 weeks), when the patient returned to have the catheter removed. The patient´s progress was excellent, the kidney was preserved and no complications have occurred in a 6 months follow up after the injury. The non-surgical treatment in renal trauma with injury to the excretory system is effective and advisable in children, provided the patients are hemodynamically stable.
Chronic Immune Thrombocytopenic Purpura (cITP) has become a field of multiple therapeutic assays. More than 20 types of treatment have been developed to obtain a favorable and prolonged platelet response. The treatment of cITP is oriented to inhibit the antiplatelet antibodies production by interference with the macrophage of the reticulum endothelial system and a blockade of the antigenic response with a decrease in the amplification of the immunological response. Steroids of the glucocorticoids type and splenectomy constitute the first line of treatment. Failure of these treatments leads to the use of second line drugs such as non steroid immunosupressors and the immunoglobulins type IgG and anti-D. Therapeutic assays with others immunomodulators have been reported. The introduction of new drugs destined to increase the megakaryocytic bone marrow platelet production, has opened a new way to treat the cITP. However, the splenectomy remains as the simplest, safest and most effective treatment in cITP. The principal criteria does not have to be focused on obtaining a normal platelet count, but to reach safe hemostatic levels in absence of hemorrhage, for a prolonged time. On the other hand, despite the persistence of thrombocytopenia, the hematologist can choose to maintain the patient with no treatment and with only a strict clinical observation. It is obvious that the cost-benefit from the different treatments is inclined towards those of lower cost and minimal secondary effects.
Atherosclerosis is an inflammatory disease of the arterial wall, where both cellular and humoral immunity mechanisms are involved. Vascular endothelial dysfunction and lipoproteins retention into the arterial intima have been reported as the earliest events in atherogenesis, promoting cytokines and chemokines releases; both responsible of leukocytes recruitment. Arterial proteoglycans retain and modify the lipoproteins, increasing their phagocytosis into macrophages through class A and class B scavenger receptors in the case of oxidized lipoproteins (LDLox), causing the production of cytokines like Tumoral Necrosis Factor (TNF)- α, Interleukin (IL)-1 β, IL-6, IL-12 and IL-18, among others. This secretion generates T cells activation into T helper lymphocytes (Th1), able to recognize the LDLox and heat shock protein as autoantigens, amplifying the inflammatory response. Macrophages that have uptaken lipoproteins become foam cells and their accumulation produces the formation of fatty streaks, the first step into atherosclerosis. Due to the biological and clinical importance of these events, the purpose of the present review is to offer recent information on the inflammatory reactions that occur around the establishment of the atheromatous plaque, exhibiting experimental evidences of the physiologic role of leukocytes and their interaction with the extracellular matrix. Furthermore, to emphasize about the major inflammatory biomarkers on the prognosis of cardiovascular diseases.
