Con el objetivo de determinar factores que modifican lipemia postprandial en respuesta a ingesta mixta de carbohidratos-lípidos en adultos jóvenes de diferentes etnias, se realizó estudio descriptivo no aleatorio con 51 sujetos (23.8± 4años) sanos, con registro de datos personales, clasificación étnica; Test Corto Insulina, prueba de lipemia postprandial, cálculo de índice masa corporal (IMC) y Área Bajo la Curva(ABC) para evaluación postprandial. Alta porcentaje (80%) con antecedentes familiares de ECNT y sedentarismo. Los resultados mostraron promedios altos de ABC de glucosa, triglicéridos y Ácidos Grasos No Esterificados(AGNE); El ABC de glucosa con diferencias por edad(p<= 0.05), sin diferencias por etnia, género, IMC o sensibilidad a insulina. La curva de trigliceridemia mostró dos picos en mestizos (40%) y tres en negros (27%). Sujetos con patrón de cuatro picos tenían IMC elevado; igualmente fue mayor en blancos y mestizos que en negros (p<0,05); mayor en grupo mayores de 29 años y en hombres(p<0.05). El cambio en trigliceridemia sin diferencias por etnia, mayor en hombres(p<0.05) y en sujetos mayores (p<0.05). Correlación (r²=0.70) entre IMC y trigliceridemia, entre IMC y cambio en trigliceridemia en etnias blanca y negra. Los AGNE descendieron hasta las 2 horas y a las 6-7h alcanzaron concentración mayor que la basal . ABC de Triglicéridos y AGNE mayores que lo reportado. Por alta correlación en casi todas las etnias IMC sería un posible indicador de magnitud de trigliceridemia postprandial. Existe respuesta anormal a dieta de grasa, con liberación temprana de AGNE.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Postprandial lipemia in colombian young adults from different ethnias. Postprandial lipemia has been associated with atherogenesis an other non infectious chronic diseases. A descriptive, non aleatory study of 51 healthy young adults (23.8 ± 4 years) of different ethnic background was carried out to identify possible personal or life style factors associated with the response of plasma lipids after a mixed carbohydrate and lipid load. Personal, family, life styles including use of drugs and activity and ethnic background were recorded. Anthropometrical measurements, a short insulin and postprandial lipemia tests were done. From these observations body mass index (BMI) and area under the curve (AUC) for lipids and glucose were calculated. High mean values were observed in AUC for glucose, triacylglycerol and nonesterified fatty acids (NEFA). Postprandial glucose with differences by age (p=0.05 Bonferroni) but no ethnic, gender, BMI or insulin sensitivity related significant differences were found. Mestizos (40%) showed two, and Negroes three (27%) postprandial triglyceridemia peaks. Noteworthy, subjects presenting four peaks had elevated BMI. Higher triglyceridemia values were found in white and mestizos (p<0.05), in subjects older than 29 and in males(p<0.05). Statistical correlation (r²=0.70) between BMI and triglyceridemia and change in postprandial trigliceridemia was found in white and black subjects. NEFA declined up to the second hour, but at 6-7 hours they reached levels higher than base values. Higher than reported values for UCA and BMI and an abnormal response to fat intake with an early release of NEFA was found. <body><![CDATA[<P ALIGN="JUSTIFY">Recibido: 20-10-2003</P>     <P ALIGN="JUSTIFY">Aceptado: 08-07-2004</P>     <P ALIGN="justify">INTRODUCCION</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Estudios realizados en poblaci&oacute;n hisp&aacute;nica y cauc&aacute;sica muestran que tanto ni&ntilde;os como adultos de origen hispano son m&aacute;s obesos, tienen mayor relaci&oacute;n cintura/cadera (C/C), mayor resistencia a la insulina , niveles similares de adiposidad y mayor distribuci&oacute;n central de grasa (1). En general estos grupos en pa&iacute;ses de altos ingresos son los de menor ingreso y no representan una etnia en particular.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; M&aacute;s recientemente se ha demostrado la importancia de las alteraciones en la lipemia postprandial como factor de riesgo de mortalidad por enfermedad cardiovascular (ECV) en poblaci&oacute;n aparentemente sana. Los estudios sugieren que en adultos las alteraciones en el metabolismo postprandial pueden contribuir o ser ellas mismas factores determinantes en el desarrollo y progresi&oacute;n de lesiones de ateroesclerosis (2) y tambi&eacute;n que las alteraciones del perfil lip&iacute;dico como hipertrigliceridemia, bajas concentraciones de lipoprote&iacute;nas de alta densidad (HDL-C) y la presencia de part&iacute;culas de lipoprote&iacute;nas de baja densidad (LDL-C) peque&ntilde;as y densas est&aacute;n relacionadas con la duraci&oacute;n prolongada de la lipemia postprandial (3).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En ni&ntilde;os se ha observado que existe asociaci&oacute;n entre una prolongada respuesta postprandial de triglic&eacute;ridos (TG) a una ingesta de grasa y concentraci&oacute;n alta de triglic&eacute;ridos con concentraciones bajas de HDL-C en ayunas. En adultos, esto mismo se atribuye a interacciones metab&oacute;licas entre HDL-C y lipoprote&iacute;nas ricas en triglic&eacute;ridos. De acuerdo con lo anterior se podr&iacute;a pensar en predictores comunes de concentraciones postprandiales para adultos y ni&ntilde;os (4). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Debido a la escasez de estudios epidemiol&oacute;gicos y a la observaci&oacute;n de que la concentraci&oacute;n de &aacute;cidos grasos no esterificados (AGNE) es muy variable entre individuos dependiendo de su<I> </I>estado nutricional, de su actividad f&iacute;sica, de situaciones de stress y del h&aacute;bito de fumar, no se han considerado las concentraciones elevadas de AGNE en plasma como factores de riesgo para enfermedad coronaria, pero se piensa que las alteraciones en los mecanismos de regulaci&oacute;n de estos niveles como por ejemplo resistencia a insulina, e hipertrigliceridemia entre otros, particularmente en el per&iacute;odo postprandial si podr&iacute;an estar relacionadas con el desarrollo de factores de riesgo (5). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Actualmente en Colombia, el 35% de las causas de muerte lo constituyen las Enfermedades cr&oacute;nicas no trasmisibles (ECNT) como diabetes, infarto agudo del miocardio, accidente cerebrovascular no hemorr&aacute;gico, c&aacute;ncer de col&oacute;n y c&aacute;ncer de seno (6-9) con el consecuente alto costo social reflejado en la disminuci&oacute;n de a&ntilde;os de vida productiva y tratamientos m&eacute;dicos de costos elevados.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En estudios realizados en varios pa&iacute;ses se ha reportado que algunos grupos &eacute;tnicos tienen un mayor riesgo de desarrollar estas enfermedades, cuando son sometidos a cambios en el estilo de vida caracterizado por dietas de alta densidad cal&oacute;rica y poca pr&aacute;ctica de actividad f&iacute;sica. Lo anterior sugiere que estas poblaciones posiblemente presentan alteraciones en el metabolismo de l&iacute;pidos, tales como una mayor permanencia de estos en el plasma y una disminuci&oacute;n en la concentraci&oacute;n de las lipoprote&iacute;nas de alta densidad (HDL-C), lo cual influye en el desarrollo de ECV. Se ha encontrado por ejemplo que la alteraci&oacute;n en el metabolismo postprandial de TG en respuesta a la ingesti&oacute;n de grasa puede estar asociada a resistencia a insulina y al riesgo de padecer ECV (10). Igualmente se ha observado en estas poblaciones una mayor prevalencia de resistencia a insulina asociada al desarrollo de ECNT (11,12). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Como es poco el conocimiento que se tiene sobre el manejo postprandial de l&iacute;pidos y sobre la existencia en la poblaci&oacute;n latinoamericana en este caso de nacionalidad colombiana, de posible asociaci&oacute;n entre alteraciones en el estado postprandial y el pertenecer a un grupo &eacute;tnico, el objetivo del presente estudio fue determinar la probable influencia del origen &eacute;tnico sobre: la lipemia postprandial, el comportamiento del proceso postprandial en respuesta a una comida rica en grasa, y la relaci&oacute;n con factores de riesgo de ECNT como son el &Iacute;ndice de Masa Corporal (IMC), la edad, el g&eacute;nero y la resistencia a insulina.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="justify">METODOS</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Estudio descriptivo no aleatorio efectuado en adultos j&oacute;venes aparentemente sanos seleccionados de un grupo de 115 sujetos a quienes previamente se les hab&iacute;a aplicado el test corto de tolerancia a insulina (TCTI) (13) y los siguientes criterios de exclusi&oacute;n: nacionalidad diferente a la colombiana, el padecer alg&uacute;n tipo de ECNT, consumir una dieta especial y haber presentado un cambio reciente de peso. Se les solicit&oacute; su participaci&oacute;n voluntaria con la intenci&oacute;n de contar con un n&uacute;mero similar de sujetos en cada uno de los grupos &eacute;tnicos. Aceptaron 15 blancos, 14 negros, 20 mestizos y 8 ind&iacute;genas. El protocolo fue sometido al Comit&eacute; de &eacute;tica de la Universidad del Valle – Facultad de Salud y todos los participantes firmaron previamente un consentimiento informado. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; A todos los participantes se les aplic&oacute; una encuesta para recolecci&oacute;n de datos personales, antecedentes familiares y personales de ECNT, h&aacute;bito de tabaquismo, consumo de medicamentos y sedentarismo, catalogando a un sujeto como sedentario cuando realiza una actividad f&iacute;sica constante durante 30 min. y menos de 3 veces a la semana. Se tomaron las siguientes medidas antropom&eacute;tricas: peso en kilogramos y un decimal con una balanza marca Health o meter talla en metros y dos decimales con un tall&iacute;metro marca TANITA y se calcul&oacute; el IMC mediante la formula: peso en kg/talla en metros al cuadrado. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Para la clasificaci&oacute;n por grupo &eacute;tnico, se utiliz&oacute; el m&eacute;todo de auto clasificaci&oacute;n, datos del lugar de nacimiento, apellidos, procedencia geogr&aacute;fica de los padres y observaci&oacute;n directa del sujeto por los investigadores para determinar caracter&iacute;sticas del fenotipo. Los grupos &eacute;tnicos definidos fueron: blanco, negro, mestizo e ind&iacute;gena. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Despu&eacute;s de 12 a 14 h de ayuno se canaliz&oacute; la vena antecubital derecha y se recolect&oacute; una muestra basal de sangre de 7 ml en tubo seco. Enseguida se proporcion&oacute; un desayuno que suministr&oacute; el 25% de los requerimientos cal&oacute;ricos diarios totales (calculados de acuerdo al g&eacute;nero, edad, peso, talla, IMC y actividad f&iacute;sica) con 64% de kcal totales derivadas de grasa, 26% de carbohidratos y 8% de prote&iacute;na para un total promedio de 632<FONT FACE=Symbol>±</FONT> 140 kcal. (14). Despu&eacute;s del desayuno se recolectaron muestras de sangre a los 20, 40, 60, 90, 120, 180, 240, 270, 300, 360 y 420 minutos durante los cuales los sujetos solamente tomaron agua (15,16). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Las muestras de sangre fueron conservadas a 4° C (por un per&iacute;odo menor a 1 hora) hasta separaci&oacute;n del plasma por centrifugaci&oacute;n a 3000 rpm por 15 min., y almacenadas a –20º C.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; A la muestra en ayunas se le determin&oacute; colesterol total (CT), triglic&eacute;ridos (TG), colesterol HDL (HDL-C), glucosa y AGNE y en las muestras del per&iacute;odo postprandial se cuantificaron triglic&eacute;ridos, AGNE y glucosa en el autoanalizador BTS-370 de Biosystems<FONT FACE=Symbol>&acirc;</FONT> , todas las muestras se cuantificaron por m&eacute;todos enzim&aacute;ticos, el HDL-C previa precipitaci&oacute;n con reactivos de Biosystems<FONT FACE=Symbol>&acirc;</FONT> (17,18). Para el c&aacute;lculo de colesterol VLDL y Colesterol LDL, se utilizaron las ecuaciones de Friedewald (19): VLDL = TG/5 y C-LDL = CT – C- HDL – VLDL. En cada una de las t&eacute;cnicas los coeficientes de variaci&oacute;n no exced&iacute;an de 2%. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Los AGNE se determinaron por el m&eacute;todo de Acil-CoA Sintetasa / Acil CoA Oxidasa / Peroxidasa, mediante qu&iacute;mica h&uacute;meda marca Randox<FONT FACE=Symbol>&Ograve;</FONT> (20) y se cuantificaron en el autoanalizador BTS-370 de Biosystems. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Como gu&iacute;a para la evaluaci&oacute;n de los datos de concentraci&oacute;n obtenidos se tuvieron en cuenta as consideraciones del Panel de Expertos en el Tercer Reporte Nacional del Programa de Educaci&oacute;n en Colesterol de Septiembre de 2002 (21).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La lipemia postprandial, se evalu&oacute; seg&uacute;n la l&iacute;nea de Dawson (22). Para glucosa, TG y AGNE (mmolxL<SUP>-1</SUP>xmin) se analiz&oacute; el ABC, llamada <I>lipemia postprandial total </I>o <I>magnitud de lipemia postprandial </I>o<I> trigliceridemia postprandial.</I> Para los TG se analiz&oacute; adem&aacute;s el &aacute;rea entre la curva y una l&iacute;nea imaginaria trazada a partir de la concentraci&oacute;n en ayunas, este valor corresponde al llamado <I>cambio en trigliceridemia. </I>Tambi&eacute;n se tuvo en cuenta el comportamiento de las curvas promedio para cada uno de los metabolitos analizados, n&uacute;mero de picos de aumento de triglic&eacute;ridos durante la lipemia postprandial y la capacidad de retornar al valor en ayunas.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">An&aacute;lisis estad&iacute;stico</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Se utiliz&oacute; el programa estad&iacute;stico SPSS 10.01. Las variables se analizaron en forma estratificada por grupo &eacute;tnico, y cuando se consider&oacute; necesario se evaluaron con respecto a la edad, g&eacute;nero, sensibilidad a insulina e IMC. Se identificaron los casos y la proporci&oacute;n de ellos con valores de mayor riesgo. Se tomaron tambi&eacute;n los cuartiles extremos de la distribuci&oacute;n para el an&aacute;lisis y un valor de significancia p<FONT FACE=Symbol>&lt;</FONT> 0.05</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Las diferencias en la lipemia postprandial se evaluaron mediante ANOVA y se utilizaron pruebas <I>(post hoc)</I> de Bonferroni y Dunnet T3. Se determin&oacute; la correlaci&oacute;n de Pearson entre las variables de lipemia postprandial y el IMC y se calcul&oacute; el valor predictivo positivo (VPP) para los valores que mostraron correlaciones significantes.</P> <B>    <P ALIGN="justify">RESULTADOS</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Al iniciar el estudio se contaba con la participaci&oacute;n de 8 sujetos de la etnia ind&iacute;gena pero en el momento de la prueba 6 de ellos se hab&iacute;an retirado por razones personales y familiares; se reportan entonces los resultados obtenidos con los dos sujetos restantes de este grupo &eacute;tnico, pero no se utilizan para sacar conclusiones con respecto a las caracter&iacute;sticas metab&oacute;licas.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Todos los participantes eran individuos cl&iacute;nicamente sanos con edad promedio de 24 a&ntilde;os y bajo consumo de tabaco. Todos se hab&iacute;an sometido previamente a un test corto de insulina. El 51% realiza actividad f&iacute;sica pero de modo poco frecuente. Se observa una distribuci&oacute;n homog&eacute;nea de los grados de sensibilidad a insulina en los grupos &eacute;tnicos (<B>Tabla 1</B>). <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; El patr&oacute;n de las curvas postprandiales de TG fue menos homog&eacute;neo (<B><a href="#fig1">Figura 1</a></B>). Se present&oacute; un aumento progresivo de la concentraci&oacute;n hasta alcanzar un valor m&aacute;ximo a las 3 o 4, en la mayor&iacute;a de los sujetos y disminuci&oacute;n posterior. En los menores de 20 a&ntilde;os el valor m&aacute;ximo se alcanz&oacute; a las 2 h. El 41% del grupo present&oacute; un pico de aumento, el 37% dos, 18% tres y el 4% cuatro. Este &uacute;ltimo grupo se encontr&oacute; en el cuartil superior de IMC (<B><a href="#fig3">Figura 3</a></B>). Los dos ind&iacute;genas presentaron un pico a las 2h y otro a las 4 h. <P ALIGN="justify">&nbsp;&nbsp;&nbsp; El 31%<I> </I>de los sujetos tuvieron a las 7h del per&iacute;odo postprandial un valor de TG mayor a su valor en ayunas y ten&iacute;an adicionalmente otras caracter&iacute;sticas comunes como pertenecer principalmente a la etnia mestiza (44%), estar ubicados en los grupos de mayor edad (56%) y en los cuartiles superiores de IMC. Adem&aacute;s hubo porcentajes importantes de alteraciones en el perfil lip&iacute;dico como 30% con triglic&eacute;ridos elevados, 62% con LDL-C aumentado y 87.5% con valores bajos de HDL-C.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Los niveles de trigliceridemia postprandial fueron mayores en blancos que en negros ajustando por g&eacute;nero (Dunnett T3 &lt; 0,05) y mayores en blancos y mestizos que en negros ajustando por IMC y g&eacute;nero (Bonferroni p&lt;=0.05). Fueron m&aacute;s elevados en individuos &gt;29 a&ntilde;os al comparar con los tres primeros grupos de edad (<FONT FACE=Symbol>£</FONT> 20 a&ntilde;os, 21 a 23 a&ntilde;os y 24 a 26 a&ntilde;os) (Dunnett &lt; 0.05). La trigliceridemia fue mayor en los hombres (Bonferroni &lt;0.05) (<b><a href="#fig4">Figura 4</a></b>).</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Las concentraciones plasm&aacute;ticas de AGNE disminuyeron en las primeras dos horas de la ingesta y ascendieron en las horas 6 y 7 a niveles superiores al basal (<a href="#fig1"><b>Figura No. 1</b></a>). La etnia negra present&oacute; dos incrementos. Las curvas postprandiales muestran gran variaci&oacute;n de los valores de AGNE entre los grupos considerados por etnia, edad, g&eacute;nero y por grado de sensibilidad a insulina. No se encontraron diferencias en el &aacute;rea bajo la curva de AGNE para ninguna de las variables analizadas.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La correlaci&oacute;n de Pearson fue significante entre IMC y trigliceridemia; entre IMC y el cambio en trigliceridemia en blancos y entre IMC y el cambio en trigliceridemia en etnia negra. (Tabla 4). Se ha sugerido que ciertas poblaciones o grupos &eacute;tnicos que se han adaptado a condiciones desfavorables de alimentaci&oacute;n y con labores f&iacute;sicas desgastantes llegan a ser m&aacute;s susceptibles a las ECNT cuando se exponen al estilo de vida occidental caracterizado por sobrenutrici&oacute;n y disminuci&oacute;n de la actividad f&iacute;sica. Existe una considerable evidencia de que en Colombia como en otros pa&iacute;ses en desarrollo el aumento en la esperanza de vida esta asociado con cambios predecibles en el patr&oacute;n de enfermedad y salud, lo anterior tiene gran relevancia con respecto a la situaci&oacute;n en Colombia ya que las ECNT han aumentado dram&aacute;ticamente en los &uacute;ltimos 20 a&ntilde;os (23). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; En el presente estudio se eval&uacute;a la respuesta postprandial entre los diferentes grupos &eacute;tnicos en sujetos que se han mantenido en su pa&iacute;s de origen y se pretende determinar de esta manera si lo reportado en otros estudios con respecto a poblaci&oacute;n hispana se debe a caracter&iacute;sticas propias de esta poblaci&oacute;n o a los cambios sufridos por &eacute;sta al desplazarse a otros pa&iacute;ses.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Las dificultades para la identificaci&oacute;n de etnias se hicieron aparentes desde un principio como ha sido reportado en varios estudios (24). En la poblaci&oacute;n colombiana, seg&uacute;n las publicaciones de la Expedici&oacute;n Colombia realizada por la Universidad Javeriana, hay una gran mezcla de etnias. Por lo tanto la clasificaci&oacute;n &eacute;tnica se efectu&oacute; por caracter&iacute;sticas fenot&iacute;picas y de origen (25,26). Si bien es cierto que el grupo investigado constituido por adultos j&oacute;venes no muestra ninguna evidencia de asociaci&oacute;n entre la etnia y la mayor&iacute;a de las variables de la lipemia postprandial, al examinar los individuos dentro de cada etnia para la trigliceridemia postprandial<I>, </I>se observaron diferencias al ajustar con otras variables como IMC y g&eacute;nero. Lo anterior est&aacute; de acuerdo con un estudio realizado en Estados Unidos (27) con mujeres afro-americanas y blancas en el cual no se hallaron diferencias en la lipemia postprandial entre estos dos grupos pero el aumento de &eacute;sta en respuesta a una ingesta alta en grasa fue similar. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Se han encontrado diferencias en los niveles de ingreso asociados con la presencia de enfermedades cr&oacute;nicas en varios pa&iacute;ses del mundo; poblaciones hispana, africana, asi&aacute;tica que habitan en pa&iacute;ses de altos ingresos generalmente tienen un nivel socioecon&oacute;mico inferior a la poblaci&oacute;n blanca (28), pero este hecho que parec&iacute;a justificar la alta prevalencia de estas enfermedades en esos grupos, no explica el por qu&eacute; en este estudio en sujetos colombianos se encontraron alteraciones en la lipemia postprandial; la adopci&oacute;n por parte de la poblaci&oacute;n hispana de los estilos de vida de propios pa&iacute;ses desarrollados en cuanto a nutrici&oacute;n y actividad f&iacute;sica se refiere podr&iacute;a ser una hip&oacute;tesis.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; La comparaci&oacute;n de este grupo de individuos con otros de estudios similares se ha dificultado por la falta de caracter&iacute;sticas comunes. En este caso la mayor&iacute;a de los participantes fueron adultos j&oacute;venes posiblemente con mayor capacidad de adaptaci&oacute;n fisiol&oacute;gica frente al proceso de disminuci&oacute;n de sensibilidad a la insulina, al desarrollo de hipertensi&oacute;n y a estados de dislipidemias excepto en el caso de HDL-C que fue la &uacute;nica variable del perfil lip&iacute;co que se encontr&oacute; con valores considerados de riesgo (21). Solamente se registr&oacute; como observaci&oacute;n general actividad f&iacute;sica poco frecuente y esto &uacute;ltimo podr&iacute;a explicar en parte el hecho de que estos individuos manejaran valores postprandiales m&aacute;s altos que los esperados. La importancia de la actividad f&iacute;sica como factor preventivo se demostr&oacute; en un estudio realizado en mujeres postmenop&aacute;usicas por Gill <I>et al</I> (29) en el cual evaluaron el efecto que sobre la lipemia postprandial tenia el ejercicio comparado con restricci&oacute;n dietaria y se demostr&oacute; un mejor manejo de los l&iacute;pidos en el per&iacute;odo postprandial en las mujeres que realizaron actividad f&iacute;sica frente a un grupo control y frente a las mujeres que recibieron una dieta con menor cantidad de energ&iacute;a; igualmente se encontr&oacute; disminuci&oacute;n de la lipemia postprandial, de los triglic&eacute;ridos en ayunas y de las concentraciones postprandiales de insulina en las mujeres que hicieron ejercicio. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; El comportamiento observado de la glucosa postprandial similar al reportado por K. Frayn (30), permite confirmar que la glucosa es el metabolito mejor regulado en el organismo y que por tanto su cuantificaci&oacute;n en estado de ayuno se debe acompa&ntilde;ar con la determinaci&oacute;n de otras variables para la identificaci&oacute;n temprana de factores de riesgo de ECNT. Con respecto a la aparici&oacute;n del segundo pico de aumento de glucosa en los ind&iacute;genas, esta observaci&oacute;n no se puede generalizar debido al escaso n&uacute;mero de participantes pertenecientes a esta etnia, pero debe ser tenido en cuenta en estudios similares. El mayor valor de ABC de la glucosa en los sujetos <FONT FACE=Symbol>³</FONT> 30 a&ntilde;os posiblemente refleja un estado inicial de la resistencia a insulina y la incapacidad de responder eficientemente a &eacute;sta ya que estos individuos se encontraban en los grupos de menor sensibilidad a la hormona (resultados no mostrados). </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Una observaci&oacute;n preocupante en los resultados del presente estudio con participantes que son latinoamericanos de nacionalidad colombiana, fue la trigliceridemia postprandial con valores mayores a los reportados en estudios con el mismo tipo de an&aacute;lisis realizado en sujetos de Inglaterra y Estados Unidos (15,31) pues se conoce que en individuos sanos el aumento de la concentraci&oacute;n de triglic&eacute;ridos en plasma despu&eacute;s de una ingesta de grasa es 2 veces la concentraci&oacute;n basal e implica una serie de cambios coordinados en el metabolismo de l&iacute;pidos que llevan de un lado a la supresi&oacute;n del paso de triglic&eacute;ridos al plasma y del otro al aumento de la captura de triglic&eacute;ridos plasm&aacute;ticos; entonces, cualquier alteraci&oacute;n en estos dos procesos seguramente lleva a perturbaci&oacute;n de la lipemia postprandial situaci&oacute;n reconocida como factor de riesgo de ECV. Condiciones como edad, g&eacute;nero, obesidad, diabetes e ingesta de az&uacute;cares simples y la carga de grasa de la dieta, son factores que afectan la lipemia, por ejemplo, los carbohidratos parecen ser determinantes de la naturaleza pero no de la magnitud de la lipemia postprandial (32). Tambi&eacute;n vale la pena resaltar que un alto porcentaje de estos individuos que no fueron capaces de retornar a su valores basales en un periodo de 7 horas, presentaron valores alterados de perfil lip&iacute;dico (21) para este rango de edad lo cual constituye un factor de riesgo si no se toman las medidas adecuadas de prevenci&oacute;n. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Los resultados sobre la respuesta postprandial de triglic&eacute;ridos de manera similar a los reportados en otros estudios muestran un aumento inicial gradual, y un pico entre 2 y 5h seguido de disminuci&oacute;n hasta el valor basal en la mayor&iacute;a de los sujetos. Concentraciones entre 0.66-1.76 mmoles/L se consideran normales en individuos sanos con media de 1.1 mmoles/L. Cuando se proporciona una comida con 30% de energ&iacute;a proveniente de grasa, que representa 1/3 de la ingesta de energ&iacute;a diaria del individuo, la concentraci&oacute;n postprandial de triglic&eacute;ridos alcanza un pico de 1.65-1.98 mmoles/L entre 3 y 5 h m&aacute;s tarde. Se consideran hipertriglicerid&eacute;micos los sujetos con valor en ayunas por encima de 2.2 mmoles/L. Generalmente, estos individuos alcanzan valores de 3.3 &oacute; 4.4 mmoles/L un poco m&aacute;s tarde. Los sujetos del presente estudio no encajaban dentro de la clasificaci&oacute;n de hipertriglicerid&eacute;micos al utilizar el valor en ayunas, pero al analizar los valores postprandiales se observaron concentraciones mucho mayores que las esperadas, evidenciando un manejo postprandial alterado.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Aun cuando en algunos estudios se reporta que la curva de triglic&eacute;ridos postprandiales es monof&aacute;sica, tambi&eacute;n se han reportado hasta 3 picos, n&uacute;mero que es independiente del valor en estado de ayuno. Se ha observado por ejemplo, que la curva bif&aacute;sica aparece en individuos que consumen una comida de prueba 5 h despu&eacute;s de otra como el desayuno; otros pocos sujetos presentan esta modalidad de curva despu&eacute;s del per&iacute;odo de ayuno. Seg&uacute;n Fielding <I>et al. </I>la composici&oacute;n de la dieta es uno de los determinantes de estos patrones que tambi&eacute;n reflejan la velocidad de absorci&oacute;n intestinal. La curvas con m&aacute;s de un pico ya fueron reportadas y como lo sugieren Shishehbor <I>et al.</I> este patr&oacute;n refleja por un lado la velocidad de absorci&oacute;n intestinal de grasa influenciada por m&uacute;ltiples factores y por el otro la grasa previamente almacenada en las paredes del intestino delgado o de los vasos linf&aacute;ticos y que se puede liberar a la circulaci&oacute;n. Fielding <I>et al.</I> sugieren que el patr&oacute;n de picos de la curva de triglic&eacute;ridos postprandiales refleja la composici&oacute;n en grasa de la comida anterior a la prueba y que en condiciones de ayuno no se observa m&aacute;s de un pico. Sin embargo, en estudios anteriores como el de Cohn <I>et al. </I>aparecen reportadas curvas de triglic&eacute;ridos postprandiales con m&aacute;s de un pico en sujetos en estado de ayuno que han recibido carga de grasa, esta &uacute;ltima condici&oacute;n es similar a la del presente estudio con individuos cuya costumbre alimentaria estilo occidental se caracteriza por un menor tiempo transcurrido entre el consumo de comidas y alto contenido de grasa en las mismas (datos no incluidos en el presente reporte). As&iacute; pues, si se tiene un consumo constante de grasa, la liberaci&oacute;n de &eacute;sta a la circulaci&oacute;n muestra patrones con m&aacute;s de un pico de aumento e incapacidad para volver al valor basal como se pudo observar (15,32-34). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Con respecto a los &aacute;cidos grasos no esterificados (AGNE), K. Frayn (5) ha reportado que su concentraci&oacute;n en plasma var&iacute;a considerablemente y tiene un marcado ritmo circadiano: m&aacute;s alta en la ma&ntilde;ana y va disminuyendo durante el d&iacute;a. Hay una disminuci&oacute;n considerable post-ingesta y aumento gradual durante la noche, estos niveles se ven afectados por h&aacute;bitos dietarios como frecuencia de ingesta de alimentos y composici&oacute;n de los mismos. En el presente estudio se observ&oacute; la liberaci&oacute;n temprana de AGNE a la sangre cuando a&uacute;n no se ha terminado el per&iacute;odo postprandial. Esto indicar&iacute;a que los sujetos inician tempranamente s&iacute;ntesis de C-VLDL cuando a&uacute;n tienen circulando en sangre los quilomicrones de la dieta. Ahora, la permanencia en plasma de C-VLDL permite que las C-LDL y HDL-C se enriquezcan en triglic&eacute;ridos y que el aumento en el recambio de triglic&eacute;ridos y de &eacute;steres de colesterol entre estas lipoprote&iacute;nas favorezca el proceso aterog&eacute;nico (10). Por otro lado, la liberaci&oacute;n de AGNE a la circulaci&oacute;n portal puede desempe&ntilde;ar un papel muy importante en la alteraci&oacute;n del metabolismo hep&aacute;tico propio del s&iacute;ndrome de resistencia a insulina y favorecer los estados de dislipidemia, hiperinsulinemia e intolerancia a glucosa (5). </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es interesante que como lo describe L. Berglund, los sujetos con patrones postprandiales alterados tienen varias caracter&iacute;sticas del s&iacute;ndrome de resistencia a la insulina, incluyendo altas concentraciones de triglic&eacute;ridos e insulina en ayunas, altos dep&oacute;sitos de grasa visceral, y un alto &iacute;ndice arterial (CT / HDL-C), variables que se consideran actualmente mejores indicadores de alteraciones en el metabolismo (35). Adicionalmente, en este estudio se hizo evidente que estas variables asociadas a otros indicadores como IMC podr&iacute;an ser considerados predictores del comportamiento postprandial y tambi&eacute;n que el metabolismo postprandial puede no necesariamente tener como &uacute;nica caracter&iacute;stica la obesidad. </P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Es preocupante que estos sujetos j&oacute;venes ya evidencien factores de riesgo para el desarrollo de ECNT como son alteraci&oacute;n postprandial de triglic&eacute;ridos, sedentarismo y antecedentes familiares de ECNT. Es probable entonces, que la alteraci&oacute;n en el metabolismo de l&iacute;pidos como factor de riesgo sea un fen&oacute;meno m&aacute;s com&uacute;n de lo esperado en la poblaci&oacute;n joven colombiana lo cual ser&iacute;a parte de la explicaci&oacute;n del aumento de tasas de mortalidad espec&iacute;fica por enfermedades cr&oacute;nicas en este pa&iacute;s. El conocimiento de las diferencias en el comportamiento postprandial ayudar&aacute; a la identificaci&oacute;n en poblaciones vulnerables de factores de riesgo para ECNT y a la elaboraci&oacute;n y ejecuci&oacute;n de programas preventivos como parte de las pol&iacute;ticas de salud. </P> <B>    <P ALIGN="justify">CONCLUSIONES</P> </B>    <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; No se encontr&oacute; asociaci&oacute;n detectable entre el pertenecer a un determinado grupo &eacute;tnico y las variables de lipemia postprandial excepto para los triglic&eacute;ridos. Existe una alta correlaci&oacute;n y un alto valor predictivo positivo (VPP) entre la magnitud de la trigliceridemia postprandial y el IMC en las etnias blanca y negra, lo cual hace de esta caracter&iacute;stica antropom&eacute;trica un buen indicador de posibles alteraciones postprandiales en estos grupos de sujetos.</P>     <P ALIGN="JUSTIFY">&nbsp;&nbsp;&nbsp; Se encontr&oacute; en todos los participantes una respuesta anormal a una carga de grasa, con liberaci&oacute;n de &aacute;cidos grasos no esterificados en el per&iacute;odo postprandial cuando a&uacute;n no se ha realizado por completo la remoci&oacute;n de triglic&eacute;ridos de la circulaci&oacute;n; este hecho no presenta ninguna asociaci&oacute;n con otras variables estudiadas. Contrario a lo esperado, se encontr&oacute; una proporci&oacute;n muy alta de alteraciones metab&oacute;licas y la presencia de otros factores de riesgo en individuos j&oacute;venes aparentemente sanos. El comportamiento postprandial de la glucosa en estos sujetos no se evidencia como factor de riesgo. Se observaron niveles mas altos de lipemia postprandial que los reportados en otros estudios y una proporci&oacute;n elevada de alteraciones metab&oacute;licas en el grupo de adultos j&oacute;venes<I>.</I> Este hallazgo aun cuando la muestra no es representativa, podr&iacute;a sugerir una de las explicaciones a la elevada mortalidad por enfermedades cr&oacute;nicas en la poblaci&oacute;n colombiana menor de 60 a&ntilde;os. </P> <B>    <P ALIGN="justify">AGRADECIMIENTOS</P> </B>    ]]></body>
