Alrededor del 15 - 20% de las mujeres comienzan el embarazo con un índice de masa corporal que las definiría como obesas. Los embarazos en mujeres obesas tienen un mayor riesgo de complicaciones como diabetes gestacional, trastornos hipertensivos, parto pretérmino y cesárea. Existen pruebas convincentes de que la obesidad aumenta tres veces el riesgo de preeclampsia y en los países desarrollados es el principal riesgo atribuible para el trastorno. Las embarazadas obesas tienen riesgo de sufrir hipertensión y preeclampsia por la reducción de vascularización y flujo sanguíneo placentario, mediado por la insulinorresistencia. Adipocinas, hormonas metabólicas, perfil de citocinas e insulinorresistencia tienen impacto directo sobre la función placentaria predisponiendo a la aparición de preeclampsia. En esta revisión se exploran las pruebas de los fuertes vínculos entre obesidad, la insulinorresistencia que esta produce, y su papel en la preeclampsia.<hr/>Prevalence of obesity during pregnancy is increasing exponentially. About 15-20% of women begin pregnancy with a body mass index that would define them as obese. Pregnancies in obese women have an increased risk of complications such as gestational diabetes, hypertensive disorders, preterm delivery and cesarean section. There is convincing evidence that obesity increases three times the risk of preeclampsia and in developed countries is the main risk attributable to the disorder. Obese pregnant women are at risk of hypertension and preeclampsia due to reduced vascularity and placental blood flow, mediate in part, by insulin resistance. Adipokines, metabolic hormones, cytokine profile, and insulin resistance have a direct impact on placental function, predisposing to the appearance of preeclampsia. This review explores the evidence of strong links between obesity, insulin resistance that this produces, and its role in preeclampsia. <![CDATA[Relatonship between biopsychosocial factors in patients with type ii diabetes mellitus considering sex]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1690-31102017000300003&lng=pt&nrm=iso&tlng=pt Objetivo: Describir la relación entre depresión, redes de apoyo social, estilo de comunicación, adherencia al tratamiento y hemoglobina glucosilada según el sexo, en pacientes con diabetes mellitus tipo 2. Métodos: Participaron 278 pacientes con promedio de 59 años de edad. Fue una investigación no experimental, con diseño transeccional-correlacional. Se aplicaron los instrumentos: cuestionario tridimensional para la depresión, escala social network and support, autoinforme de actitudes y valores en las interacciones sociales e inventario para medir el estilo de vida en diabéticos. Resultados: Por medio del coeficiente de correlación de Pearson se encontraron las siguientes relaciones en ambos sexos: mayor depresión con menor adherencia (M= -0,391; H= -0,307), con menor estilo asertivo (M= -0,391; H= -0,485) y con mayor estilo pasivo-agresivo (M= 0,429; H= 0,563). Hubo correlación de mayor estilo asertivo con mayor adherencia (M= 0,236; H= 0,247). Los estilos de comunicación se asociaron entre sí, excepto el estilo pasivo que no se relacionó con agresivo ni con pasivo-agresivo. Únicamente en el sexo femenino, un mayor estilo pasivo-agresivo se relacionó con menor estilo pasivo (-0,181), con menor adherencia (-0,273) y con un mal control metabólico (0,160). Mayor estilo asertivo se relacionó con un mejor control metabólico (-0,168) y mayores redes de apoyo social con mayor adherencia (0,207). Conclusión: En función del sexo, existen diferencias en la relación entre las variables estudiadas en los pacientes con diabetes tipo 2. Se sugiere confirmar estos resultados en estudios posteriores.<hr/>Objective: To describe the relationship between depression, social support network, communication style, adherence to treatment and glycosylated hemoglobin by sex in patients with Type 2 diabetes mellitus. Methods: A total of 278 patients averaging 59 years old participated in a non-experimental research with a cross-sectional correlational design. Four instruments were applied: The Three Dimensional Questionnaire for Depression, The Network and Support social scale, an attitudes and values in social interactions self-report, and an inventory to measure lifestyle in diabetics. Results: Through the Pearson correlation coefficient the following relationships were found in both sexes: increased depression with less adherence (M= -0,391; H= -0,307), less assertive style (M= -0,391; H= -0,485) and more passive-aggressive style (M= 0,429; H= 0,563). Increased assertive style with more adherence (M= 0,236; H= 0,247). Communication styles were associated amongst themselves, except the passive style which was not associated with the aggressive nor the passive-aggressive style. Only in females, increased passive-aggressive style was associated with less passive style (-0,181), with less adherence (-0,273) and with poor metabolic control (0,160). Increased assertive style was associated with improved metabolic control (-0,168) and increased social support network with more adherence (0,207). Conclusion: Depending on sex, differences arise in the relationship between the studied variables in patients with type 2 diabetes. It is suggested to confirm these results in subsequent studies. <![CDATA[<b>Anthropometric parameters</b>: <b>predictive capacity and utility as diagnostic tests of insulin resistance in adolescents</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1690-31102017000300004&lng=pt&nrm=iso&tlng=pt Objetivo: Determinar la capacidad del índice de masa corporal (IMC), índice circunferencia abdominal/talla (ICAT) y circunferencia abdominal (CA) para predecir resistencia insulínica (RI) según sitio de medición de la CA y determinar su eficacia para discernir entre sujetos sanos y sujetos con RI. Métodos: Se estudiaron 798 adolescentes de ambos sexos (404 hombres), entre 15 y 19 años de edad, de 31 instituciones de educación media, diversificada y educación universitaria del Municipio Iribarren, Estado Lara, Venezuela. Los jóvenes firmaron el consentimiento informado. Se evaluó estrato socioeconómico, peso, talla y CA, éste último en tres sitios anatómicos: encima de crestas ilíacas, punto medio y debajo de reborde costal. Se calculó IMC e ICAT, para cada sitio de CA. Se determinó insulina basal y se calculó Homeostasis Model Assessment Insulin Resistance (HOMA-IR). Se realizaron pruebas no paramétricas, regresión logística y curvas receiver operating characteristic (ROC). Resultados: Se demostró que la RI es una función del ICAT, CA e IMC, siendo esta asociación más fuerte con el ICAT. La capacidad predictiva del ICAT fue mejor en segmento superior y medio del abdomen. Como pruebas diagnósticas, todas presentaron igual eficacia para discernir entre sujetos sanos y sujetos con RI, no obstante, en adolescentes permiten clasificar un mayor número de sujetos sanos. Conclusión: Todos los parámetros antropométricos son eficaces, pero en adolescentes identifican un mayor número de sujetos sanos. Se propone unificar sitio de medición de la CA, inmediatamente debajo del reborde costal o punto medio del abdomen, por ser los sitios con mayor probabilidad de predecir RI.<hr/>Objective: To determine the capacity of the body mass index (BMI), waist to height ratio (WHtR) and abdominal circumference (AC) to predict insulin resistance (IR) according to the measurement site of the AC, and determine the effectiveness of such indicators to distinguish between healthy subjects and subjects with IR. Methods: In 31 institutions of secondary and university education at the Iribarren Municipality, Lara State, Venezuela, 798 adolescents of both sexes (404 men) between 15 and 19 years of age were enrolled. Adolescents signed informed consent. Socioeconomic stratum, weight, height and AC were evaluated in three anatomical sites: above the iliac crests, midpoint and below the costal margin. We calculated BMI and WHtR for each site. Basal insulin was determined and Homeostasis Model Assessment Insulin Resistance (HOMA-IR) was calculated. Non-parametric tests, logistic regression and receiver operating characteristic (ROC) curves were performed. Results: It was demonstrated that IR is a function of WHtR, AC and BMI, being this association stronger with WHtR. The predictive capacity of WHtR was better in the upper and middle segments of the abdomen. As diagnostic tests, all had equal efficacy to distinguish between healthy subjects and subjects with IR, however, in adolescents allow to classify a greater number of healthy subjects. Conclusion: All anthropometric parameters are effective, but in adolescents, they identify a greater number of healthy subjects. It is proposed to unify AC measurement site, immediately below the costal ridge or midpoint of the abdomen, because they are the sites most likely to predict IR. <![CDATA[Stress and cortisol before and after elections in a group of voters from Carabobo State, Venezuela]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1690-31102017000300005&lng=pt&nrm=iso&tlng=pt Objetivo: Comparar los niveles de estrés y cortisol sérico en un grupo de votantes del estado Carabobo, antes y después del proceso electoral de abril 2013. Métodos: El estudio fue de tipo observacional, correlacional y de corte longitudinal. La muestra estuvo conformada por 122 votantes inscritos en el registro electoral para las elecciones presidenciales de abril 2013, en edades comprendidas entre 25 y 55 años residenciados en los municipios Carlos Arvelo, Naguanagua, Miguel Peña y Valencia del Estado Carabobo, Venezuela. Se les aplicó el instrumento “Índice de Reactividad al Estrés” el cual mide las respuestas individuales de tipo cognitiva, emocional, conductual y somáticas del organismo cuando se encuentra en situaciones reconocidas como estresantes. Se determinó el cortisol sérico antes y después, en un lapso no mayor a 8 días de la fecha electoral. Resultados: El 67,2% de los votantes pertenecía al sexo femenino. Se encontraron resultados significativos al comparar el estrés total, las respuestas de tipo emocional, cognitivo y conductual (p= 0,000; p= 0,028; p= 0,010; p= 0,009 respectivamente). Así mismo, se evidenció que no hubo significación con el cortisol y el estrés somático antes y después del proceso electoral. Conclusión: El proceso electoral de abril 2013 tuvo incidencia sobre el votante pues este presentó estrés leve, moderado y severo, así como las diferentes respuestas (emocional, conductual y cognitivo) antes y después del proceso electoral, esta situación puede generar daños en el organismo.<hr/>Objective: Compare levels of stress and serum cortisol in a group of voters in the state of Carabobo, before and after the electoral process in April 2013. Methods: The study was observational, correlational and longitudinal. The sample consisted of 122 voters registered in the electoral registry for the presidential elections of April 2013, aged from 25 to 55 years old residing at Carlos Arvélo, Naguanagua, Miguel Peña and Valencia, municipalities from Carabobo State, in Venezuela, to which the instrument "Reactivity Index to Stress" was applied, which measures the individual responses of cognitive, emotional, behavioral and somatic type in the organism when it is in recognized as stressful situations. Serum cortisol was determined before and after the electoral date, in a period not exceeding 8 days. Results: The 67.2% of the voters were female. We found significant results when comparing total stress, emotional, cognitive and behavioral responses (p = 0.000, p = 0.028, p = 0.010, p = 0.009 respectively). Likewise, it was evidenced that there was no significance with cortisol and somatic stress before and after the electoral process. Conclusion: The electoral process of April 2013 had an impact on the voter, who presented mild, moderate and severe stress, as well as the different responses (emotional, behavioral and cognitive) before and after the electoral process; this situation can cause damages in the organism. <![CDATA[Tumor in pancreas and thyroid as metastasic presentation of renal carcinoma of clear cells: About a case]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S1690-31102017000300006&lng=pt&nrm=iso&tlng=pt Objetivo: Describir el caso inusual de paciente con carcinoma renal de células claras, con metástasis a páncreas y tiroides, que inicialmente se diagnosticaron como entidades aisladas. Caso Clínico: Paciente femenina de 63 años, con enfermedad actual caracterizada por dolor en hipocondrio derecho, fuerte intensidad, permanente, sin acalmia, quien es evaluada por el servicio de cirugía. Por hallazgos en TC de abdomen de tumoración en cuerpo de páncreas de 6x4 cm y dos lesiones más peripancreáticas de 1x1,5 cm, realizan pancreatectomía corpocaudal. La biopsia reporta tumor de células de islotes de páncreas, con células claras. En los antecedentes personales se obtiene que la paciente 27 años antes tuvo un carcinoma renal de células claras, y hace 4 años, carcinoma insular de tiroides pobremente diferenciado, con abundantes células claras. Se plantea carcinoma renal de células claras como entidad primaria, con metástasis a tiroides y páncreas. Conclusión: El carcinoma renal de células claras constituye una neoplasia rara, los sitios de invasión más frecuentes se localizan en pulmón, hígado y cerebro, menos frecuente en piel, tiroides y páncreas, reflejando un desafío en el diagnóstico diferencial de enfermedad tumoral aislada o metastásica.<hr/>Objective: To describe the unusual case of a patient with clear cell renal carcinoma, with metastases to the pancreas and thyroid, which were initially diagnosed as isolated entities. Case Report: A 63-year-old female patient with a current illness characterized by pain in the right hypochondrium, strong intensity, permanent, without calming. She was evaluated by the surgical service, who, due to abdominal CT findings, that show a tumor of 6x4 cm in the pancreatic body and two more lessions of 1x1,5 cm in the peripancreatic area, perform a corpocaudal pancreatectomy. The biopsy reports tumor of islet cells of pancreas, with clear cells. The patient 27 years ago had a clear cell renal carcinoma and, 4 years ago, a poorly differentiated insular thyroid carcinoma with abundant clear cells. A clear cell renal cell carcinoma as primary entity with metastasis to the thyroid and pancreas is raised. Conclusions: Clear cell renal carcinoma constitutes a rare neoplasm, the most frequent invasive sites are located in lung, liver and brain, and with less frequency skin, thyroid and pancreas, reflecting a challenge in the differential diagnosis of isolated or metastatic tumor disease.
