Caspofungin and micafungin appear to be similarly efficacious in salvage therapy in aspergillosis; anidulafungin has excellent in vitro activity against Aspergillus species but as yet there are no sufficient clinical data for anidulafungin in this disease state. Each drug has minor advantages and disadvantages compared to the others of the same classe; however, there are large differences in the approved indications for the different drugs. The formulary selection process should consider the direct and indirect costs of the single agents; the characteristics of the patient population at risk for invasive mycosis, such as frequent use of interacting drugs and the burden of monitoring plasma drug levels of drugs; and the implications of using products for indications which have not been still approved (off-label indications). <![CDATA[<b>Riesgos en la práctica odontológica</b>: <b>Uso del Mercurio</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000300003&lng=es&nrm=iso&tlng=es El mercurio es un elemento metálico que ha sido catalogado como un material peligroso debido a los graves daños que ocasiona a la salud y al ambiente. La inhalación de vapor de mercurio por un periodo prolongado causa el mercurialismo, el cual es una enfermedad que se caracteriza por temblores finos y eretismo. En odontología, el mercurio es utilizado para la elaboración de las amalgamas empleadas en la restauración de los dientes tratados por caries. La intoxicación en los consultorios dentales, generalmente es de carácter crónico causada por la exposición prolongada a vapores de mercurio y ocurre por no tomar las precauciones durante la manipulación del metal en el proceso previo a la preparación de amalgama. Se ha demostrado que las buenas prácticas en el trabajo odontológico reducen los niveles de mercurio en la orina relacionados con afecciones del comportamiento. La Asociación Dental Americana (ADA) considera que los riesgos de exposición mercurial en el trabajo de Odontología son escasos si el personal que trabaja en dicha área cumple con las normas de higiene mercurial existentes.<hr/>Mercury is a metallic element that has been listed as a hazardous material because of the severe damage it causes to health and the environment. Inhalation of mercury vapor for an extended period causes mercury poisoning, which is a disease characterized by tremors erethism fine. In dentistry, mercury is used for the preparation of amalgams used in the restoration of carious teeth treated. Poisoning in dental offices, is generally caused by a chronic exposure to mercury vapor and occurs for not taking precautions when handling the metal in the process leading to the preparation of amalgam. It has been shown that good dental work practices reduce levels of mercury in urine associated with behavioral disorders. The American Dental Association (ADA) believes that the risks of mercury exposure in the work of Dentistry are limited if the personnel working in that area meets the standards of existing mercury hygiene. <![CDATA[<b>Diagnóstico micológico en pacientes infectados por el Virus</b> <b>de la Inmunodeficiencia Humana</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000300004&lng=es&nrm=iso&tlng=es Con el descubrimiento del virus de la inmunodeficiencia humana (VIH) en 1981, la frecuencia de las infecciones oportunistas en pacientes inmunodeprimidos, especialmente los infectados por el VIH, ha experimentado un notable incremento. Dentro de éstas se encuentran las de etiología fúngica, las cuales presentan un alto grado de mortalidad si no son diagnosticadas y tratadas a tiempo. En nuestro país con la instauración de la terapia antirretroviral altamente efectiva a partir del año 2001, se ha notado una reducción significativa en la morbilidad y la mortalidad por infecciones oportunistas, en particular por las micosis, muy comunes en este tipo de pacientes. En el presente estudio se realizó el diagnóstico micológico procedente de pacientes infectados por el VIH/sida, durante el período comprendido entre enero 2005 y diciembre de 2009. De un total de 1173 muestras biológicas procesadas, solamente resultaron positivas 324 para un 27,6% de positividad. La candidiasis orofaríngea y la meningoencefalitis criptococóccica fueron las infecciones diagnosticadas con una mayor incidencia. En las muestras analizadas la concordancia en la positividad entre el examen directo y el examen por cultivo micológico fue de un 56,2%, lo que confirma la importancia del empleo del examen directo en la visualización de estructuras fúngicas a partir de muestras clínicas.<hr/>With the discovery of the human immunodeficiency virus (HIV) in 1981, the frequency of the opportunistic infections in the immunosuppressed patients, especially the infected ones by the HIV, it has experienced a notable increase. Inside these there are those of micotic aetiology, which present a high grade of mortality, if they are not diagnosed and treated on time. In our country with the instauration of the highly effective antirretroviral therapy since the year 2001, a significant reduction has been seen in the morbidity and the mortality of the opportunistic infections, specialle those caused by fungus, in this type of patients. In the present study the positividad was analyzed in the mycological diagnosis realized by conventional methods of the different clinical samples received in the National Laboratory of Reference of Mycology, from patients infected with HIV/aids during the period between January 2005 and December 2009. Of a whole of 1173 biological samples processed only 324 turned out positive, for 27, 6 % of positivity, being the oropharingeal candidiasis and the cryptococcal meningoencefalitis, those of major incidence observed. In the clinical samples analyzed, a concordance of 56,2 % was obtained in the positividad of the samples studied by means of direct examination and mycologycal cultures, which confirms the importance of direct preparations of clinical samples for the visualization of fungal elements from clinical samples. <link>http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000300005&lng=es&nrm=iso&tlng=es</link> <description/> </item> </channel> </rss> <!--transformed by PHP 10:09:53 22-09-2026-->
