Por otro lado, en estos niños inmunosuprimidos la infección suele ser subclínica. Suele diagnosticarse al observar elevación de aminotransferasas. Objetivo: evaluar la respuesta al tratamiento de hepatitis en niños con antecedentes oncológicos. Materiales: 80 pacientes con antecedentes de tratamiento hematoncológico, del año 2000 a 2008 con serologías positivas para hepatitis viral. Resultados: 39 (48,75%) presentan Hepatitis B, 19 (23,75%) Hepatitis C, 22 (27,75%) coinfección Hepatitis B y C. En el grupo HBV, 7 negativizaron espontáneamente DNAHBV. 20 recibieron tratamiento con diferentes esquemas: 3 IFN, 4 lamivudina, 3 lamivudina + IFN, 10 Lamivudina + PEG; 7/20 negativizaron ADN VHB. 6/10 con tratamiento combinado negativizaron DNAHBV (60%). En el grupo con Hepatitis C, todos genotipo 1; 3 negativizaron RNAHCV espontáneamente; 10 recibieron tratamiento, 3/10 tratados con IFN + RIBAVIRINA y 7 con PEG + RIBAVIRINA. 1/3 y 4/7 (57,14%), negativizaron RNAHCV, respectivamente. En el grupo con coinfección, 14 tratados: 2 IFN; 4 IFN + RIBAVIRINA; 8 PEG + RIBAVIRINA. 11/12 con esquema combinado culminaron tratamiento; negativizaron 1/11 (9%) DNAHBV y 5/11 (45,45%) RNAHCV. 8/80 desarrollaron complicaciones, cirrosis y hepatocarcinoma. Conclusión: los pacientes pediátricos presentan respuesta al tratamiento para hepatitis B y C crónicas, similar al del adulto. Hubo diferencia estadísticamente significativa en la respuesta al tratamiento relacionado con el grado de inflamación en pacientes coinfectados. Cirrosis y hepatocarcinoma también se presentan en este grupo de edad.]]></p></abstract>

La  distribución por el tipo de tumor se muestra en la <a href="#tab10">tabla 10</a>,  observándose que la leucemia y el linfoma son las patologías más frecuentes. <p ALIGN="justify"><font face="Verdana" size="2">Todos los pacientes presentaron  genotipo 1 del HCV; 3 negativizaron el RNAHCV espontáneamente, se inició  tratamiento a 13 pacientes, de los cuales, 3 se encuentran recibiendo  tratamiento actualmente, 3 recibieron tratamiento con IFN + RIBAVIRINA y 7 con  PEG + RIBAVIRINA. De los que recibieron IFN + RIBAVIRINA sólo 1/3 negativizó el  RNA, mientras que 4/7 del grupo de PEG (57,14%), negativizaron RNA. Si unimos  los dos grupos de tratamiento, de 10 pacientes tratados, respondieron 5 (50%).</font></p>     <p ALIGN="justify"><font face="Verdana" size="2">Al aplicar la prueba exacta de  Fisher en la relación entre el tiempo de infección con la respuesta al  tratamiento (<a href="#tab12">tabla 12</a>), valores de ALT y respuesta al  tratamiento (<a href="#tab13">tabla 13</a>), tiempo entre la finalización de la  quimioterapia, el inicio del tratamiento y la respuesta final (<a href="#tab14">tabla  14</a>), no hay diferencia estadísticamente significativa. <p ALIGN="justify"><font face="Verdana" size="2"><b>COINFECCIÓN HEPATITIS B Y C</b></font></p>     <p ALIGN="justify"><font face="Verdana" size="2">En el grupo de coinfección de  HBV y HCV, se encuentran 22 pacientes (27,75% del grupo de pacientes  oncológicos), siendo del sexo masculino 12, con un rango de edad de 2 a 15, con  una media de 7,91 años (<a href="#tab20">tabla 20</a>). En la <a href="#tab21"> tabla 21</a> se muestra la distribución según el tipo de tumor, observándose, al  igual que en los anteriores, predominio de linfoma y leucemia. En cuanto a la  distribución de la muestra según el tratamiento recibido (<a href="#tab22">tabla  22</a>), 14 pacientes recibieron tratamiento, 2 con IFN, 4 con IFN + ribavirina,  8 con PEG + ribavirina, correspondiendo 54,6% a tratamiento combinado IFN  (estándar o PEG) + ribavirina.</font></p>     <p ALIGN="center"><a name="tab20"> <img border="0" src="/img/fbpe/gen/v64n2/art05tab20.gif" width="546" height="195"></a></p>

<p ALIGN="justify"><font face="Verdana" size="2">El paciente tratado con  Telbivudina, es un paciente que presenta cirrosis, con várices esofágicas, por  lo que el tratamiento con PEG es restringido. 11 pacientes recibieron  tratamiento combinado, 4 IFN + RIBAVIRINA, de los cuales 1 suspendió el  tratamiento y 7 PEG+ RIBAVIRINA, de los cuales 1 suspendió tratamiento. De estos  pacientes, 1/11 (9%) negativizó el DNAHBV, y 5/11 (45,45%) negativizaron el  RNAHCV (<a href="#tab23">tabla 23</a>).</font></p>     <p ALIGN="center"><a name="tab23"> <img border="0" src="/img/fbpe/gen/v64n2/art05tab23.gif" width="574" height="204"></a></p>     
<p ALIGN="justify"><font face="Verdana" size="2">Del total de pacientes con  coinfección, 4/22 (21%) desarrollaron complicaciones tales como, 3 cirrosis y 1  HCC, en un lapso de tiempo de 2 a 10 años.</font></p>     <p ALIGN="justify"><font face="Verdana" size="2">Se relacionó el tiempo de  infección, los valores de ALT, el tiempo transcurrido entre el final de la  quimioterapia y el inicio del tratamiento y la respuesta al final del  tratamiento, tanto para el virus C como el virus B, no encontrándose diferencia  estadísticamente significativa (<a href="#tab24">tablas 24</a>, <a href="#tab25"> 25</a>, <a href="#tab26">26</a>, <a href="#tab27">27</a>, <a href="#tab28">28</a>, <a href="#tab29">29</a>). De igual manera se relacionó el grado de fibrosis e  inflamación y los valores de ALT (<a href="#tab30">tabla 30</a> y <a href="#tab31">31</a>), no encontrándose diferencia estadísticamente  significativa.</font></p>     <p ALIGN="center"><a name="tab24"> <img border="0" src="/img/fbpe/gen/v64n2/art05tab24.gif" width="550" height="221"></a></p>

<p ALIGN="justify"><b><font face="Verdana" size="2">DISCUSIÓN</font></b></p>     <p ALIGN="justify"><font face="Verdana" size="2">En los pacientes con Hepatitis  B que recibieron tratamiento combinado de Lamivudina e Interferón (ya sea  estándar o pegilado), se obtuvo una respuesta del 60%, que es mayor si la  comparamos con el total de pacientes tratados con los diferentes esquemas (35%).  Lo reportado por la literatura en pacientes adultos, es que la negativización  del DNA HBV oscila entre el 13% y 95% con diferentes esquemas de tratamiento<sup>(10)</sup>.  En un trabajo realizado por D’Antiga y colaboradores, reportan en niños  inmunotolerantes infectados perinatalmente con el virus de la hepatitis B, AMT  normales, cambios histológicos leves, tratados con Lamivudina durante 3 meses  como monoterapia y posteriormente IFN durante 10 meses, obtuvieron una respuesta  del 78%<sup>(11)</sup>. Esto se asemeja más a nuestros resultados cuando tomamos  en cuenta sólo los pacientes tratados con esquema combinado.</font></p>     <p ALIGN="justify"><font face="Verdana" size="2">Hay controversia en la  literatura en cuanto al seguimiento y evolución a largo plazo de los pacientes  sobrevivientes a un cáncer e infección con HCV. Algunos estudios muestran leve  afectación hepática después de 10 a 30 años de seguimiento, mientras que otros  reportan cambios patológicos importantes en pocos años<sup>(12,13,14,15,16)</sup>.</font></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify"><font face="Verdana" size="2">En un estudio realizado por  Castellino y col., donde realizaban seguimiento de 122 niños que habían  sobrevivido a un cáncer y 81,1% presentaban infección por HCV siendo el genotipo  1 el más frecuente, con una media de edad de 29 años, observó que 60 (28,8%) de  los pacientes, con una media de 12,4 años después del diagnóstico del cáncer,  presentaban fibrosis leve y 35,6% fibrosis moderada; 13,6% tenían cirrosis. El  tratamiento fue realizado en 38 pacientes. Los esquemas de tratamiento variaron  dependiendo de las terapias antivirales disponibles (IFN, IFN + Ribavirina,  PEGIFN + Ribavirina), logrando respuesta viral sostenida en 17 (44%). Concluyen  que a pesar de la temprana adquisición del HCV, la progresión de la enfermedad  hepática es comparable a la observada en los pacientes adultos<sup>(17)</sup>.</font></p>     <p ALIGN="justify"><font face="Verdana" size="2">La coinfección HCV-HBV en  varios estudios de seroprevalencia, se encontró que era de 21,3 – 38,2% de los  casos, y la enfermedad mostró un curso benigno, sin evidencia de descompensación  o desarrollo de HCC en un periodo de 10 años<sup>(18)</sup>. Sin embargo, Cesaro  y col. observaron el desarrollo de cirrosis en el 21% de los pacientes adultos  con coinfección, a pesar del curso leve de la enfermedad. El tratamiento con IFN  alfa ha sido inefectivo en la supresión de la replicación de ambos virus en la  coinfección<sup>(20,21,22,23)</sup>. El tratamiento combinado con IFN y  Ribavirina ha mejorado la respuesta viral. Sin embargo, Marrone y col. trataron  con IFN y lamivudina a 8 pacientes adultos jóvenes coinfectados HBV y HCV,  observaron una respuesta viral sostenida en 50% para HCV y seroconversión a Anti-HBe  en 20% de los pacientes(24). a diferencia que en nuestro trabajo, donde se  obtuvo una negativización del DNAHBV sólo en un paciente (9%) y del RNAHCV en  45,45%. Urganci y col. trataron 6 pacientes pediátricos con coinfección HCV y  HBV con IFN y Ribavirina, obteniendo seroconversión a anti-HBe, negativización  del DNAHBV y RNAHCV en 20% de los pacientes<sup>(18)</sup>.</font></p>     <p ALIGN="justify"><font face="Verdana" size="2">En nuestro trabajo, se observó  el desarrollo de complicaciones como cirrosis o HCC, tanto en pacientes  infectados por el virus de Hepatitis B, como en pacientes con coinfección HBV-HCV.  González - Peralta<sup>(25)</sup> describe el desarrollo de HCC en pacientes con  Hepatitis C, sin embargo en el grupo de hepatitis C no se observó estas  complicaciones.</font></p>     <p ALIGN="justify"><font face="Verdana" size="2"><b>CONCLUSIONES</b></font></p>     <blockquote>       <p ALIGN="justify"><font face="Verdana" size="2">• La respuesta al tratamiento    no mostró diferencias significativa al relacionar los niveles de ALT, grados    de inflamación o cirrosis, tiempo de la infección.</font></p>       <p ALIGN="justify"><font face="Verdana" size="2">• Las complicaciones como    cirrosis o HCC han sido descritas en pacientes pediátricos como muy raras, sin    embargo, pueden ocurrir en la edad pediátrica, por lo que debe realizarse un    seguimiento a estos pacientes.</font></p>       <p ALIGN="justify"><font face="Verdana" size="2">• La Hepatitis B, en    pacientes con antecedentes Hematooncológicos, responde mejor a una disminución    previa de la carga viral.</font></p>       <p ALIGN="justify"><font face="Verdana" size="2">• La hepatitis C tiene un    curso lento</font></p>       <p ALIGN="justify"><font face="Verdana" size="2">• La coinfección es    frecuente. Las complicaciones (cirrosis, HCC) se presentan más tempranamente    en presencia del virus de la HEPATITIS B</font></p>       ]]></body>
