The purpose of the present work was to determine the manometric changes that are produced in the low esophageal sphincter after total and partial laparoscopic fundoplications in patients with gastroesophageal reflux disease, achalasia, and giant paraesophageal hiatal hernia; and to establish the relation of this changes with the disease control and postoperative complications. Among 42 patients operated 28 are included in the study (5 Nissen, 19 Toupet and 4 Dor) in which an esophageal manometry was practiced at 8 weeks of posoperative period. In the three kinds of fundoplications statistically significant elevations of low esophageal sphincter pressures was observed (Wilcoxon test), however there weren’t differences between the groups. All the patients with Nissen fundoplications had postoperative disfagia, and this didn’t occur in the cases with partial fundoplications. One patient with a Toupet fundoplication presented recurrence of the symptoms due to slipped wrap toward the mediastinum at two years of the operation. This required a new antireflux surgery practiced successfully by laparoscopy. All the remaining patients have reflux control without medication. We concluded that total and partial fundoplications produced enough pressure elevations in the low esophageal sphincter to control the reflux, however the first ones had the handicap of postoperative disfagia, reason why we recommend the Toupet procedure as the main option in patients with gastroesophageal reflux disease.
A study of 13 patients with thyroid patology was carried out, to which thyroidectomy subtotal or lobectomy were practiced in the Service of General Surgery of the Hospital Risquez of Caracas during the year 2000. The objective was to locate anatomically, to identify histologically and to preserve the parathyroid glands during the thyroidectomy. A total of 27 glands, were located: 25 (92.6 %) in its normal anatomical position, while 2 (7.4 %) they weren’t. Per-operative biopsy was practiced to the 27 parathyroids, being these identified histologically in 100 % of the cases. We concludes that the location of the parathyroid glands for the surgeon during the thyroidectomy is of great importance, as well as being familiarized with the macroscopic characteristics, since in this way they can be preserved during the surgical act to avoid severe complications.
The heart weight is the most important mass measure as it constitutes the indispensable element to diagnose heart hypertrophy, risk factor for acute cardiovascular events. At the moment we do not have normal values for the national heart weigths and we use data from other ethnic groups as a reference. Our goal was to analyse the weight in a sample of 141 macro and microscopical "normal" hearts, from the Forensic medical service of the Metropolitan area. The estimated linear equation with 33 hearts’ weights and the age from people under 16 years old was 27+10.6x. On the 108 hearts from people between 16 and 80 years old, 95 % confidence limits vary from 276.2 g to 294.4 g and a general (both genders) mean weight of 285.7 g. For males the mean heart weight was 294.4 g, 36 g more than the female mean heart weight (258.4 g). This difference reach statistical significance (P< 0.01). Adult hearts weights were normally distributed, according to the goodness of fit test (P= 0.83) and the asimetry and kurtosis values. No matter the limitations we had, our results are similar to those found by foreign authors. Concepts as critical, physiologic and pathologic hypertrophy are discuss and also factors which influence upon the increase of the heart weight, as obesity, myocardial infiltrations and other changes proper to the old aged.
Background: the atrial fibrillation is characterized in the electrocardiogram by absence of wave P, presence of wave f and variation in the intervals R-R. Objectives: to register and to compare the differences of voltage of the complex R-R in the atrial fibrillation as in sinoatrial rhythm. Methods: 21 electrocardiograms were evaluated with atrial fibrillation and 23 with sinoatrial rhythm, registering the voltage of the waves R, the depth of the waves Q and the sum of R with Q. Results: Of the patients with sinoatrial rhythm 56.5 % was female and the age average was of 50.7 ± 21.2 years. Of the patients with atrial fibrillation 81,0 % was male and the age average was of 67.4 ±13.4 years. For wave R, as for wave Q and their additions in all the derivations, excepted derivation D2 of the wave R (P= 0.095), it was found statistically significant differences (P= 0.02 or less) among the electrocardiograms of sinoatrial rhythm and atrial fibrillation. Conclusions: a voltage difference exists among the registered complexes in the electrocardiogram between sinoatrial rhythm and atrial fibrillation.
