Numerosos factores ambientales se han asociado al riesgo de obesidad y es importante que todos ellos sean considerados en las políticas de prevención. Se ha demostrado que existe un nexo entre la publicidad de alimentos en televisión con la obesidad infantil. La gran cantidad de anuncios publicitarios de alimentos poco saludables dirigidos a los niños a través de la televisión y las repercusiones que esto podría tener en la salud, ha llevado a algunos países a legislar al respecto. Sin embargo, falta definir a nivel internacional un marco conceptual de referencia que permita una legislación que logre un impacto real en la prevención de la obesidad infantil. Este trabajo revisa las evidencias científicas disponibles sobre la relación entre publicidad de alimentos y obesidad infantil como base para el desarrollo de políticas públicas de regulación del mercadeo de alimentos a través de la televisión.]]></p></abstract>

Para el a&ntilde;o 2010 se estim&oacute; que exist&iacute;an 43 millones de ni&ntilde;os con sobrepeso y obesidad en todo el mundo, de los cuales 35 millones viven en pa&iacute;ses en v&iacute;as de desarrollo (1). En Chile, los datos del Ministerio de Salud del 2010 muestran que la poblaci&oacute;n menor de 6 a&ntilde;os de edad controlada en el sistema p&uacute;blico de salud presenta una prevalencia de 22,6 % de sobrepeso y de 9,6 % de obesidad, aumentando la obesidad a 12,5 % en el grupo de 4 a 5 a&ntilde;os (2).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Para combatir la epidemia de obesidad se han aplicado diversas estrategias de base poblacional, incluyendo medidas educativas orientadas a promover una alimentaci&oacute;n saludable y la actividad f&iacute;sica, as&iacute; como actividades de prevenci&oacute;n secundaria y de tratamiento. En Chile, entre otras actividades, se ha implementado desde el 2005 la Estrategia de Intervenci&oacute;n Nutricional a trav&eacute;s del ciclo vital y desde el 2006 la Estrategia Global Contra la Obesidad (EGO), que no han logrado los resultados esperados (3).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Mientras los gobiernos invierten esfuerzos y recursos para revertir la situaci&oacute;n, los individuos de todas las edades y clases sociales est&aacute;n sometidos diariamente a una enorme oferta de alimentos poco saludables a trav&eacute;s de avisos publicitarios en televisi&oacute;n, prensa y otros medios, dentro de campa&ntilde;as cuidadosamente dise&ntilde;adas (4).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">El informe de la Organizaci&oacute;n para la Cooperaci&oacute;n y el Desarrollo Econ&oacute;mico (OCDE) muestra que las intervenciones en salud efectuadas desde la atenci&oacute;n primaria y dirigidas a la poblaci&oacute;n infantil, son de alto costo y pueden tomar mucho tiempo para tener un real impacto. Las estrategias m&aacute;s favorables desde el punto de vista costo-efectividad, se encuentran fuera del sector salud, especialmente en el &aacute;mbito regulatorio y fiscal (5).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Este trabajo revisa las evidencias cient&iacute;ficas actuales sobre la relaci&oacute;n entre publicidad de alimentos y obesidad infantil como base para el desarrollo de pol&iacute;ticas p&uacute;blicas de regulaci&oacute;n del mercadeo de alimentos a trav&eacute;s de la televisi&oacute;n.</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <b><font face="Verdana" size="2">PUBLICIDAD DE ALIMENTOS Y OBESIDAD INFANTIL</font>                     </b>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">La evidencia sugiere que el tiempo que los ni&ntilde;os ven televisi&oacute;n y la prevalencia de obesidad infantil est&aacute;n relacionadas (6). Informaci&oacute;n cient&iacute;fica sobre el tema existe desde la d&eacute;cada de los setenta y actualmente ha tenido un repunte, concentr&aacute;ndose fundamentalmente en cuatro categor&iacute;as: a) an&aacute;lisis del contenido de la publicidad televisiva de alimentos; b) efectos de la publicidad televisiva de alimentos en el comportamiento alimentario de los ni&ntilde;os; c) publicidad televisiva de alimentos y obesidad infantil; y d) uso de televisi&oacute;n y patr&oacute;n de consumo alimentario (7).</font>                     </span>                         ]]></body>
<body><![CDATA[<p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Estudios internacionales muestran que los ni&ntilde;os entre 2 y 17 a&ntilde;os de edad ocupan en promedio 2,5 horas por d&iacute;a en ver televisi&oacute;n (8). Dentro de ese tiempo est&aacute;n expuestos a un promedio de una publicidad de alimentos cada 5 minutos (cerca de 30 por d&iacute;a), que son principalmente de alimentos no saludables, emitidos con t&eacute;cnicas avanzadas de mercadeo y persuasi&oacute;n (7). Las estrategias publicitarias m&aacute;s utilizadas son las promociones y la asociaci&oacute;n de los productos alimenticios a emociones positivas: diversi&oacute;n, felicidad, juego, fantas&iacute;a, imaginaci&oacute;n, integraci&oacute;n social y aceptaci&oacute;n de los pares (9, 10). La publicidad de alimentos es m&aacute;s intensa durante el periodo de programas infantiles que durante la banda dirigida a la audiencia general (11-14).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">En la mayor parte de los pa&iacute;ses los alimentos m&aacute;s ofertados son ricos en calor&iacute;as, grasas, az&uacute;cares simples y sodio y rara vez se incluyen alimentos nutritivos de bajo costo (15). Taras & Gage encontraron que de los alimentos publicitados en televisi&oacute;n el 69% son altos en az&uacute;car refinada, 35% altos en grasa y 20% altos en sodio (16). Existe una gran discrepancia entre la calidad nutricional de los productos alimenticios publicitados y la dieta normal recomendada para la poblaci&oacute;n infantil (17 – 19).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <b><font face="Verdana" size="2">EFECTO DE LA PUBLICIDAD TELEVISIVA DE ALIMENTOS</font>                     </b>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">La publicidad de alimentos en televisi&oacute;n se correlaciona con el requerimiento que los ni&ntilde;os hacen a sus padres y el patr&oacute;n de alimentaci&oacute;n de los ni&ntilde;os expuestos (7). Se ha observado asociaci&oacute;n significativa entre el n&uacute;mero de horas que el ni&ntilde;o ve televisi&oacute;n e ingesta de energ&iacute;a (20, 21), l&iacute;pidos y az&uacute;cares (22, 23), el &Iacute;ndice de Masa Corporal (IMC) (24), el porcentaje de grasa corporal (25) y la prevalencia de obesidad (26 - 29). Un estudio ecol&oacute;gico que incluy&oacute; informaci&oacute;n de Australia, Estados Unidos y ocho pa&iacute;ses europeos, demostr&oacute; asociaci&oacute;n entre la proporci&oacute;n de ni&ntilde;os con sobrepeso y el n&uacute;mero de anuncios por hora en la televisi&oacute;n (r = 0,81, P <0,005) (30).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">En Estados Unidos se desarroll&oacute; un modelo matem&aacute;tico de simulaci&oacute;n para estimar los posibles efectos de la disminuci&oacute;n de la exposici&oacute;n a la publicidad televisiva de alimentos en la prevalencia de sobrepeso y obesidad en ni&ntilde;os de 6 a 12 a&ntilde;os. Si la exposici&oacute;n se redujera a cero la prevalencia de obesidad disminuir&iacute;a de 17,8 a 15,2 % (IC 95%: 14,8-15,6) en varones y de 15,9 a 13,5 % (IC 95%: 13,1-13,8) en ni&ntilde;as. Uno de cada tres a uno de cada siete ni&ntilde;os obesos podr&iacute;a no haber desarrollado obesidad, en la ausencia de publicidad de alimentos poco saludables en la televisi&oacute;n (31).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <b><font face="Verdana" size="2">HACIA UNA POL&Iacute;TICA DE REGULACI&Oacute;N DE LA PUBLICIDAD TELEVISIVA DE ALIMENTOS</font>                     </b>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Las pol&iacute;ticas p&uacute;blicas destinadas a promover un comportamiento saludable no han logrado contrarrestar los cuantiosos recursos invertidos en la producci&oacute;n y comercializaci&oacute;n de alimentos que no promueven una mejor salud. El resultado es una "econom&iacute;a propicia a la obesidad", en la que los ni&ntilde;os son un objetivo prioritario (32).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">La publicidad de alimentos no regulada, la alta disponibilidad y bajos precios de alimentos no saludables, la utilizaci&oacute;n de medios de transporte a motor, la falta de espacios urbanos abiertos, el d&eacute;ficit de actividad f&iacute;sica y la falta de educaci&oacute;n nutricional, son factores ambientales de la vida moderna que determinan la ingesta y el gasto energ&eacute;tico, contribuyendo al incremento de la prevalencia de obesidad infantil. La clave del &eacute;xito de las pol&iacute;ticas de prevenci&oacute;n radica en que se consideren todos los elementos involucrados (4).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">En 2004, la Organizaci&oacute;n Mundial de la Salud (OMS) pidi&oacute; a los gobiernos tomar acciones para reducir los mensajes de mercadeo de alimentos no saludables (33). Para los pa&iacute;ses de Am&eacute;rica Latina y El Caribe la Organizaci&oacute;n Panamericana de la Salud (OPS), ha establecido directrices y normas respecto de la comercializaci&oacute;n y publicidad de alimentos que van dirigidas a los ni&ntilde;os (34). Desde entonces la industria ha desarrollado m&eacute;todos de autorregulaci&oacute;n, la sociedad civil ha desarrollado campa&ntilde;as por restricciones legales, y los gobiernos han generado una serie de propuestas de regulaci&oacute;n legal. Sin embargo no ha sido f&aacute;cil legislar y las nuevas reglamentaciones no han sido lo suficientemente claras y fuertes y no se han basado en la evidencia cient&iacute;fica ni en un marco de referencia &eacute;tico (35).</font>                     </span>                         <p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">Actualmente 16 pa&iacute;ses de Europa (Alemania, Austria, B&eacute;lgica, Dinamarca, Espa&ntilde;a, Finlandia, Francia, Grecia, Irlanda, Italia, Luxemburgo, Noruega, Pa&iacute;ses Bajos, Portugal, Reino Unido y Suecia), dos de Am&eacute;rica del Norte (Canad&aacute; y Estados Unidos) y dos de Ocean&iacute;a (Australia y Nueva Zelanda), cuentan con marcos regulatorios sobre publicidad televisiva de alimentos relacionados con los ni&ntilde;os. Se incluyen normas de autorregulaci&oacute;n por parte de la industria y/o restricciones que van desde limites parciales a la cantidad, horarios y tipo de los anuncios emitidos, hasta la prohibici&oacute;n total de la publicidad como en el caso de Suecia (36). En Am&eacute;rica Latina Brasil, Chile y Ecuador han tomado algunas iniciativas (34). Sin embargo hasta en los pa&iacute;ses donde se han implementado estrictas normativas de prohibici&oacute;n o de autorregulaci&oacute;n como en el Reino Unido y Canad&aacute; respectivamente, no se han observado modificaciones importantes a la publicidad, debido a que no est&aacute; claramente definido el concepto de los alimentos menos saludables que no deben ser publicitados. (37).</font>                     </span>                         ]]></body>
<body><![CDATA[<p align="justify" style="line-height: 100%"><span>                     <font face="Verdana" size="2">En la <a href="#TABLA 1"> tabla 1</a> se presentan las regulaciones que se implementaron o que se encontraban en etapa de discusi&oacute;n o de propuesta en algunos pa&iacute;ses entre los a&ntilde;os 2004 y 2007. <p style="line-height: 100%" align="justify"><font face="Verdana" size="2">El proyecto Pan- Europeo concluye que se observa un patr&oacute;n ineficaz e incoherente de regulaci&oacute;n en todos los pa&iacute;ses (38) y que pocos gobiernos imponen fuertes restricciones. La mayor&iacute;a prefiere convencer a la industria a actuar voluntariamente con responsabilidad. En base a estos hallazgos se elaboraron recomendaciones que incluyen modificaciones en la televisi&oacute;n de la Uni&oacute;n Europea sin fronteras para prohibir toda publicidad televisiva de alimentos poco saludables para los ni&ntilde;os, la adopci&oacute;n de una definici&oacute;n com&uacute;n de alimento "poco saludable", y el establecimiento de un mecanismo de Observatorio Pan-Europeo que vigile la naturaleza, el alcance y la regulaci&oacute;n de la publicidad y comercializaci&oacute;n de alimentos destinados a la poblaci&oacute;n infantil.</font>                         <p style="line-height: 100%" align="justify"><font face="Verdana" size="2">En 2006 el Grupo Internacional de Trabajo sobre Obesidad (IOTF), elabor&oacute; los Principios de Sydney (39), basados en los derechos del ni&ntilde;o y cuyos objetivos son orientar la acci&oacute;n para el cambio de las pr&aacute;cticas de comercializaci&oacute;n de alimentos y bebidas dirigidas a los ni&ntilde;os: 1) apoyar los derechos de los ni&ntilde;os, 2) ofrecer una protecci&oacute;n sustancial a los ni&ntilde;os, 3) ser de naturaleza legal, 4) adoptar una definici&oacute;n amplia de promoci&oacute;n comercial, 5) dar garant&iacute;a a una ni&ntilde;ez libre de comerciales, 6) incluir los medios de comunicaci&oacute;n transfronteriza, y 7) evaluar, controlar y hacer cumplir los acuerdos (40).</font>                     </p>                         <p style="line-height: 100%" align="justify"><font face="Verdana" size="2">En Chile no se cuenta a&uacute;n con suficiente investigaci&oacute;n que relacione la obesidad infantil con la exposici&oacute;n a la publicidad de alimentos. Un estudio de Uribe y cols., revel&oacute; que el 9,5% del tiempo de programaci&oacute;n de TV corresponde a propaganda dirigida a los ni&ntilde;os. Dentro de los comerciales las categor&iacute;as m&aacute;s publicitadas son alimentos, juguetes y art&iacute;culos electr&oacute;nicos. (41). Desde el a&ntilde;o 2008 se est&aacute; tramitando un proyecto de ley que pretende regular la publicidad de alimentos dirigida a menores de 14 a&ntilde;os de edad. Sin embargo, ha enfrentado una intensa objeci&oacute;n por parte de la industria de alimentos y bebidas (42). El Instituto Libertad y Desarrollo elabor&oacute; un documento donde argumenta que en base a la legislaci&oacute;n vigente no se justifica una ley de regulaci&oacute;n de la publicidad de alimentos (43). Las empresas del rubro agrupadas en Chilealimentos han reaccionado implementando a contar de mayo del 2008 la Gu&iacute;a Diaria de Alimentaci&oacute;n (GDA), est&aacute;ndar de etiquetado nutricional utilizado en varios pa&iacute;ses del mundo. La industria se apoya en el argumento de que el p&uacute;blico tiene derecho a estar informado para escoger libremente los productos que desea consumir (44).</font>                     </p>                         <p style="line-height: 100%" align="justify"><b><font face="Verdana" size="2">CONCLUSIONES</font>                     </b></p>                         <p style="line-height: 100%" align="justify"><font face="Verdana" size="2">La prevalencia de obesidad infantil es cada vez mayor y los programas de prevenci&oacute;n implementados no han logrado revertir la situaci&oacute;n. Los ni&ntilde;os est&aacute;n expuestos a un intenso mercadeo de alimentos en la televisi&oacute;n cuyo contenido nutricional es perjudicial para la salud, lo que se asocia a conductas alimentarias inadecuadas y un mayor riesgo de obesidad infantil. Se hace imprescindible regular la publicidad de alimentos que emiten los canales de televisi&oacute;n y que va dirigida a los ni&ntilde;os. Los gobiernos que han optado por los c&oacute;digos de autorregulaci&oacute;n voluntaria de la publicidad de alimentos por parte de la industria, no han logrado la reducci&oacute;n del mercadeo alimentario orientado a los ni&ntilde;os. Por otro lado los pa&iacute;ses donde se ha implementado una regulaci&oacute;n restrictiva y prohibitiva tampoco han conseguido resultados sustanciales ya que sus legislaciones han sido poco objetivas o incoherentes, las restricciones no han sido lo suficientemente fuertes y no se han basado en un marco de conocimiento cient&iacute;fico ni fundamentos &eacute;ticos.</font>                     </p>                         <p style="line-height: 100%" align="justify"><font face="Verdana" size="2">A la hora de decidir si legislar por la restricci&oacute;n o la autorregulaci&oacute;n de la publicidad televisiva de alimentos dirigida a la poblaci&oacute;n infantil, es necesario tomar las medidas para que la implementaci&oacute;n de la regulaci&oacute;n tenga un real impacto en la salud y estado nutricional infantil (45, 46). Para esto es relevante la adopci&oacute;n de una definici&oacute;n lo m&aacute;s precisa posible y com&uacute;n a todos los pa&iacute;ses de “alimento saludable y no saludable” y la generaci&oacute;n de informaci&oacute;n cient&iacute;fica s&oacute;lida que sirva como un marco conceptual de base. Considerando que en Chile no se cuenta con evidencias cient&iacute;ficas suficientes que favorezca la toma de decisiones en pol&iacute;ticas p&uacute;blicas, se hace necesario el desarrollo de esta &aacute;rea de investigaci&oacute;n sobre publicidad de alimentos y su relaci&oacute;n con la obesidad infantil a nivel nacional.</font>                     </p>                         ]]></body>
