Informes recientes han demostrado un aumento de la prevalencia de carbapenemasas en distintos géneros de enterobacterias y más recientemente de una nueva metaloenzima denominada New-Delhi metalobetalactamasa (NDM). Se presentan los dos primeros casos de Escherichia coli productoras de metaloenzimas tipo NDM en el Hospital Dr. Raúl Leoni Otero de Ciudad Guayana en Venezuela, en pacientes hospitalizados en los servicios de traumatología y cirugía. Las cepas fueron aisladas de secreciones en los sitios de lesión y fueron resistentes a todos los antibióticos excepto tigeciclina. Por ensayos fenotípicos se evidenció la producción de metaloenzima, la cual fue caracterizada por PCR como carbapenemasa tipo NDM. En uno de los aislados se detectó la coexistencia de la betalactamasa CTX-M-1. Es necesario reforzar estrategias para prevenir y controlar la aparición y propagación de microorganismos resistentes a los antimicrobianos en los hospitales.<hr/>The emergence of carbapenem-resistant Enterobacteria, as a result of the acquisition of genes encoding carbapenemase is an alarming global problem, associated with high mortality rates, big levels of resistance to other antimicrobial and enormous spreading potential. Recent reports have shown an increased prevalence of carbapenemases in different enterobacterial genus and more recently a new type named New Delhi metalloenzyme (NDM) was characterized. The first two isolates of Escherichia coli producing NDM type metalloenzymes were recovered by culture of wound secretions from patients hospitalized in the Trauma and Surgical wards at Dr. Raul Leoni Hospital in Ciudad Guayana, Venezuela. The two strains were resistant to all antibiotics exception made to tigecycline. Phenotypic assays showed metalloenzyme production, which was characterized by PCR as NDM-type carbapenemase. In one of the isolates, coexistence of NDM-type carbapenemase with CTX-M-1 beta-lactamase was detected. It is necessary to strengthen strategies to prevent and control the emergence and spread of antimicrobial resistant microorganisms in hospitals. <![CDATA[<b>Vaginosis citolítica en pacientes con diagnóstico clínico de vulvovaginitis candidiásica</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562016000200004&lng=es&nrm=iso&tlng=es La microbiota vaginal en la edad fértil está dominada por Lactobacillus spp. Su sobrecrecimiento provoca el aumento en la producción de ácido láctico, disminución del pH vaginal y lisis de células del epitelio vaginal, causando vaginosis citolítica, la cual se confunde con vulvovaginitis candidiásica (VVC) por la aparición de signos y síntomas similares. Se determinó la prevalencia de vaginosis citolítica, así como sus características clínicas y epidemiológicas en pacientes que consultaron con clínica sugestiva de VVC durante el período agosto-septiembre del año 2015. Se encontraron 12 pacientes con clínica compatible de VVC. En 11 (91,7%) se realizó el diagnóstico de vaginosis citolítica y en 1 (8,3%) de vulvovaginitis candidiásica. El 63,7% de las pacientes (7) tenían diagnóstico previo de VVC recurrente. Los síntomas más frecuentes fueron prurito vaginal o vulvar y secreción vaginal blanquecina de aspecto grumoso como leche cortada. La clínica iniciaba entre los 4-6 días posteriores a la menstruación, y desaparecía días antes o justo con el inicio de una nueva fase menstrual. En conclusión, se encontró que en las pacientes examinadas la sintomatología de ambas patologías resultó indistinguible, por tanto el diagnóstico clínico no es suficiente y debe recurrirse al diagnóstico de laboratorio para distinguir entre vaginosis citolítica y vulvovaginitis candidiásica.<hr/>The vaginal microbiota in fertile age is dominated by Lactobacillus spp. Its overgrowth causes increased production of lactic acid, decreased vaginal pH and lysis of cells of the vaginal epithelium, causing cytolytic vaginosis, which can be confused with vulvovaginal candidiasis (VVC) due to the similar signs and symptoms. The prevalence of cytolytic vaginosis, as well as its clinical and epidemiological characteristics was determined in patients who consulted with suggestive signs and symptoms of VVC during the period August to September of the year 2015. Twelve patients with signs and symptoms compatible with VVC were considered. Of those, 11 (91.7%) were diagnosed as cytolytic vaginosis and 1 (8.3%) corresponded to vulvovaginal candidiasis. From the 12 patients included, 7 (63.7%) had previous diagnosis of recurrent VVC. The most frequent symptoms recorded were vaginal or vulvar pruritus and whitish, cheesy or curd-like vaginal discharge. Signs and symptoms started between 4 to 6 days after menstruation, and disappeared days before or just with the onset of a new menstrual cycle. In conclusion, it was found that in the patients examined, signs and symptoms of both pathologies were indistinguishable. Therefore, the clinical diagnosis is not enough and laboratory studies should be used to distinguish between cytolytic vaginosis and vulvovaginal candidiasis. <![CDATA[<b>Fusión intergénica de protoplastos de <i>Saccharomyces cerevisiae</i> y <i>Hanseniaspora guillermondii</i></b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562016000200005&lng=es&nrm=iso&tlng=es La fusión de protoplastos ha facilitado la obtención de nuevas cepas de levaduras con propiedades biotecnológicas muy interesantes. El principal objetivo de esta investigación fue obtener una levadura híbrida intergénica con potencialidades enológicas características de dos géneros diferentes. Para ello se fusionaron protoplastos de Saccharomyces cerevisiae autóctona de la región zuliana con Hanseniaspora guillermondii CECT 11102 de origen comercial. Saccharomyces es una levadura que produce altas concentraciones de etanol pero el perfil aromático es sencillo y común. Hanseniaspora no resiste las concentraciones de etanol, pero puede generar aromas agradables e intensos. La identificación de las levaduras antes y después de la fusión de protoplastos se realizó con la técnica PCR-RFLP del gen 5.8S rADN y las regiones intergénicas adyacentes ITS1 e ITS2 del ADN extraído, sometiendo los productos amplificados a un análisis de restricción con las enzimas HinfI, HaeIII, CfoI y DdeI. El polietilenglicol fue usado para inducir la fusión de protoplastos. La cepa híbrida presentó características de ambas levaduras parentales debido a que resistió altas concentraciones de etanol como S. cerevisiae y fue capaz de metabolizar el salicín como H. guillermondii. El análisis molecular PCR-RFLP de la levadura híbrida mostró un patrón de bandas diferente al de las levaduras parentales.<hr/>Protoplast fusion has facilitated the development of new yeast strains with very interesting biotechnological properties. The main objective of this research was to obtain hybrid yeast with potentialities of two different genera, that could be used in the wine manufacture. Saccharomyces cerevisiae protoplasts from the Zulian region were fused with Hanseniaspora guillermondii CECT 11102 of commercial origin. Saccharomyces is a yeast that produces high levels of ethanol but the aromatic profile is simple and common. Hanseniaspora does not withstand ethanol concentrations, but it can generate pleasant and intense aromas. Identification of yeast before and after the fusion of protoplasts was performed using the PCR-RFLP technique of the 5.8S rDNA gene and the adjacent ITS1 and ITS2 intergenic regions of the extracted DNA, subjecting the amplified products to a restriction with the enzymes HinfI, HaeIII, CfoI and DdeI. Polyethylene glycol was used to induce fusion of protoplasts. The hybrid strain showed characteristics of both parental yeasts because it resisted high concentrations of ethanol as S. cerevisiae and was able to metabolize the salicin as H. guillermondii. PCR-RFLP molecular analysis of hybrid yeast showed a different band pattern than those pertaining to parental yeasts. <![CDATA[<b>Especies de <i>Candida</i> asociadas a lesiones bucales en pacientes con diabetes tipo 2</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562016000200006&lng=es&nrm=iso&tlng=es La candidiasis es una enfermedad infecciosa provocada por levaduras del género Candida, principalmente por C. albicans. En la cavidad bucal del paciente diabético convergen condiciones fisiológicas que propician el desarrollo de candidiasis. Sin embargo, son escasos los estudios microbiológicos que involucren a otras especies de Candida como posibles agentes causales. El objetivo del trabajo fue determinar la frecuencia de las especies de Candida asociadas a lesiones bucales en pacientes con diabetes tipo 2, que acudieron al Servicio de Endocrinología del Instituto Autónomo Hospital Universitario de Los Andes del estado Mérida, durante el segundo trimestre del 2015. Se realizó la evaluación clínico-estomatológica con el respectivo levantamiento de la ficha clínica. La recolección de la muestra de la mucosa bucal fue realizada mediante un raspado con una espátula 7A estéril, realizándose examen directo al fresco y cultivo. Las levaduras recuperadas fueron identificadas utilizando HiChrome Candida Differential agar y agar harina de maíz. De 172 pacientes examinados, solo 59 presentaron lesiones sugestivas de candidiasis, obteniéndose 17 muestras positivas para Candida spp., donde 15 correspondieron a C. albicans y 2 a C. glabrata. Los resultados coinciden con los reportados en la literatura mundial donde C. albicans es la principal especie causante de candidiasis bucal en pacientes con diabetes tipo 2.<hr/>Candidiasis is an infectious disease caused by yeasts of the genus Candida, mainly C. albicans. In the oral cavity of the diabetic patient, physiological conditions that promote the development of candidiasis converge. However, there are few microbiological studies involving other Candida species as potential causative agents. The objective of this study was to determine the frequency of Candida species associated with oral lesions in patients with type 2 diabetes who attended the Endocrinology Service of the Instituto Autónomo Hospital Universitario of Los Andes University of the State of Mérida, during the second quarter of 2015. Clinical record and oral cavity examination was performed. Specimen from the oral mucosa was collected by scraping with a sterile 7A spatula for wet direct examination and culture. The recovered yeasts were identified using HiChrome Candida differential agar and corn flour agar. Of the 172 patients examined, only 59 presented lesions suggestive of oral candidiasis, obtaining 17 samples positive for Candida spp. Of which 15 corresponded to C. albicans and 2 to C. glabrata. The results coincide with those reported in the world literature where C. albicans is the main species responsible for oral candidiasis in patients with type 2 diabetes. <![CDATA[<b>Mutaciones K130M y V131I del gen X en aislados venezolanos del virus de hepatitis B</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562016000200007&lng=es&nrm=iso&tlng=es Aproximadamente el 50% de los carcinomas hepatocelulares (CHC) en el mundo están etiológicamente asociados con la infección por el virus de hepatitis B (VHB). Se han descrito 10 genotipos del VHB (A-J). En Venezuela y en varios países latinoamericanos predomina el genotipo F. Las mutaciones K130M y V131I presentes en la proteína HBx del VHB han sido asociadas al desarrollo del CHC. El objetivo de este trabajo fue estudiar la variabilidad genética de la proteína HBx del VHB circulante en pacientes venezolanos, con el fin de correlacionar estas mutaciones con los parámetros clínicos y virológicos de la enfermedad. Se analizó la secuencia del gen X del VHB, mediante amplificación por PCR de un fragmento de ese gen, en 45 pacientes infectados (35 crónicos y 10 agudos). Se observó una mayor frecuencia de las mutaciones K130M y V131I en pacientes de 25 o más años y con infección crónica. La presencia de estas mutaciones fue significativamente menor en el subgenotipo F3, comparado con el genotipo C. Estos resultados refuerzan la hipótesis de que el subgenotipo F3, predominante en Venezuela, podría estar asociado a una progresión menos severa de la enfermedad que la descrita para otros subgenotipos americanos, como F1b o F2.<hr/>Approximately 50% of the hepatocellular carcinomas (HCC) in the world are etiologically associated to hepatitis B virus (HBV) infection. Ten HBV genotypes (A-J) have been described in Venezuela and in other Latin American countries where the F genotype predominates. The K130M and V131I mutations present in the HBx protein of HBV have been associated with the development of HCC. The aim of this work was to study the genetic variability of HBx protein from HBV circulating in Venezuelan patients, in order to correlate these mutations with clinical and virus factors involved in the disease. The X HBV gene sequence was analyzed by PCR amplification of that gene in 45 infected patients (35 with chronic and 10 with acute stages of hepatitis). A higher frequency K130M and V131I mutations was observed in subjects 25 years of age and older with chronic infection. The presence of these mutations was significantly lower in the F3 subgenotype compared with genotype C. These results support the hypothesis that the F3 subgenotype, predominant in Venezuela, could be associated with a less severe progression of the disease than that described for other American subgenotypes, such as F1b or F2. <![CDATA[<b>Vaginosis citolítica</b>: <b>presentación de un caso clínico</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562016000200008&lng=es&nrm=iso&tlng=es La vaginosis citolítica, descrita hace 30 años como citólisis de Döderlein, es frecuente en mujeres en la edad reproductiva y, por las características de flujo vaginal blanquecino y síntomas clínicos, es indistinguible de la vulvovaginitis micótica. Se presenta el caso de una paciente, con diagnóstico clínico presuntivo de vulvovaginitis por Candida spp. a repetición, tratada empíricamente con antifúngicos por año y medio sin ninguna mejoría. Luego de estudios microbiológicos, la coloración de Gram, demostró la presencia de 50 bacilos grampositivos por campo y abundantes núcleos celulares desnudos. El cultivo resultó puro para Lactobacillus spp., lo que permitió confirmar el diagnóstico de vaginosis citolítica. La paciente fue tratada con ampicilina-sulbactam y no ha vuelto a presentar recidivas. En conclusión, es fundamental determinar el pH vaginal de las pacientes en la consulta, así como practicar una coloración de Gram de la secreción vaginal para poner en evidencia los cambios celulares por el exceso de ácido en la vagina y así evitar tratamientos antifúngicos innecesarios que acrecentarán los trastornos de la microbiota vaginal.<hr/>Cytolytic vaginosis, described 30 years ago as Döderlein cytolysis, is common in women of reproductive age and, due to the characteristics of whitish vaginal discharge and clinical symptoms, is indistinguishable from mycotic vulvovaginitis. We describe the case of a patient with presumptive clinical diagnosis of recurrent vulvovaginitis by Candida spp. treated empirically with antifungal agents for one and a half years without improvement. After microbiological studies, Gram staining demonstrated the presence of 50 Gram-positive bacilli per field and abundant nude cell nuclei. The culture recovered pure Lactobacillus spp. which permitted the diagnosis of cytolytic vaginosis. The patient was treated with ampicillin-sulbactam and since, has not had recurrences.
