Se estudiaron 364 embarazadas aleatoriamente seleccionadas en toda la provincia. La hemoglobina (Hb) promedio ± DS descendió progresivamente de 118.4 ±11.0 a 112.1± 11.5 g/L (p = 0.015) entre el 1er y el 3er trimestre gestacional. La prevalencia de anemia (Hb < 110 g/L) fue de 17.4, 26.5 y 35.8% del 1º al 3º. La prevalencia en el 2o trimestre fue 14.% usando Hb <105g/L como se sugiere recientemente. Los promedios geométricos de ferritina para los trimestres gestacionales progresivos fueron 28.9; 16.0 y 11.1 µg/L. En los respectivos trimestres, 21.7, 40.5 y 60.5% tenían ferritina <12 µg/L y 39.1, 58.9 y 83.2% tenían ferritina < 20 µg/L (p<0.004 entre cada trimestre en ambos casos). La prevalencia de deficiencia de hierro y la anemia fueron significativamente mayores en mujeres con nivel de instrucción inferior a primaria completa y con intervalo intergenésico < 1 año. Setenta y cinco porciento de las mujeres tenía adecuada frecuencia de controles prenatales pero solo 23% tomaban suplementos de hierro y 10% los habían suspendido. La prevención de la anemia y DH debe incluir intervenciones desde el período preconcepcional y durante el embarazo, mejorando la adherencia a la toma de suplementos de hierro con motivación a los equipos de salud y comunidad.]]></p></abstract>

The prevention and correction of gestational iron deficiency and anemia must focus on the preconceptional period (inter-pregnancy spacing and increasing iron intake using all available means) as well as during pregnancy improving adherence to iron supplements by means of motivation of health workers and community.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">Recibido: 27-11-2001 Aceptado: 28-06-2002</P> <B>    <P>INTRODUCCION</P> </B>    <P ALIGN="JUSTIFY">    La deficiencia de hierro (DH) es una condici&oacute;n sist&eacute;mica que afecta especialmente a embarazadas, ni&ntilde;os menores de 2 a&ntilde;os, adolescentes y mujeres en edad f&eacute;rtil. Es la m&aacute;s frecuente de las deficiencias nutricionales espec&iacute;ficas que se manifiesta, tard&iacute;amente, por anemia ferrop&eacute;nica (AF). La DH con o sin anemia, sobre todo en edades tempranas afecta m&uacute;ltiples funciones con serias consecuencias en el desempe&ntilde;o y en el desarrollo humano y en su productividad. <B><SUP>(1)</P> </B></SUP>    <P ALIGN="JUSTIFY">    La anemia durante el embarazo tiene repercusiones en la salud de la madre y del producto de la gestaci&oacute;n. <B><SUP>(2)</B></SUP> Existen evidencias del aumento de riesgo de mortalidad materna en mujeres con anemia severa en el &uacute;ltimo trimestre. <B><SUP>(3)</B></SUP> La DH y la AF materna en la primera mitad del embarazo, aumentan el riesgo de retardo de crecimiento intrauterino, prematurez y consecuentemente la mortalidad perinatal. <B><SUP>(4)</B></SUP> </P>     <P ALIGN="JUSTIFY">    Los ni&ntilde;os nacidos de madres an&eacute;micas muestran reservas de hierro disminuidas cuando estas se eval&uacute;an a la edad de 3 o mas meses <B><SUP>(5)</B></SUP> y por consiguiente, tienen aumentado el riesgo de desarrollar AF, con sus consecuencias negativas en el desarrollo f&iacute;sico y mental, algunas de las cuales parecen ser permanentes. <B><SUP>(6)</B></SUP> La DH y la AF tambi&eacute;n producen apat&iacute;a, astenia, alteraciones en la funci&oacute;n muscular, y disminuciones en la calidad de vida de la mujer durante y despu&eacute;s del embarazo, disminuyendo su rol social y su capacidad de brindar cuidados a su familia. Se ha reportado que los hijos de mujeres no suplementadas con hierro durante el embarazo tienen menores dep&oacute;sitos de hierro durante el primer a&ntilde;o de vida, en relaci&oacute;n a los hijos de madres suplementadas. <B><SUP>(7,8)</P> </B></SUP>    <P ALIGN="JUSTIFY">    La provincia de Chaco, donde se realiz&oacute; este estudio, es una de las provincias argentinas con mayor concentraci&oacute;n de pobreza. Cuarenta porciento de su poblaci&oacute;n viv&iacute;a en hogares con necesidades b&aacute;sicas insatisfechas (NBI) seg&uacute;n el Censo Nacional de Poblaci&oacute;n y Vivienda (CNPV) de 1991. <B><SUP>(9)</B></SUP> Su tasa de mortalidad infantil en 1996 (34,4%) fue la m&aacute;s alta del pa&iacute;s. <B><SUP>(10)</P> </B></SUP>    <P ALIGN="JUSTIFY">    En 1997, el Gobierno de Chaco inici&oacute; un programa de reducci&oacute;n de la mortalidad materna e infantil, en cooperaci&oacute;n con UNICEF. Uno de los componentes del programa fue la prevenci&oacute;n de carencias de micronutrientes. En 1998 se realiz&oacute; una encuesta poblacional, en embarazadas y ni&ntilde;os, con el objeto de conocer la prevalencia de carencias de micronutrientes, y el acceso de la poblaci&oacute;n a servicios y programas, como punto de partida para la definici&oacute;n y realizaci&oacute;n de intervenciones. </P>     <P ALIGN="JUSTIFY">    Se presentan aqu&iacute; los resultados obtenidos en embarazadas en dicha encuesta, focalizados en la evoluci&oacute;n de la DH y de la AF en los distintos trimestres, el acceso y adherencia a suplementos de hierro y en las asociaciones entre DH y AF y diversas variables biol&oacute;gicas y sociales.</P> <B>    <P ALIGN="JUSTIFY">Poblaci&oacute;n</P> </B>    <P ALIGN="JUSTIFY">    Se estudiaron 364 embarazadas, representativas del total de mujeres con autopercepci&oacute;n de embarazo que habitaban la provincia de Chaco, Argentina, en octubre de 1998. Esta provincia est&aacute; localizada en la zona Nor-Este de la Rep&uacute;blica. Su altitud media es de aproximadamente 100 metros sobre el nivel del mar. Ciertas zonas est&aacute;n infectadas por uncinaria pero en todos los casos la infecci&oacute;n es de poca severidad. No hay un mapa epidemiol&oacute;gico de esta parasitosis.</P> <B>    ]]></body>
<body><![CDATA[<P>MATERIAL Y METODOS</P>     <P ALIGN="JUSTIFY">Muestra</P> </B>    <P ALIGN="JUSTIFY">    Se dise&ntilde;&oacute; un estudio transversal en una muestra aleatoria de n = 440 embarazadas obtenida en tres etapas abarcando la totalidad del territorio provincial, seleccionando 44 radios censales en la primer etapa, 10 hogares dentro de los radios censales en la segunda e individuos dentro de los hogares en la tercera. Este tama&ntilde;o muestral se consider&oacute; necesario para realizar la estimaci&oacute;n de una proporci&oacute;n (prevalencia) con una precisi&oacute;n m&iacute;nima del +/- 10% con el 95% de confianza, postulando un coeficiente de correlaci&oacute;n alto intra-clase (0,40). <B><SUP>(11)</B></SUP> Con coeficientes de correlaci&oacute;n intra-clase mas bajos la precisi&oacute;n m&iacute;nima disminuye. </P>     <P ALIGN="JUSTIFY">    Los 44 radios censales, &oacute; puntos muestra (PM), fueron seleccionados sistem&aacute;ticamente, con dos arranques aleatorios, con probabilidades proporcionales al tama&ntilde;o, seg&uacute;n la poblaci&oacute;n correspondiente al CNPV 1991. El listado de los radios fue ordenado seg&uacute;n su hacinamiento. <B><SUP>(12)</B></SUP> En todos los PM se seleccionaron 15 hogares elegibles, de manera de reemplazar hasta 5 rechazos. </P>     <P ALIGN="JUSTIFY">    Se realiz&oacute; una campa&ntilde;a comunicacional para sensibilizar e informar a la poblaci&oacute;n. Cada embarazada fue encuestada asign&aacute;ndole un c&oacute;digo y se le extrajo sangre venosa antecubital en su domicilio luego de dar su consentimiento informado. La proporci&oacute;n de rechazos entre etapa de listado y encuesta fue de 12.6%. </P>     <P ALIGN="JUSTIFY">    Todas las mujeres incluidas en la muestra fueron informadas sobre los resultados hematol&oacute;gicos obtenidos y las an&eacute;micas recibieron tratamiento dentro de las dos semanas siguientes a la encuesta. Luego de esta etapa se resguardaron los registros que permit&iacute;an identificar a la persona por su c&oacute;digo. El estudio fue aprobado en sus aspectos &eacute;ticos por el Ministerio de Salud de la Provincia. </P> <B>    <P ALIGN="JUSTIFY">Procesamiento de las muestras de sangre</P> </B>    <P ALIGN="JUSTIFY">    Las muestras de sangre fueron tomadas por punci&oacute;n venosa. Parte de la muestra se colect&oacute; en conos de pl&aacute;stico blanco, sobre anticoagulante EDTA para la determinaci&oacute;n de hemoglobina (Hb) por el m&eacute;todo de cianmetahemoglobina usando un contador hematol&oacute;gico automatizado. Otra parte de la sangre extra&iacute;da fue centrifugada en terreno antes de las 6 horas de extra&iacute;da y el plasma se congel&oacute; hasta realizar la determinaci&oacute;n de ferritina.</P>     <P ALIGN="JUSTIFY">    La ferritina fue determinada por Enzimo Inmuno Ensayo (MEIA) en analizador IMX (Abbot) Se realizaron controles de calidad internos con cada corrida realizada y externos en un centro de referencia.</P>     <P ALIGN="JUSTIFY">   El total de muestras con determinaci&oacute;n de Hb fue 359 y con ferritina 356.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">L&iacute;mites de inclusi&oacute;n para Hb y ferritina plasm&aacute;tica</P> </B>    <P ALIGN="JUSTIFY">    Los valores de Hb &lt;110 g /L se consideraron anemia. <B><SUP>(13)</B></SUP> Para ferritina se consideraron 2 l&iacute;mites de inclusi&oacute;n: &lt; 12 (g./L deficiencia de hierro severa y &lt;20 (g./L deficiencia de hierro moderada. <B><SUP>(1,12)</B></SUP> Los valores de ferritina se transformaron logar&iacute;tmicamente para normalizar su distribuci&oacute;n dada su desviaci&oacute;n hacia la derecha. Todas las estad&iacute;sticas se realizaron con los valores transformados. Los promedios as&iacute; obtenidos son promedios geom&eacute;tricos. Se proces&oacute; la informaci&oacute;n mediante el programa SPSS para Windows versi&oacute;n 11. </P> <B>    <P>RESULTADOS</P> </B>    <P ALIGN="JUSTIFY">    En la <a href="#tab1">Tabla 1</a> se presentan las caracter&iacute;sticas de la poblaci&oacute;n de embarazadas estudiadas seg&uacute;n variables seleccionadas. El n&uacute;mero de controles en relaci&oacute;n a la edad gestacional fue igual &oacute; superior a la norma en el 75% de los casos. </P>     <P ALIGN="JUSTIFY">    23% estaba tomando suplemento de hierro en el momento de la encuesta y 10,4% hab&iacute;an tomado y suspendieron. Entre &eacute;ste &uacute;ltimo grupo la causa de la suspensi&oacute;n m&aacute;s mencionada fue la falta de indicaci&oacute;n para continuarlo. </P>     <P ALIGN="JUSTIFY">    Del total de embarazadas, 73% refirieron que se hicieron an&aacute;lisis de sangre en el embarazo en curso y 31% que se les inform&oacute; su condici&oacute;n de an&eacute;micas. </P>     <P ALIGN="JUSTIFY">    En la <a href="#tab2">Tabla 2</a> y en el <a href="#graf1">Gr&aacute;fico 1</a> se observan progresivas disminuciones de los valores de hemoglobina y ferritina y el aumento en la proporci&oacute;n de mujeres que presentaron tanto deficiencia de hierro como anemia, conforme avanza el embarazo. La prevalencia de anemia en el tercer trimestre fue significativamente m&aacute;s alta que en el primero (p = 0.004) y las prevalencias de deficiencia de hierro en cada trimestre fueron significativamente diferentes para ambos l&iacute;mites de inclusi&oacute;n (12µg /L o 20 µg /L). Entre 1º y 2º trimestres: p = 0.00 y p = 0.006, entre 2º y 3 º trimestre p = 0.002 y p &lt; 0.001, respectivamente. </P>     <P ALIGN="CENTER"><a name="tab2"></a><b>TABLA 2</b></P>     ]]></body>

<body><![CDATA[<P ALIGN="JUSTIFY">    Los promedios de Hb en el segundo y tercer trimestres fueron significativamente m&aacute;s bajos que en el primero, respectivamente (p = 0.03 y &lt; 0.001). <P ALIGN="JUSTIFY">    Las diferencias entre los promedios geom&eacute;tricos de ferritina en cada trimestre difieren significativamente (p &lt; 0.001).</P>     <P ALIGN="JUSTIFY">    La distribuci&oacute;n de los valores de ferritina muestra el desplazamiento de la poblaci&oacute;n hacia la izquierda en los sucesivos trimestres de embarazo (<a href="#graf3">Gr&aacute;fico 3</a>). <P>&nbsp;</P>     <P>    La sensibilidad de la ferritina (&lt; 12 µg /L) en relaci&oacute;n a la anemia por hemoglobina &lt;110 g/ L fue de 66% (IC 55.6-75.1) y la especificidad de 68%. (IC 61.4-73.2). El valor predictivo positivo fue 43,5% (IC: 35.7-51.9) y el negativo 84%. (IC 78.1-88.56)</P>     <P ALIGN="JUSTIFY">    Las caracter&iacute;sticas que se asociaron significativamente con mayor prevalencia de anemia (<a href="#tab3">Tabla 3</a>) fueron el bajo nivel de instrucci&oacute;n y el intervalo intergen&eacute;sico menor a 12 meses. <body><![CDATA[<P ALIGN="JUSTIFY">    El intervalo intergen&eacute;sico inferior a 1 a&ntilde;o, se asoci&oacute; significativamente (p=0.0053) con niveles de ferritina &lt;12 µg/L. La instrucci&oacute;n materna inferior a primaria completa y la pertenencia a un hogar NBI muestran tendencia asociativa con ferritina baja, pero la probabilidad de error alfa es superior al 5% en ambas. </P> <B>    <P>DISCUSION</P> </B>    <P ALIGN="JUSTIFY">    La proporci&oacute;n de mujeres que pertenec&iacute;an a hogares con NBI en la muestra (64%) result&oacute; sensiblemente superior a la de hogares con NBI en la provincia en el &uacute;ltimo Censo (40%). Puede interpretarse que la muestra tiene un sesgo, debido a que los rechazos en la etapa de campo pudieron ser selectivos de los hogares no pobres, &oacute; que la mayor natalidad entre los pobres llev&oacute; a que el subuniverso de embarazadas concentre mayor proporci&oacute;n de pobreza que la poblaci&oacute;n total. En este estudio la proporci&oacute;n de mujeres con 4 y m&aacute;s hijos entre las pertenecientes a hogares con NBI fue de 26,4% y entre las no pobres de 3,1%, lo que refuerza esta explicaci&oacute;n. </P>     <P ALIGN="JUSTIFY">    No obstante, la prevalencia de anemia en embarazadas en el total de la muestra (27.9%) es similar a la hallada en la ciudad de Usuahia (31,6%) en 1994, <SUP>(14)</SUP> poblaci&oacute;n con muy buenos indicadores de salud y a la estimaci&oacute;n hecha por la OPS (35%) para Am&eacute;rica Latina <SUP>(15)</SUP> la que proviene de estudios parciales realizados en su gran mayor&iacute;a en poblaciones con bajo nivel socioecon&oacute;mico. </P>     <P ALIGN="JUSTIFY">    La prevalencia de AF en mujeres no pobres (22%), y la ausencia de asociaci&oacute;n significativa entre pobreza y DH y AF (p = 0.116), demuestra la existencia de un problema que afecta a las mujeres embarazadas con relativa independencia de su nivel socioecon&oacute;mico. Este hallazgo explicar&iacute;a la escasa variabilidad encontrada en dos estudios en nuestro pa&iacute;s con diferentes niveles de pobreza.</P>     <P ALIGN="JUSTIFY">    El acceso de las embarazadas a los servicios de salud fue aceptable, en tanto tres de cada cuatro ten&iacute;an una frecuencia de controles prenatales acorde a las normas, y se les hab&iacute;a realizado al menos un an&aacute;lisis de sangre. </P>     <P ALIGN="JUSTIFY">    La suplementaci&oacute;n diaria con hierro (60 mg/d&iacute;a) y folatos (0.4 mg/d&iacute;a) integra la norma de cuidado prenatal en la provincia. No obstante, menos de la cuarta parte de las mujeres estaban tomando hierro en el momento de la encuesta y 10% hab&iacute;an tomado pero interrumpieron. La causa m&aacute;s frecuente (79%) invocada para explicar la suspensi&oacute;n en este &uacute;ltimo grupo fue la falta de indicaci&oacute;n &oacute; provisi&oacute;n del suplemento por el equipo de salud, en tanto que la intolerancia al medicamento fue mencionada en el 13%. Esta negligencia no puede atribuirse solo a una dificultad en asumir una conducta prolongada de car&aacute;cter preventivo ya que tambi&eacute;n en las mujeres que hab&iacute;an sido informadas de su condici&oacute;n de an&eacute;micas y que debieron ser tratadas, la mitad no recib&iacute;a suplemento de hierro. </P>     <P ALIGN="JUSTIFY">    Se ha usado como l&iacute;mite de inclusi&oacute;n para anemia valores de Hb &lt; 110 g /L en todo el embarazo a fin de hacer comparables estos resultados con otros reportados. Si se toma 105 g /L en el segundo trimestre de embarazo tal como recomienda el CDC, en base a los resultados de estudios de suplementaci&oacute;n en poblaciones europeas, <B><SUP>(16)</B></SUP> la prevalencia de anemia en ese per&iacute;odo se reduce a 14.1%. </P>     <P ALIGN="JUSTIFY">    Dada la controversia existente con respecto a los l&iacute;mites de inclusi&oacute;n de par&aacute;metros hematol&oacute;gicos durante el embarazo para definir anemia y DH, se presentan las frecuencias acumuladas de casos por trimestre de embarazo tanto para Hb como para ferritina. (<a href="#graf2">Gr&aacute;ficos 2</a> y <a href="#graf3">3</a>). </P>     <P ALIGN="JUSTIFY">    En ambos se observa un desplazamiento en la distribuci&oacute;n de Hb y ferritina hacia valores mas bajos en los sucesivos trimestres de embarazo. El desplazamiento es m&aacute;s notable especialmente para ferritina entre el 1º y 2º trimestre (un incremento de 87% de mujeres con valores &lt; 12 µg/L) el cual no es explicable por la expansi&oacute;n plasm&aacute;tica acelerada entre las semanas 12-13 hasta las semanas 20-21 de embarazo <B><SUP>(17)</B></SUP> ya que el incremento relativo de valores bajos es mayor que el de Hb en el mismo per&iacute;odo (52%). Sin embargo, no nos es posible determinar en qu&eacute; medida el desplazamiento de las curvas refleja el agotamiento de las reservas de hierro, con un real descenso de la Hb, en relaci&oacute;n con la hemodiluci&oacute;n por expansi&oacute;n plasm&aacute;tica.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">    En el <a href="#graf4">Gr&aacute;fico 4</a> se calcularon para la edad gestacional promedio en cada trimestre los promedios de hemoglobina y se los compar&oacute; con los valores de referencia del CDC en mujeres europeas suplementadas con hierro durante el embarazo para igual momento de la gestaci&oacute;n. Puede observarse que en todos los puntos los promedios obtenidos en nuestro estudio son inferiores a los esperados en mujeres sanas suplementadas con hierro. Las diferencias en el segundo y tercer trimestre son esperables, en tanto el uso de suplementos en las embarazadas de nuestro estudio fue muy bajo. Pero los datos del primer trimestre indican claramente que nuestras mujeres entran al embarazo con 10 g/L menos de Hb que la poblaci&oacute;n de referencia, sugiriendo un estado de d&eacute;ficit de hierro y AF a&uacute;n antes del embarazo. Diversos estudios realizados en Salta, Corrientes, el Gran Buenos Aires y Tierra del Fuego muestran 41%, 34%, 28% y 11% de anemia en mujeres en edad f&eacute;rtil, respectivamente. <B><SUP>(18)</B></SUP> En las dos &uacute;ltimas localidades las reservas de hierro estaban depauperadas (ferritina s&eacute;rica &lt; 12 µg/L) en 36% y 18%. Estas cifras sugieren que una gran proporci&oacute;n de mujeres en edad f&eacute;rtil entran al embarazo con reservas de hierro inadecuadas. La pendiente entre los promedios de Hb entre 1º y 2º trimestre es m&aacute;s plana que la de la referencia, lo que podr&iacute;a indicar una menor capacidad de expansi&oacute;n plasm&aacute;tica en este per&iacute;odo. Entre los trimestres 2º y 3º, el promedio de Hb tiende a subir en la poblaci&oacute;n de referencia en tanto que en los datos aqu&iacute; presentados contin&uacute;a descendiendo. Este comportamiento tanto de la Hb como de la ferritina entendemos refleja el r&aacute;pido deterioro de las reservas de, ya precarias al inicio del embarazo, como consecuencia de la alta demanda en la segunda mitad del mismo. El alto valor predictivo negativo encontrado (84%) para ferritina en relaci&oacute;n a Hb indica la importancia de mantener ferritinas por encima de 12 µg/L para eitar la aparici&oacute;n de anemia. </P>     <P ALIGN="JUSTIFY">Teniendo en cuenta que los requerimientos de hierro en el primer trimestre de embarazo son similares a los previos al mismo (19) y que la expansi&oacute;n plasm&aacute;tica es incipiente, puede asumirse que el estado nutricional de hierro encontrado en las embarazadas en ese per&iacute;odo refleja la situaci&oacute;n de las mujeres previa al embarazo. </P>     <P ALIGN="CENTER"><a name="graf4"></a><b>GRAFICO 4</b></P>     <P ALIGN="CENTER">Concentraci&oacute;n de Hb por trimestre de embarazo en embarazadas de referencia y en Chaco</P>     <P ALIGN="CENTER">&nbsp;</P>     <P ALIGN="CENTER"><IMG SRC="/img/fbpe/alan/v52n4/art2img4.gif" WIDTH=513 HEIGHT=316 align="center"></P>     
<P ALIGN="CENTER">&nbsp;</P>     <P ALIGN="JUSTIFY">    Por lo tanto, 17% de las mujeres en edad f&eacute;rtil no embarazadas en la provincia son an&eacute;micas y 40% deficientes de hierro o con reservas insuficientes. Esta situaci&oacute;n es particularmente grave en tanto se trata de una poblaci&oacute;n con una alta proporci&oacute;n de mujeres con corto intervalo intergen&eacute;sico (45% &lt; 2 a&ntilde;os) y 23% de embarazadas con menos de 20 a&ntilde;os de edad, muchas de las cuales transcurren su embarazo durante su adolescencia, per&iacute;odo en el cual confluyen 2 momentos de altas necesidades de hierro. <B><SUP>(20)</P> </B></SUP>    <P ALIGN="JUSTIFY">    La DH y la AF materna se han asociado en estudios epidemiol&oacute;gicos con incremento del riesgo de prematurez y retardo del crecimiento intrauterino, especialmente cuando se presentan en la primera mitad del embarazo. <B><SUP>(21-24)</B></SUP> A&uacute;n con las consideraciones hechas precedentemente sobre la dificultad de interpretar la proporci&oacute;n de mujeres con DH y anemia, la proporci&oacute;n de ellas con DH en la primera mitad del embarazo afecta a m&aacute;s de la mitad de las embarazadas. Es posible que este riesgo est&eacute; involucrado en la alta proporci&oacute;n de ni&ntilde;os nacidos con peso inferior a 2.500 g (9.7%), obtenido en este estudio en la encuesta de ni&ntilde;os en relaci&oacute;n a 7% en el promedio nacional. <B><SUP>(25)</B></SUP> </P>     <P ALIGN="JUSTIFY">    Es de destacar la baja prevalencia de casos con Hb &lt;90 g/L (<a href="#graf2">Gr&aacute;fico 2</a>), l&iacute;mite de Hb a partir del cual los riesgos de mortalidad y complicaciones materno-fetales aumentan exponencialmente. <B><SUP>(3)</B></SUP> Se reconoce que la deficiencia de hiero de origen alimentario exclusivo no produce anemias severas (Hb &lt; 70 g/l) excepto en situaciones extremas, no encontradas en la poblaci&oacute;n estudiada. La anemia severa se observa cuando existen otros factores etiol&oacute;gicos como malaria, uncinariasis severa &oacute; SIDA. <B><SUP>(26)</B></SUP> </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">    Tambi&eacute;n los valores altos de Hb (superiores a 125 g /L) en el tercer trimestre de embarazo se han asociado a riesgo de enfermedad hipertensiva (pre-eclampsia y eclampsia) y a insuficiente crecimiento fetal probablemente debido a la incapacidad de la mujer de expandir su volumen sangu&iacute;neo con la consiguiente disminuci&oacute;n de la perfusi&oacute;n placentaria. <B><SUP>(20,27)</B></SUP> En nuestra poblaci&oacute;n se encontr&oacute; un 12,5 % de mujeres con valores de Hb por encima de este valor en el tercer trimestre, proporci&oacute;n similar a la reportada en otros estudios. </P>     <P ALIGN="JUSTIFY">    El nivel educativo materno y el intervalo intergen&eacute;sico inferior a 1 a&ntilde;o son las variables que mostraron mayor fuerza de asociaci&oacute;n tanto con anemia como con DH. A su vez, el espaciamiento entre embarazos es una variable fuertemente asociada al nivel educativo. Entre las mujeres que no completaron la escolaridad primaria 15% eran prim&iacute;paras y 40% ten&iacute;a un hijo menor de un a&ntilde;o en tanto que en las madres que hab&iacute;an completado la secundaria estas proporciones fueron 42% y 21% respectivamente. La proporci&oacute;n de madres con deficiencia de hierro en alg&uacute;n momento del embarazo result&oacute; cercano a 50% cuando el intervalo intergen&eacute;sico fue inferior a 3 a&ntilde;os, mostrando a partir de este l&iacute;mite un progresivo descenso hasta 30% en las prim&iacute;paras. </P>     <P ALIGN="JUSTIFY">    Por todo lo anterior es necesario realizar intervenciones que contemplen tanto el per&iacute;odo preconcepcional como el embarazo y el post-parto, o todo el per&iacute;odo intergen&eacute;sico. </P>     <P ALIGN="JUSTIFY">    Dado que 33% de las mujeres pertenecen a hogares con ingresos per c&aacute;pita mensuales diarios cercanos a $1 por d&iacute;a, es posible que la promoci&oacute;n del consumo de carnes y otras fuentes de hierro hem&iacute;nico encuentre algunas restricciones. </P>     <P ALIGN="JUSTIFY">    La fortificaci&oacute;n de alimentos, especialmente la harina de trigo, resulta as&iacute; una medida de elecci&oacute;n por la escasa o nula incidencia de esta medida en el costo de la canasta de alimentos familiar y en el sistema de salud. Otras alternativas a explorar son la suplementaci&oacute;n preventiva con hierro y otros nutrientes (por ejemplo &aacute;cido f&oacute;lico y zinc) en mujeres en edad f&eacute;rtil, <B><SUP>(28)</B></SUP> durante el embarazo y la lactancia. <B><SUP>(26,29)</P> </B></SUP>    <P ALIGN="JUSTIFY">    A mediano plazo la intervenci&oacute;n m&aacute;s recomendable es mejorar el nivel educativo de las mujeres, indicador universalmente asociado positivamente con mejoras en salud y nutrici&oacute;n.</P>     <P ALIGN="JUSTIFY">    La baja adherencia a la suplementaci&oacute;n durante el embarazo requiere que las estrategias a implementar contemplen la capacitaci&oacute;n de los equipos de salud y el empoderamiento de las comunidades. Algunas de las estrategias aqu&iacute; propuestas est&aacute;n en ejecuci&oacute;n en la provincia de Chaco, como parte de la cooperaci&oacute;n entre el gobierno de dicha provincia y UNICEF.
