Se tomó el peso, talla, circunferencia cefálica y del brazo al nacer. La clasificación antropométrica se realizó a través del indicador "Peso para la edad gestacional" y se utilizó la referencia venezolana de Henríquez, además de Lubchenco y Williams, esta última recomendada por la OMS. La edad gestacional se calculó por Capurro, tomando solo RN de 40 semanas. Se consideró pequeño para edad gestacional (PEG) al RN con peso al nacer £ p10 de los valores de referencia y grande para edad gestacional (GEG) al RN ³ p90. El Kappa se utilizó para conocer el grado de acuerdo. La prevalencia del PEG fue mayor para Williams (25,5%), seguido de Lubchenco (16,7%) y Henríquez (13,7%). Situación contraria ocurrió con los GEG. El mejor acuerdo se presentó entre Henríquez y Lubchenco (Kappa=0,71). Este estudio evidenció divergencias en la clasificación antropométrica del RN al utilizar valores internacionales y locales, lo que sugiere la necesidad de identificar la referencia que permita garantizar el adecuado diagnóstico nutricional.<hr/>Abstract To examine discrepancies in the anthropometric classifications of at term newborns using different reference values, 102 newborns were studied, from a public Maternity Hospital in Valencia Venezuela (1998-2000). Weight, height, and mid-upper arm circumference were assessed. "Weight for gestational age" was used as an indicator for anthropometric classification, using the Venezuelan reference of Henríquez; as well as Lubchenco’s and William’s, this last recommended by WHO. The Capurro method was used to assess gestational age, including in the study only those infants born at 40 weeks. Newborns, whose birth weight was £ the 10th percentile of the different reference values, were considered small for gestational age (SGA). Those with birth weight ³ the 90th percentile were classified as large for gestational age (LGA). Kappa coefficient was used to determine which of the references showed the best agreement. The SGA newborn prevalence was higher when using William’s reference (25.5%), followed by Lubchenco’s (16.7%), and Henríquez’s with the lowest prevalence (13.7%). The best agreement was between Henríquez and Lubchenco (Kappa= 0.71). Opposite results were found for LGA newborns. This study showed discrepancies in the newborns’ anthropometric classification when using local and international reference values, suggesting the need to identify the reference that provides an adequate nutritional diagnosis. <![CDATA[<b>Relación del peso al nacer con la edad gestacional y la antropometría materna</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-07522006000100003&lng=es&nrm=iso&tlng=es Para establecer la relación entre el peso al nacer con la edad gestacional y con las siguientes variables antropométricas maternas: peso preconcepcional, IMC preconcepcional, ganancia de peso gestacional y talla, se evaluaron 1027 madres y sus recién nacidos, producto de embarazos simples, sin patología asociada, de hábitos saludables, estrato socioeconómico IV. Se calcularon descriptivos básicos, coeficiente de correlación de Pearson, estadístico Chi Cuadrado, test de Levene, ANOVA, Scheffé y se construyeron modelos de regresión múltiple y logística. El 65,3% de las gestantes eran eutróficas según índice de masa corporal preconcepcional, 61,9% de los recién nacidos fue "peso adecuado", 33,5% "peso deficitario" y 4,6% "peso bajo". Todas las correlaciones entre el peso al nacer con las variables antropométricas y la edad gestacional resultaron positivas y significativas. Hubo diferencia estadísticamente significativa de los valores de peso e IMC preconcepcional, peso al final del embarazo y edad gestacional entre los peso bajo vs. peso adecuado y peso deficitario vs. peso adecuado. Los modelos de regresión múltiple construidos no resultaron suficientes para predecir con precisión el peso al nacer. Las variables independientes del modelo de regresión logística con valor predictivo para peso deficitario fueron el peso preconcepcional materno (OR=1,04), el incremento ponderal (OR=1,07) y la edad gestacional (OR=1,20). Los indicadores antropométricos del estado nutricional materno resultaron indicadores predictivos y efectivos del peso deficitario al nacer, no así para el peso bajo. Se recomienda desarrollar modelos de regresión múltiple que analicen el poder predictivo de otros indicadores del estado nutricional materno o de otras variables maternas y fetales.<hr/>Abstract The objective was to determine the relationship between birth weight and gestational age and the following anthropometric variables: pre-pregnancy weight, pre-pregnancy body mass index, weight gain, and height. The sample consisted of 1,027 healthy mothers, of socioeconomic stratum IV, with newborns resulting from single pregnancies. The descriptive standards, Pearson correlation test, Levene test, ANOVA, and Scheffé were calculated. Multiple and logistic regression models were constructed. 65.3% of the pregnant women were well-nourished, according to pre-pregnancy BMI. 61.9%, 33.5% and 4.6% of newborns had appropriate, deficient, and low weight at birth, respectively. All the correlations between weight at birth and the mothers’ anthropometric variables were positive and significant. There was a significant difference of the values of pre-pregnancy weight, pre-pregnancy BMI, weight at the end of pregnancy and gestational age between low weight vs. appropriate weight at birth, and deficient weight vs. appropriate weight at birth. The multiple linear models constructed were not enough to accurately predict the weight at birth. The independent variables of the logistic regression model with a predictive value for deficient weight at birth were pre-pregnancy weight (OR = 1.04), weight increase (OR = 1.07) and gestational age (OR = 1.20). The anthropometric indicators of the maternal nutritional condition resulted effective and predictive of deficient weight at birth, but not of low weight. We recommend the development of multiple regression models that are able to analyze the predictive capability of other maternal nutritional status indicators, or of other maternal and fetal variables. <![CDATA[Variantes esqueléticas menores en niños y adolescentes de estratos bajos de Caracas]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-07522006000100004&lng=es&nrm=iso&tlng=es Se consideran variantes esqueléticas menores (VEM) a las pseudoepífisis (PE), braquimesofalanges (BMP), epífisis en cono (EC), epífisis supernumerarias (ES) y la fusión ósea (FO). El objetivo de esta investigación fue evaluar en 3.596 radiografías de la mano y muñeca izquierda de un grupo de niños y adolescentes entre 2 a 15 años, la presencia de VEM analizando la frecuencia de éstas y su asociación (chi-cuadrado) con  el  sexo,  edad cronológica, edad gestacional, estado nutricional al nacer, la presencia de  déficit primarios del crecimiento y con maduración ósea. Se encontró que la prevalencia general de VEM fue de 27,7%; PE: 23,9% más frecuentes en varones; EC: 3,3% más frecuentes en niñas; BMF: 3,1%; ES: 0,08% y FO: 0,2%.  Sólo 2,5% de los sujetos con VEM presentaron algún déficit primario de crecimiento. No se encontró asociación entre VEM y prematuridad, talla baja al nacer, estadio de maduración ósea, aunque si entre pequeño simétrico para la edad gestacional  con PE, BMF y EC, y retardo del crecimiento intrauterino con PE, BMF. La prevalencia de las VEM encontradas corresponde fundamentalmente a población sin déficit primario de crecimiento de comienzo prenatal y se encuentra dentro de los rangos que reporta la literatura internacional, así como también, la frecuencia de las localizaciones y las tendencias en uno u otro sexo. En aquellos niños con retardo del  de crecimiento al nacer, se encontró asociación con PE, BMF y EC.<hr/>Abstract The frequency of Minor Skeletal Variants (MSV): pseudo-epiphysis (PSE), cone-shaped epiphysis (CSE), brachymesophalangy (BMP), supernumerary epiphysis (SE), and bone fusion (BF) was analyzed in 3,596 subjects between 2-15 years of age by means of a radiological study of left hands and wrists according to sex, age, and the location and presence of growth primary deficit. Chi-square relation was established between the MSV at gestational age, nutritional status at birth and skeletal maturation status. The MSV prevalence found was 27.6%; PSE: 23.9% more frequent in boys; CSE: 3.3% more frequent in girls; BMP: 3.1%; SE: 0.08% and BF: 0.2%. Only 2.5% showed some growth primary deficit. No relation was found between prematurity with MSV, low stature at birth, epiphysis maturation status; however, some relation was found between small symmetric for gestational age with PSE, BMP, and CSE and intrauterine retarded growth with PSE and BMP. The prevalence of the different MSV found in this study corresponds to children and adolescents without primary growth failure of prenatal onset and are within the ranges reported in international literature, as well as the location and trend frequencies for either one or the other sex. In the children with intrauterine retarded growth an association was found with PSE, BMP and CSE. <![CDATA[Susceptibilidad a la oxidación de las lipoproteínas de baja y muy baja densidad del plasma en escolares]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-07522006000100005&lng=es&nrm=iso&tlng=es La modificación oxidativa de las lipoproteínas plasmáticas es un paso importante en la formación de la placa de ateroma. Por otra parte, existen evidencias de que la aterogénesis comienza durante etapas tempranas de la vida, por lo que es de interés evaluar la susceptibilidad al estrés oxidativo de las lipoproteínas del plasma en la infancia. Se estudiaron 24 niños escolares aparentemente sanos (7 a 9 años de edad)   que asistieron a unidades educativas de los Estados. Carabobo y Lara (Venezuela) durante el periodo escolar 2003-2004. La VLDL y  LDL del plasma se sometieron a oxidación  in vitro con cobre y se midió la concentración de las sustancias reactivas al ácido tiobarbitúrico (TBARS) luego de tres horas de incubación.  La susceptibilidad a la oxidación se relacionó a variables antropométricas (peso-talla, talla-edad, área grasa), hematológicas (hemoglobina y hematocrito) y bioquímicas (concentración plasmática de colesterol total, LDL-c, HDL-c, triglicéridos y ferritina).  Bajo las condiciones de nuestro experimento, 50% de los niños estudiados presentó oxidación de la LDL, 62,5% oxidación de la VLDL y 33% oxidación de ambas lipoproteínas. La susceptibilidad a la oxidación no se asoció a  las variables antropométricas y hematológicas estudiadas.  La oxidación de la LDL se correlacionó negativamente con la concentración de ferritina plasmática (r= -0,61, p<0,05). Esto último  contrasta con el rol del hierro como agente prooxidante. Nosotros proponemos que posiblemente los niños ferropénicos presentan otros déficit nutricionales que condicionan una baja reserva antioxidante, probablemente debido al consumo de dietas bajas en antioxidantes y/o cofactores de enzimas antioxidantes.<hr/>Abstract The oxidative modification of plasma lipoproteins is an important step in the formation of the atherosclerotic plaque. On the other hand, there are evidences that atherogenesis begins during early stages of life, therefore it is of interest to evaluate the susceptibility to oxidative stress of plasma lipoproteins in childhood. Twenty-four children apparently healthy (7 to 9 years of age) who attended to elementary schools in Carabobo and Lara States (Venezuela) during the school period 2003-2004 were studied. Plasma VLDL and LDL were submitted to oxidation in vitro with copper and concentrations of the resulting thiobarbituric acid reacting substances (TBARS) were determinated after three hours of incubation. The susceptibility to oxidation was related to anthropometrics (weigh-height, height-age, fat area), haematologics (hemoglobin and hematocrit) and biochemicals variables (plasmatic concentration of total cholesterol, LDL-c, HDL-c, triglycerides and ferritin).  Under the conditions of our experiment, 50% of the studied children displayed oxidation of LDL, 62.5% oxidation of VLDL and 33% oxidation of both lipoproteins. The susceptibility to oxidation was not associated to anthropometrics and haematologic variables.  The oxidation of LDL was negatively correlated with concentration of plasmatic ferritin (r = -0.61, p<0.05). This last one contrasts with the role of  iron as a prooxidant agent. We propose that possibly ferropenics children present other nutritional deficits that condition a low antioxidant reserve, probably due to the intake of  diets low in antioxidants and/or cofactors of antioxidant enzymes. <![CDATA[<b>Síndrome de comedor nocturno</b>: <b>un nuevo trastorno de la conducta alimentaria</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-07522006000100006&lng=es&nrm=iso&tlng=es La anorexia y  bulimia son los clásicos Trastornos de la Conducta Alimentaria, debido a las alteraciones patológicas  del comportamiento de comer, como ayunos, dietas extremas o ingesta compulsivas de alimentos. El síndrome de comedor nocturno, es una entidad que cursa con estas alteraciones sumadas a variaciones en los patrones del sueño. Los estudios demuestran que a mayor Indice de Masa Corporal mayor incidencia del mismo, por lo que se está considerando la posibilidad de incluir la obesidad como un desorden de alimentación. El síndrome se caracteriza por presentar: anorexia matutina, hiperfagia nocturna e insomnio; con  modificación  del patrón neuroendocrino (Eje Hipófisis Pituitario Adrenal, cortisol, melatonina y leptina) el cual  tiene función reguladora con ritmos circadianos propios que modulan diversas funciones metabólicas y psicológicas.  La exacerbación de la sintomatología coincide con períodos de ganancia de peso y episodios de vida estresante. Los tipos de comedor nocturno son: Comedor compulsivo de tarde y noche,  nocturno ansioso, por antojos y el que tiene la creencia del todo o nada sobre el comedor nocturno. El diagnostico diferencial: la bulimia,  el comedor compulsivo y el desorden del sueño relacionado con la ingesta nocturna. Todo esto es relevante considerarlo en la evaluación nutricional del  paciente que asiste a la consulta, ya que es bien conocido el fracaso de los tratamientos   para la obesidad a corto o largo plazo. por esta razón el objetivo de la revisión es presentar las características del Síndrome del Comedor Nocturno a fin de diagnosticarlo y obtener los resultados esperados del tratamiento.<hr/>Abstract Bulimia and Anorexia are the classic eating disorders due to pathologic alterations of eating behavior like fasting, extreme dieting or compulsive eating. The night eating syndrome (NES) is an entity that characterizes itself by these alterations added to variations in the sleeping pattern. Several studies show the fact, that at a higher body mass index, a higher incidence of the syndrome, one of the reasons to consider obesity as an eating disorder. The three main signs of NES are: morning anorexia, nocturnal overeating and insomnia with alteration of the neuroendocrine pattern (Hyphotalamic-pituitary-adrenal axis, cortisol, melatonin, leptin) which has a principal role modulating some metabolic and psychological functions. The exacerbation of the symptoms coincides with weight gaining periods and stress episodes. There are four types of night eaters: the compulsive evening and nightime overeater, the anxious/ agitated night eater, the cravings night eater, the all or nothing belief about sleep night eater. The differential diagnoses are bulimia, compulsive eater and nocturnal sleep-related eating disorder. All of the above are relevant to consider in the nutrition assessment of the patient, specially because of the failure in the short and long treatment of the obese patient.
