ABSTRACT Introduction: Adherence to antiretroviral therapy (ART) is crucial for achieving viral suppression, preventing resistance, and reducing HIV-related morbidity and mortality. Objective: To determine factors associated with non-adherence to ART in people living with HIV treated at a referral hospital in Machala, Ecuador. Materials and methods: Quantitative, descriptive, cross-sectional study in 100 patients on ART. Adherence was assessed using the Simplified Medication Adherence Questionnaire (SMAQ). Sociodemographic, behavioral, and treatment variables were collected. Odds ratios (OR) and 95% confidence intervals (95% CI) were estimated in bivariate analyses. Results: Men predominated (72.0%), as did the 30-42 age group (38.0%). Alcohol use was reported by 38.0% and smoking by 20.0%. Non-adherence was mainly observed as failure to follow dosing schedules (77.0%) and missing more than two doses in the past week (44.0%). Smoking was associated with greater non-adherence (OR 3.00; 95% CI 1.00-9.04; p=0.049). Conclusions: Non-adherence was mainly linked to dosing schedule deviations and recent missed doses; smoking emerged as an associated factor. Nursing interventions focused on routine organization, reminders, and behavioral counseling should be strengthened. Access and use of information and communication technologies to facilitate the performance of activities of daily living in older adults: A systematic review http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S2665-01502026000100018&lng=pt&nrm=iso&tlng=pt RESUMEN Introducción: El envejecimiento de la población plantea nuevos desafíos para el mantenimiento de la autonomía y funcionalidad en las personas mayores.
ABSTRACT Introduction: Antisocial Personality Disorder (ASPD) represents a significant clinical challenge due to its behavioral complexity and the limited therapeutic adherence commonly observed in patients. In this context, Cognitive-Behavioral Therapy (CBT) has been widely applied in the treatment of various personality disorders; however, its specific effectiveness in managing ASPD remains a subject of ongoing debate within the scientific literature. Objective: To examine the impact of Cognitive-Behavioral Therapy in the treatment of Antisocial Personality Disorder, assessing its clinical usefulness and current therapeutic alternatives. Materials and Methods: A documentary review was conducted based on recent academic literature indexed in scientific databases such as PubMed. Studies were selected according to their thematic relevance and recency, with the aim of analyzing available evidence regarding the effectiveness of CBT in ASPD and exploring emerging therapeutic approaches. Results: The review revealed limited literature directly linking CBT with ASPD and the absence of unified conclusions. Nevertheless, the prevailing consensus suggests limited effectiveness of CBT in this population, mainly due to low patient adherence to the therapeutic process. Furthermore, a growing trend toward alternative approaches was identified, including Mentalization-Based Therapy (MBT), Virtual Reality Therapy (VR), and Schema Therapy (ST), supported by technological innovations and integrative models. Conclusion: Current evidence indicates that CBT demonstrates limited effectiveness in the treatment of ASPD, highlighting the need for further research to strengthen its empirical validation or to consolidate alternative therapeutic strategies aimed at optimizing clinical care for this population. Dentigerous cyst associated with mandibular transmigration and the presence of a supernumerary tooth.
ABSTRACT Introduction: Dental transmigration is a rare anomaly characterized by the migration of more than half of the length of an impacted tooth across the midline of the maxilla or mandible. Its etiology is multifactorial and has been associated with genetic factors, developmental disturbances, and environmental conditions. Objective: To describe the diagnosis and surgical management of a case of mandibular canine transmigration associated with an odontogenic cystic lesion. Materials and Methods: A clinical case report is presented involving a 15-year-old male patient referred to the maxillofacial surgery service by orthodontics following an incidental radiographic finding. Clinical and imaging evaluations were performed using panoramic radiography, complemented by diagnostic studies, followed by surgical intervention and histopathological examination of the lesion. Results: Intraoral examination revealed absence of tooth 3.3 and persistence of the primary tooth 7.3, with swelling at the level of the mandibular symphysis. Imaging studies confirmed transmigration of the left mandibular canine (3.3), classified as type II according to Mupparapu, associated with a pericoronal radiolucent image compatible with a dentigerous cyst and the presence of a supernumerary tooth. Surgical extraction of the impacted tooth and biopsy of the lesion were performed, and histopathological analysis confirmed a dentigerous cyst. Postoperative evolution was favorable. Conclusion: Canine transmigration is an uncommon anomaly that requires early diagnosis and multidisciplinary management to prevent complications associated with tooth impaction and concomitant lesions. In this case, individualized and timely surgical treatment resulted in a favorable clinical outcome.
ABSTRACT Introduction: The allocation of critical healthcare resources represents one of the most complex dilemmas in contemporary bioethics, particularly in contexts marked by structural limitations such as Ecuador. The bioethical principle of justice plays a central role in guiding clinical decisions related to the equitable distribution of scarce resources, including intensive care unit (ICU) beds, from ethical, legal, and deontological perspectives. Objective: To analyze the application of the bioethical principle of justice in the allocation of critical healthcare resources through the examination of a clinical case, integrating a hermeneutic, ethical, and legal approach within the Ecuadorian context. Materials and Methods: An argumentative hermeneutic essay was developed using a qualitative and reflective approach, structured according to the hermeneutic cycle in three phases: (1) pre-understanding, focused on the ethical dilemma “Ethical decision-making in ICU allocation for critically ill patients”; (2) micro-hermeneutic analysis of the case components; and (3) macro-hermeneutic argumentation aimed at achieving a comprehensive understanding of the phenomenon and its ethical and legal foundations. Results: The analysis demonstrated that the application of the principle of justice requires distribution criteria grounded in equity, proportionality, and non-discrimination, moving beyond purely utilitarian or administrative approaches. It also highlighted the need to integrate deontological principles and national regulatory frameworks to strengthen clinical decision-making in situations of resource scarcity. Conclusions: The bioethical principle of justice constitutes a fundamental ethical pillar that compels healthcare systems to ensure equitable resource distribution, guaranteeing access to care with respect, equality, and human dignity, regardless of age, gender, or clinical condition. Its proper application requires solid ethical training and clear normative criteria to guide responsible decision-making in critical contexts.
ABSTRACT Introduction: Simulation-based education enables safe, controlled practice and is increasingly used in dentistry. In pediatric local anesthesia training, which requires anatomical accuracy, psychomotor control, and interaction with caregivers, self-efficacy is a relevant educational outcome because it influences students’ effort, persistence, and readiness to perform complex tasks. Objective: To evaluate changes in self-efficacy among dental students after progressive use of a dental anesthesia simulator during the preclinical Pediatric Dentistry course. Materials and methods: An analytical pretest posttest single-group study was conducted in sixth-semester dental students (n = 53). Self efficacy was measured before and after simulation training, delivered progressively in sessions structured into briefing, simulated practice, and debriefing, using the Nissin® anesthesia training model with 11 electronic sensory points providing visual and auditory feedback. Activities included anatomical landmark identification, four-handed practice, needle insertion and aspiration technique, and simulated interaction with parents/caregivers. Results: After confirming normality of paired differences (Shapiro Wilk: W = 0.96; p = 0.095), a paired t-test was applied. Mean self-efficacy increased from 26.23 (SD = 3.78) to 30.25 (SD = 4.64) (t (52) = 4.79; p = 0.000014), with a moderate effect size (Cohen’s d = 0.66). The post hoc power was 0.997 (α = 0.05). After training, 96.2% of students were classified as having medium or high self-efficacy. Conclusion: Progressive simulation with briefing and debriefing was associated with increased self efficacy, supporting its structured incorporation into preclinical Pediatric Dentistry training.
ABSTRACT Introduction: This research addresses the experience of minibus users in the highlands of Ancash, considering its impact on well-being and quality of life. Objective: To analyze how service conditions, staff treatment, and fare costs influence user experience. Specifically, the study examined the influence of overcrowding and vehicle conditions on comfort and safety, the effect of drivers' and fare collectors' behavior on emotional well-being, the importance of price in transportation choice, and the psychological consequences associated with travel. Methods: A qualitative study with a phenomenological design was conducted, based on semi-structured interviews with 14 frequent users, whose data were processed through inductive categorization. Results: Findings revealed overcrowding, poor maintenance, insecurity, verbal abuse, stress, and frustration; however, low cost remains the main reason for use. Conclusion: Passenger experience is shaped by physical, economic, and relational factors that negatively affect well-being. Strengthening supervision, improving vehicle maintenance, training staff, and implementing visible reporting mechanisms are recommended.
