<body><![CDATA[<P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">RESUMO<U>:</U></font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Artigo presente, &eacute; uma revis&atilde;o sobre v&aacute;rias considera&ccedil;&otilde;es em refer&ecirc;ncia para placa dent&aacute;ria localizada na pr&oacute;pria superf&iacute;cie de dentaduras como agente de condi&ccedil;&atilde;o etiol&oacute;gica<B> </B>de Estomatite de Dentadura, e os microorganismos que podem ser achados nesta placa.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">KEY WORDS:</font></B><font face="Times New Roman" size="3">Denture Stomatitis, dental plaque, microorganisms, dentures.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3"><b>Recibido</b> <I> para arbitraje: 25/11/2001</I></font>&nbsp; <font face="Times New Roman" size="3"><b>Aceptado</b> <I> para publicaci&oacute;n: 07/02/2002 </I></font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El t&eacute;rmino de Estomatitis Sub-Prot&eacute;sica (E.S.P.) es usado para describir cambios patol&oacute;gicos en la mucosa bucal en el &aacute;rea de soporte de la dentadura. Estos cambios se caracterizan por la presencia de un eritema en los tejidos, siendo com&uacute;nmente encontrada en el maxilar superior y raramente en el inferior<SUP>1,2,3,4,5,6,7</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    La mucosa se encuentra brillante, eritematosa y eventualmente hemorr&aacute;gica. Puede presentarse adem&aacute;s, con m&uacute;ltiples petequias localizadas en el paladar duro<SUP>8,9</SUP>.</font></P>     <P ALIGN="JUSTIFY">        <font face="Times New Roman" size="3">        La mucosa inflamada y enrojecida est&aacute; muchas veces limitada a los tejidos en contacto directo con la dentadura. La mucosa bucal inflamada y lisa puede estar asociada con una sensaci&oacute;n de quemadura, ardor o de prurito<SUP>7,9,10,11</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Es bien conocido el hecho de que la E.S.P. suele estar acompa&ntilde;ada con Candidiasis Aguda Pseudomembranosa, Queilitis Angular, Glositis, as&iacute; como con Aftas bucales, bucofar&iacute;ngeas y esofagofar&iacute;ngeas<SUP>12</SUP>.</font></P> <FONT COLOR="#008000">    <P ALIGN="JUSTIFY">     </FONT><font face="Times New Roman" size="3">A&uacute;n cuando se sabe que<FONT COLOR="#008000"> </FONT>las evidencias sobre la etiolog&iacute;a de la E.S.P. est&aacute;n inconclusas y son a menudo contradictorias, la mayor&iacute;a de los autores coinciden en se&ntilde;alar que &eacute;sta es multifactorial<SUP>2,7,13,14,15</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Diversos reportes publicados en la literatura indican que el tiempo de las dentaduras, su uso cont&iacute;nuo, as&iacute; como los h&aacute;bitos de higiene influyen en el inicio y progresi&oacute;n de la E.S.P.<SUP>16,17,18,19</SUP>.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Hoy d&iacute;a, existen suficientes evidencias para poder establecer la relaci&oacute;n que existe entre la placa dental y la E.S.P.<SUP>12,18,20,21</SUP>. Esto est&aacute; ampliamente soportado por estudios cl&iacute;nicos y epidemiol&oacute;gicos que demuestran la correlaci&oacute;n entre los &iacute;ndices de placa dental y la presencia y severidad de la E.S.P.<SUP>17,18,22</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Se han realizado estudios en relaci&oacute;n con la colonizaci&oacute;n de los materiales utilizados para las bases de las pr&oacute;tesis, demostrando que la pr&oacute;tesis constituye un reservorio de microorganismos<SUP>23,24</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Penhall<SUP>25</SUP>, sugiri&oacute; que la dentadura era considerada como un &quot;aplicador de placa&quot;. Esta, contiene conglomerados de placa que se encuentran en contacto con la mucosa bucal por un extenso per&iacute;odo de tiempo y por lo tanto, sus efectos t&oacute;xicos se evidencian al aparecer las lesiones de E.S.P. en la cavidad bucal.</font> </P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Tambi&eacute;n se ha demostrado mediante estudios de microscop&iacute;a &oacute;ptica y electr&oacute;nica, que esta placa tiene un aspecto muy particular y es similar a la observada en los dientes naturales<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se ha descrito a la placa dental sub-prot&eacute;sica como una acumulaci&oacute;n microbiana no mineralizada que se adhiere a la pr&oacute;tesis. Dicha placa presenta estructuras con formas diversas (filamentosas, bacilares y cocoides) y est&aacute; constitu&iacute;da por una matriz org&aacute;nica derivada de las glicoprote&iacute;nas salivales y de los productos minerales extracelulares. Su presencia puede ser demostrada mediante el empleo de reveladores de placa dental<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Si se observa al microscopio electr&oacute;nico de barrido, la superficie interna de la resina acr&iacute;lica de la pr&oacute;tesis en pacientes que han usado sus dentaduras durante muchos a&ntilde;os, la placa dental muestra una superficie uniforme, encontr&aacute;ndose en ella cocos, bacilos y filamentos<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    De la misma forma, el estudio al microscopio electr&oacute;nico de barrido de las resinas acr&iacute;licas provenientes de pr&oacute;tesis muy usadas, permite verificar la presencia de placa dental sub-prot&eacute;sica, en las zonas m&aacute;s rugosas y porosas de la superficie interna de la pr&oacute;tesis<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    La presencia de <I>Candida albicans</I> podr&iacute;a ser considerada como el factor principal en la aparici&oacute;n de E.S.P.<SUP>7,8,23</SUP>. El hongo se localiza en el borde y preferiblemente sobre la superficie de la placa microbiana de la pr&oacute;tesis, y la lesi&oacute;n ser&aacute; el resultado de la producci&oacute;n de toxinas extremadamente irritantes<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">       <font face="Times New Roman" size="3">       La mayor&iacute;a de los estudios han demostrado la presencia de diversas especies de <I>Candida</I> en altas proporciones en pacientes con E.S.P. Sin embargo, estas especies no son los agentes etiol&oacute;gicos en todos los casos<SUP>7,12,26</SUP>. Otros estudios han demostrado la presencia de <I>Candida</I> en 100% de los casos de sujetos con E.S.P.<SUP>27</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Aunque esto permite sugerir que los microorganismos del G&eacute;nero <I>Candida</I> constituyen como tal un factor causal de E.S.P., si estos est&aacute;n presentes como comensales en la cavidad bucal, no sorprender&iacute;a que se encontraran presentes en la mayor&iacute;a de los casos de E.S.P.<SUP>7</SUP>.</font> </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Por su parte, Santarpia y col.<SUP>28</SUP> se&ntilde;alan que en los a&ntilde;os comprendidos entre 1960 y 1980, la presencia de infecciones mic&oacute;ticas entre los pacientes con E.S.P. ha sido reportada entre 40% y 70%. Los reportes m&aacute;s recientes indican que actualmente, el porcentaje verdadero oscila entre 60% y 100%.</font></P>     <P ALIGN="JUSTIFY">  <font face="Times New Roman" size="3">  Se ha sugerido que la E.S.P. puede deberse a bacterias presentes habitualmente en la cavidad bucal, tales como <I>Streptococcus, Staphylococcus</I> y <I>Klebsiella</I><SUP>8,23</SUP>, o bien a una combinaci&oacute;n de <I>C. albicans</I> y otros microorganismos<SUP>29</SUP>. M&aacute;s recientemente se aislaron <I>&quot;in vitro&quot; </I>cepas de los G&eacute;neros <I>Streptococcus </I>y <I>Staphylococcus</I> a partir de muestras de pr&oacute;tesis y paladar de pacientes con E.S.P.<SUP>30</SUP>.</font></P>     <P ALIGN="JUSTIFY">         <font face="Times New Roman" size="3">         Shinada y col.<SUP>31</SUP> afirmaron que la E.S.P. es causada principalmente por la infecci&oacute;n producida por <I>C. albicans</I> y <I>Streptococcus mutans</I>. Estos investigadores demostraron que <I>S. mutans</I> sintetiza Gluc&aacute;n Acuo-Insoluble, concomitantemente con su adhesi&oacute;n a la superficie de acr&iacute;lico de la pr&oacute;tesis dental en conjunto con <I>C. albicans</I> y observaron adem&aacute;s que la adherencia de <I>C. albicans</I> a la superficie de la pr&oacute;tesis era mediada por interacciones c&eacute;lula-c&eacute;lula con <I>S. mutans</I>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se ha podido determinar el grado de acidez, as&iacute; como los tipos de &aacute;cidos carbox&iacute;licos de la placa dental adherida a la superficie de la pr&oacute;tesis dental en sujetos con y sin E.S.P. Los principales &aacute;cidos identificados fueron: Acetato, Lactato, Propionato, Succinato, Formato y Piruvato. Las muestras de placa dental de los sujetos con E.S.P. ten&iacute;an un pH m&aacute;s &aacute;cido y conten&iacute;an concentraciones significativamente menores de iones lactato que las muestras de placa de los sujetos sin E.S.P. Estos resultados demuestran que los &aacute;cidos carbox&iacute;licos producidos por la microflora de la placa dental ubicada en la superficie de la pr&oacute;tesis est&aacute;n involucrados en la patog&eacute;nesis de la E.S.P<SUP>32</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Por otra parte, se realiz&oacute; una investigaci&oacute;n con la finalidad de determinar la composici&oacute;n de la pel&iacute;cula adquirida que se forma sobre la superficie de las dentaduras, en pacientes con E.S.P. y en pacientes sanos. Se encontr&oacute; que la pel&iacute;cula adquirida estaba constitu&iacute;da en ambos grupos de pacientes como una capa que se adhiere selectivamente a la superficie de las dentaduras. Esta conten&iacute;a: Amilasa de origen salival, mucina de alto peso molecular (MG1), lisozima, alb&uacute;mina e IgA salival. No se detectaron histatinas salivales, prote&iacute;nas ricas en prolina ni mucina de bajo peso molecular (MG2). Tambi&eacute;n se encontr&oacute; que la composici&oacute;n de amino&aacute;cidos de la pel&iacute;cula adherida sobre la superficie de las dentaduras era muy similar a la de la pel&iacute;cula que se adhiere sobre la superficie del esmalte dental. Cabe destacar adem&aacute;s que, en la pel&iacute;cula adquirida adherida sobre la superficie de las dentaduras en los pacientes con E.S.P. se detectaron componentes adicionales del suero, productos de degradaci&oacute;n y <I>C. albicans</I>, los cuales no se identificaron en los pacientes sanos<SUP>33</SUP>.</font>           </P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se ha demostrado que hay una significativa alta densidad y frecuencia de colonizaci&oacute;n por <I>Candida</I> mayormente en sujetos con E.S.P. que en individuos sanos. Asimismo, se sabe que <I>Candida</I> puede adherirse  tanto a la superficie de la mucosa bucal como a la de las dentaduras<SUP>34,35</SUP>. Tambi&eacute;n se ha comprobado que la prevalencia de <I>C. albicans</I> en la dentadura es significativamente mayor que en la mucosa del paladar en pacientes con E.S.P<SUP>35</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Avila<SUP>36</SUP> en su estudio, concluy&oacute;  que existe cierta diferencia entre la flora microbiana bucal de un paciente dentado y la de un ed&eacute;ntulo, tanto en su organizaci&oacute;n como en la composici&oacute;n de la misma. En pacientes con dientes, la flora est&aacute; formada  predominantemente por cocos con escasos hongos. Por el contrario, en individuos portadores de pr&oacute;tesis total, se encontr&oacute; inversi&oacute;n de la flora microbiana, ya que hay aumento del n&uacute;mero de hongos y disminuci&oacute;n de las formas coc&aacute;ceas. Del mismo modo, refiere que la proliferaci&oacute;n de bacterias sobre la dentadura se encuentra altamente influenciada por 2 factores determinantes: la higiene bucal y la dieta empleada.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Est&aacute; claramente demostrado que no existen diferencias significativas en la morfolog&iacute;a de la placa dental que se adhiere a la mucosa del paladar, tanto en pacientes con E.S.P. relacionada con dentaduras como en pacientes cl&iacute;nicamente sanos. En todos los casos, la superficie de la placa dental estaba compuesta predominantemente por bacterias en forma de cocos y bacilos y solamente por algunas levaduras<SUP>37</SUP>.</font> </P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Davenport<SUP>38</SUP> realiz&oacute; un estudio donde tom&oacute; muestras tanto del paladar como de la pr&oacute;tesis en 50 pacientes con E.S.P. El encontr&oacute; abundancia de <I>Candida </I>en todas las muestras. La mayor proporci&oacute;n de c&eacute;lulas de <I>Candida</I> fueron vistas sobre las muestras de las dentaduras  que sobre las de mucosa. Esto sugiere que el tratamiento intentado para reducir la cantidad de <I>Candida</I> presente, deber&iacute;a estar dirigido tanto a la mucosa afectada como a la dentadura.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Por otra parte, en el mismo estudio se indica que la E.S.P. est&aacute; asociada con una proliferaci&oacute;n de <I>Candida</I>, la cual se encuentra principalmente dentro de la matriz de la placa m&aacute;s que encima de la placa depositada sobre la dentadura<SUP>38</SUP>.</font> </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se ha relacionado la presencia de <I>C. albicans</I> con la incidencia de E.S.P. en pacientes ancianos, encontr&aacute;ndose 75,53% de incidencia de este hongo en portadores de pr&oacute;tesis sin E.S.P. Esa incidencia se elev&oacute; a 96,77% en ancianos portadores de pr&oacute;tesis con manifestaciones de E.S.P. De la misma forma se demostr&oacute;  experimentalmente, que largos per&iacute;odos de contacto de la pr&oacute;tesis con la mucosa bucal, aumentan la posibilidad de que productos del metabolismo de <I>C. albicans</I> causen inflamaci&oacute;n<SUP>39</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Para demostrar el papel que juega <I>C. albicans</I> en la etiolog&iacute;a de la E.S.P., se tomaron muestras de paladar en 41 pacientes (31 con E.S.P. y 10 como grupo control), encontr&aacute;ndose esta especie como agente causal de E.S.P. en 35,4% de los cultivos en donde se demostr&oacute; el crecimiento de levaduras<SUP>40</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se ha establecido que en sujetos con pr&oacute;tesis nuevas (sobre una semana de instaladas), no les fue aislada <I>C. albicans</I>, mientras que en sujetos con pr&oacute;tesis muy usadas (sobre 1 mes de instaladas), fue conseguida esta especie en un 42,8% de los casos, habl&aacute;ndose de la presencia de E.S.P. en estos &uacute;ltimos pacientes<SUP>41</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Berdicevsky y col.<SUP>42</SUP> compararon la presencia de <I>Candida</I> en la cavidad bucal de portadores de dentaduras, con un grupo control dentados, del mismo sexo y la misma edad que los primeros. Encontraron <I>Candida</I> en 88% de portadores de dentadura, en contraste con 52% conseguido en el grupo control. No hallaron diferencias en  la qu&iacute;mica de la saliva entre los portadores de dentaduras y el grupo dentado portador del hongo.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Por su parte, Cardash y col.<SUP>27</SUP> determinaron la prevalencia de <I>C. albicans</I> en sujetos portadores de dentaduras que se encontraban en un hospital geri&aacute;trico de Israel. Se seleccionaron 37 pacientes, de los cuales 25 ten&iacute;an E.S.P. y los 12 restantes ten&iacute;an la mucosa bucal cl&iacute;nicamente normal. La prevalencia de <I>C. albicans</I> fue demostrada en todos los pacientes (100%) con E.S.P. y en 82% de los pacientes del grupo control. Tambi&eacute;n refirieron que se encontraron grandes cantidades de levaduras del G&eacute;nero <I>Candida</I> en los pacientes con E.S.P. Estos microorganismos se hallaron en grandes concentraciones en la base de las dentaduras, m&aacute;s no as&iacute; en el paladar de los sujetos estudiados. Se&ntilde;alaron adem&aacute;s que se encontr&oacute; <I>Candida</I> en zonas de la base de las dentaduras que no estaban en contacto con la lesi&oacute;n.</font></P>     <P ALIGN="JUSTIFY">      <font face="Times New Roman" size="3">      En Venezuela, se ha reportado la presencia de <I>C. albicans</I> en pacientes con E.S.P. portadores de pr&oacute;tesis totales y parciales removibles en una alta proporci&oacute;n, tanto en muestras tomadas de la mucosa bucal afectada, como en la dentadura, no encontr&aacute;ndose correlaci&oacute;n entre h&aacute;bitos de higiene bucal y positividad para esta especie<SUP>43</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Renner y col.<SUP>14</SUP> realizaron una investigaci&oacute;n para determinar si las concentraciones de <I>C. albicans</I> sobre la mucosa del  paladar constitu&iacute;an un diagn&oacute;stico significativo en la identificaci&oacute;n y tratamiento de la E.S.P. Concluyeron, que  pacientes  positivos a <I>C. albicans</I>, con concentraciones de 1 x 10<SUP>2</SUP> organismos por cm<SUP>2</SUP>, por material de muestras, no se medican; porque en el grupo control (pacientes sanos sin E.S.P.)  hallaron  esos resultados. En cambio, en pacientes con E.S.P. se obtuvieron cifras de 1 x 10<SUP>4</SUP> organismos por cm<SUP>2</SUP>, siendo necesario en estos pacientes la indicaci&oacute;n de terapia antimic&oacute;tica, ya que <I>C. albicans</I> infecta cuando alcanza concentraciones superiores a 1 x 10<SUP>4</SUP>  organismos por cm<SUP>2</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Se ha se&ntilde;alado en un estudio que es la propia superficie de la dentadura superior, la que sirve de principal reservorio de la levadura en pacientes con Candidiasis Cr&oacute;nica Sub-Prot&eacute;sica, pero se desconocen los mecanismos por los cuales <I>C. albicans</I> se adhiere a la superficie del acr&iacute;lico. Sin embargo, se hece referencia en este mismo estudio que la incubaci&oacute;n de <I>C. albicans</I> en sacarosa y glucosa por 18 horas, aument&oacute; significativamente su adhesi&oacute;n al acr&iacute;lico<SUP>44</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Se han identificado diversas especies de <I>Candida</I> en pacientes con E.S.P., tanto a partir de muestras provenientes de paladar como de la placa dental que se encuentra en la superficie interna de las pr&oacute;tesis. La especie que se aisl&oacute; con mayor frecuencia fue <I>C. albicans</I>, en tanto que, otras especies halladas fueron: <I>C. tropicalis</I>, <I>C. glabrata</I>, <I>C. parapsilosis</I> y <I>C.</I> <I>guillermondi</I><SUP>45,46</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Tambi&eacute;n se ha se&ntilde;alado la posibilidad de que la colonizaci&oacute;n de las superficies de la boca (incluyendo la superficie de ajuste de las dentaduras) por parte de <I>Candida</I>, puede servir como reservorio para la diseminaci&oacute;n de procesos patol&oacute;gicos tales como neumon&iacute;a por aspiraci&oacute;n e infecciones gastrointestinales<SUP>47</SUP>.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Destacan algunas investigaciones que el tipo de material empleado en la construcci&oacute;n de la pr&oacute;tesis, tambi&eacute;n puede influir en la acumulaci&oacute;n de levaduras sobre las dentaduras y sobre el paladar<SUP>14,48</SUP>. Es as&iacute; como la cantidad de levaduras es mayor, tanto en el paladar como en la dentadura de los sujetos portadores de pr&oacute;tesis obturadoras de silicona que en los sujetos portadores de pr&oacute;tesis obturadoras de acr&iacute;lico. Estas diferencias pudieran deberse a diversos factores. La silicona es rugosa y porosa por lo que es m&aacute;s dif&iacute;cil su limpieza; esto pudiera favorecer la formaci&oacute;n inicial de placa y evita que los microorganismos sean desalojados de la pr&oacute;tesis. Tambi&eacute;n hay que a&ntilde;adir el hecho de que los obturadores de silicona son inestables y se mueven considerablemente durante su funcionamiento y con el habla, por lo que ha de causar irritaci&oacute;n y trauma en el paladar. Estos cambios, seg&uacute;n lo expresan los autores antes nombrados, pueden estar asociados con una alta incidencia de E.S.P. y de colonizaci&oacute;n por <I>Candida</I><SUP>49</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    La rugosidad presente en las pr&oacute;tesis vencidas y la presencia de detritus alimenticios, facilita la adherencia y la maduraci&oacute;n de la placa dental a la pr&oacute;tesis. As&iacute;, la placa se adhiere a la superficie del acr&iacute;lico y a la placa se adhiere <I>Candida</I><SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <font face="Times New Roman" size="3">     Las levaduras se adhieren f&aacute;cilmente a las irregularidades de la superficie de acr&iacute;lico de las dentaduras. Cuando las condiciones inherentes al medio ambiente bucal, tales como pH y factores nutricionales son &oacute;ptimas, su crecimiento es promovido<SUP>49</SUP>.</font></P>     <P ALIGN="JUSTIFY">        <font face="Times New Roman" size="3">        Otros microorganismos tales como <I>Pseudomonas, Klebsiella </I>y <I>Neisseria,</I> pueden migrar al  interior del acr&iacute;lico  por  muchos  mecanismos y llegar a una profundidad de 1,2 mm. Este fen&oacute;meno de migraci&oacute;n puede atribuirse a la utilizaci&oacute;n metab&oacute;lica del carbono de la resina acr&iacute;lica por parte de los microorganismos<SUP>8</SUP>.</font></P>     <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Es as&iacute; como, las rugosidades de la superficie del acr&iacute;lico y la higiene defectuosa favorecen la adhesi&oacute;n de la placa dental Sub-Prot&eacute;sica, penetrando los microorganismos dentro de la resina. De esta forma, la pr&oacute;tesis constituye un reservorio de microorganismos que facilita la aparici&oacute;n de E.S.P.<SUP>38</SUP>.</font></P>     <P ALIGN="JUSTIFY">   <font face="Times New Roman" size="3">   Recientemente, se hizo un estudio donde se compar&oacute; la adherencia de <I>C. albicans</I> sobre la superficie del pol&iacute;mero de base de las pr&oacute;tesis dentales y sobre la superficie de la resina de base de las pr&oacute;tesis reforzada con fibras de vidrio-E, pudi&eacute;ndose comprobar que dicha adherencia fue menor sobre las fibras de vidrio-E que sobre la matriz de pol&iacute;mero de las pr&oacute;tesis<SUP>50</SUP>.</font></P>     <P ALIGN="JUSTIFY">      <font face="Times New Roman" size="3">      Se piensa que las interacciones que permiten la coagregaci&oacute;n entre <I>C. albicans</I> y bacterias de la cavidad bucal, como <I>Streptococcus sanguis </I>y <I>Actinomyces naeslundii</I>, juegan un papel crucial en la colonizaci&oacute;n por parte de estos microorganismos sobre la superficie de acr&iacute;lico de las dentaduras, conllevando a la aparici&oacute;n de E.S.P.<SUP>51</SUP>.</font><I> </P> </I>    <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Girard y col.<SUP>52</SUP> reportaron  que <I>C. albicans</I>, prolifera en el espacio entre la mucosa y la pr&oacute;tesis en pacientes con E.S.P. Esta proliferaci&oacute;n pareciera ser un factor preponderante en el desarrollo de la enfermedad. Sostienen estos autores que si bien otras especies del G&eacute;nero <I>Candida </I>han sido aisladas recientemente de pacientes con E.S.P. (<I>C. tropicalis </I>y<I> C. glabrata</I>), todo lleva a creer que solamente <I>C. albicans</I> posee una superficie adherente importante para la superficie de la pr&oacute;tesis, facilitando as&iacute; su multiplicaci&oacute;n. No obstante, la infecci&oacute;n por <I>Candida</I> no constituye por s&iacute; s&oacute;la la causa &uacute;nica de E.S.P.<SUP>7</SUP>.</font></P>     <P ALIGN="JUSTIFY">     <B><font face="Times New Roman" size="3">CONCLUSI&Oacute;N.</font></P> </B>    <P ALIGN="JUSTIFY">    <font face="Times New Roman" size="3">    Tomando en consideraci&oacute;n todo lo expresado anteriormente, es obvio se&ntilde;alar que la placa dental que se forma sobre la superficie interna de la pr&oacute;tesis, constituye como tal un factor etiol&oacute;gico local altamente significativo en la patogenia de la E.S.P., por lo que su remoci&oacute;n es fundamental en la prevenci&oacute;n de esta patolog&iacute;a.</font></P>     ]]></body>
