MATERIALES Y MÉTODOS: Previo consentimiento informado se les realizó la Historia Clínica 30 embarazadas que incluyó examen clínico para diagnosticar las manifestaciones bucales. El análisis estadístico fue descriptivo utilizando la Clasificación Jerárquica de Ward. RESULTADOS: la manifestación bucal observada con mayor frecuencia en las embarazadas fue la gingivitis 16 (53;3%), seguida de candidiasis pseudomembranosa y queilitis angular ambas con 9 (30%), aftas recurrentes 6 (20%), eritema marginal lineal 5 (16,67%), xerostomia y candidiasis eritematosas 3 (6,67%) y parotiditis con 1 (3,3%) .CONCLUSIÓN: Las manifestaciones bucales observadas en las embarazadas VIH/SIDA resaltan la importancia del odontólogo en el diagnóstico y manejo de las mismas en nuestra consulta.]]></p></abstract>

Tests concerning proportions were descriptive using jerarquica ward classification. RESULTS: observed oral manifestation most frequently in the pregnant women was gingivitis 16 (53; 3%),, candidiasis pseudomembranosa 9 (30%), queilitis angular 9 (30%), la aftas recurrentes con 6 (20%), lineal erithema con 5 (16,67%), la xerostomia y candidiasis eritematosas 3 (6,67%); parotiditis 1 (3,3%) CONCLUSION: The presence of oral manifestations that we saw in pregnant HIV women gives relevance to the dentistry in the diagnosis and handling of the infection.]]></p></abstract>

<body><![CDATA[<p align="justify"><b><span style="font-family: Verdana"><font size="2"> INTRODUCCIÓN:</font></span></b></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">En las  embarazadas las&nbsp; manifestaciones del VIH/sida son frecuentes en cabeza, cuello y  cavidad bucal estas incluyen infecciones oportunistas, neoplasias y adenopatías  reactivas difusas. Algunos tipos de lesiones bucales que aparecen como primer  signo clínico de la infección por VIH son indicadores importantes del estadio  VIH y del estado de inmunosupresión (3;4)</font></span></p>     <p style="margin-left: 0cm; margin-right: 0cm; margin-top: 0cm; margin-bottom: .0001pt" align="justify"> <span style="font-family: Verdana"><font size="2">El espectro de las  manifestaciones orales de la infección por VIH depende del estado de  inmunosupresión, la vía de contagio y algunos factores étnicos, geográficos,  sociales y demográficos en regiones concretas (5;6)</font></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">A medida  que la epidemia de VIH/sida se traslada cada vez más a la población de mujeres,  también va en aumento el número de casos de mujeres embarazadas infectadas. Esta  situación particular es una oportunidad importante para hacer el diagnóstico de  la mujer durante el control prenatal y prevenir los casos perinatales, por lo  que se hace necesario solicitar prueba del VIH siempre en la primera visita  prenatal, siempre con consentimiento informado (10)</font><span class="bodytext1"></span></span></p>     <p style="margin-left: 0cm; margin-right: 0cm; margin-top: 0cm; margin-bottom: .0001pt" align="justify"> <span class="bodytext1"><span style="font-family: Verdana"><font size="2">La  Situación Nacional de embarazadas se señala en el <a href="#tab">cuadro No 1</a>,  según el Informe de La República Bolivariana de Venezuela, periodo 2003 –  2005(16)</font></span></span></p>     <p style="margin-left: 0cm; margin-right: 0cm" align="center"><a name="tab"> <img border="0" src="/img/fbpe/aov/v47n3/art14tab.gif" width="305" height="189"></a></p>     
<p align="justify"><span class="bodytext1"><span style="font-family: Verdana"> <font size="2">Incidencia de Embarazadas VIH (+) registradas en Venezuela 2001 -  2005</font></span></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Mucho se ha  escrito en la literatura sobre la transmisión vertical de VIH/sida, al igual que  la relación entre las manifestaciones bucales y el VIH/sida pero poco hemos  encontrado &nbsp;en la literatura, sobre &nbsp;la presencia de manifestaciones bucales en  la embarazada VIH/sida por lo que nos propusimos hacer este estudio.</font></span></p>     <p style="text-autospace: none" align="justify"> <span style="font-family: Verdana"><font size="2">Toda mujer tiene derecho a  comprender las ventajas y los riesgos de someterse a la detección del VIH y de  participar en un programa nacional. La decisión de realizarse las pruebas de  detección y de participar en un programa nacional le corresponde enteramente a  ella. La negativa de la mujer a someterse a pruebas de detección del VIH o a  participar en un programa nacional, no debe limitar su acceso a atención médica,  ni estar sujeta a sanción legal alguna (20).</font></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">En  Venezuela según la Resolución 292- Ministerio de Salud y Desarrollo Social, 7 de  agosto de 2000, Gaceta Oficial No. 37.009, por la cual se instituyen las pruebas  de anticuerpos contra el VIH con carácter obligatorio para&nbsp; toda embarazada que  concurra a la consulta prenatal en centros públicos o privados, con el  diagnóstico, segundo trimestre, tercer trimestre y al momento del parto (21).&nbsp;</font></span></p>     ]]></body>
<body><![CDATA[<p align="justify"><b><span style="font-family: Verdana"><font size="2"> OBJETIVO:</font></span></b></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Determinar  las manifestaciones bucales más frecuentes en las embarazadas VIH/SIDA. que  acudieron al servicio de Ginecología y Obstetricia del Hospital Universitario de  Caracas de enero a diciembre 2006</font></span></p>     <p align="justify"><b><span style="font-family: Verdana"><font size="2"> MATERIALES Y MÉTODOS:</font></span></b></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Estudio&nbsp;  descriptivo y transversal en el cual&nbsp; recolectamos una serie de datos, obtenidos  de un examen clínico de los tejidos blandos bucales, en embarazadas VIH (+) para  especificar las características de las manifestaciones bucales observadas.</font></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">El criterio  de exclusión incluye embarazadas en el último trimestre de la gestación, que  fueron diagnosticadas para infección por VIH/Sida, y cuya nacionalidad fuera  extranjera.</font></span></p>     <p style="margin-left: 0cm; margin-right: 0cm; margin-top: 0cm; margin-bottom: .0001pt" align="justify"> <span lang="ES-TRAD" style="font-family: Verdana"><font size="2">Se seleccionó  para este estudio a todas las embarazadas venezolanas con diagnóstico  confirmatorio de infección por VIH, con menos de seis meses de gestación que  &nbsp;acudieron a la consulta de alto riesgo del Servicio de Obstetricia del Hospital  Universitario de Caracas y fueron referidas para su control bucal al Centro de  Atención a Pacientes con Enfermedades Infectontagiosas “Dra. Elsa La Corte” de  la Facultad de Odontología de la Universidad Central de Venezuela, durante el  año 2006. La muestra quedó constituida por 30 embarazadas VIH (+)</font></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Se les  realizó la Historia Clínica, siguiendo el protocolo establecido en el CAPEI/UCV;  el cual consiste en:</font></span></p>     <p align="justify"><font size="2"><span style="font-family: Verdana">1)</span></font><span style="font-style: normal; font-variant: normal; font-weight: normal; font-family: Verdana"><font size="2">&nbsp;&nbsp;</font></span><span style="font-size: 11.0pt; font-family: Calibri"><font size="2"><span style="font-family: Verdana">Antes  del examen: Interconsulta con el obstetra e infectologo tratante, consentimiento  escrito, en el cual se describía con palabras sencillas el estudio a realizar,  efectuar buches&nbsp; por 1 minuto con enjuague bucal</span></font></span></p>     <p align="justify"><font size="2"><span style="font-family: Verdana">2)</span></font><span style="font-style: normal; font-variant: normal; font-weight: normal; font-family: Verdana"><font size="2">&nbsp;</font></span><span style="font-size: 11.0pt; font-family: Calibri"><font size="2"><span style="font-family: Verdana">Interrogatorio  para determinar variables: sociodemográficas, antecedentes personales y  familiares, fecha del diagnóstico y status clínico e inmunológico de la  infección que fue imposible determinar en el momento del examen pues las  personas involucradas no disponían de estos estudios.</span></font></span></p>     <p align="justify"><font size="2"><span style="font-family: Verdana">3)</span></font><span style="font-style: normal; font-variant: normal; font-weight: normal; font-family: Verdana"><font size="2">&nbsp;</font></span><span style="font-size: 11.0pt; font-family: Calibri"><font size="2"><span style="font-family: Verdana">Examen  clínico: se realizó con la paciente sentada en la unidad dental y con luz  apropiada, se examinó de manera sistemática&nbsp; la cavidad bucal&nbsp; y sus anexos:  cara interna de carrillos, lengua, piso de la boca, paladar blando, paladar  duro, encías, ganglios cervicofaciales, músculos, anexos y saliva.</span></font></span></p>     ]]></body>
<body><![CDATA[<p align="justify"><span style="font-family: Verdana"><font size="2">El examen  clínico fue realizado por un solo examinador, responsable del estudio,  especialista en Odontología Infantil de la Universidad Central de Venezuela&nbsp;  previa calibración intraevaluador e interevaluador por un Patólogo&nbsp; Bucal e  Infectólogo, con siete años de experiencia en exámenes bucales en el CAPEI/UCV.</font></span></p>     <p style="margin-left: 0cm; margin-right: 0cm; margin-top: 0cm; margin-bottom: .0001pt" align="justify"> <span lang="ES-TRAD" style="font-family: Verdana"><font size="2">Se tomó la  clasificación de Manifestaciones Bucales por Infección de VIH/SIDA emanada por  los organismos internacionales Organización Panamericana de la Salud, Oficina  Sanitaria Panamericana y Oficina Regional de la Organización Mundial de la Salud  1995 (18)</font></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">El análisis  estadístico se realizó mediante el método de Clasificación Jerárquica de Ward,  aplicado a todas las manifestaciones bucales estudiadas (<a href="#gra1">Gráfico  No 1</a> y <a href="#tab1">Tabla No 1</a>).</font></span></p>     <p align="center"><a name="gra1"> <img border="0" src="/img/fbpe/aov/v47n3/art14gra1.gif" width="471" height="375"></a></p>

<p align="justify"><span style="font-family: Verdana; color: black"> <font size="2">Las edades estuvieron comprendidas entre 14 y 42 años de edad,  &nbsp;en el <a href="#gra1">grafico No 1</a> &nbsp;se observa la distribución de frecuencia y su curva normal,  además se puede ver que la distribución de los datos tiene tres moda, también se  puede notar que los datos están dispersos de la media, es decir la </font> </span><font face="Verdana" size="2">desviación típica es grande.<span style="color: black">  Por otra parte, como </span></font><font face="Verdana"> <span style="position: relative; top: 6.0pt"><font size="2"><i>Q</i><sub>3</sub>-<i>Q</i><sub>2</sub>=5.5&gt;3.75=<i>Q</i><sub>2</sub>-<i>Q</i><sub>1</sub></font></span></font><span style="font-size: 11.0pt; font-family: Calibri"><!--[if gte mso 9]><![endif]--></span><font size="2"><span style="font-family: Verdana">,  entonces podemos concluir que la distribución de los datos es <span style="color: black">asimétrica positiva</span>. Ubicándose el mayor  número de embarazadas en el grupo entre 24 y 27 años de edad, con una media 27,2  y una mediana de 26,5 años</span></font></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Observamos  en el cuadro No 2, que la manifestación bucal observada con mayor frecuencia en  las embarazadas fue la gingivitis leve con 16 (53;3%) casos, luego le siguen la  candidiasis pseudomembranosa y la queilitis angular ambas con 9 (30%), la aftas  recurrentes con 6 (20%), la eritema lineal con 5 (16,67%), la xerostomia&nbsp; y  canadidiasis eritematosas 3 (6,67%) y por ultimo la parotiditis con 1 (3,3%) .</font></span></p>     <p align="justify"><b><span style="font-family: Verdana"><font size="2"> DISCUSIÓN:</font></span></b></p>     <p style="text-autospace: none" align="justify"> <span style="font-family: Verdana"><font size="2">En relación a la edad de las  embarazadas objeto de esta investigación 15(50%) estaban entre 14 y 25 años de  edad, sin embargo el grupo más alto se ubico entre 24 y 27 años.</font><span style="color: #231F20"><font size="2">  Al igual que los informes internacionales que reportan que además de cerca de la  mitad de las personas infectadas con el VIH son mujeres que se infectan&nbsp; a un  ritmo más acelerado que los hombres. En varios países de África y el Caribe, las  tasas de infección entre las mujeres menores&nbsp; de 24 años es de dos a seis veces  mayor que la de los hombres del mismo grupo de edad. (23)</font></span></span></p>     <p align="justify"><span style="font-family: Verdana"><font size="2">Debemos  resaltar que la gingivitis y agrandamientos gingivales han sido reportados por  diversos autores como trastornos frecuentes durante el embarazo de una mujer,  que para algunos médicos y odontólogos es considerado como “normal” el que  sangren y se agranden las encías de sus pacientes. La inflamación gingival puede  alcanzar cifras de prevalencia entre el 50 y el 98.25%. (30; 31;32;33; 34). Sin  embargo en este estudio a pesar que la manifestación bucal más frecuente fue la  gingivitis, reportándose con un 53,33% (16), se encuentra por debajo del  reportado en estudios de embarazadas seronegativas como López y cols. en el  2002(35).</font></span></p>     ]]></body>
<body><![CDATA[<p align="justify"><span style="font-family: Verdana"><font size="2">En el 2004  (36) La candidiasis bucal es una de la presentaciones clínicas de mayor  prevalencia entre las infecciones causadas por <i>Candida</i>, sobre todo en  pacientesVIH/SIDA, sin embargo en&nbsp; este estudio la candidiasis bucal se observo  en: un 30% (9) pseudomembranosa, 30%&nbsp; (9) y 6,6% (3) eritematosa, ubicándose por  debajo de lo reportado por otros estudios, en los cuales se ha descrito como la  forma más frecuente de manifestaciones bucales en pacientes VIH/SIDA es  candidiasis, como forma aguda pseudomembranosa, típicamente con placas blancas  superficiales en la mucosa bucal y lengua, o como queilitis angular. <span style="color: red">&nbsp;</span>Es interesante resaltar que nuestros resultados  son similares a los reportados por Gileva y cols en el 2004 en pacientes  VIH/sida (37)</font></span></p>     <p style="margin-left: 0cm; margin-right: 0cm; margin-top: 0cm; margin-bottom: .0001pt" align="justify"> <span style="font-family: Verdana"><font size="2">Las aftas o ulceras &nbsp;orales  recurrentes aparecen entre un 2 y un 6% &nbsp;de la población infectada (38;39). En  este estudio se observaron&nbsp; en un 20% (6) Estas lesiones aparecen como&nbsp; áreas  necróticas en la mucosa bucal en labio, paladar blando y lengua, casi siempre  con&nbsp; dolor. Con frecuencia pueden sufrir sobreinfecciones. Los factores  predisponentes son: el estrés, déficit vitamínico, traumatismos, disfunción  inmunitaria, infecciones víricas, alergias o cambios hormonales como ocurre  durante el embarazo (39). Otras úlceras inespecíficas surgen y proliferan por la  faringe y el paladar, y no siguen el patrón reconocido de estomatitis aftosa  recurrente. Suelen aparecer en períodos agudos de la enfermedad dada las  alteraciones locales y sistémicas, provocadas por el VIH/SIDA. Otras son de  origen iatrogénico causadas por medicamentos o neutropenia.
