ABSTRACT Since 1988, when Bianco and Colleagues published findings about the beneficial effect of furosemide in exercise-induced asthma, there has been an increasing interest about the function of this drug over the airway. The purpose of this article is to gather data in prevously published studies about the putative mechanisms for the potential protective role of furosemide in asthma. Even though there are multiple potential mechanisms of action, there is not significant data that could allow us to accept or deny any hypothesis, because there are important discrepancies between existing studies, so that, to the present day, there are not relevant clinical advantages on exercise-induced asthma. It is noteworthy to establish that furosemide is not a bronchodilator agent, so that its use in acute exacerbations of asthma is not justified.
ABSTRACT We are including revision on atherosclerosis, the different types of tisular changes described in this pathology, the theories that could explain its physiopathology, the risk factors and its complications. The cholesterol metabolism and transport is described and the findings of several studies demonstrating cholesterol and triglyceride high serum levels as risk factors for atherosclerosis and coronary events. Description is made of the endothelial dysfunction and its relation with early cardiovascular events. The action and use of statins (competitive inhibitors of the action of the hepatic enzyme hydroxymethyl glutaryl coenzyme A reductase -HMG CoA reductase-, stopping the conversion of HMGCoA to mevalonic acid, the principal precursor of endogenous cholesterol) in the prevention of cardiovascular events. How statins have actions in the production of nitric oxide, interaction white blood cell and the endothelium, antioxidant, antithrombotic and cytoprotective actions. There are also included other effects described for these drugs and the benefits when administered for the prevention of cardiovascular diseases. Emergence of drug resistance is usually associated with changes in the expression of an specific membrane P-glycoprotein, but also includes physiological responses with high complexity. In the present review we summarize results which emphasize that the comprehension of the molecular pharmacology of drug-resistant phenotype must include, as a way for identifying new strategies for the control of the disease, the understanding of the multiple biochemical and functional parasite mechanisms involved. Molecular Pharmacology of Chemo-Resistant Leishmania http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642003000100004&lng=es&nrm=iso&tlng=es ABSTRACT Infectious diseases leishmaniosis among them, constitute a leading cause of death world wide, especially in the developing world, where they remain as an important cause of concern and has become a serious problem because of the everyday enhanced risk of co infection with HIV and the increasing frequency of resistance development of the parasites to the drug agents.
ABSTRACT Autoimmune Cholangitis is a disease of unknown etiology, with clinical, biochemical and histological findings of colestasis.Autoimmune Cholangitis has been considered a Primary Biliary Cirrhosis without Antimitochondrial Antibodies.
ABSTRACT Revision is made of the mechanism of action of inhibitors of the ciclooxygenase (COX), their efficacy and adverse effects when used alone or combined. Emphasis is made in the risk-benefit when using inhibitors of COX-2 and the clinical implications in cardiovascular physiology. We include also revision of COX isoenzymes (COX-1 y COX-2), their genetic coding, tissue expression and functions in the vascular wall. Comparison is made of mechanism of action for aspirin, non selective NSAID’s, coxibs (selective inhibitors of ciclooxygenase) and acetaminophen including clinical studies. We conclude that further clinical studies in vivo are required to study the combination of low dosage of aspirin with ANSAID, coxibs and acetaminophen, in order to determine their interactions and cardiovascular effects. We can also point out that the use of NSAIDS as drugs for the prevention of cardiovascular events has not been adequately investigated, and for that reason we recommend to carry on further investigations in this area.
ABSTRACT Diverse investigations have established a relationship between an altered lipid profile and higher risk of cardiovascular diseases, which have their highest incidence in adult people. For this reason it was considered necessary to know the lipid profile behavior in a sample of this population. 186 individuals over 40 years of age and from Maracaibo, were chosen at random order. They were divided by age groups (40-49y, 50-59y, 60y and over) and by gender (94 men, M, 92 women, W). Serum concentrations of total cholesterol (TC), Triacilglycerides (TAG) and HDL-col were determined; the Friedwald formulas were used for the remaining fractions. TC (mg/dl) showed a significant statistical difference in relation with gender in the age group 60y and over (176,9±4,7-M Vs 200,1±4,9-W), and also between men of each age group (193,5±9,0 Vs 192,2 ±11,2 Vs 176,9±4,7). Tryacilglicerides (mg/dl) showed a significant statistical difference between men age groups (221,15±21.84 Vs 170,96±17,94 ± Vs 131,45±6,871) and according to sex in the age group 40-49y (221,1±21,8- M Vs 149,8±9,7-W) and 60y and over 131,4±6,8-M Vs 157,7±9,8-W). HDL-col (mg/dl) levels showed differences according to sex in all age groups, 40-49y (39,3±1,5-M Vs 48,3±1,7-W), 50-59y (37,3±1,21-M Vs 45,2±1,7-W), 60y and over (41,4±1,4-M Vs 45,8±1,21-W). VLDL-col (mg/dl) concentrations showed difference in each of the men’s age groups (44,2±4,3 Vs34,1±3,5 Vs 26,3±1,3), also between sex for the age groups 40-49y (44,2±4,3-M Vs 29,9±1,9-W) and 60y and over (26,2±1,3-M Vs 31,5±1,91-W). The sutdy showed a decrease in the TC and TAG levels in the age groups of 60y and over, likewise there were significant differences in the HDL-col levels between sex for all age groups studied. These differences were probably due to genetic and environmental factors.
ABSTRACT A prospective study was made in the Hospital "Domingo Luciani" to evaluate the efficiency of the Hyaluronic Acid (HA) on the articute cartilage. For this were taken 25 patients (30 knees), in patients of bouth sexs and ages between 40-60 years, with diagnostic of Gonarthrosis Incipient (Grades I-II), according to Ahlbäck radiological classification, In which were evaluated the following parameters: Pain, degree of flexion limit, degree of extention limit and damage of the articular cartilage, using arthroscopy before and after the treatment. The study was divided in 3 steeps: Diagnosis, Treatment (15 knees with HA and 15 knees without HA or control). Evaluation and Following (4 months after treatment). The results when bouth groups were compared shaw a big improvement in the evaluated parameters of the patient treated with HA, stadistically demontrated (varianze analysis p<0,001), except in the parameter of degree of extension limit, where was evidenced clinical improvement, but could’nt be demonstrated stadistically. We can conclude from our study that this working protocol was successful in the manegement of Gonarthrosis Incipient and that the HA produces an clinical improvement and promotes the scarring of the articular catilage wounds.
ABSTRACT To explore the antioxidant potential of tomato we submitted adults rats to three diets: I, II or III, containing 0%, 5% or 10% w/w tomato extract, respectively. After 9 continuos weeks of treatment, the animals were exanguinated by heart punction. Liver microsomes and plasma high-density lipoproteins were obtained by preparative ultracentrifugation. Fatty acid composition of HDL and microsomes was determined by gas-liquid chromatography. Both, HDL and liver microsomes samples were challenged with an oxidant stress by exposing to a mixture of Fe2+/ascorbate. Thereafter, MDA/mg protein was analized. Results show that in diet I group, MDA production was significantly increased as compared to II and III. Inter-dietary results of fatty acids showed that there was no difference in fatty acids n-6 series. On the contrary, n-3 series was higher in HDL of diet I group. In conclusion, tomato consumption in the rat diminished the susceptibility to the oxidation of hepatic microsomes and HDL.
ABSTRACT It has been demonstrated that genetic factors can be influenciated by race, whereas people with African origin tend to have high body mass index (BMI) with normal lipid profile (high HDL-c and low triglyceride). The purpose of this study was to analyze insulin resistance through a mathematical model, Homeostasis Model Assessment (HOMA), in two Venezuelan communities with African origin. Seventy-two afrovenezuelan women from the populations of Santa María (SM) y San José (SJ), Zulia, Venezuela, with age between 36 and 48 years, were used. A medical chart was made along with BMI so that they could be divided into 2 groups: obese and lean; glycemia, lipid profile and HOMA IR were determined. Our results indicated that there was not significative different in glycemia (obese group 85,1 ± 3,5 SM y 83 ± 2,9 SJ; lean 77,3 ± 2,2 SM y 78,9 ± 2,8 SJ mg/dl), Total Cholesterol (obese group 194,4 ± 13,4 SM y 190,1 ± 9,1 SJ; lean 184,4 ± 8.8 SM y 193,4 ± 7,4 SJ mg/dl), Triglycerides (obese group 109,8 ± 16,3 SM y 114,8 ± 21,9 SJ; lean 64,9 ± 7,6 SM y 77,5 ± 10,8 SJ mg/dl), LDL-c (obese group 126,6 ± 14,8 SM y 126,9 ± 8,6 SJ; lean 115,5 ± 8,3 SM y 133,6 ± 11,4 SJ mg/dl) and serics HDL-c (obese group 46,3 ± 4,0 SM y 40,2 ± 3,4 SJ; lean 55,5 ± 2,8 SM y 44,1 ± 2,3 SJ mg/dl).
ABSTRACT Introduction and objectives: The Wolff-Parkinson-White syndrome (Short PR interval and functional blockade of branch) is related with accessory pathways bundle (atrial/ventricular) identified originally through 12D-EKG as patterns A, B and C. Gallagher, using electrophysiological methods identified 10 sites, but without direction and orientation of the ventricular vectors. The purpose of this study was to establish the initial point and spatial orientation of the initial ventricular vector plus delta wave and abnormal bundle using the 12D-EKG and pointing the left or right, top or bottom and anterior or posterior positions. Methods: Were studied 78 patients with ages between 2 and 65 years old. 71 of them do not show any other disease and 5 show hypertension and 2 Ebstein disease. Results: According to bundle position, patients were classified in two groups: a) with left ventricular bundles: bundles 9 (C1) and 7 (A1), according to the SIC, with 11 and 10 cases respectively. b) With right ventricular bundles: bundles 1 (B2), 2 (B1), 3(A2), 4, and 6 (A3), according to the Sociedad Internacional de Cardiología (SIC) with 26, 5, 21 and 5 cases respectively. All the results agree with electrophysiological patterns: C1(5), A1(4), B2(7), B1(4), A2(7), A3(5 cases). Conclusions: Detection of abnormal bundles using 12D-EKG will help the electrophysiological studies and thereby the solution of the main complication, arrhythmias, for patient benefit.
