MÉTODO: 22 pacientes con cáncer epitelial avanzado de ovario fueron elegidas en base a los criterios de Vergote: enfermedad voluminosa, implantes múltiples, enfermedad metastásica parenquimatosa; se administró 3 ó 4 ciclos de quimioterapia con carboplatinotaxanos, y posterior cirugía. RESULTADOS: Todas las pacientes fueron catalogadas como estadio III-C o IV según la clasificación de la FIGO. El tipo histológico predominante fue el adenocarcinoma sin especificación con el 45,5 %. El 63,6 % eran tumores poco diferenciados. Todas las pacientes respondieron a la quimioterapia neoadyuvante, incluso en 5 pacientes se reportó respuesta patológica total. Se logró cirugía cito- reductora óptima en 81,8 % de los pacientes. Para un seguimiento promedio de 18 meses: 20 pacientes están vivas (90,9 %), 14 libres de enfermedad. En 4 se produjo recidiva, 4 presentaron persistencia y fallecieron 2 de ellas. La morbilidad perioperatoria fue mínima. La sobrevida libre de enfermedad registró una media de 11,45 meses. El único factor relacionado con sobrevida libre de enfermedad fue la capacidad de realizar cirugía óptima (P= 0,006). CONCLUSIONES: La cirugía citorreductora primaria sigue siendo la primera opción en cáncer de ovario epitelial avanzado, la quimioterapia neoadyuvante seguida de cirugía de intervalo es una opción válida en un subgrupo de pacientes con factores de riesgo adversos.<hr/>SUMMARY OBJECTIVE: To determine the feasibility of optimal cytoreduction after neoadjuvant chemotherapy in epithelia advanced ovarian cancer and relation it with the global survival and disease free survival. METHOD: 22 patients with advanced epithelial cancer of ovary selected according Vergote´s indications: bulky disease, multiply implants, and parenchyma disease metastasis, it administrated 3 or 4 cycles of chemotherapy with carboplatinumtaxans and after interval surgery. The statistical analysis was based on porcentual relations, variable analysis and Pearson variable correlation test. RESULTS: All the patients were catalogued as stage III-C or IV by FIGO classification. Adenocarcinoma without specification was the most important histological subtype with the (45.5 %). 63.6 % were poor differentiation tumors. All the patients responded to neoadyuvant chemotherapy, even there was a complete pathological response in five. Optimal cytoreduction surgery was achieved in 81.8 % of the patients. The median in follow up was 18 months, 20 patients are lived (90.9 %) and 14 without disease. In 4 patients had persistence of the disease, 2 of them died because of these, in 4 patients recurrent disease was described, Disease free survival registers a media of 11.45 months. The unique factor relationated with disease free survival was the capacity to realize optimal cytoreductive surgery (P= 0.006). CONCLUSIONS: The primary cytoreductive surgery is still the gold standard in treatment of advanced epithelian ovarian carcinoma, neoadjuvant chemotherapy and interval debulky surgery is a valid option in a subgroup of patients with adverse prognostic factors. <![CDATA[<b>Carcinoma del tiroides</b>: <b>Resultado del tratamiento quirúrgico</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300003&lng=pt&nrm=iso&tlng=pt OBJETIVOS: Analizar la experiencia en el tratamiento quirúrgico del carcinoma tiroideo. MÉTODOS: Se evaluaron 85 pacientes con carcinoma de tiroides tratados en el Servicio de Cirugía General Nº 1, del Hospital Universitario Miguel Pérez Carreño, entre octubre de 1970 y 2003. Se analizaron: la clínica, diagnóstico, tratamiento, complicaciones y seguimiento. La información se procesó con el paquete estadístico BMDP. Los resultados se expresan en: distribución de frecuencia, estadísticas descriptivas. Se utilizó el Chi-cuadrado; la prueba "t" para valorar diferencias entre promedios. La sobrevida con la prueba de Mantel-Cox, valor de P= 0,05. RESULTADOS: El diagnóstico histológico fue papilar en el 64,70 %, folicular en 17,64 %, medular en el 10,58 %, indiferenciado en el 7,05 %. En 76 pacientes, un nódulo palpable fue el único signo de enfermedad. Edad promedio fue 48 años. La variedad papilar se presentó en pacientes más jóvenes que el folicular (P= 0,03). Prevaleció en el sexo femenino en las menores de 49 años (3,6:1) comparado con (2,3:1) en las pacientes > de 50 años. La gammagrafía reveló un 74 % de tumores no captantes. La ultrasonografía demostró un nódulo solitario en el 96 % de los casos. La tiroidectomía total fue realizada en el 82 %, hemitiroidectomía o tiroidectomía subtotal en el 12 %, los restantes recibieron tratamiento paliativo. La recurrencia alcanzó un 6 %. CONCLUSIONES: Es necesario realizar estudios prospectivos aleatorizados para mejorar los conocimientos sobre la evolución y tratamiento del carcinoma del tiroides.<hr/>SUMMARY OBJECTIVES: To analyze the experience in the surgical treatment of the thyroid carcinoma. METHODS: We evaluated 85 patients with carcinoma of thyroids treated in the Service of General Surgery Nº 1 of the University Hospital Miguel Perez Carreño, between 1970 and 2003. They were analyzed: the clinic, diagnosis, treatment, complications and follow up. The information was processed with the BMDP statistical package. The results are expressed in: frequency allocation, descriptive statistics. Comparisons of groups were by the Chi-square test, and Student t- test was used to value differences between averages. The survival curves were compared by the Mantel-Cox test. RESULTS: The histological diagnoses were papillary 64.70 %, follicular 17.64 %, medullar 10.58 %, undifferentiated 7.05 %. In 76 patients a palpable neck mass was the only clinical presentation of disease. The mean age was 48 years. Patients with papillary carcinoma were younger than those with follicular (P= 0.03). The female predominance being even more pronounced among patients less than 50 years old (3.6/1) compared with (2.3/1) among patients above 50. Thyroid Gammagrama revealed 74 % of none captivate nodes. Ultrasound test demonstrated a solitary node in 96 % of the cases. A total thyroidectomy was performed in 82 % of the patients, hemithyroidectomy or subtotal thyroidectomy in 12 %, the rest received palliative treatment. The recurrence rate was 6 %. CONCLUSIONS: It is necessary randomized and prospective studies to improve the knowledge on the evolution and treatment of the carcinoma of the thyroid. <![CDATA[<b>Asociación de catepsina D con factores pronósticos en melanoma maligno </b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300004&lng=pt&nrm=iso&tlng=pt OBJETIVOS: Determinar la asociación de la catepsina D, como factor pronóstico inmunohistoquímico, para mejorar nuestro arsenal terapéutico. MÉTODOS: Se revisaron las historias clínicas de 23 pacientes con diagnóstico de melanoma, tratados en el Instituto de Oncología Dr. Miguel Pérez Carreño, en el lapso comprendido de enero de 1998 y diciembre de 2001, a los cuales, se les realizó la determinación de catepsina D en los bloques de parafina de las biopsias realizadas. RESULTADOS: El procedimiento de marcaje inmunohistoquímico de catepsina D se realizó en 23 pacientes según la técnica de revelado con aminoetilcarbazol. Se relacionaron los resultados con la presencia de factores pronósticos. Los pacientes con inmunomarcajes 41,17 % son Clark IV, 41,17 % Breslow > 3 mm, 70,58 % presentaron ulceración, 52,94 % eran estadio III. CONCLUSIONES: Se evidenció un paralelismo entre los factores histológicos y el inmunomarcaje con catepsina D, pero se debe determinar la evolución natural de aquellos pacientes en estadios iniciales para poder determinar su valor como factor pronóstico.<hr/>SUMMARY OBJECTIVES: To determine the association of cathepsin D, like immunehistochemical prognosis factor, in order to improve our therapeutic arsenal. METHODS: We reviewed the clinical histories of 23 patients with diagnosis of malignant melanoma treated in the Instituto de Oncología Dr. Miguel Perez Carreño, in the lapse of January of 1998 and December of 2001, to which, the determination of cathepsin D was made in the paraffin blocks of the made biopsies RESULTS: The inmunehistochemical mark procedure with cathepsin D was made in 23 patients according to the revealed technique of aminoetilcarbazol. The results were related to the presence of factors prognoses. The patients with positive inmunemarked 41.17 % are Clark IV, 41,17 % Breslow > 3 mm, 70,58 % presented ulceration, 52,94 % were stage III. CONCLUSIONS: A parallelism between the histological factors and the inmunemarked with cathepsin D was demonstrated, but the natural evolution of those patients in initial stages is due to determine to be able to determine its value like factor prognosis. <![CDATA[<b>Evolución del tratamiento con radioterapia convencional del cáncer de próstata desde 1984 hasta el 2000</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300005&lng=pt&nrm=iso&tlng=pt INTRODUCCIÓN: Es un trabajo retrospectivo, descriptivo de la evolución de los esquemas de tratamientos utilizados en carcinoma de próstata entre 1984 a 2000 en el Servicio de Radioterapia del HUC. Métodos: En un período de 16 años se revisaron 177 casos de carcinoma de próstata, reportándose estadio, esquemas de tratamiento, dosis total, fracción y tipo de máquina. Resultados: Entre 1984 y 2000, el 51,41 % de los pacientes pertenecían al estadio D, el 24,86 % al estadio C, 23,16 % al estadio B y 0,56 % al estadio A. El 42,85 % de los tratamientos aplicados fueron con fines paliativos y, 20,51 % con fines curativos, Se utilizó cobalto 60 en el 52,80 % de los tratamientos aplicados y 47,19 % en acelerador lineal 4Mv (AC 4Mv), la tomografía pélvica se utilizó para la planificación en el 100 % de los casos a partir de 1990. La irradiación de mamilas se usó sólo entre 1984-1989, la radioterapia radical a toda la pelvis hasta 4500 cGy y boost hasta 6600-6800 cGy se programó en el 25,08 % de los casos y, la radioterapia radical localizada hasta 6800-7000 cGy en un 20,51 %. El esquema de fraccionamiento paliativo fue 3000 cGy en 10 fracciones en 76,47 %. Conclusión: El Servicio de Radioterapia del HUC mantiene protocolos de tratamientos que han demostrado una excelente efectividad, correspondiéndose la inclusión de los métodos de estudio y la transformación del tratamiento.<hr/>SUMMARY INTRODUCTION: It is a retrospective and descriptive study about the evolution of the treatments schemes used in prostate carcinoma between 1984 and 2000 at the Radiotherapy Service of the HUC. Methods: In a period of 16 years, 177 cases of prostate carcinoma were reviewed, reporting stage, treatment schemes, total dose, fraction and type of radiotherapy machine. Results: Between 1984 and 2000, 51.41 % of the patients belonged to stage D, 24.86 % to stage C, 23.16 % to stage B and 0.56 % to stage A. 42.85 % of the applied treatments were with palliative aims and, 20.51 % with curative aims, It was used Cobalto 60 in the 52.80 % of the total treatments applicated and 47.19 % in 4Mv linear accelerator (AC 4Mv). The pelvic tomography was used for the treatment planning in 100 % of the cases since 1990. The irradiation of the mamilas was used only between 1984 and 1989. The radical radiotherapy to the whole pelvis up to 4500 cGy and boost until 6600-6800 cGy was programmed in 25.08 % of the cases and, the located radical radiotherapy up to 6800-7000 GY in 20.51 %. The scheme of palliative treatment was 3000 cGy in 10 fractions in 76.47 %. Conclusion: The Radiotherapy Service of the HUC keeps treatment protocols since 16 years and has proved an excellent effectivity agree with the inclusion of the study methods and the changes of treatments as it is signed for the clinic evidence and the international literature. <![CDATA[<b>Adenocarcinoma ductal infiltrante de mama en paciente masculino.</b> <b>A propósito de un caso</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300006&lng=pt&nrm=iso&tlng=pt INTRODUCCIÓN: Se presenta un paciente masculino, quien presentó un adenocarcinoma ductal infiltrante de mama, y quien fuera atendido en el Servicio de Cirugía 2, Servicio Autónomo Hospital Central de Maracay, Venezuela. CASO CLÍNICO: Paciente masculino de 57 años de edad quien inició su enfermedad actual 3 meses antes de su ingreso con nódulo de 1 cm de diámetro, retroereolar izquierda, no doloroso, el cual, tiene un crecimiento rápido hasta alcanzar 5 cm x 5 cm de diámetro. Posteriomente dicha lesión se ulcera, drenando exudado serohemático en moderada cantidad, motivo por el cual consulta y le realizan biopsia que reportó carcinoma ductal infiltrante. Se realizó mastectomía radical izquierda, evidenciando lesión ulcerada de 4 cm de diámetro que infiltra hasta aponeurosis del pectoral mayor. Además de plastrón ganglionar con ganglios de 1 cm de diámetro, de consistencia aumentada. DISCUSIÓN: Se presenta el caso debido a lo infrecuente de encontrar adenocarcinoma de mama en un hombre. Los pacientes con cáncer de mama pueden ser sometidos a procedimientos quirúrgicos que van desde el extremo conservador de una tumorectomía hasta el opuesto de una mastectomía radical ampliada, dependiendo del criterio o preferencia del cirujano.<hr/>SUMMARY INTRODUCTION: We present a masculine patient, who presented adenocarcinoma ductal infiltrante of breast, and who was attended in the Surgery Service 2, Autonomous Independent Service Central Hospital of Maracay, Venezuela. CLINICAL CASE: Masculine patient of 57 years old, who initiated his present disease 3 months before its entrance with nodule of 1 cm of diameter, at retroereolar left zone, non painful, which, it has a fast growth until reaching 5 cm x 5 cm of diameter. After this lesion was ulcerate, draining serohematic exuded in moderate amount, reason by which it, he consults and we made biopsy that report infiltrate ductal carcinoma. Left radical mastectomy was made, demonstrating ulcerate injury of 4 cm of diameter, that infiltrates until greater pectoral aponeurosis. In addition, fixed axillary’s lymph nodes with lymph nodes of 1 cm of diameter, of increased consistency. DISCUSION: We present this case due to the infrequent of finding an adenocarcinoma of breast in a man. The patients with breast cancer can be put under surgical procedures that go from the preservative end of a lumpectomy to the opposed one of a radical mastectomy, depending on the criterion or preference of the surgeon. <![CDATA[<b>Tumor neuroectodérmico primitivo (PNET) de la mama.</b> <b>Reporte de un caso </b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300007&lng=pt&nrm=iso&tlng=pt El tumor neuroectodérmico primitivo, se incluye entre las neoplasias de células redondas y azules, forma parte de la familia de tumores de Ewing. El tumor neuroectodérmico primitivo es uno de los tumores que pertenecen a la familia de neoplasias formadas por células redondas, pequeñas, indiferenciadas que presentan una expresión fenotípica neuroectodérmica. Aparece con mayor frecuencia en la segunda década de la vida. Tiene varias formas de presentación, la más frecuente es en hueso, seguida de partes blandas, cavidad abdominal, área torácico-pulmonar, columna vertebral y sistema nervioso central. Excepcionalmente, se han descrito casos en miocardio, vulva y meninges. El caso que presentamos a continuación, es el segundo caso reportado en la literatura mundial diagnosticado en la glándula mamaria, así como, la revisión de la literatura en relación con esta patología.<hr/>SUMMARY The primitive neuroectodermic tumor, is included among the blue and round cell’s neoplasm’s, it is a part of The Ewing Tumor’s Family. The primitive neuroectodermic tumor is one of the tumors that belong to the family of neoplasm’s form by round, small, undifferentiated cells, which present a neuroectodermic phenotypic expression. It is more frequently found during the second decade of life. It has several presentations, been the bones the more frequent place, followed by soft tissues, abdominal cavity, lung and thoracic area, spinal cord, and Central Nervous System. There have been exceptional cases in the myocardium, vulvae area and meninges. The case we are presenting here by is the second one reported world wide; diagnosed in the mammary gland, as well as the literature review related with to this pathology. <![CDATA[<b>Cáncer papilar primario en quiste del conducto tirogloso. A propósito de un caso</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300008&lng=pt&nrm=iso&tlng=pt El conducto tirogloso es una anomalía congénita resultado de la retención del tracto epitelial entre la tiroides, el foramen cecum y el piso de la faringe. La aparición de un carcinoma en el quiste del conducto tirogloso es rara, menor del 1 %. Hasta donde sabemos se han publicado 150 casos en la literatura internacional. Presentamos un caso de carcinoma papilar en un quiste del conducto tirogloso tratado en el Servicio de Cabeza y Cuello del Instituto de Oncología Dr. Miguel Pérez Carreño (centro de referencia nacional), el cual, representa el único caso tratado en nuestro Instituto en el lapso comprendido desde 1986 y el 2003, (17 años).<hr/>SUMMARY A thyroglossal duct cyst is a congenital anomaly resulting from retention of epithelial tract between the thyroid and its origin, the foramen cecum, at the floor of the pharynx. Carcinoma papillary originating in thyroglossal duct cyst is a rare event, minor of 1 %. As far as we know, only 150 cases of thyroglosal duct cyst carcinoma have been reported in the international literature. We present a case of papillary carcinoma of thyroglossal duct cyst treated in the Head and Neck Surgery Service of the Instituto Oncológico Dr. Miguel Pérez Carreño (national reference center), which, it represents the only case treated in our Institute in the laps included from 1986 and the 2003, (17 years). <![CDATA[<b>Pseudotumor inflamatorio del higado</b>: <b>en paciente con adenoma tubulo velloso del colon</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300009&lng=pt&nrm=iso&tlng=pt El pseudotumor inflamatorio del hígado es un tumor benigno de origen desconocido. En este trabajo se reporta el caso de un paciente masculino de 68 años de edad con malestar general, fiebre y dolor torácico. Se identificó un tumor hepático mediante estudios ecosonográficos y tomográficos. Asimismo se detectó un adenoma tubulo velloso en el colon.<hr/>SUMMARY Inflammatory pseudo tumor of the liver is a rare benign tumor of unknown origin. In this report we describe the case of a 68 year old male that presented with history of malaise, fever and chest pain. An hepatic mass was revealed by abdominal ultrasonography and computed tomography. A tubulovillous adenoma of the colon was identified by colonoscopy. <![CDATA[<b>Nuevas indicaciones de ganglio centinela en cáncer de mama</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822005000300010&lng=pt&nrm=iso&tlng=pt El pseudotumor inflamatorio del hígado es un tumor benigno de origen desconocido. En este trabajo se reporta el caso de un paciente masculino de 68 años de edad con malestar general, fiebre y dolor torácico. Se identificó un tumor hepático mediante estudios ecosonográficos y tomográficos. Asimismo se detectó un adenoma tubulo velloso en el colon.<hr/>SUMMARY Inflammatory pseudo tumor of the liver is a rare benign tumor of unknown origin.
