Se estudió la influencia del nivel de calcio (Ca) de la dieta, durante preñez y lactancia, sobre el contenido materno de zinc (Zn) en hueso y en sangre. Ratas Wistar, hembras, entre 250 a 350 g de peso, se alimentaron desde el comienzo de la preñez hasta el destete con una dieta conteniendo/100 g: 0.2, 0.6 ó 0.9 g Ca (grupos BAJO, NORMAL y ALTO respectivamente). Al parto y al destete se determinó Zn en sangre (ZnS). Al destete se determinó en fémur Zn (ZnF) y Ca (CaF). Los resultados promedio ± ESM fueron : para ZnS (my g/ml), al parto y al destete respectivamente: BAJO: 8.73 ± 1.05; 12.8 ± 2.02; NORMAL: 3.49 ± 0.19; 3.73 ± 0.37; ALTO: 3.21 ± 0.19; 3.85 ± 0.27. Al destete para ZnF (my g/100 mg): BAJO: 30.2 ± 0.9; NORMAL: 24.1 ± 0.3; ALTO: 24.1 ± 0.9; CaF (mg/100 mg): BAJO:19.2 ± 0.8; NORMAL: 21.4 ± 0.6; ALTO: 20.4 ± 1.1. ZnS en BAJO fue estadísticamente mayor (p<0.0001) a NORMAL y ALTO al parto y al destete. En BAJO, ZnF fue mayor y CaF menor a NORMAL y ALTO (p<0.0001). ZnS, ZnF y CaF no mostraron diferencias entre NORMAL y ALTO. Estos resultados evidencian que durante gestación y lactancia no existió efecto negativo al incrementar en 50% el nivel de calcio de la dieta; el bajo aporte de calcio con zinc constante, aumentó tanto el contenido en fémur como los niveles sanguíneos de zinc.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY"><b>Aceptado:</b> 24-04-2003</P>     <P ALIGN="JUSTIFY"><B>INTRODUCCION</P> </B>     <P ALIGN="JUSTIFY">La buena nutrici&oacute;n es uno de los pilares sobre los que se cimenta el desarrollo de los pueblos, siendo la salud materna durante el embarazo y la lactancia uno de los indicadores que diferencia el grado de desarrollo de los pa&iacute;ses (1). </P>     <P ALIGN="JUSTIFY">Argentina no cuenta con datos nacionales que proporcionen un panorama completo del estado nutricional. Algunas encuestas han revelado un elevado porcentaje de poblaci&oacute;n, de diferentes edades y estados fisiol&oacute;gicos, que presenta bajas ingestas de calcio (Ca), lo cual podr&iacute;a ser una de las causas de deterioro del tejido &oacute;seo materno y consiguiente riesgo de posterior osteoporosis en la edad adulta (2-5). Respecto de mujeres gestantes y lactantes del Gran Buenos Aires, otras encuestas recientes han evidenciado tambi&eacute;n elevada prevalencia de baja ingesta de Ca, conjuntamente con ingestas marginales de zinc (Zn) (6).</P>     <P ALIGN="JUSTIFY">Durante el embarazo y lactancia est&aacute;n incrementadas las necesidades de nutrientes y las deficiencias nutricionales implican un riesgo para la salud materno-infantil. Las adaptaciones fisiol&oacute;gicas que ocurren durante el embarazo tienden a lograr un adecuado desarrollo fetal, a&uacute;n a expensas de la salud materna (7). En el primer semestre se produce un sustancial aumento del volumen plasm&aacute;tico y de la masa eritrocitaria, liber&aacute;ndose substancias vasodilatadoras y de inhibici&oacute;n de la agregaci&oacute;n plaquetaria, que convierten el lecho vascular en un sistema de baja resistencia. La falla en estos mecanismos de adaptaci&oacute;n a los cambios fisiol&oacute;gicos puede conducir a hipertensi&oacute;n inducida por el embarazo (HIE) (8-10), cuyas causas no est&aacute;n perfectamente esclarecidas, aunque existen evidencias de su  relaci&oacute;n con la baja ingesta de  Ca (11-12). A su vez, bajas ingestas de Zn y/o bajas concentraciones de dicho micronutriente en plasma o eritrocitos podr&iacute;an relacionarse con ciertas complicaciones durante la gestaci&oacute;n como aumento de la incidencia de abortos, alteraciones en el desarrollo del feto, nacimientos prematuros, disminuci&oacute;n del peso del reci&eacute;n nacido y efectos tard&iacute;os en el crecimiento infantil (13). </P>     <P ALIGN="JUSTIFY">Se ha documentado que los suplementos de Ca reducen el riesgo de HIE y de preeclampsia (12). Sin embargo, las complejas interacciones Ca-Zn (14) hacen necesario prestar atenci&oacute;n al posible efecto adverso de los suplementos de Ca sobre la utilizaci&oacute;n del Zn, as&iacute; como estudiar la influencia de las bajas ingestas de Ca, aspecto sobre el cual existe escasa informaci&oacute;n. En este sentido, Kenney y McCoy, suplementando dietas bajas en Ca con cantidades moderadas de Zn, en ratas, encontraron que se hallaba alterado el intercambio Zn/Ca en f&eacute;mur,  lo que se traduc&iacute;a en variaciones en sus dimensiones, contenido mineral y propiedades mec&aacute;nicas (15). Adem&aacute;s, Dursun and Aydojan observaron en ratas alimentadas con dietas bajas en Ca que no se modificaba significativamente la absorci&oacute;n porcentual de Zn, aunque exist&iacute;a un incremento en su concentraci&oacute;n en duodeno, cerebro, h&iacute;gado y sangre  (16).</P>     <P ALIGN="JUSTIFY">Teniendo en cuenta los antecedentes mencionados, el objetivo del presente trabajo longitudinal fue evaluar, en un modelo experimental en ratas, la influencia del nivel de Ca de la dieta consumida durante pre&ntilde;ez y lactancia, sobre el contenido materno de Zn en sangre y f&eacute;mur.</P> <B>     <P ALIGN="justify">MATERIALES Y METODOS</P>     <P ALIGN="JUSTIFY">Animales</P> </B>    <P ALIGN="JUSTIFY">Ratas adultas de la cepa Wistar fueron criadas y mantenidas en bioterio con condiciones estandarizadas de temperatura (21°C), de humedad (70%)  y con ciclos de luz-oscuridad de 12 hs, regulados autom&aacute;ticamente (de 8-20 hs). Cuando alcanzaron un peso  entre 250 y 350 g (aproximadamente 5 meses de edad) se aparearon en la relaci&oacute;n de 4 hembras por macho. Una vez comprobado el estado de pre&ntilde;ez, mediante el hallazgo de espermatozoides en los extendidos vaginales, se asignaron 7 ratas hembras a cada grupo experimental y se alojaron en jaulas individuales de acero inoxidable, administr&aacute;ndoles "ad-libitum&quot; agua desionizada y las dietas experimentales. Al nacimiento de las cr&iacute;as se registraron el peso y el n&uacute;mero de las mismas, las que se ajustaron a 8 por madre. </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">El peso de las madres se registr&oacute; al inicio de la gestaci&oacute;n, al parto y al destete. A la tercera semana de  la gestaci&oacute;n y al destete se tomaron muestras de sangre materna de la vena caudal, con heparina como anticoagulante, entre las 10 y 12 hs para evitar variaciones circadianas. Luego del destete, las madres se sacrificaron y se les extrajo el f&eacute;mur derecho. Toda la experiencia fue realizada de acuerdo a la Gu&iacute;a para cuidado y uso de Animales de Laboratorio del National Institute of Health, USA (17).</P> <B>     <P ALIGN="JUSTIFY">Dietas experimentales</P> </B>    <P ALIGN="JUSTIFY">El presente modelo experimental para gestaci&oacute;n y lactancia fue aplicado en estudios previos, anteriores a 1993 (18), a&ntilde;o en que se publicaron las &uacute;ltimas recomendaciones para la preparaci&oacute;n de las dietas para animales de experimentaci&oacute;n (19). Por lo tanto, para poder comparar los presentes resultados con aquellos, se utilizaron dietas basadas en las recomendaciones de 1976/1980 para la pre&ntilde;ez y lactancia (17,20), modificando el porcentaje de l&iacute;pidos y variables en el contenido de Ca. Se utiliz&oacute; una mezcla de sales libres de Ca (4 g/100 de dieta que aport&oacute; por 100g de dieta 0.6 g de P y 3.5 mg de Zn). Se agreg&oacute; carbonato de Ca anhidro para alcanzar una concentraci&oacute;n de Ca en la dieta de 0.2; 0.6 y 0.9 g/100 g de dieta (grupos BAJO, NORMAL y ALTO respectivamente). <body><![CDATA[<P ALIGN="JUSTIFY">7-Refiner&iacute;as de maiz, Baradero, Argentina.</P>     <P ALIGN="JUSTIFY"><B>Ingesta de nutrientes</P> </B>    <P ALIGN="JUSTIFY">El consumo de alimento se registr&oacute; tres veces por semana. En base al consumo y a la composici&oacute;n de las dietas, se calcularon las ingestas de Ca y Zn. </P> <B>     <P ALIGN="JUSTIFY">Determinaciones bioqu&iacute;micas</P> </B>    <P ALIGN="JUSTIFY">En sangre entera se determin&oacute; Zn. Se extrajeron los f&eacute;mures derechos de las madres, al destete, liber&aacute;ndolos de tejido muscular y sec&aacute;ndolos durante 72 hs. en  estufa  a 100<FONT FACE=Symbol>&#176;</FONT> C. Se desengrasaron con mezcla de  cloroformo:metanol (3:1) y se secaron en estufa durante  48 hs. a 100<FONT FACE=Symbol>&#176;</FONT> C.  Sobre  la muestra  desengrasada y seca se determin&oacute; el peso. Posteriormente, se mineralizaron por v&iacute;a seca  (seg&uacute;n el m&eacute;todo de AOAC) (21), hasta aspecto blanco cristalino. Las cenizas se disolvieron en ClH cc p.a. y se llevaron a volumen adecuado con agua desionizada para determinar Ca y Zn.</P>     <P ALIGN="JUSTIFY">Se determinaron Ca y Zn por espectrofotometr&iacute;a de absorci&oacute;n at&oacute;mica (EAS) (espectrofot&oacute;metro Varian, modelo SpectrAA-20, con llama de aire-acetileno) con el agreAltodo de Cl<SUB>3</SUB>La, en una concentraci&oacute;n de 6500 ppm, en la soluci&oacute;n de lectura, como supresor de interferencias (22). </P>     <P ALIGN="JUSTIFY">Los diferentes pesos se determinaron con una balanza anal&iacute;tica Mettler con precisi&oacute;n <FONT FACE=Symbol>&#177;</FONT>  0.1 mg</P> <B>     <P ALIGN="JUSTIFY">An&aacute;lisis estad&iacute;stico de los resultados</P> </B>    <P ALIGN="JUSTIFY">Los datos se expresaron como media <FONT FACE=Symbol>&#177;</FONT>  error est&aacute;ndar de la media. Se aplic&oacute; an&aacute;lisis de varianza y los test de Student-Newman-Keuls para las comparaciones intragrupo "a posteriori" (23). </P> <B>     <P ALIGN="justify">RESULTADOS</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Consumo de dieta y nutrientes </P> </B>    <P ALIGN="JUSTIFY">La <a href="#tab2"> Tabla 2 </a> muestra los consumos promedio de dieta (g/rata/d&iacute;a), Ca (mg/rata/d&iacute;a) y Zn (<FONT FACE=Symbol>&#109;</FONT> g/rata/d&iacute;a) desglosados en dos per&iacute;odos: desde el inicio de la pre&ntilde;ez hasta el parto  y desde el parto hasta el d&iacute;a 15 de lactancia. A partir de los 17 d&iacute;as de edad las cr&iacute;as comienzan a consumir dieta s&oacute;lida, por lo cual no se incluy&oacute; el consumo materno a partir del d&iacute;a 15 de la lactancia. Durante la pre&ntilde;ez, el consumo de dieta en el grupo BAJO, fue significativamente mayor que en el  ALTO y el NORMAL (p = 0.0001), pero no hubo diferencias significativas entre los grupos durante la lactancia. El mayor consumo de dieta en el grupo BAJO durante  la pre&ntilde;ez se tradujo en mayor ingesta de Zn (p&lt;0.0005). Los valores de Zn no mostraron diferencias <B> </B>significativas entre los grupos ALTO y NORMAL. <P ALIGN="JUSTIFY">Alto, normal y bajo: grupos alimentados con dietas conteniendo 0.9, 0.6 y 0.2 g de Ca/100 g respectivamente y con 3.5 mg de Zn/100 g de dieta.</P>     <P ALIGN="JUSTIFY">*: p&lt;0.001 con respecto a Alto y Normal</P>     <P ALIGN="JUSTIFY"><B>DISCUSION</P> </B>     <P ALIGN="JUSTIFY">La interacci&oacute;n Ca/Zn es un tema discutido, pero no aclarado. Se ha postulado que el Zn y el Ca podr&iacute;an competir a nivel intestinal por un mismo transportador, por existir un mecanismo de absorci&oacute;n transcelular com&uacute;n a ambos cationes y que, adem&aacute;s, el Ca podr&iacute;a disminuir la absorci&oacute;n del Zn reduciendo el transporte pasivo a trav&eacute;s del duodeno (25). </P>     <P ALIGN="JUSTIFY">Si bien el mecanismo de la absorci&oacute;n del Ca es conocido, los mecanismos involucrados en la absorci&oacute;n del Zn no son tan claros (26), puesto que las dificultades metodol&oacute;gicas hacen dif&iacute;cil evaluar su biodisponibilidad. Las determinaciones de Zn en muestras biol&oacute;gicas como suero, plasma, gl&oacute;bulos rojos y pelo son las m&aacute;s utilizadas como indicadores de estado nutricional, presentando cada una diversos inconvenientes. La concentraci&oacute;n en suero o plasma se incrementa cuando existe hem&oacute;lisis, a&uacute;n imperceptible y puede estar alterada en el caso de infecciones o de ejercicio intenso (27-29). En el caso particular de la gestaci&oacute;n se ha evidenciado una disminuci&oacute;n que guarda relaci&oacute;n con el incremento del volumen plasm&aacute;tico, que es dif&iacute;cil de interpretar (30). La concentraci&oacute;n en eritrocitos es un reflejo del Zn disponible en el momento de la eritropoyesis;  sin embargo, la determinaci&oacute;n de Zn en sangre entera ha sido poco estudiada como indicador de estado nutricional, aunque es &uacute;til para estudios en humanos (31). </P>     <P ALIGN="JUSTIFY">Nuestros resultados preliminares en mujeres gestantes indicaron una relaci&oacute;n inversa entre la ingesta de Ca y la concentraci&oacute;n de Zn en sangre entera durante el curso del embarazo (32). No obstante, la existencia de diversas variables en las dietas humanas hace dif&iacute;cil interpretar los resultados. Por ello, en el presente modelo experimental, la &uacute;nica variable fue la concentraci&oacute;n de Ca de la dieta, que en el grupo<B> </B>NORMAL se ajust&oacute; a las recomendaciones para la rata en per&iacute;odo de gestaci&oacute;n y lactancia y en los otros dos grupos represent&oacute;, respectivamente, 38% y 173% de aquella cifra. Adem&aacute;s, la concentraci&oacute;n de Zn en las tres dietas experimentales fue constante y suficiente para cubrir los requerimientos de la rata durante el per&iacute;odo reproductivo. </P>     <P ALIGN="JUSTIFY">Los presentes resultados evidencian que, al igual que en las gestantes con baja ingesta de Ca, en el modelo experimental en ratas, el grupo con BAJO nivel de Ca present&oacute; un aumento de la concentraci&oacute;n sangu&iacute;nea de Zn. El efecto del incremento del consumo de dieta en ese grupo con respecto a los otros dos se descartar&iacute;a debido a que el aumento en la ingesta de Zn en la pre&ntilde;ez (38%) no ser&iacute;a suficiente para explicar el gran aumento observado en los niveles de Zn en sangre (250%) (<a href="#tab2">Tablas 2</a> y <a href="#tab4">4</a>). Adem&aacute;s, los niveles de Zn en sangre continuaron increment&aacute;ndose durante la lactancia aunque en ese per&iacute;odo no hubo diferencias en el consumo de dieta entre los 3 grupos estudiados. Estos resultados confirmar&iacute;an la interacci&oacute;n entre el Ca y el Zn de la dieta a bajos niveles de Ca, durante el embarazo y lactancia. Esta acci&oacute;n  competitiva fue observada por Rossowska, en las ves&iacute;culas del ret&iacute;culo sarcopl&aacute;smico de m&uacute;sculo esquel&eacute;tico de ratas j&oacute;venes (33).</P>     <P ALIGN="JUSTIFY">Murray y col observaron en ratas, un incremento del contenido de Zn en hueso, con sustituci&oacute;n parcial del Ca &oacute;seo por el Zn, ante dietas bajas en Ca (34). Posteriormente, los trabajos de Kenney y McCoy, que suplementaron dietas bajas en Ca con cantidades moderadas de Zn, tambi&eacute;n en ratas, encontraron alteraciones en la relaci&oacute;n Zn/Ca en f&eacute;mur,  lo que se traduc&iacute;a en variaciones en sus dimensiones, contenido mineral y propiedades mec&aacute;nicas (15). Los presentes resultados de composici&oacute;n de f&eacute;mur concuerdan con los mencionados, ya que evidencian en la dieta baja en Ca un incremento de la relaci&oacute;n Zn/Ca en f&eacute;mur (<a href="#fig1">Figura 1</a>), lo cual corroborar&iacute;a la existencia de un reemplazo mineral del Ca por el Zn, ante dietas bajas en Ca, durante la gestaci&oacute;n y lactancia. </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">A pesar de la esencialidad del Zn tanto para el crecimiento fetal como para la salud materna, no ser&iacute;an deseables niveles demasiado altos de Zn en el organismo. Las concentraciones elevadas de Zn en f&eacute;mur, cuando la dieta materna es baja en Ca conducir&iacute;an a efectos negativos en las propiedades f&iacute;sicas y mec&aacute;nicas del hueso, as&iacute; como podr&iacute;an afectar otros procesos metab&oacute;licos (35,36). Aunque no hay evidencia, en humanos, acerca de efectos adversos debidos a cantidades elevadas de Zn provenientes de la dieta, recientemente se han establecido cifras m&aacute;ximas tolerables para el Zn proveniente de alimentos fortificados y suplementos dietarios. Dichos efectos adversos se asocian con alteraciones de la respuesta inmune y disminuci&oacute;n de la absorci&oacute;n del cobre (35) y no se han publicado estudios sobre sus relaciones con los niveles en fluidos o tejidos. Sin embargo, en funci&oacute;n de esos conocimientos es de importancia tener en cuenta no s&oacute;lo una ingesta de Ca adecuada, sino tambi&eacute;n una relaci&oacute;n Zn/Ca equilibrada, que permita mantener las concentraciones de Zn en los rangos fisiol&oacute;gicos normales.</P>     <P ALIGN="JUSTIFY">Los resultados del presente trabajo permiten concluir que, en el modelo experimental estudiado durante la gestaci&oacute;n y la lactancia, la interacci&oacute;n entre el Zn y el Ca, reflejada en el incremento de Zn en sangre y f&eacute;mur, existir&iacute;a a bajas concentraciones de Ca de la dieta materna. Por otra parte, el aumento del Ca de la dieta hasta los niveles utilizados en el presente estudio no afect&oacute; los niveles de Zn en hueso ni en sangre.
