Para estudiar la relación entre la antropometría materna y la ganancia neta de peso gestacional con el peso de nacimiento, y su asociación con los riesgos de Peso Bajo al Nacer (PBN) , Pequeño para la Edad Gestacional (PEG) y Recién Nacido (RN) Prematuro se evaluaron restrospectivamente 9613 registros del Sistema Informático Perinatal (OPS/OMS) del Hospital Sardá entre 1994 y 1995. Criterios de exclusión : muerte fetal, gemelar, malformaciones congénitas, ausencia del control prenatal y falta del registro de peso y talla. Se observó un 9,6 % del total de madres y 15% de adolescentes con bajo IMC preconcepcional (media 24,8 + 4,3 Kg/ m2), mientras que el sobrepeso-obesidad alcanzó al 28%. La ganancia neta de peso (media 9,25 + 4,9 Kg) representó el 16% del peso previo y fue mayor a menor IMC (p < 0,001). El peso al nacer (media 3375 + 467 g) fue menor a menor IMC (p < 0,001) y el riesgo de BPN (p<0,05), PEG (p< 0,05) y RN Prematuro (p = 0,05) fue mayor a menor peso e IMC así cuanto menor era la ganancia neta de peso( p< 0,001). El mejor predictor del riesgo de Peso Bajo al Nacer, Pequeño para la Edad Gestacional y RN Prematuro fue el peso preconcepcional "bajo" - 40-51 Kg - (OR ajustados 1,72 [IC 95% 1,48-1,95], 2,12 [1,82-2,41] y 1,46 [1,12-1,79] respectivamente). La ganancia neta de peso y diversas variables predictoras no explicaron más del 10,8% de la variabilidad del PN. <body><![CDATA[<P ALIGN="left"></B><font face="Times New Roman" size="3">A pesar de los progresos en la esfera socioecon&oacute;mica y en el cuidado obst&eacute;trico prenatal en los pa&iacute;ses desarrollados, la tasa de nacimientos de Peso Bajo al Nacer (PBN) se mantiene en alrededor del 6% (1)..</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En la Argentina esta tasa alcanza valores entre el 7 al 16% del total de nacimientos. Esto representa unos 49.000 a 112.000 ni&ntilde;os que demandan asistencia preferencial y que presentan un riesgo elevado de muerte o de morbilidad a corto o largo plazo (2).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En 1990 un informe del Instituto de Medicina (IOM) (3) de los EE.UU. concluy&oacute; que la ganancia de peso gestacional se asocia positivamente con el peso de nacimiento (PN) y que los extremos de esta ganancia eran predictores de la mortalidad neonatal, a&uacute;n despu&eacute;s de controlar para la edad gestacional (4-6).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El crecimiento fetal est&aacute; determinado en parte por caracter&iacute;sticas maternas como el peso preconcepcional, talla,<B> </B>nutrici&oacute;n materna, tabaquismo, abuso de alcohol y otras drogas.<B> </B>Adem&aacute;s est&aacute; influenciado por infecciones maternas, la salud del feto, fisiolog&iacute;a de la placenta, y la presencia de anomal&iacute;as cong&eacute;nitas.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El &Iacute;ndice de Masa corporal (IMC:<B> </B>kg/m<SUP>2</SUP>) preconcepcional  ha sido utilizado en numerosos estudios sobre resultados del embarazo, especialmente en la literatura europea y estadounidense (7,8) pero, para nuestro conocimiento, no ha sido evaluado en conjunci&oacute;n con la ganancia total de peso<B> </B>(GTP) y/o la ganancia neta de peso (GNP) en mujeres argentinas por lo que se decidi&oacute; estudiar sus efectos independientes (IMC, GTP y GNP) sobre diferentes resultados perinatales.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Diversos estudios han demostrado la importancia del riesgo<I> </I>para BPN, PEG y RN Prematuro<I> </I>asociados con en el peso preconcepcional y la ganancia de peso durante el embarazo (9), aunque el mecanismo biol&oacute;gico de la asociaci&oacute;n contin&uacute;an siendo en su mayor&iacute;a desconocidos. Aquellas mujeres con bajo peso preconcepcional para la talla est&aacute;n expuestas a un riesgo aumentado de resultados perinatales adversos (3).</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Intentar explicar c&oacute;mo la antropometr&iacute;a materna y la ganancia de peso gestacional  se asocian con el riesgo de diversos resultados perinatales adversos y c&oacute;mo podr&iacute;a usarse efectivamente esta informaci&oacute;n en el cuidado prenatal es lo que nos motiv&oacute; a realizar  la presente investigaci&oacute;n. Los <I>objetivos</I> del presente estudio<B> </B>fueron: estudiar la relaci&oacute;n entre la antropometr&iacute;a materna preconcepcional y la ganancia de peso durante la gestaci&oacute;n con el peso de nacimiento, y calcular el riesgo de Peso  Bajo al Nacer, Peque&ntilde;o para la Edad Gestacional y Reci&eacute;n Nacido Prematuro<B> </B>asociado a diferentes condiciones maternas, especialmente  la antropometr&iacute;a  y  la ganancia de peso  gestacional.</font></P> <B>     <P ALIGN="left"><font face="Times New Roman" size="3">MATERIAL Y METODOS</font></P> </B>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El dise&ntilde;o del estudio fue de tipo observacional y retrospectivo. Los datos fueron recolectados de la Base de Datos del <I>Sistema Inform&aacute;tico Perinatal</I> (S.I.P., OPS/OMS) (10) del Hospital Materno Infantil Ram&oacute;n Sard&aacute; de Buenos Aires desde 1988 (cobertura 97,7 %). Por consiguiente la unidad de an&aacute;lisis fue  el registro de esta Base de Datos. El profesional encargado del volcado de los datos constata la consistencia de los datos, procediendo a su correcci&oacute;n en caso de ser necesario, luego de consultar la Historia Cl&iacute;nica con el m&eacute;dico responsable. A pesar de este doble control, existe un porcentaje peque&ntilde;o de datos con subregistros e inconsistencias que fueron eliminados para  este estudio.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Se<B> </B> incluy&oacute; una muestra de 12860 reci&eacute;n nacidos (RN) vivos consecutivos con una edad gestacional (EG) entre 25 y 43 semanas y m&aacute;s de 500 gramos de peso al nacer desde el 1<SUP>o</SUP> de enero de 1994 al 31 de diciembre de 1995.  Se excluyeron<B> </B>3247 casos (25,2%) debido a la presencia de muerte fetal, embarazo m&uacute;ltiple, anomal&iacute;as cong&eacute;nitas y ausencia de cualquier variable usada en este estudio, particularmente el peso materno preconcepcional, la talla y la ganancia de peso. Estas exclusiones llevaron a una muestra final de 9613<B> </B>casos.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La exclusi&oacute;n de casos por falta de informaci&oacute;n podr&iacute;a haber introducido sesgos de estimaci&oacute;n<I> </I>de los efectos de diferentes factores maternos, debido a que estas mujeres pueden diferir de las que finalmente fueron incluidas tanto en variables mensurables como en aquellas desconocidas. Sin embargo, trabajos previos de Maddala (11) y Meng (12) indican que la exclusi&oacute;n de casos por datos faltantes, sumado al elevado tama&ntilde;o muestral ( n = 9613), no provoca sesgos de estimaci&oacute;n en los diferentes modelos.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para sostener lo anterior seleccionamos de la Base de Datos una muestra aleatoria de mujeres con datos faltantes (n = 1360); &eacute;stas  presentaron, en comparaci&oacute;n con las mujeres con datos completos, una significativa mayor proporci&oacute;n de tabaquismo ( 8,9% vs 6,7%, p = 0,0003) y PN ligeramente inferior  (3211 <U>+ </U>668 vs 3375 <U>+</U> 467 g, p &lt; 0,001), mientras que la edad materna, adolescencia, primiparidad, BP anterior, hipertensi&oacute;n, diabetes y sexo del RN no mostraron diferencias estad&iacute;sticas.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Las variables dependientes (resultados) de este estudio fueron: clasificaci&oacute;n del peso al nacer, defini&eacute;ndose Peso Bajo al Nacer (PBN) menor a 2500 gramos,<I> </I>clasificaci&oacute;n de peso seg&uacute;n edad gestacional, defini&eacute;ndose Peque&ntilde;o para la Edad Gestacional (PEG) si el peso estaba por debajo del percentil 10 para la edad gestacional -usando las Tablas de Referencia locales (13)<B>-</B> y clasificaci&oacute;n del reci&eacute;n nacido seg&uacute;n la edad gestacional defini&eacute;ndose Reci&eacute;n Nacido  Prematuro  al menor de 37 semanas de gestaci&oacute;n.</font></P>     <P ALIGN="JUSTIFY"> <font face="Times New Roman" size="3"> Las variables independientes incluidas fueron: edad materna, paridad, talla, n&uacute;mero de cigarrillos fumados por d&iacute;a, hipertensi&oacute;n previa o inducida por el embarazo (cualquier tipo), diabetes (de cualquier grado y duraci&oacute;n, incluyendo la diabetes gestacional), edad gestacional y peso pregestacional clasificado en tres categor&iacute;as (bajo: 40-51 kg, medio: 52-64 kg y alto: &#62; &#61;  65 kg), asumiendo que los puntos de corte corresponden al primero y tercer cuartiles de la distribuci&oacute;n.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El Indice de Masa Corporal<B> </B>(IMC&#61;  peso/talla<SUP>2</SUP>) preconcepcional se operacionaliz&oacute; en tres categor&iacute;as: bajo (<U>&lt;</U>  21,54 kg / m<SUP>2</SUP>), medio (2,55 a 2,57 kg / m<SUP>2 </SUP>)  y  alto (&gt; 26,57 kg / m<SUP>2</SUP> ) asumiendo que los puntos de corte corresponden al primero y tercer cuartiles de la distribuci&oacute;n.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La ganancia total de peso (GTP, en kg)  se calcul&oacute; restando del peso registrado en el &uacute;ltimo control prenatal el peso previo al embarazo referido por cada paciente. Si no se dispone del peso corporal medido hasta dos meses antes de la concepci&oacute;n, un sustituto aproximado del peso antes del embarazo puede ser el recuerdo de la madre o  una medici&oacute;n efectuada durante el primer trimestre del embarazo. (3) En la presente investigaci&oacute;n esto se vio corroborado  por la elevada correlaci&oacute;n entre el peso por recordatorio y el registrado en la 1ª consulta prenatal <I>antes</I> de la 20ª semana de gestaci&oacute;n (r  = 0,96, R<SUP>2</SUP> = 92%, p&lt; 0,05).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Debido a que el peso de un reci&eacute;n nacido a t&eacute;rmino representa aproximadamente m&aacute;s del 25% de la ganancia de peso materna durante la gestaci&oacute;n, se calcul&oacute; la Ganancia Neta de Peso (GNP), sustrayendo el peso al nacer a la  GTP y se categoriz&oacute; en baja (&lt; 6 kg), media (6 – 12 kg) y alta (&gt;12 kg), adoptando como puntos de corte el primer y  tercer cuartiles de la distribuci&oacute;n.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La duraci&oacute;n de la gesta fue calculada a partir del primer d&iacute;a de la &uacute;ltima menstruaci&oacute;n<I> </I>o una estimaci&oacute;n obst&eacute;trica basada en el resultado de la ultrasonograf&iacute;a. En caso de dudas se adopt&oacute; la edad gestacional por el examen f&iacute;sico del RN calculada utilizando el &Iacute;ndice de Capurro (14).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El an&aacute;lisis bivariado incluy&oacute; el test de Student para la  diferencia de las medias de variables continuas y  el de Kruskal-Wallis para la comparaci&oacute;n de medianas.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La contribuci&oacute;n independiente (no confundida) de diversos covariados sobre el crecimiento fetal se analiz&oacute; mediante el an&aacute;lisis de regresi&oacute;n lineal m&uacute;ltiple. Si las variables independientes eran significativas al nivel p&lt; 0,01 en el an&aacute;lisis de regresi&oacute;n lineal simple, entonces eran ingresadas al modelo utilizando el procedimiento &quot;inclusi&oacute;n hacia adelante&quot;<I> </I>(<I>forward</I> <I>stepwise</I>).</font> </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Debido al elevado sesgo de las distribuciones, la talla, IMC y cigarrillos por d&iacute;a fueron transformadas usando el log <SUB>10</SUB>  y los resultados se expresan luego de su transformaci&oacute;n antilogar&iacute;tmica.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para el prop&oacute;sito de la modelizaci&oacute;n en el an&aacute;lisis de regresi&oacute;n se incluyeron las siguientes variables de interacci&oacute;n  potencialmente &uacute;tiles en la predicci&oacute;n del peso de nacimiento: No. cigarrillos cuadr&aacute;ticos, &quot;raz&oacute;n&quot; (GNP/EG), &quot;proporci&oacute;n&quot; (GNP/peso preconcepcional x 100), GTP x talla,  GNP x IMC, y GTP x talla.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El peso preconcepcional y la ganancia total del<I> </I>peso fueron forzados en todos los modelos, dada su trascendencia sobre los objetivos del estudio. La hipertensi&oacute;n y la diabetes no se incluyeron como variables independientes en el modelo final debido a que  los coeficientes de la regresi&oacute;n no se modificaron sustancialmente al incluirlas, probablemente debido a su baja prevalencia.</font>  </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Se estim&oacute; el riesgo de Peso Bajo al Nacer, Peque&ntilde;o para la edad gestacional y RN Prematuro<B> </B>de acuerdo a la antropometr&iacute;a materna preconcepcional y la ganancia de peso (variables de exposici&oacute;n) durante el embarazo  mediante el an&aacute;lisis de regresi&oacute;n log&iacute;stica m&uacute;ltiple<I>, </I>ajustando el odds ratio (ORa)  para  las mismas variables independientes  utilizadas en la regresi&oacute;n lineal m&uacute;ltiple. Para evaluar la  bondad del ajuste de los modelos se utiliz&oacute; el &quot;likelihood ratio test&quot; (un estad&iacute;stico chi cuadrado).</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El an&aacute;lisis estad&iacute;stico se realiz&oacute; utilizando el programa Statistica (Version 5,1, Statsoft, Tulsa, OK, USA). Debido al tama&ntilde;o de la muestra y el n&uacute;mero de asociaciones testeadas, solamente un valor p &lt; 0,01 fue considerado estad&iacute;sticamente significativo.</font></P> <B>     <P ALIGN="left"><font face="Times New Roman" size="3">RESULTADOS</font></P> </B>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La <a href="#tabla1"> Tabla 1</a> contiene las estad&iacute;sticas descriptivas para la madre y el reci&eacute;n nacido, se&ntilde;alando el perfil de marginalidad de las mujeres atendidas:  elevada prevalencia de adolescentes, 40% casadas, bajo nivel educativo (84% hab&iacute;an alcanzado menos de 12 a&ntilde;os de educaci&oacute;n) y edad gestacional a la primera visita avanzada (24 <U>+</U> 8 semanas). La  relaci&oacute;n masculino / femenino<B> </B>y la edad gestacional media de los reci&eacute;n nacidos fue de 1,005 y 39,0 semanas respectivamente. Utilizando el patr&oacute;n internacional del IMC preconcepcional propuesta por el IOM (3) de los EE.UU.  la incidencia de bajo IMC materno (&lt;19,8 kg/m2) alcanz&oacute; 9,68% mientras que la del sobrepeso/ obesidad (<U>&gt;</U> 26,1 kg/m2) ascendi&oacute; a 28,5%. La mediana<B> </B>de la ganancia total  de peso se ubic&oacute; en 12 kg (9 – 16) mientras que la de la ganancia neta  fue menor, 9 kg  (6 –12), representando el  16% del peso previo y fue mayor a menor IMC (p &lt; 0,001). El peso medio al nacer  fue de  3375 <U>+</U> 467 gramos y  fue menor a menor IMC (p &lt; 0,001).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La <a href="#tabl2"> Tabla 2</a> resume la predicci&oacute;n del peso de nacimiento a partir de diferentes variables maternas utilizando el modelo de regresi&oacute;n lineal simple. Todas fueron significativas excepto cigarrillos por d&iacute;a.  Los mayores efectos sobre el PN se observaron con el IMC (0,390 Kg por cada kg/m<SUP>2   </SUP>materno  al comienzo del embarazo) y la ganancia neta de peso ajustada para la edad gestacional, evitando de esta manera el sesgo de la prematurez  (0,393 Kg por cada kg ganado por semana de EG). Como era previsible el h&aacute;bito de fumar ejerci&oacute; un efecto negativo (disminuci&oacute;n) sobre el peso de nacimiento.  En general los valores  R<SUP>2  </SUP>hallados fueron peque&ntilde;os y explicaron entre el 1% y 5% de la variabilidad del peso al nacer.</font></P>      ]]></body>

<P ALIGN="left"><font face="Times New Roman" size="3">DISCUSION</font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Este estudio muestra que tanto la antropometr&iacute;a materna as&iacute; como la ganancia de peso<I> </I>inadecuada durante el embarazo son predictoras de diferentes resultados perinatales –Peso al Nacer, Peque&ntilde;o para la Edad Gestacional y Prematurez- ajustadas para diversas condiciones reconocidas por afectar el crecimiento fetal, confirmando los hallazgos de otros autores sobre los efectos de diversas variables maternas sobre el crecimiento fetal y la duraci&oacute;n de la gestaci&oacute;n. (15,16).</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En comparaci&oacute;n con el patr&oacute;n  de referencia de la OMS (1983)  que especifica un peso de 55 kg y una talla de 16,7<B> </B>cm para una mujer tipo no embarazada, en los pa&iacute;ses en v&iacute;as de desarrollo, como la Argentina, la mayor&iacute;a de las mujeres tienen promedios inferiores. Los valores de nuestro estudio difieren fundamentalmente en la talla y al compararlos con los resultados de una encuesta nutricional de 263 embarazadas efectuada en cinco Centros de Salud pertenecientes a hospitales p&uacute;blicos de la ciudad de Buenos (17), se pudo  comprobar una mayor prevalencia de mujeres con talla baja (&lt; 150cm, 25%  vs 9,5%).  Esto ser&iacute;a el reflejo de una peque&ntilde;a pero importante contribuci&oacute;n gen&eacute;tica, pero principalmente una desnutrici&oacute;n cr&oacute;nica que comenz&oacute;, en alguna de ellas, en la vida intrauterina y luego perpetuadas hasta la edad reproductiva por  la carencia marginal de energ&iacute;a y nutrientes, a la reiteraci&oacute;n de procesos infecciosos agudos y a las condiciones socioecon&oacute;micas desfavorables en que vive la ni&ntilde;a, adem&aacute;s de cierta restricci&oacute;n en el tama&ntilde;o de la pelvis.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El segundo problema  nutricional detectado en este estudio en t&eacute;rminos de frecuencia fue el sobrepeso, coincidiendo con una encuesta antropom&eacute;trica  realizada en la ciudad de Buenos Aires (18) sugiriendo <A NAME="texto"></A>cambios en la conducta alimentaria de la poblaci&oacute;n joven.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Una limitaci&oacute;n del presente estudio es que los cuartiles del IMC y la ganancia de peso utilizados con fines de modelizaci&oacute;n podr&iacute;an haber sido demasiado r&iacute;gidos como predictores para permitir la detecci&oacute;n de una m&aacute;s s&oacute;lida asociaci&oacute;n entre el  peso al nacer y el IMC o la ganancia de peso. La exclusi&oacute;n del IMC en los modelos de predicci&oacute;n del PN (<a href="#tabl3">tabla 3</a>) y estimaci&oacute;n de riesgos (<a href="#tabla4">tabla 4</a>) podr&iacute;a explicarse por su colinealidad con el peso y la talla.</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La relativamente baja prevalencia de PBN, PEG y RN prematuro  en comparaci&oacute;n con la poblaci&oacute;n general asistida (19) se atribuye a los criterios de selecci&oacute;n adoptados.</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Una probable explicaci&oacute;n de la falta de asociaci&oacute;n estad&iacute;stica entre la talla materna con el peso de nacimiento y los riesgos de PBN, PEG y RN prematuro es que esta asociaci&oacute;n estar&iacute;a mediada por el peso preconcepcional as&iacute; como por el bajo peso para la talla. As&iacute;, se observ&oacute; una elevada prevalencia de sobrepeso/obesidad en mujeres de talla baja en comparaci&oacute;n con las de talla normal (43,3% vs 28,5%, p &lt; 0.001).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Esto concuerda con el estudio de Adair (20), mientras que Stein demostr&oacute; que la talla no modificaba la incidencia de  la prematurez, aunque su incremento reduc&iacute;a las tasas de RCIU. (21). Por &uacute;ltimo estudios recientes incluyendo un metan&aacute;lisis concluyen que la estatura materna aisladamente no es un buen predictor del peso de nacimiento, sino de complicaciones en el trabajo de parto. (22-24).</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los efectos del peso preconcepcional, talla y ganancia de peso, aunque peque&ntilde;os en magnitud (10 a 60 g ), son importantes desde el punto de vista cl&iacute;nico. De todos modos, solamente una peque&ntilde;a fracci&oacute;n de la varianza del PN era explicada utilizando diferentes medidas. En el estudio del <I>National Collaborative Perinatal Project </I> (25), sobre una muestra de 7829 mujeres blancas que inclu&iacute;an en el modelo al peso previo, ganancia de peso, cigarrillos por d&iacute;a, talla,  nivel socioecon&oacute;mico y  tensi&oacute;n arterial sist&oacute;lica se hall&oacute; un R2 ajustado = 0,386 para la predicci&oacute;n del PN, valor pr&aacute;cticamente similar al calculado en el presente estudio (0,373).</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Nuestros resultados tambi&eacute;n se corresponden con lo comunicado por Hickey en que el modelo de regresi&oacute;n explicaba como m&aacute;ximo el 32% (p&lt;0,004) de la variabilidad del PN, ajustado a una baja ganancia de peso por trimestre, edad, talla, IMC, tabaquismo, alcohol, RN previo con PN &lt;2750 gramos y sexo. (26).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Por consiguiente una de las principales conclusiones de este estudio  es que una proporci&oacute;n de la variabilidad del PN a diferentes edades gestacionales permanece sin explicaci&oacute;n. Parte de esta variabilidad podr&iacute;a reflejar errores aleatorios en las mediciones (especialmente la talla, aunque su <I>coeficiente de variaci&oacute;n </I>es bajo –3.8%), pero la mayor&iacute;a representa verdadera variabilidad biol&oacute;gica, cuyas causas no pueden indagarse con el presente dise&ntilde;o.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El hecho de que en el modelo logit (<a href="#tabla4">Tabla 4</a>) la &uacute;nica condici&oacute;n materna asociada con un incremento <I>biol&oacute;gicamente </I>importante del riesgo de procrear un RN con peso menor a 2500 gramos, peque&ntilde;o para la edad gestacional o prematuro fue el peso preconcepcional entre 40 y 51 kg - para este estudio categorizado como <I>bajo</I>-  concuerda con el estudio colaborativo de la OMS (22).</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El hallazgo de un  elevado riesgo de PEG en mujeres  portadoras de peso bajo  coincide con  las  evidencias de que cuanto mayor es la incidencia de <I>peso bajo </I> <I>al nacer</I> en una poblaci&oacute;n, mayor es la proporci&oacute;n del PEG, permaneciendo relativamente constante el n&uacute;mero de RN prematuros.  Por esto las investigaciones sobre <I>factores de riesgo</I>  asociados al PBN en pa&iacute;ses &quot;emergentes o en desarrollo&quot; como la Argentina deber&iacute;an enfocarse hacia el PEG. (27).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La ganancia neta de peso, aunque estad&iacute;sticamente significativa, present&oacute; un m&iacute;nimo efecto protector, es decir, que por cada 1 kg de aumento de peso durante la gestaci&oacute;n el riesgo de peso bajo  al nacer disminu&iacute;a en promedio 0,06 unidades. Debido a que la ganancia total de peso se registra hasta  el comienzo del trabajo de parto prematuro algunos autores (28) postulan que ser&iacute;a  inapropiado el uso de la ganancia neta de peso  a ra&iacute;z de que no corresponder&iacute;a  sustraer el peso de nacimiento de la ganancia total porque en  la mayor&iacute;a de los casos de internaci&oacute;n con amenaza de parto prematuro  el peso fetal (desconocido) es sustancialmente menor que al momento del nacimiento. Sin embargo, al repetir el modelo utilizando la ganancia total en lugar de la neta el OR ajustado no vari&oacute; significativamente (datos no presentados).</font>   </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Varios estudios han demostrado que la ganancia inadecuada de peso durante la segunda mitad de la gestaci&oacute;n es predictor  de resultados perinatales adversos (29-30). Sin embargo, no existe  un claro<B> </B>mecanismo biol&oacute;gico que soporte un rol causal de la escasa ganancia de peso, aunque en el caso de parto prematuro podr&iacute;a ser un &quot;marcador&quot;, como lo destaca  una revisi&oacute;n cr&iacute;tica de esta  relaci&oacute;n (31). Una baja ganancia de peso reflejar&iacute;a una inadecuada ingesta durante el embarazo y, consecuentemente, de necesidades b&aacute;sicas insatisfechas; esto desencadenar&iacute;a la producci&oacute;n local de prostaglandinas y parto prematuro (32) .</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Sin embargo la controversia surge cuando estas evidencias se intentan extrapolar a comunidades con bajos recursos. As&iacute; Kusin y col. estudiando una poblaci&oacute;n de 982 mujeres y sus RN de Java , sostienen que no hay acuerdo aun en la relaci&oacute;n entre la nutrici&oacute;n materna y los resultados reproductivos en comunidades con ingesta cal&oacute;rica marginal, ya sea &eacute;sta estacional o cr&oacute;nica. La opini&oacute;n actual, sostiene este autor, es que los requerimientos energ&eacute;ticos para la reproducci&oacute;n son relativamente bajos y pueden compensarse por mecanismos protectores como una baja tasa metab&oacute;lica<I> </I>basal y disminuci&oacute;n de la actividad f&iacute;sica durante el embarazo (33).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">De manera similar el h&aacute;bito de fumar<I> </I> durante la gestaci&oacute;n, aunque mostr&oacute; un leve incremento del riesgo, no detect&oacute; ning&uacute;n efecto biol&oacute;gico destacable a pesar de la significaci&oacute;n estad&iacute;stica.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Debido a que los factores antropom&eacute;tricos y nutricionales operan presumiblemente durante toda la gestaci&oacute;n, nuestros resultados no resuelven la especificidad y momento de la malnutrici&oacute;n materna u otras influencias adversas ambientales (adem&aacute;s de la hipertensi&oacute;n arterial, tabaquismo, etc.) sobre el  tama&ntilde;o al nacer.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Como implicancias para la Salud P&uacute;blica en el logro del peso de nacimiento &oacute;ptimo el presente estudio sugiere que se requiere una diferencia cuantitativa en el estado nutricional materno <I>antes </I>del embarazo y mantenido <I>a trav&eacute;s</I> de la gestaci&oacute;n.
