OBJECTIVE: Compare infiltrante lobulillar carcinoma and ductal carcinoma in long-term follow up as stage surgical treatment, neoadyuvante, adjuvant therapy. METHODS: we studied in CECLINES database 841 patients; lobulillar represents 7.25 % (61) and 51.24 % ductal (431). The overall follow up was up to twenty two years with an average of five years. RESULTS: The overall survival for lobulillar was 68.1 % and for ductal 60.9 % (P=0.772), the survival according to state was: I lobulillar 96.3 % ductal 90.5, II 94.4 % vs. 88.4 III 90.5 and 83.2 respectively (P=0.023). Estrogen receptors positive expression for lobulillar compared to ductal was 87.7 % vs. 74.7 %, (P=0.031) and its survival 96.9%vs. 94.0%(P=0.033). The breast conservingsurgery for lobulillar compared to ductal was 57.4 % vs. 63.2 % (P=0.949) survival 85.4 % vs. 82.9 % (P=0.001). The survival reported for patients with lobulillar who received neoadyuvante chemotherapy adjuvant chemotherapy radiotherapy and adjuvant hormonotherapy compared to ductal was 93.4 % 91.3 %, 91.5 % 89.7 %, 92.5 % 89.8 % 92.5 % 88.4 %, respectively P<0.05. CONCLUSIONS: The overall survival is equal, survival favors. The lobulillar have more positive receptors and survival is better. The tendency to conservative treatment in lobulillar is increasingly. Given the hormonal profile and survival according to estrogen receptors patients with ILC, are probably good candidates to participate in neoadyuvante hormone therapy protocols. Usually the treatment is the same or similar for both groups.
Colon cancer is one of the most common tumors in our country. Following surgery, adjuvant chemotherapy has shown benefit for patients at high risk and node-positive, so it is considered standard modality. OBJECTIVE: The aim of this study is to evaluate the experience of our institution as far as this treatment modality, referred for five years. RESULTS: Preliminarily we evaluated total of 118 patients, the gender distribution was equal. Left colon tumors, occupied the highest percentage (45.76 %) and the most common histology was adenocarcinoma. With regard to TNM, T3 was the most frequent (53.1 %) and N0 also (46.9 %). Of the sample assessed, 63.2 % received adjuvant chemotherapy, being the most widely used schemes, Folfox 4 62.5 %, Xelox 18.1 % and Capecitabine 13.9 %. 66.7 % of patients had adverse events, being the most frequent diarrhea, nausea, vomiting, and peripheral neuropathy, all G1/2 in most cases. There were no treatment-related deaths. 100 % of patients had follow-up, for the year 2009, 55.93 % of them were alive. The OS was greater in the group that received adjuvant treatment with regard to that it did not receive it, without statistics significant (P=0.098) since up to the date there has not been reached the median of global survival. CONCLUSIONS: Similar results were obtained with regard to the DFS, being this major in the group that received chemotherapy, versus the group that did not receive it, reached significant statistics (P=0.045).
OBJECTIVE: The objective of this work is estimate the risk of mortality for cancer, considering the standards of mortality in Lara and Venezuela. RESULTS: Of the whole of deaths for cancer in the period it happened in men 50.11 % and in woman49.89 %, the highest rate was the calculated for 2003 and lowest in 1997 (P=0.0001). Most frequent anatomical location was in the digestive system and the high rate in the state during the period appeared in Crespo municipality. On having calculated the reason between the rates of every municipality with those of Lara, were observed statistically significant differences between the rate of the state and those of the municipalities Andrés Eloy Blanco, Iribarren, Palavecino and Simón Planas. CONCLUSIONS: On having compared the mortality for cancer with the standards of Lara state of Venezuela, in the municipalities Iribarren and Jiménez the risk was high and on having fitted with the standard world population an increase was observed in the rates of mortality by cancer in all the municipalities. It hopes that the results orientate on the reality of the mortality for cancer according to municipalities of the Lara state and use as base for future investigations.
In adult life, the kidney carcinomas associated with Xp11.2 translocations are an infrequent neoplasia representing less than the 1 % of all kidneys neoplasias. OBJECTIVE: To analyze the morphologic characteristics of a carcinoma with Xp11.2 translocation and to establish the probable differential diagnostics of this kidney tumour. METHODS: A 45 years-old female patient, who at May 2008 was admitted to emergency by left flank pain. An abdominal ultrasound showed a left renal lithiasis and a solid lesion at the inferior pole, which was confirmed by the abdominal tomography. A left laparoscopic nephrectomy was performed without complications. The surgical specimen was send to the pathology department in 10 % buffered formalin and processed to histology and immunohistochemestry. RESULTS: Macroscopically was observed at the inferior kidney pole a yellow orange tumor lesion with necrosis and 45 mm x 40 mm x 35 mm in size. The histopathologic sections showed an epithelial lesion with papillary pattern. The tumour cells showed discrete cytoplasmatic borders, with granular eosinophil to vacuolated appearance. Nuclei showed variability in form and size. The immunohistochemical study showed a nuclear positivity to TFE3. CONCLUSION: The correct classification of these carcinomas is important due to the prognostic and the predictive factors, since these neoplasias could be sensible to the chemotherapeutics agents that affect the micro tubules.
The pleomorphic adenoma is the most common tumor type localized in the salivary gland and is rarely located in the breast.
