Cutaneous tuberculosis (TB) is an uncommon infectious disease (1-2% of cases) that has been increased due to the human immunodeficiency virus (HIV) pandemic and to the increased use of pharmacological immunosuppression. We describe an unusual case of cutaneous TB in a Cuban patient with acquired immunodeficiency syndrome (AIDS), who presented ulcerative/exudative lesions in his left ear and penis. Samples of tissue and secretion from ulcers were cultured prior to decontamination. For the culture the conventional method in Löwenstein-Jensen medium (L-J) and the automated method Bact/Alert 3D were used. After 2-3 weeks the presence of a slowgrowing non-pigmented mycobacterial strain was observed that could be identified as Mycobacterium tuberculosis by determination of TBAg MPT64 Bioline. According to the clinical characteristics of the patient with ulcerative and mutilating lesions, tuberculosis cutis of the lupus vulgaris variety was diagnosed. The patient was given anti-tuberculosis treatment and antiretroviral therapy to improve his quality of life. The early diagnosis and treatment of Mycobacterium extrapulmonary infections can prevent the spread of infection in both immunocompetent and immunodeficient patients.
