Los resultados indican que el 35% de éstos(as) inmigrantes presenta niveles altos de CT ( > o =  5,16 mmol/L) y el 51% concentraciones elevadas de LDL-C ( > o =  2,84 mmol/L). Asimismo, en el 36% de la muestra se evidenciaron niveles elevados de Tg (> 1,69 mmol/L) y solamente en el 15% concentraciones bajas de HDL-C (<= 0,90 mmol/L). La proporción de hombres con concentraciones elevadas de CT, LDL-C y Tg y niveles bajos de HDL-C fue mayor que la de mujeres; aunque esta diferencia no llegó a ser significativa. El 41% de la población estudiada presentó valores mayores de 4,5 para el Índice de Castelli (CT/HDL-C). En general, cerca del 80% de los (as) chinos (as) inmigrantes consumen vegetales 5-6 veces/semana y alrededor del 50% consumen frutas con esa misma frecuencia. Un alto porcentaje de los inmigrantes consumen alimentos fuente de proteína animal más de 3 veces/semana y alrededor del 23% consumen frecuentemente comidas rápidas y snacks. Nuestros datos sugieren que los inmigrantes chinos mantienen algunas características de su cultura alimentaria. Sin embargo, han modificado su patrón alimentario convirtiéndolo en uno más aterogénico. Es importante que los nutricionistas y otros profesionales del área de la salud, evalúen con mayor detalle los factores asociados al cambio del patrón alimentario en inmigrantes, con el propósito de evitar un aumento en las tasas de morbi-mortalidad por enfermedad coronaria en ese grupo de población.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">Recibido: 15-01-2001 Aceptado: 26-04-2002</P> <B>     <P ALIGN="left">INTRODUCCION</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La prevalencia de enfermedad coronaria en China es una de las m&aacute;s bajas del mundo, seg&uacute;n lo ha demostrado el proyecto MONICA de la Organizaci&oacute;n Mundial de la Salud (1,2). Las caracter&iacute;sticas de su dieta han contribuido con este fen&oacute;meno. El consumo de grasa en China es menos del 50% de lo reportado en  los Estados Unidos, mientras que el consumo de fibra es  tres veces mayor. Asimismo, el consumo de prote&iacute;na  de origen animal es de tan s&oacute;lo un 10% del consumo reportado en los Estados Unidos (3). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La tasa de mortalidad por enfermedad coronaria es respectivamente 16,7 y 5,6 veces mayor en los hombres y mujeres estadounidenses comparado con sus contrapartes de China (4). Sin embargo, los chinos que han inmigrando a los Estados Unidos han modificado su estilo de vida y cultura alimentaria incorporando una serie de factores aterog&eacute;nicos a la misma (5-7). Esto se ha manifestado como un cambio en su perfil de morbi-mortalidad, pues la occidentalizaci&oacute;n de la dieta esta fuertemente asociada con una mayor susceptibilidad a los efectos pro-aterog&eacute;nicos y a un incremento en la frecuencia de la enfermedad coronaria en individuos  asi&aacute;ticos (8).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp; Recientemente se ha publicado que no existen diferencias importantes en los niveles de colesterol entre los asi&aacute;ticos inmigrantes y aquellos nacidos en los Estados Unidos (5). Adem&aacute;s, se ha informado que la incidencia de infarto al miocardio ha incrementado de forma importante entre los asi&aacute;ticos que inmigraron a este pa&iacute;s (9-11).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La enfermedad cardiovascular en Costa Rica tiene una prevalencia tan elevada como la reportada en Estados Unidos y Canad&aacute; (12). Por tanto, los chinos que han inmigrado al pa&iacute;s  est&aacute;n expuestos a la dieta aterog&eacute;nica caracter&iacute;stica de los costarricenses (13) y por ende a un mayor riesgo de desarrollar alteraciones en su perfil lip&iacute;dico.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; El objetivo de este estudio fue  analizar el perfil de l&iacute;pidos y lipoprote&iacute;nas como indicador de riesgo coronario en chinos inmigrantes a Costa Rica.</P> <B>    <P ALIGN="left">MATERIALES Y METODOS</P> </B>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Para lograr la participaci&oacute;n de los inmigrantes chinos en el estudio, se recurri&oacute; a la Asociaci&oacute;n China de Costa Rica para lograr la motivaci&oacute;n de los mismos. Se reclutaron en total 120 chinos adultos. Sin embargo, solamente 70 cumplieron con el requisito de inclusi&oacute;n en el estudio el cual fue tener ascendencia china de primera y segunda generaci&oacute;n. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La firma de cada participante en el consentimiento informado fue requerida para su inclusi&oacute;n en el estudio tal y como lo establecen el Comit&eacute; &Eacute;tico Cient&iacute;fico del Instituto Costarricense de Investigaci&oacute;n y Ense&ntilde;anza en Nutrici&oacute;n y Salud (INCIENSA) y de la Universidad de Costa Rica (UCR).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La prevalencia de sobrepeso se determin&oacute; utilizando el &Iacute;ndice  de  Masa  Corporal (IMC). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Para esto los participantes fueron pesados y medidos siguiendo los lineamientos definidos en Lohman et al. (14).  La talla fue medida al 0,1 cm m&aacute;s cercano y el peso al 0,1 kg m&aacute;s pr&oacute;ximo.  Dos mediciones fueron independientemente obtenidas para peso y talla.  El promedio de ambas lecturas fue utilizado para el an&aacute;lisis de los datos.  El punto de corte utilizado para determinar sobrepeso fue 23 kg/m<SUP>2</SUP>, de acuerdo a lo recomendado para la prevenci&oacute;n de la enfermedad coronaria en asi&aacute;ticos (15).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; El perfil de l&iacute;pidos se determin&oacute; a partir de una muestra de sangre extra&iacute;da por venipuntura despu&eacute;s de 12-14 horas de ayuno utilizando tubos vacutainer (Becton, Dickinson &amp; Co).  El suero fue obtenido por centrifugaci&oacute;n a 6000 rpm/5 min a 25ºC.  El HDL-Colesterol (HDL-C) fue separado utilizando &aacute;cido fosfot&uacute;ngstico/Mg++ (Wiener Lab, Argentina) como reactivo precipitante y centrifugado a 2000 rpm/15 min. La reacci&oacute;n colorim&eacute;trica fue determinada en un espectofot&oacute;metro Shimadzu UV1201 a 505nm y 25ºC. El Colesterol Total (CT) y los Triglic&eacute;ridos (Tg) fueron determinados por m&eacute;todos enzim&aacute;ticos (Wiener Lab, Argentina). El LDL-Colesterol (LDL-C) se calcul&oacute; utilizando la ecuaci&oacute;n de Friedewald et al. (16) Se utilizaron sueros control &quot;normal&quot; y &quot;alto&quot; (DMA Arlington TX) de origen humano para el control de calidad. En aquellos casos donde las muestras exced&iacute;an ± 2 DS el valor promedio del control, se realizaron nuevamente las determinaciones. Las concentraciones de CT, LDL-C, HDL-C y Tg fueron clasificadas de acuerdo a los puntos de corte recomendados para la prevenci&oacute;n de la enfermedad coronaria en asi&aacute;ticos (17). CT deseable: <FONT FACE=Symbol>&#60;</FONT>  4,39 mmol/L, CT l&iacute;mite: 4,39-5,15 mmol/L y CT elevado: <FONT FACE=Symbol>&#179;</FONT>  5,16 mmol/L.  LDL-C deseable: <FONT FACE=Symbol>&#60;</FONT>  2,33 mmol/L, LDL-C l&iacute;mite: 2,33-2,83 mmol/L y LDL-C elevado: <FONT FACE=Symbol>&#179;</FONT>  2,84 mmol/L. HDL-C deseable: <FONT FACE=Symbol>&#62;</FONT> 0,90 mmol/L y Tg deseable: <FONT FACE=Symbol>&#60;</FONT>  1,69 mmol/L. Se calcul&oacute; el Indice de Castelli utilizando la f&oacute;rmula: CT/HDL-C. Valores mayores de 4,5 se consideraron de riesgo coronario.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; El patr&oacute;n de alimentaci&oacute;n se evalu&oacute; utilizando una frecuencia de consumo de alimentos autoadministrada. El formulario contempl&oacute; 50 alimentos incluidos en once grupos de alimentos (3-11 alimentos/grupo). El instrumento fue validado previamente en poblaci&oacute;n similar a la del estudio. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Los datos fueron analizados en el programa SPSS para Windows utilizando las pruebas t-student para comparar los promedios de las variables continuas y Chi-cuadrado para las variables categ&oacute;ricas. Un valor de p<FONT FACE=Symbol>&#60;</FONT> 0,05 fue considerado significativo.</P> <B>    <P ALIGN="left">RESULTADOS</P> </B>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En total se incluyeron en el estudio 50 adultos chinos, 40% hombres y 60% mujeres con edades entre  30 y 70 a&ntilde;os. La edad promedio fue  42 <FONT FACE=Symbol>&#177;</FONT>   3,5  a&ntilde;os, sin diferencia entre sexos. El IMC promedio fue 24,2 <FONT FACE=Symbol>&#177;</FONT>  3,8 kg/m<SUP>2</SUP>.  El 65% de los hombres (n=13) y el 47% de la mujeres (n=14)  presentaron sobrepeso (IMC <FONT FACE=Symbol>&#62;</FONT> 23) <a href="#tab1">(Tabla 1)</a>.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Los niveles promedio de CT, LDL-C y HDL-C fueron 4,68 ± 1,0 mmol/L; 2,84 ± 0,9 mmol/L y 1,16 ± 0,3 mmol/L respectivamente. La concentraci&oacute;n media de Tg fue 1,44 ± 0,8 mmol/L. No se observaron diferencias significativas entre los valores promedio de los l&iacute;pidos al analizar los datos por sexo (<a href="#tab1">Tabla 1</a>).  </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En la <a href="#tab2">Tabla 2</a> se presenta la distribuci&oacute;n de chinos (as) inmigrantes seg&uacute;n los puntos de corte para l&iacute;pidos establecidos para la prevenci&oacute;n de la enfermedad coronaria en asi&aacute;ticos. Solamente el 40% de los chinos (as) inmigrantes present&oacute; concentraciones deseables de CT (<FONT FACE=Symbol>&#60;</FONT>  4,39 mmol/L) y cerca del 25% niveles deseables de LDL-C (<FONT FACE=Symbol>&#60;</FONT>  2,33 mmol/L). El 35 % de los (as) inmigrantes present&oacute; niveles altos de CT (<FONT FACE=Symbol>&#179;</FONT>  5,16 mmol/L) y el 51% concentraciones elevadas de LDL-C (<FONT FACE=Symbol>&#179;</FONT>  2,84 mmol/L). <body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En promedio el Indice de Castelli (CT/HDL-C) fue 4,3 <FONT FACE=Symbol>&#177;</FONT>  1,2; no obstante este valor fue significativamente mayor en los hombres con respecto al  promedio evidenciado en las mujeres (4,8 <FONT FACE=Symbol>&#177;</FONT>  1,5 y 3,9 <FONT FACE=Symbol>&#177;</FONT>  1,2 respectivamente, p= 0,018). Adem&aacute;s, la proporci&oacute;n de hombres con un Indice de Castelli <FONT FACE=Symbol>&#62;</FONT> 4,5 fue significativamente mayor que la de mujeres (53% y 29% respectivamente, p= 0,0012).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En la <a href="#tab3">Tabla 3</a> se presentan  los resultados de la frecuencia de consumo de alimentos. Cerca del 80% de los (as) chinos (as) inmigrantes consumen vegetales 5-6 veces/semana. Asimismo, alrededor del 30% consume con la misma frecuencia productos del mar y cerca del 20% consume pollo con la misma frecuencia. En general, alrededor del 50% de los inmigrantes consumen frutas 5-6 veces/semana. Sin embargo, la proporci&oacute;n de hombres y mujeres que mantienen este patr&oacute;n de consumo es significativamente diferente (42% y 55% respectivamente, p=0,001). Por otro lado, solamente cerca del 30% de los inmigrantes  consumen 1-2 veces por semana productos de soya (tofu, nato, miso y leche de soya), el 70% restante no los consume. Cabe destacar que alrededor del 23% de los inmigrantes consumen frecuentemente comidas r&aacute;pidas (hamburguesas, perros clientes, papas fritas, pollo frito, pizza, burritos, tacos, empanadas y arreglados) y snacks (chocolates, estruidos de ma&iacute;z y trigo, pasteles rellenos, golosinas,  y reposter&iacute;a en pasta de hojaldre). La proporci&oacute;n de hombres que consumen estos alimentos 5-6 veces/ semana fue significativamente mayor que la proporci&oacute;n de mujeres que incluyen con esa frecuencia tales alimentos a la dieta (13% y 2% respectivamente, p=0,001). Cerca del 35% de los inmigrantes chinos consumen carne de res m&aacute;s de tres veces por semana y el 96%  utiliza exclusivamente aceite de soya para cocinar y el 4% manteca de palma africana. M&aacute;s del 60% nunca consume leguminosas, embutidos, gaseosas, comidas r&aacute;pidas y snacks. </P> <B>     <P ALIGN="left">DISCUSION</P> </B>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Este estudio evidencia que m&aacute;s del 50% de los inmigrantes chinos, principalmente los hombres, presentan un perfil lip&iacute;dico adverso. El nivel promedio de colesterol evidenciado en nuestra muestra es similar al observado en chinos residentes en Hong Kong (15), donde los niveles plasm&aacute;ticos de l&iacute;pidos son cercanos a los reportados en occidente. Para 1997 el nivel promedio de colesterol total en adultos de Hong Kong fue 5,04 mmol/L(15), tan s&oacute;lo 0,21 mmol/L menos que el promedio reportado para adultos estadounidenses (4). <body><![CDATA[<P ALIGN="JUSTIFY">1 </SUP>Incluye: leche de soya, tofu, miso y nato; <SUP>2</SUP> Incluye: hamburguesas, perros clientes, papas fritas, pollo frito, pizza, burritos, tacos, empanadas y arreglados. <SUP>3 </SUP>Incluye: chocolates, estruidos de ma&iacute;z y trigo, pasteles rellenos, golosinas,  y reposter&iacute;a en pasta de hojaldre.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Nuestros datos se&ntilde;alan que en el patr&oacute;n alimentario de estos inmigrantes, principalmente en el de los hombres se han incorporado alimentos ricos en grasa saturada propios de la cultura occidental como comidas r&aacute;pidas (perros calientes, hamburguesas, papas fritas, pollo frito, pizza, entre otros) y los snacks (chocolates, estru&iacute;dos de ma&iacute;z y trigo, pasteles rellenos, confites y otros). Esto podr&iacute;a explicar, al menos en parte, por qu&eacute; el perfil de l&iacute;pidos y lipoprote&iacute;nas tiende a ser m&aacute;s adverso en los hombres.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Adem&aacute;s, nuestros datos muestran un consumo frecuente de prote&iacute;na animal. En el &aacute;rea rural de China, donde habita m&aacute;s del 75% de la poblaci&oacute;n (1,2), el aporte de prote&iacute;na animal a la dieta es menos de 11% del consumo total de prote&iacute;nas (aproximadamente 66g/d) (3). De acuerdo con los resultados de la frecuencia de consumo, se puede se&ntilde;alar que en la dieta de los inmigrantes chinos el aporte de prote&iacute;na animal es mayor. Esto contribuye a explicar las concentraciones de l&iacute;pidos y lipoprote&iacute;nas evidenciados, pues ha sido demostrado que en chinos que mantienen una dieta tradicional, un consumo peque&ntilde;o de carne genera un incremento importante en el nivel de colesterol plasm&aacute;tico (3). Campbell,  <I>et al.</I> (4) han demostrado que el consumo de carne, est&aacute; asociado con un incremento en el nivel de la apolipoprote&iacute;na B, la cual es marcadamente aterog&eacute;nica (18).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Por otro lado, la limitada disponibilidad de algunos alimentos de soya (tofu, nato, miso y otros) en Costa Rica ha hecho que los inmigrantes modifiquen sin otra alternativa su patr&oacute;n de consumo. Algunos estudios indican que el consumo de soya puede ser utilizado como un marcador de factores asociados con inmigraci&oacute;n o con el grado de occidentalizaci&oacute;n de la dieta (19). De hecho, se ha evidenciado que el consumo de alimentos de soya se reduce en m&aacute;s del 90% en los inmigrantes asi&aacute;ticos (19). Este comportamiento alimentario se refleja en nuestros datos de consumo, pues solamente una minor&iacute;a de los inmigrantes chinos los consume y con una frecuencia de tan solo 1-2 veces/semana.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es interesante  destacar que m&aacute;s del 60% de los inmigrantes han dejado de consumir leguminosas, un alimento tradicional de su dieta (3), a pesar de la amplia disponibilidad de estos alimentos (incluyendo frijol de soya) en el mercado costarricense. Esto es particularmente importante pues el consumo de leguminosas esta inversamente asociado con los niveles de la apolipoprote&iacute;na B (4). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La modificaci&oacute;n del patr&oacute;n alimentario de los inmigrantes chinos tiene importantes repercusiones en el perfil de l&iacute;pidos, pues la reducci&oacute;n del consumo de soya y leguminosas se asocia a un mayor riesgo coronario (20-,22). Asimismo, la inclusi&oacute;n de alimentos ricos en grasa saturada y prote&iacute;na animal, favorecen el desarrollo de las lesiones ateroscler&oacute;ticas (23). Esto es de mayor inter&eacute;s en los hombres, pues adem&aacute;s de presentar con mayor frecuencia un perfil lip&iacute;dico adverso, tambi&eacute;n mostraron una mayor prevalencia de sobrepeso. En Costa Rica se reporta para algunas &aacute;reas urbanas una prevalencia de obesidad en hombres adultos superior al 60% (24) y para mujeres  a nivel nacional una prevalencia del 46% (25). Adem&aacute;s, Klatsky y Armstrong al estudiar los factores de riesgo cardiovascular entre asi&aacute;ticos americanos del norte de California, encontraron tambi&eacute;n &iacute;ndices de masa corporal m&aacute;s altos en hombres que en mujeres (11).  De continuar con el estilo de vida actual, la incidencia de infarto al miocardio podr&iacute;a incrementarse en el grupo de sexo masculino, pues el exceso de grasa corporal,  es un factor de riesgo independiente de la enfermedad cardiovascular (26).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Para este estudio utilizamos los par&aacute;metros establecidos para evaluar las concentraciones de l&iacute;pidos y lipoprote&iacute;nas en habitantes del continente Asi&aacute;tico. Sin embargo, se podr&iacute;a pensar que debido a su inmigraci&oacute;n ser&iacute;a m&aacute;s conveniente analizar los datos seg&uacute;n los criterios occidentales del Programa Nacional de Educaci&oacute;n en Colesterol (NCEP) de los Estados Unidos (27). No obstante, los niveles establecidos para asi&aacute;ticos, est&aacute;n basados en sus h&aacute;bitos alimentarios, los cuales se asocian a una tasa de mortalidad por enfermedad coronaria inferior al 15% (3). Por tanto, consideramos que la evaluaci&oacute;n con puntos de corte m&aacute;s elevados, ser&iacute;a aceptar como adecuado el incremento de la aterogenicidad de la dieta. Adem&aacute;s, recientemente se ha evidenciado que la disfunci&oacute;n endotelial ocurre en individuos sanos con  niveles &quot;normales&quot; de colesterol (<FONT FACE=Symbol>&#60;</FONT>  5,17 mmol/L) (28,29). Se ha indicado, por lo tanto, que para prevenir completamente  la enfermedad coronaria deber&iacute;a mantenerse el colesterol plasm&aacute;tico en niveles inferiores a 3,88 mmol/L (4), un criterio m&aacute;s estricto que el par&aacute;metro establecido para asi&aacute;ticos (<FONT FACE=Symbol>&#60;</FONT>  4,39 mmol/L) (14).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es bien conocido  que la producci&oacute;n de estr&oacute;genos en las mujeres de edad f&eacute;rtil estimula la s&iacute;ntesis de HDL-Colesterol incrementando sus valores en plasma (18). Las mujeres de origen chino presentaron niveles promedio de HDL-Colesterol similares  a los hombres (1,21 mmol/L y 1,08 mmol/L   respectivamente; p= 0,1230), a pesar de que la mayor&iacute;a de ellas probablemente sean pre-menopa&uacute;sicas. Este fen&oacute;meno podr&iacute;a deberse al bajo poder que tiene la muestra del estudio (n=50). Probablemente si el n&uacute;mero de individuos participantes en esta investigaci&oacute;n hubiera sido mayor esa diferencia observada  en los niveles de HDL-C s&iacute; hubiera sido significativa entre ambos sexos.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es esencial que los nutricionistas y otros profesionales del &aacute;rea eval&uacute;en con mayor detalle los factores asociados al cambio de patr&oacute;n alimentario en inmigrantes para definir cual variable tiene mayor impacto en la modificaci&oacute;n de su perfil de l&iacute;pidos. La prevenci&oacute;n o retraso de esa modificaci&oacute;n  puede evitar un aumento en las tasas de morbi-mortalidad por enfermedad coronaria en ese grupo. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es importante promover en los inmigrantes chinos el valor de su cultura alimentaria, particularmente en los ni&ntilde;os descendientes de &eacute;stos, pues ellos adoptan m&aacute;s r&aacute;pidamente la &quot;nueva&quot; cultura alimentaria seg&uacute;n lo demuestran los estudios con refugiados de Cambodia, Laos y del sur de Vietnam en los Estados Unidos (28,31). El proceso de occidentalizaci&oacute;n de la dieta en los adultos es m&aacute;s lento pues su cultura alimentaria es s&iacute;mbolo de etnicidad  y deseo de mantener una identidad de grupo. Adem&aacute;s, es una forma de uni&oacute;n con el pasado (30,32). Por tanto, la persuasi&oacute;n para mantener su patr&oacute;n alimentario es una tarea relativamente sencilla para los nutricionistas.</P> <B>    ]]></body>
