Sin embargo, la mayoría de los Obstetras y Odontólogos prefieren que el tratamiento dental se realice durante el segundo trimestre, manteniéndose al mínimo durante los otros períodos de la gestación.]]></p></abstract>

La cavidad bucal como parte integral del ser humano, presenta morbilidad que en algunas ocasiones puede coincidir con el embarazo, siendo motivo de angustia para la mujer, la disyuntiva de ser tratada por el Odontólogo dado que como auxiliar de tratamiento, utilizan anestésicos locales (1).</span></p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; En la revisión de la literatura internacional existen informes aislados y basados en revisiones bibliográficas, en cuanto al uso de anestésicos locales durante la gestación (2); como consecuencia incluso el mismo odontólogo cuando debe utilizarlos en una mujer embarazada, tiene duda en cuanto a la inocuidad del fármaco, tanto para la madre como para el feto, ya que en algunos folletos está proscrito su uso durante la gestación (3).</span></p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Tradicionalmente los Odontólogos han ofrecido las opciones conservadoras del tratamiento a mujeres embarazadas. Actualmente, ya esto no es así, dejándose sólo para el tratamiento electivo (cosmético), el que se debe aplazar hasta después del parto.</span></p>      ]]></body>
<body><![CDATA[<p class="MsoNormal" style="text-align: justify">&nbsp;</p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <b>Anestésicos Locales</b></span></p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; La anestesia local, es la pérdida de la sensación sin pérdida de la conciencia ni del control central de las funciones vitales. Los anestésicos locales actúan sobre cualquier parte del sistema nervioso y sobre cualquier fibra nerviosa. Tiene una acción reversible, con recuperación total de la función sin daño alguno para las fibras (4).</span></p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Desde el punto de vista histórico, los anestésicos locales han sido algunos de los fármacos de mayor importancia en medicina y odontología. La capacidad de producir anestesia, es decir la pérdida de la sensación en zonas específicas del cuerpo, revolucionó las dos ciencias mencionadas, ya que no necesitó la anestesia general para practicar todos los procedimientos (5). Einhorn y colaboradores (6) permitieron sintetizar anestésicos locales más inocuos que no crearan dependencia.</span></p>      <p class="MsoNormal" style="text-align: justify"><span lang="ES" style="font-size: 12.0pt">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Los anestésicos locales contienen tres componentes principales, que son: un centro hidrófilo, un centro hidrófobo separados por una cadena alquílica intermedia. El centro hidrófobo suele ser un grupo aromático, y la unión con este grupo determina algunas de las propiedades farmacológicas de éstos. Los grupos intermedios más prevalentes, y con ello las dos clases principales de anestésicos locales son ésteres y amidas. De tal manera, de acuerdo al tipo de grupo químico los anestésicos locales se clasifican (4,7,8):</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: -18.0pt; margin-left: 53.4pt"><span lang="ES" style="font-size: 12.0pt; font-family: Symbol">·<span style="font-style: normal; font-variant: normal; font-weight: normal; font-size: 7.0pt; font-family: Times New Roman">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </span></span><span lang="ES" style="font-size: 12.0pt">Aminoésteres: cocaína, procaína, novocaína, cloroprocaína y tetracaína.</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: -18.0pt; margin-left: 53.4pt"><span lang="ES" style="font-size: 12.0pt; font-family: Symbol">·<span style="font-style: normal; font-variant: normal; font-weight: normal; font-size: 7.0pt; font-family: Times New Roman">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </span></span><span lang="ES" style="font-size: 12.0pt">Aminoamidas: lidocaína, mepivacaína, prilocaína, bupivacaína, etidocaína y ropivacaina.</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Los anestésicos locales se diferencian por el período de latencia, la duración de la acción, la toxicidad, la potencia y la sensibilidad de bloqueo. Las propiedades deseables del anestésico local ideal son: ausencia de irritabilidad y toxicidad local, ausencia de toxicidad sistémica, tiempo de instauración de la anestesia breve, y duración del efecto anestésico prolongado (4,9).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Los anestésicos locales impiden la producción del impulso nervioso actuando sobre la membrana celular, su acción consiste en un bloqueo de la conducción nerviosa al reducir la permeabilidad de la membrana a los iones sodio (9).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">La duración de la acción de un anestésico local depende del tiempo de contacto real con los tejidos nerviosos. Por tal motivo, los anestésicos locales llevan habitualmente un complemento vasoconstrictor, en general adrenalina o noradrenalina. Sin embargo, es punto de controversia el empleo de vasoconstrictores en fórmulas de anestésicos locales empleados en odontología, por el peligro conocido de los efectos adversos después de la absorción sistémica (4). Los vasoconstrictores disminuyen la velocidad de absorción sistémica del anestésico, de forma que aumentan la duración del efecto anestésico, disminuye la dosis del anestésico necesario, disminuye la hemorragia en el área infiltrada y disminuye el riesgo general de la anestesia local (5).</span></p>      ]]></body>
<body><![CDATA[<p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Cabe destacar que la presencia de inflamación disminuye la eficacia de los anestésicos locales, ya que la inflamación hace que el pH sea más ácido, lo que disminuye la cantidad de anestésico&nbsp; en forma lipófila y disminuye la capacidad de estos fármacos para atravesar la membrana nerviosa.</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Los anestésicos locales intervienen en la función de todos los órganos en los que existe conducción o transmisión de impulsos nerviosos. Las reacciones adversas de los anestésicos locales dependen del equilibrio entre la velocidad de absorción y la de destrucción; se conocen cuatro reacciones y son: tóxicas, sin relación con el fármaco, de idiosincrasia y alérgicas (5,10).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">De tal forma, debido a la mayor eficacia y menor incidencia de complicaciones de las aminas, como la lidocaína, se ha limitado el uso de los tipos ésteres.</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt">&nbsp;</p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt"><b>Embarazo y Anestesia Local.</b></span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">La mayoría de los Obstetras y Odontólogos, prefieren que el tratamiento dental sea realizado durante el segundo trimestre del embarazo, si es posible. Manteniéndose al mínimo dichos procedimientos durante el primer y tercer trimestre de la gestación (11, 12).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Se debe evitar la práctica de procedimientos dentales en el primer trimestre del embarazo, ya que es el período de la organogénesis, y aunque el tratamiento dental no puede necesariamente dañar estos órganos, se deben disminuir al mínimo los riesgos potenciales (9,1). Existen estudios donde se analizan la exposición a anestésicos locales en la embarazada, y no se han encontrado una mayor incidencia de malformaciones congénitas (14, 15)</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Igualmente durante el tercer trimestre de la gestación, y fundamentalmente la segunda mitad de éste, se debe disminuir los procedimientos dentales, ya que además de ser muy incómodo para la paciente, el colocar la espalda en la silla dental durante este período del embarazo, el útero puede aumentar la presión sobre la vena cava inferior, produciendo alteraciones hemodinámicas al binomio madre-feto, que se traducirían en hipotensión (producto de la posición materna y no por efecto del medicamento), así como dificultad para respirar. Por ende, si en última instancia el tratamiento debe ser practicado en este período, se debe permitir que la paciente cambie de posición cada 3 a 7 minutos (9-13)</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Como regla general lo mejor es no utilizar ningún fármaco durante el embarazo, especialmente, durante el primer trimestre, si no es imprescindible, independientemente de la seguridad del medicamento. A veces, sin embargo se deben administrar en el curso de tratamientos necesarios. Afortunadamente, la mayoría de los medicamentos empleados en odontología, no están contraindicados durante el embarazo. Para mayor seguridad y por la dificultad para la obtención de información sobre la teratogenicidad de los fármacos la Food and Drug Adm (FDA) estableció cinco categorías (A, B, C, D, X), que indican el potencial de una droga para causar defectos fetales. De tal manera, cuando el odontólogo se encuentre en la disyuntiva de prescribir o no un medicamento a una paciente embarazada, se deben seleccionar los medicamentos en las categorías A y B, y los medicamentos pertenecientes a las categorías restantes deben evitarse (16, 17).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Los anestésicos locales inyectados a la gestante pueden cruzar la barrera placentaria, sin embargo la lidocaína parece ser segura en la paciente embarazada, ya que las dosis pequeñas de lidocaína empleados en odontología, probablemente no afectaran al feto independientemente del trimestre del embarazo (1,9,18, 19).</span></p>      ]]></body>
<body><![CDATA[<p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Existe literatura que reporta que los agentes anestésicos locales exhiben un inicio de acción más rápido y duración más prolongada en el embarazo (20). Generalmente la lidocaína tiene efecto en 2 &#150; 5 minutos y es efectiva por 1 &#150; 2 horas dependiendo de la dosis, del método de la administración, de cada paciente y de otras variables (1,9).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">El uso de vasoconstrictores con anestésicos locales durante el embarazo es todavía controversial. El útero grávido tiene una mayor sensibilidad a las propiedades vasoconstrictoras de la adrenalina, que otros órganos. Sin embargo, esta sensibilidad disminuye en el embarazo tardío (21). Los agentes adrenérgicos comprometen el flujo sanguíneo uterino (21-24); sin embargo esto parece no tener efectos deletéreos en el feto&nbsp;&nbsp;&nbsp;&nbsp; (24, 25). La adrenalina es usada rutinariamente en la anestesia de cualquier tipo en obstetricia sin complicaciones. Por tanto, su uso durante la administración de anestesia local para tratamientos odontológicos no estaría contraindicado, sin embargo se debe evitar las inyecciones intravasculares (18).</span></p>      <p class="MsoNormal" style="text-align: justify; text-indent: 35.4pt"><span lang="ES" style="font-size: 12.0pt">Actualmente existen numerosas publicaciones acerca del uso de anestesia con acupuntura para la práctica odontológica, como alternativa a la anestesia convencional. Sin embargo, cabe destacar que el embarazo puede ser una contraindicación para la acupuntura, especialmente si el punto Hoku ha de ser utilizado; la inserción de una aguja en este punto puede&nbsp; causar palpitaciones y contracciones en músculos lisos, incluyendo el útero; lo podría inducir a la pérdida del embarazo.
