Se utilizaron datos de su crecimiento, morbilidad y características de la alimentación además de los valores de hemoglobina en diversos momentos (6,9,12,18 y 24 meses de edad) medidos en un hemoglobinómetro digital portátil (Hemocue). Para definir anemia se consideró un valor inferior a 11,0g/dL. Se realizó análisis simple por regresión logística con variables socioeconómicas, grupo etario, estado nutricional, enfermedades, ingestión de leche materna e introducción de alimentos. Se observó que la ingestión de leche de vaca a los 4 meses de edad y el orden de nacimiento superior a 4 fueron factores de riesgo para la anemia (OR ~ 2). A su vez la edad superior a 18 meses, la ingestión de fórmula infantil, de alimentos fuentes de vitamina C, de carne y de frijoles a los 6 meses fueron factores de protección (OR ~ 0.5). Se comprobó la importancia de una adecuada introducción de alimentos en el sexto mes de vida, para prevención de la anemia ferropriva en la infancia.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY"><B>INTRODUCCION</B></P>     <P ALIGN="JUSTIFY">Los ni&ntilde;os menores de dos a&ntilde;os de edad presentan un elevado riesgo de anemia ferropriva debido a la alta demanda de hierro para su crecimiento y adem&aacute;s, por el aporte insuficiente y baja disponibilidad del mineral en la dieta. Por este motivo, son considerados junto con las mujeres embarazadas, los grupos de mayor  vulnerabilidad (1,2). La consecuencia m&aacute;s grave de la anemia para los ni&ntilde;os peque&ntilde;os son las alteraciones irreversibles en el desarrollo psicomotor que persisten hasta la edad escolar (3).</P>     <P ALIGN="JUSTIFY">En Brasil, ninguna encuesta nacional realizada sobre salud y nutrici&oacute;n incluy&oacute; la evaluaci&oacute;n de prevalencia de la anemia por deficiencia de hierro. Sin embargo, se llevaron a cabo estudios en distintas localidades del pa&iacute;s verificando que es la carencia nutricional que se encuentra con m&aacute;s frecuencia (4).  Particularmente en la ciudad de S&atilde;o Paulo, desde hace 30 a&ntilde;os se observa el aumento significativo en la prevalencia de anemia. En los ni&ntilde;os menores de dos a&ntilde;os, notoriamente los m&aacute;s afectados, la prevalencia pas&oacute; de 41,8% en 1973-1974 (5) a 56,9% en 1984-85 y a 67,6% en 1995-96 (6).</P>     <P ALIGN="JUSTIFY">Ese aumento no se puede explicar ni por factores socioecon&oacute;micos, tales como recursos y escolaridad materna, ni por las condiciones ambientales, saneamiento y acceso a servicios b&aacute;sicos de salud, ya que estos tuvieron una evoluci&oacute;n favorable en la ciudad. En relaci&oacute;n al consumo alimentario, a pesar de constatar la baja densidad de hierro en la dieta de los ni&ntilde;os, tampoco se identificaron cambios que justificasen el aumento de la prevalencia, concluyendo que es necesario que se lleven a cabo estudios prospectivos, capaces de evaluar m&aacute;s precisamente las caracter&iacute;sticas de la alimentaci&oacute;n infantil y su relaci&oacute;n con la presencia de la anemia ferropriva (6).  Se debe tomar en cuenta que los factores nutricionales son los principales agentes causales de la deficiencia de hierro en ni&ntilde;os de edad temprana, por tanto es prioritario conocer las pr&aacute;cticas alimentarias y de ablactaci&oacute;n, para garantizar la efectividad de las intervenciones que solucionen el problema (7,8) .</P>     <P ALIGN="JUSTIFY">En este sentido el objetivo del estudio fue analizar las caracter&iacute;sticas de salud y nutrici&oacute;n, enfatizando la lactancia materna y la introducci&oacute;n de alimentos, buscando identificar factores relacionados con el riesgo y la protecci&oacute;n de la anemia ferropriva en ni&ntilde;os menores de 24 meses de edad, de comunidades de bajos recursos, seguidos de forma prospectiva desde el nacimiento en un programa de atenci&oacute;n primaria de salud en la ciudad de S&atilde;o Paulo – Brasil.</P> <B>    <P ALIGN="justify">METODOS</P>     <P ALIGN="justify"></B>Para realizar el presente estudio se utiliz&oacute; informaci&oacute;n del banco de datos del &quot;Projeto Favela&quot; de la Universidade Federal de S&atilde;o Paulo -  UNIFESP/EPM, que es un programa de atenci&oacute;n primaria de salud desarrollado en comunidades de bajos recursos del barrio de Vila Mariana en la ciudad de S&atilde;o Paulo, Brasil. Se ejecuta con un equipo de nutricionistas que promueven la educaci&oacute;n en salud y en nutrici&oacute;n a trav&eacute;s de visitas domiciliarias, con vistas a la  prevenci&oacute;n de carencias nutricionales en el grupo materno-infantil. A partir de la identificaci&oacute;n de las mujeres embarazadas en las comunidades, se les explica en qu&eacute; consiste el acompa&ntilde;amiento de salud y nutrici&oacute;n y con su consentimiento se realizan las visitas y evaluaciones. Idealmente las visitas son mensuales en el primer a&ntilde;o de vida del ni&ntilde;o y trimestrales en el segundo, donde se acompa&ntilde;a el crecimiento, alimentaci&oacute;n y condiciones de salud. La evaluaci&oacute;n de la hemoglobina (Hb) se realiza alrededor de los 6, 9, 12, 18 y 24 meses de vida. Toda la informaci&oacute;n del seguimiento es transferida a un banco de datos (9).</P>     <P ALIGN="JUSTIFY">Para determinar el valor de la hemoglobina se utiliza un hemoglobin&oacute;metro digital port&aacute;til (HEMOCUE- <FONT FACE=Symbol>&#98;</FONT>  hemoglobin- Photometer) (10) y se toma la muestra de sangre por punci&oacute;n digital o del tal&oacute;n del ni&ntilde;o cuando &eacute;ste no camina.<SUP>  </SUP>Se clasifican como an&eacute;micos los ni&ntilde;os con valores de hemoglobina inferiores a 11,0 g/dL (1).  Si en la ocasi&oacute;n de la medici&oacute;n el ni&ntilde;o, seg&uacute;n la madre, presenta datos cl&iacute;nicos de infecci&oacute;n como fiebre u otra enfermedad, o est&aacute; en tratamiento, se eval&uacute;a en edad posterior. Se toma esta precauci&oacute;n, ya que en procesos infecciosos comunes en la infancia, ocurre una disminuci&oacute;n del valor de la hemoglobina, que puede interferir en los resultados (11) . </P>     <P ALIGN="JUSTIFY">Seg&uacute;n el resultado de la evaluaci&oacute;n de la Hb en los diferentes momentos durante el acompa&ntilde;amiento, los ni&ntilde;os reciben dosis profil&aacute;ctica o terap&eacute;utica con hierro medicamentoso. Esta conducta de prevenci&oacute;n y tratamiento de la anemia ferropriva est&aacute; incluida en la rutina de intervenci&oacute;n que se lleva a cabo en las comunidades seguidas por el &quot;Projeto Favela&quot; desde 1996 (9).  Adem&aacute;s, se orienta el consumo de alimentos fuentes de hierro hem&iacute;nico (carnes, menudos) y de hierro no hem&iacute;nico (frijoles, verduras) asociado al consumo de alimentos fuentes de &aacute;cido asc&oacute;rbico (naranja, papaya, mango), porque estos facilitan la absorci&oacute;n de este tipo de hierro. Tambi&eacute;n se orienta el horario correcto para el consumo de alimentos que contienen inhibidores de la absorci&oacute;n del hierro no hem&iacute;nico (leche de vaca, infusiones). Estas orientaciones son transmitidas durante las visitas domiciliarias y en reuniones con las madres en la comunidad. </P>     <P ALIGN="JUSTIFY">Para verificar el estado nutricional se obtiene el peso de los ni&ntilde;os en una balanza electr&oacute;nica, con escala ponderal de 50 g y se utiliza el escore Z (desviaciones est&aacute;ndar) para el indicador peso/edad usando las referencias del National Center for Health Statistics- NCHS. Para los c&aacute;lculos se utiliza el Epinut- Epi Info versi&oacute;n 6.02 (12).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">En este trabajo se incluyeron datos del seguimiento de 130 ni&ntilde;os menores de 24 meses de edad realizado entre los a&ntilde;os de 1996 y 2002. En ese per&iacute;odo nacieron 317 ni&ntilde;os en las comunidades atendidas. Sin embargo, no se incluyeron aquellos con antecedentes patol&oacute;gicos perinatales (3),  peso al nacer inferior a 2500 g (34), ni gemelaridad (2),  adem&aacute;s de los que no tuvieron seguimiento peri&oacute;dico y constante desde el nacimiento (148). En el grupo de ni&ntilde;os incluidos en el estudio se totalizaron 371 mediciones de Hb entre los 6 y los 24 meses de edad. </P>     <P ALIGN="JUSTIFY">El protocolo de este estudio fue aprobado por el Comit&eacute; de &Eacute;tica en Investigaci&oacute;n de la Universidade Federal de S&atilde;o Paulo.</P>     <P ALIGN="JUSTIFY">El an&aacute;lisis estad&iacute;stico se realiz&oacute; por regresi&oacute;n log&iacute;stica simple para estimar la probabilidad de ocurrencia de anemia, desde los 6 hasta los 24 meses de edad, en funci&oacute;n de variables que se consideraron influyentes: sexo, edad (dividida en tres grupos: 6 <FONT FACE="WP MathA">&#124;</FONT> 12,12 <FONT FACE="WP MathA">&#124;</FONT> 18,18 <FONT FACE="WP MathA">&#124;</FONT> 24 meses – donde se fij&oacute; la edad de 6 <FONT FACE="WP MathA">&#124;</FONT> 12 como de riesgo=1, para comparaci&oacute;n con las dem&aacute;s), orden de nacimiento (1º, 2º, 3º, 4º o m&aacute;s – donde se fij&oacute; el orden 1, como de riesgo=1, para comparaci&oacute;n con las dem&aacute;s), escolaridad materna (hasta o m&aacute;s de 4 a&ntilde;os de estudio) y edad materna (menor o igual a 20 a&ntilde;os o<FONT COLOR="#ff0000"> </FONT>mayores), n&uacute;mero de episodios de enfermedades (hasta 5 o m&aacute;s - hasta el momento de la evaluaci&oacute;n de Hb),  estado nutricional en el momento de la evaluaci&oacute;n de la Hb (menor o igual que -1Z o mayor - para el indicador Peso/Edad). Las variables de alimentaci&oacute;n fueron dicotomizadas  (Si o No)  seg&uacute;n la ingesta de determinados alimentos a los 4 y a los 6 meses: leche materna, leche de vaca, f&oacute;rmula infantil, alimentos fuentes de vitamina C, carnes y frijoles<FONT COLOR="#ff0000">. </FONT>No se incluy&oacute; la variable peso al nacer ya que los ni&ntilde;os con bajo peso fueron intencionalmente excluidos;  ni la toma de suplementos de hierro ya que todos los ni&ntilde;os seguidos recibieron orientaci&oacute;n y medicamentos para hacerlo, como recomienda la OMS (1).</P>     <P ALIGN="JUSTIFY">Para obtener los OR brutos (Odds Ratio) y el intervalo de confianza de cada variable, se utiliz&oacute; la regresi&oacute;n log&iacute;stica simple (13) .  Se consideraron resultados estad&iacute;sticamente significativos aquellos cuyos intervalos de confianza (de 95%) se alejaron del valor 1 (menores que 1 significaron protecci&oacute;n y mayores que 1 significaron riesgo). Para las variables de protecci&oacute;n cuyos valores de p fueron &lt;0.10 se ajust&oacute; el OR para el grupo etario, donde se fij&oacute; la edad de 6 <FONT FACE="WP MathA">&#124;</FONT> 12. Por tanto los resultados de OR ajustados muestran la protecci&oacute;n en relaci&oacute;n a la anemia, de los ni&ntilde;os en el segundo a&ntilde;o de vida (12 a 24 meses), en funci&oacute;n del consumo o no de determinado alimento a los 4 y 6 meses de vida. Por tratarse de datos repetidos (m&aacute;s de una medici&oacute;n por ni&ntilde;o) los modelos estad&iacute;sticos utilizados fueron los modelos lineales generalizado (14)  con estimaci&oacute;n  GEE (ecuaciones de estimaci&oacute;n generalizadas) (15) . Para los c&aacute;lculos se utiliz&oacute; el Stata Statistical Software (16).</P>     <P ALIGN="JUSTIFY"><B>RESULTADOS</P> </B>    <P ALIGN="JUSTIFY">De los 130 ni&ntilde;os seguidos, 69 eran del sexo masculino y 61 del femenino. El n&uacute;mero de mediciones en cada grupo etario fue: 162 de los 6 a los 12 meses, 115 de los 12 a los 18 meses y 94 de los 18 a los 24 meses. Todas las variables estudiadas se encuentran en la <a href="#TABLA 1"> Tabla 1</a>. Se verific&oacute; que los factores relacionados con el riesgo mayor de anemia ferropriva fueron el orden de nacimiento igual o superior a 4 (OR=2,1) y la ingesta de leche de vaca en el cuarto mes de vida (OR=1,5). Con relaci&oacute;n a los factores de protecci&oacute;n se verific&oacute; que el grupo etario de 18 <FONT FACE="WP MathA">&#124;</FONT>  24 meses, comparado con el de 6 <FONT FACE="WP MathA">&#124;</FONT> 12,  tuvo menos chance de presentar anemia. Sobre el consumo alimentario, la ingesta de f&oacute;rmula a los 4 y a los 6 meses proporcion&oacute; una protecci&oacute;n de cerca del 60%. Tambi&eacute;n se encontr&oacute; que los ni&ntilde;os que consum&iacute;an fuentes de vitamina C, carne y frijoles a los 6 meses, tuvieron alrededor de 2 veces menos chance de desarrollar anemia. Con el ajuste de los valores de OR para la edad (<a href="#TABLA 2">Tabla 2</a>) se pudo verificar que el efecto protector de la ingesta de f&oacute;rmula, de fuentes de vitamina C y de frijoles a los 6 meses, continuaba presente en la edad de 12 a 24 meses (p&lt;0.05). </P>     <P ALIGN="CENTER"><a name="TABLA 1">TABLA 1</a></P>     <P ALIGN="justify">Valores de Odds Ratio (OR) bruto e intervalo de confianza de variables relacionadas con la anemia ferropriva en ni&ntilde;os menores de 24 meses de bajo nivel socioecon&oacute;mico. <body><![CDATA[<P ALIGN="JUSTIFY">Frijoles 6m</FONT></TD> <TD WIDTH="21%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="justify">0,58</FONT></TD> <TD WIDTH="15%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="justify">0,031</FONT></TD> <TD WIDTH="11%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="justify">0,36</FONT></TD> <TD WIDTH="6%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="justify">–</FONT></TD> <TD WIDTH="15%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="justify">0,95</FONT></TD> </TR> </TABLE> </CENTER>  <FONT FACE="Courier New" SIZE=2> </FONT><B>    <P ALIGN="justify">DISCUSION</P> </B>     <P ALIGN="JUSTIFY">Los factores de riesgo para la anemia ferropriva encontrados en este estudio empezando por el orden de nacimiento superior a 4 y la edad menor a 18 meses, ya que estar en el grupo etario de 18-24 meses se constituy&oacute; en protecci&oacute;n, fueron confirmados por otros dos estudios realizados recientemente en el sur de Brasil con ni&ntilde;os menores de 3 a&ntilde;os de edad para evaluar la prevalencia y los factores de riesgo para la anemia. Newmann et al (17), encontraron que estar en la edad de 12 a 18 meses represent&oacute; cerca de 6 veces m&aacute;s riesgo de anemia comparado a los dem&aacute;s grupos etarios (6-12, 18-24, 24-36 meses). La existencia de m&aacute;s de 3 personas por habitaci&oacute;n utilizada para dormir fue la otra variable identificada, con un riesgo del orden de 3. Como en nuestro estudio, la anemia no se asoci&oacute; con sexo ni con escolaridad materna. </P>     <P ALIGN="JUSTIFY">En el estudio de Silva et al (18), el grupo etario de 12 a 23 meses de edad y la presencia de dos o m&aacute;s hermanos, representaron un riesgo 1,4 veces mayor de anemia. Los autores sugieren que ese riesgo aumentado por la presencia de hermanos, puede deberse a una mayor demanda por alimentos en la casa, adem&aacute;s de dividir la atenci&oacute;n dispensada al ni&ntilde;o en relaci&oacute;n a los cuidados con alimentaci&oacute;n y salud en general. Otras variables estudiadas como sexo del ni&ntilde;o, escolaridad y edad materna no fueron factores determinantes de la anemia.</P>     <P ALIGN="JUSTIFY">La anemia tampoco estuvo asociada a las enfermedades, ni a las variables antropom&eacute;tricas y ni al tipo de lactancia a los 4 meses de edad, &uacute;nica variable diet&eacute;tica evaluada en esos estudios (17,18).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">El efecto protector de la lactancia materna exclusiva hasta los 6 meses de edad sobre la anemia ferropriva en ni&ntilde;os con peso al nacer superior a 3000 g, ha sido documentado (19). Sin embargo, el hecho de que otros estudios y tambi&eacute;n el nuestro no verifiquen ese efecto, probablemente se debe a la baja duraci&oacute;n de la lactancia materna exclusiva en Brasil. La &uacute;ltima encuesta nacional sobre demograf&iacute;a y salud, realizada en el a&ntilde;o 1996, encontr&oacute; una duraci&oacute;n mediana de 1 mes. Entonces, es primordial desarrollar estrategias para aumentar la lactancia materna, no s&oacute;lo para la prevenci&oacute;n de la anemia ferropriva sino tambi&eacute;n para promover la salud infantil, principalmente en poblaciones de bajo nivel socioecon&oacute;mico.</P>     <P ALIGN="JUSTIFY">Es importante destacar que el nivel socioecon&oacute;mico, medido por la variable ingreso familiar, fue identificado como de riesgo para la anemia en los dos estudios brasile&ntilde;os mencionados. En nuestro caso, como el estudio fue realizado exclusivamente en una poblaci&oacute;n de bajo nivel socioecon&oacute;mico, no ten&iacute;a sentido estimar el impacto de esta variable. Incluso la escolaridad materna dif&iacute;cilmente ser&iacute;a identificada como un factor de influencia ya que es homog&eacute;neamente baja en esta poblaci&oacute;n.</P>     <P ALIGN="JUSTIFY">Sobre el estado nutricional y su poca o ninguna relaci&oacute;n con la anemia, un estudio con ni&ntilde;os brasile&ntilde;os de 12 a 72 meses de edad, verific&oacute; la coexistencia de un buen estado nutricional (prevalencia de d&eacute;ficit antropom&eacute;trico menor que 5%) con elevados niveles de anemia ferropriva, 68,7%. Se recomend&oacute; entonces que los programas de nutrici&oacute;n deberian ser revisados para no incluir solamente energ&iacute;a y prote&iacute;nas sino garantizar el suministro de hierro (20) .</P>     <P ALIGN="JUSTIFY">Otro factor relacionado a un riesgo mayor de anemia ferropriva en los ni&ntilde;os estudiados, fue la ingesta de leche de vaca en el cuarto mes de vida. En el estudio de G&oacute;mez et al (7),  con lactantes de 6 a 12 meses de edad de la ciudad de La Habana, se estudiaron factores de riesgo biol&oacute;gicos como el peso al nacer y enfermedades, adem&aacute;s de la duraci&oacute;n de la lactancia materna exclusiva y caracter&iacute;sticas de la ablactaci&oacute;n. Se encontraron niveles significativamente m&aacute;s bajos de hemoglobina en los ni&ntilde;os que ten&iacute;an antecedentes de enfermedad diarreica aguda en los &uacute;ltimos 3 meses, llevando a p&eacute;rdidas del mineral, y en aquellos en que se introdujo la leche de vaca antes de los 90 d&iacute;as. </P>     <P ALIGN="JUSTIFY">La utilizaci&oacute;n de leche de vaca en el primer a&ntilde;o de vida est&aacute; contraindicada tanto por el bajo contenido de hierro como por el riesgo de micro hemorragias intestinales. Ziegler et al (21), encontraron relaci&oacute;n entre el consumo de leche de vaca y la p&eacute;rdida de sangre por el tracto intestinal de ni&ntilde;os saludables con edades entre 7 y 12 meses. Los autores concluyeron que esa p&eacute;rdida desaparece al cumplir el ni&ntilde;o 1 a&ntilde;o de edad, indicando que el tracto intestinal pierde gradualmente su sensibilidad a la prote&iacute;na de la leche de vaca. </P>     <P ALIGN="JUSTIFY">Un estudio en Suecia con ni&ntilde;os de 2 a&ntilde;os y medio de edad, encontr&oacute; que  la ingesta de leche de vaca entera se relacionaba positivamente con la deficiencia de hierro a&uacute;n en esta edad (22).  Mas los resultados son conflictivos ya que otro estudio mostr&oacute; que despu&eacute;s de 1 a&ntilde;o de edad, en ni&ntilde;os con buenas reservas de hierro, aparentemente el consumo de leche de vaca, fortificada o no con hierro, no afectaba el estado nutricional del mineral (23), Kazal (24), destaca que en caso de que se utilice la leche de vaca en el segundo a&ntilde;o de vida, el consumo debe ser limitado a la cantidad de 500-700 mL al d&iacute;a. Adem&aacute;s recomienda que, si se necesita sustituir la leche materna, se debe utilizar f&oacute;rmula infantil modificada enriquecida con hierro durante los primeros 12 meses de vida y si es posible, hasta los 24 meses.</P>     <P ALIGN="JUSTIFY">Confirmando la pertinencia de esa recomendaci&oacute;n, en los ni&ntilde;os de nuestro estudio que consumieron f&oacute;rmula infantil a los 4 y a los 6 meses, se comprob&oacute; el efecto protector para la anemia ferropriva.</P>     <P ALIGN="JUSTIFY">En los Estados Unidos se logr&oacute; un gran descenso en la prevalencia de anemia en la d&eacute;cada del 80 relacionado con el inicio, en los a&ntilde;os 70, del Special Supplemental Food Program for Women, Infants and Children (WIC). Tal programa tuvo un efecto positivo en el estado nutricional de hierro en los ni&ntilde;os beneficiados, cuyas posibles razones fueron la utilizaci&oacute;n de f&oacute;rmula infantil y cereales fortificados con hierro hasta los 12 meses de edad, adem&aacute;s de la educaci&oacute;n nutricional de la familia, que aument&oacute; el consumo de hierro y vitamina C (25).</P>     <P ALIGN="JUSTIFY">Walter et al (26), estudiando ni&ntilde;os chilenos de 6-12 meses de edad, comprob&oacute; que la utilizaci&oacute;n de f&oacute;rmula infantil, aunque fortificada con baja cantidad de hierro, es eficaz en prevenir la anemia ferropriva en el primer a&ntilde;o de vida, principalmente en los lactantes a t&eacute;rmino con buenas condiciones de salud.</P>     <P ALIGN="JUSTIFY">Por otra parte, los avances tecnol&oacute;gicos utilizados en la elaboraci&oacute;n de estas f&oacute;rmulas altamente modificadas, ocasionan costos elevados, lo cual las aleja del consumo masivo en los pa&iacute;ses en v&iacute;as de desarrollo (27). En este sentido son absolutamente necesarias pol&iacute;ticas gubernamentales a nivel de salud p&uacute;blica, que garanticen el acceso a este alimento, comprobadamente m&aacute;s adecuado a las necesidades del ni&ntilde;o que la leche de vaca, en esta etapa relativamente corta pero muy importante de su crecimiento y desarrollo. </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Adem&aacute;s de la f&oacute;rmula, la variable que present&oacute; la mayor protecci&oacute;n fue el consumo de fuentes de vitamina C a los 6 meses de edad. Esto probablemente se debe al efecto estimulador de la absorci&oacute;n del hierro no hem&iacute;nico ejercido por esta vitamina. Su presencia en diferentes concentraciones adem&aacute;s de la carne, clasifica las dietas en baja, mediana y alta biodisponibilidad de hierro.  Dietas con 50 mg de &aacute;cido asc&oacute;rbico adem&aacute;s de 100 g de carne son consideradas con alta biodisponibilidad del mineral(28), por eso la promoci&oacute;n de su consumo por medio de educaci&oacute;n nutricional, es descrita como una medida de intervenci&oacute;n necesaria para combatir la anemia ferropriva  (1,2,24).</P>     <P ALIGN="JUSTIFY">Otra variable de protecci&oacute;n identificada fue la ingesta de carne a los 6 meses. El estudio de Engelmann et al (29),  con ni&ntilde;os dinamarqueses a los 8 meses de edad, compar&oacute; en una intervenci&oacute;n que dur&oacute; 2 meses,un grupo que ingiri&oacute; 10 g de carne al d&iacute;a con otro que ingiri&oacute; 27 g al d&iacute;a y verific&oacute; que estos &uacute;ltimos mantuvieron la concentraci&oacute;n de la hemoglobina, mientras que los primeros presentaron una disminuci&oacute;n significante de &eacute;sta. Se concluy&oacute; entonces, que hasta un peque&ntilde;o aumento en la ingesti&oacute;n de carne, puede prevenir la disminuci&oacute;n de la concentraci&oacute;n de Hb y que esta orientaci&oacute;n debe ser enfatizada para la prevenci&oacute;n de la anemia en la infancia.</P>     <P ALIGN="JUSTIFY">Nuestro estudio no evalu&oacute; la cantidad ingerida de carne, pero constat&oacute; que la simple presencia del alimento en el sexto mes de vida pareci&oacute; ejercer un efecto protector contra la anemia. </P>     <P ALIGN="JUSTIFY">Estudios de cohorte como el nuestro fueron realizados para evaluar conjuntamente los factores relacionados con el desarrollo de la anemia ferropriva. A pesar de la dificultad para desarrollar estudios con este dise&ntilde;o, principalmente en comunidades de bajo nivel socioecon&oacute;mico, cuyos habitantes presentan una gran movilidad que dificulta su seguimiento, dichos estudios son fundamentales para identificar m&aacute;s precisamente los factores que influyen en determinada enfermedad. Adem&aacute;s, los estudios prospectivos son los m&aacute;s adecuados para conocer las pr&aacute;cticas alimentarias y de ablactaci&oacute;n, ya que los datos son registrados en el momento preciso, lo que no sucede en los estudios retrospectivos pues las madres, en general, no retienen la informaci&oacute;n de la &eacute;poca de la introducci&oacute;n de los alimentos.</P>     <P ALIGN="JUSTIFY">El estudio con ni&ntilde;os dinamarqueses, desde el nacimiento hasta los 12 meses de edad, se llev&oacute; a cabo para caracterizar el estado nutricional de hierro y verificar la influencia del patr&oacute;n de consumo y del crecimiento. Los ni&ntilde;os fueron evaluados en m&aacute;s de una ocasi&oacute;n en cuanto al estado nutricional de hierro (2, 6 y 9 meses) y no se encontr&oacute; anemia ferropriva, solamente indicios de reservas marginales de hierro. Se verific&oacute; que la velocidad de crecimiento, adem&aacute;s del consumo de pan y leche de vaca, influenciaron negativamente en esas reservas, mientras que la ingesti&oacute;n de f&oacute;rmula infantil y la ingesta de carne y de pescado se asociaron positivamente (30).</P>     <P ALIGN="JUSTIFY">El sub estudio del Euro-Growth (31) que se llev&oacute; a cabo con una cohorte multic&eacute;ntrica de ni&ntilde;os europeos y evalu&oacute; el estado nutricional de hierro a los 12 meses de edad, identific&oacute; que el factor diet&eacute;tico de mayor influencia negativa fue la introducci&oacute;n precoz de la leche de vaca. A su vez, la duraci&oacute;n de la utilizaci&oacute;n de la f&oacute;rmula fortificada, se asoci&oacute; a una disminuci&oacute;n del riesgo de la deficiencia. Otros factores estudiados como la lactancia materna, la utilizaci&oacute;n de cereales fortificados con hierro y la ingesti&oacute;n de carne y frutas, no explicaron ninguna variaci&oacute;n en el estado nutricional de hierro a los 12 meses. </P>     <P ALIGN="JUSTIFY">Como se ve, nuestros hallazgos son semejantes a los encontrados en esos dos estudios y adem&aacute;s se pudo verificar el papel protector de los frijoles, alimento b&aacute;sico de la dieta brasile&ntilde;a (32). Esto tiene un impacto muy positivo en la orientaci&oacute;n alimentaria a nivel de salud p&uacute;blica ya que es un alimento disponible, de bajo costo y que forma parte del h&aacute;bito alimentario. Aun siendo un alimento fuente de hierro no hem&iacute;nico, el consumo junto con la vitamina C mejora su absorci&oacute;n. </P>     <P ALIGN="JUSTIFY">Aunque la alimentaci&oacute;n infantil en los dos primeros a&ntilde;os de vida incluya alimentos fuente de hierro y estimuladores de su absorci&oacute;n, la Organizaci&oacute;n Mundial de Salud y UNICEF (1,2) recomiendan la suplementaci&oacute;n de hierro en dosis de 10-15mg/d&iacute;a desde los 6 hasta los 24 meses, en pa&iacute;ses con niveles de prevalencia superior a 40%. </P>     <P ALIGN="JUSTIFY">La contribuci&oacute;n mayor de este trabajo fue identificar factores de protecci&oacute;n relacionados a la alimentaci&oacute;n infantil, en una poblaci&oacute;n de bajo nivel socioecon&oacute;mico. Por esta raz&oacute;n se sugiere que la orientaci&oacute;n alimentaria, en la &eacute;poca de la ablactaci&oacute;n, sea considerada una medida efectiva para prevenir o minimizar los efectos de la anemia ferropriva en la infancia.</P> <B>     <P ALIGN="justify">AGRADECIMIENTOS</P> </B>     ]]></body>
