Methicillin resistant Staphylococcus aureus strains (MRSA) are associated to diverse infectious processes and their growing resistance represents a serious public health problem. The objectives of this study were to detect methicillin and erythromycin resistance of 60 MRSA strains phenotypically and genotypically, and confirm methicillin resistance. This last one was confirmed by the minimal inhibitory concentration (MIC) method, screening test, cefoxitin resistance (FOX), PBP2a detection, β-lactamase production, and mecA gene detection (PCR). For erythromycin resistance, the disk diffusion method, MIC, inducible clindamycin resistance (D-test), and ermA, ermB, ermC, and msrA gene detection (PCR) were used The MIC, discard test, FOX, and PBP2a methods showed in each test a sensitivity and predictive value of 100% and 98% respectively. One strain was mecA negative, PBP2a negative, and β-lactamase positive and it was considered “borderline”. Thirty five (58.3%) were erythromycin resistant and showed MS B (6/17.1%), eML.S B (25/71.4%) and iML.S B (4/11.4%); these last ones were D-test positive. ermA gene was the most frequent. The erythromycin resistance found contitutes a serious problem since it is one of the treatment alternatives. Detection of OXA type carbapenemases in Acinetobacter baumannii from different hospital centers in Caracas, Venezuela http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562012000200004&lng=pt&nrm=iso&tlng=pt Cada vez es más frecuente detectar aislados nosocomiales de Acinetobacter baumannii resistentes a carbapenémicos.
It has become increasingly frequent to detect nosocomial carbapenem-resistant Acinetobacter baumannii isolates. In this study we detected OXA type carbapenemases in 60 A. baumannii isolates from different health centers in Caracas. The identification was done in a conventional fashion and by PCR through detection of blaOXA-51-like as a A. baumannii species marker. The susceptibility profile was done with the Kirby Bauer method according to CLSI 2009 guidelines; enzymatic activity against carbapenems was evaluated through microbiological tests; molecular detection of blaOXA-23-like and blaOXA-58-like was done by PCR. Genes that codify for OXA type carbapenemases were detected in 96.6% of the isolates; of these, 93.4% showed blaOXA-23-like and 6.6% blaOXA-58-like. In 3.3% of the positive isolates coexistence of blaOXA-23-like and blaOXA-58-like was detected. Detection of blaOXA-23-like in most of the carbapenem resistant isolates indicates that it is a determining factor in the resistance of these antimicrobials; consequently, more than one gene codifying for OXA type carbapenemases can be detected in A. baumannii, the one presented intrinsically (blaOXA-51), and those which are acquired (blaOXA-23 and blaOXA-58).
The rapid emergence of antimicrobial resistance to extended spectrum ß-lactamases (ESBL) has a significant clinical impact, and is considered a public health problem. The purpose of this study was to determine if there is clonal dissemination of ESBL producer K. pneumonia in adult patients hospitalized at the Complejo Hospitalario “Ruiz y Paez” (CHRP) of Ciudad Bolivar, Venezuela. Thirteen K. pneumoniae strains isolated from adult patients with intra-hospital infections were evaluated. The phenotypic confirmation of ESBL production was done with the double disk synergy and combined disk methods. The molecular typing of the isolates was done through pulse field electrophoresis (PFE). All the strains presented fluoroquinolone-associated resistance and 9 were aminoglucoside resistant. Twelve susceptibility phenotypes were found; phenotype I prevailed in 2 strains. PFE revealed that most of the strains presented different pulsetypes, discarding a possible clonal relationship among them, except for 2 isolates which presented undistinguishable pulsetypes, both from the same hospitalization service. It can be concluded that at the CHRP there was no clonal dissemination of ESBL producer K. pneumoniae during the March-June 2011 period. First isolation of non-O157 shiga toxin producing Escherichia coli in bovine and porcine meats in Venezuela http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562012000200006&lng=pt&nrm=iso&tlng=pt Escherichia coli productor de toxina Shiga (STEC), es un patógeno transmitido por alimentos.
Shiga toxin producing Escherichia coli (STEC) is a food-transmitted pathogen. Serotype O157:H7 is considered the most clinically important, but 50% of STEC infections correspond to non-O157 serotypes. In Venezuela, presence of non-O157 STEC strains in meat products has not been reported, which was the reason for this study. Seventy ground meat samples were analyzed (35 bovine and 35 porcine). E. coli isolation was done in MacConkey sorbitol agar, supplemented with cefixim, and the biochemical identification was done according to FDA guidelines. DNA extraction and PCR assays were used for the identification of STEC O157:H7 and not O157 strains. Of the 70 samples analyzed, 50 (71.4%) were positive for E. coli isolation, and 47 sorbitol positive and 3 sorbitol negative strains were identified. PCR showed absence of STEC O157:H7 and presence of non-O157 STEC Shiga toxin Stx1 and Stx2 producers in 4.3% of the samples analyzed. This is the first time that the circulation of non-O157 STEC strains in meat products is demonstrated in this country, which suggests that prevention strategies associated to this pathogen should be established. Counts of Staphylococcus aureus and detection of staphylococcal enterotoxins in Venezuelan white artisan “telita” type cheese sold at Caracas city markets http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562012000200007&lng=pt&nrm=iso&tlng=pt La intoxicación alimentaria estafilocócica es producida por el consumo de alimentos contaminados con enterotoxinas estafilocócicas producidas por cepas de Staphylococcus aureus, constituyéndose en una de las principales enfermedades alimentarias a nivel mundial. The diesel growth was evaluated by a redox-indicator based test, and surfactant production was estimated indirectly by both the emulsification index determination (E24) and hemolytic activity. Twenty-six strains (16.8%) were considered as capable of growing in diesel, while surfactant production was detected in 22 (14.8%), estimated according to E24. Seven strains showed high production of biosurfactants (E24 ≥ 50%), headed by Pseudomonas aeruginosa 148D-O, P. aeruginosa 87R-B and Bacillus pumilus 133S-B. No significant correlation was observed between hemolytic patterns and growth outcomes in diesel or E24. Surfactant producing strains should be studied further to assess its potential applications.
Methodological aspects for the culture and quantification of heterotrophic sulfate-reducing bacteria http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1315-25562012000200009&lng=pt&nrm=iso&tlng=pt This paper presents a comparison of various methods for culture, quantification, and maintenance of sulfate-reducing bacteria (SRB) under laboratory conditions, using liquid and semisolid media for water and soil samples. Starkey, Postgate B, API and modified Baars media were used with an incubation time of 21 days in a GasPack™ anaerobic jars type. The modified Baars medium was more efficient for the quantification of SRB in both liquid and semisolid media when compared with other culture media tested, detecting differences of three orders of magnitude in soil samples and in two orders for water samples at 8 days of incubation. The semisolid modified Baars medium in Petri dishes allowed the isolation of pure cultures of SRB by the streak plate method. It was found that strains in liquid modified Baars medium remain viable for up to three months, while in the same semisolid medium were kept only one month.
With the purpose of detecting presence of fungi and aflatoxins in pre-cooked corn flour in Aragua State, Venezuela, we sampled four commercial presentations during five weeks.
Intestinal parasitic diseases represent one of the most important health problems in Venezuela from both public health and socioeconomic development points of view. The main objective of this study was to determine the prevalence of enteroparasites in children under 12 years old at 45 communities of Anzoategui State, Venezuela. The study included the analysis of 5,876 samples taken by direct examination and Kato concentration, and the geotopographic characteristics were described by municipality, identifying the marker parasite per locality. The overall prevalence was 74.6%, significantly higher in children 4-9 years old (53.8%). Globally, protozoa predominated (82.5%): Blastocytis spp. (25.9%), Giardia intestinalis (20.6%) and Entamoeba coli (18.4%). Among helmints, the most common were Ascaris lumbricoides (6.9%), Trichuris trichiura (6.4%), and hookworms (2.7%). It was concluded that there is a high prevalence of enteroparasites in the child population of the Anzoategui State geography evaluated, which includes a mountainous region with its piedmont and the plane regions of the Guanipa mesa. The polyparasitism at the expense of commensals observed in some communities reflects their precarious sanitary conditions, especially regarding the quality of the drinking water available, suggesting that health education activities be carried out frequently to decrease this public health problem.
In this study we determined the prevalence of respiratory viruses and atypical bacteria in a population group of Zulia State between June 2008 and June 2010. The group included 224 individuals with respiratory infections due to different causes. The serum determination of IgG anti-Mycoplasma pneumoniae, Legionella pneumophila serogroup 1, Costella burnetii, Chlamydophila pneumoniae, adenovirus, influenza A and B, parainfluenza 1, 2, 3, and sincicial respiratory virus (SRV), was done by indirect immunofluorescency. In 100% of the patients analyzed we detected antibodies against the respiratory viruses evaluated (p<0.05), while in 26.3% we demonstrated seropositivity against atypical bacteria. Adenoviruses presented the highest percentage (79.4%), followed by SRV (76.7%), (p<0.001) in relation to parainfluenza 1, 2, 3, and influenza A and B (58.9%). There was greater prevalence (p<0.001) of anti-Chlamydophila pneumonia IgG (27.6%) when comparing it with the rest of the bacteria. Of the population analyzed, 33.9% (76/224) (p<0.001) presented antibodies against the pathogens. This demonstrates the circulation of respiratory viruses and atypical bacteria in the population group evaluated, which is extremely useful for knowing the dynamics and epidemiology of respiratory infections and for setting the bases for implementation of an active surveillance system at Zulia State.
Co-infection with hepatitis B (HBV) virus and/or hepatitis C (HCV) virus can induce complications in HIV+ patients. The purpose of this study was to evaluate the frequency of serologic markers in HBV and/ or HCV in plasma of HIV infected patients, and its correlation with the HIV viral status and the immunological status of the patient. The study included the evaluation of 1,846 HIV positive plasmas referred to the Instituto Nacional de Higiene “Rafael Rangel” for the determination of HBV and HCV serologic markers. The evaluation of the viral and immunological status was done by the analysis of the HIV-1 viral load and CD4+/CD8+ T lymphocyte counts, respectively, in the population studied. The frequency of HBV or HCV co-infection was 15% and 5%, respectively, while HBV/HCV co-infection was 0.16% (3/1,846) in HIV infected patients. There was no association between the presence of HBV or HCV serologic markers and low or normal values of the HIV genomic RNA (p=0.81 and p=0.31, respectively) nor low or normal values of the CD4/CD8 index (p=0.75 and p=0.06, respectively). These results suggest that HBV or HCV co-infection does not seem to influence the viral and immunological status of the evaluated population.
