Las tasas más altas de mortalidad estan en Japón, Suramérica y la Unión Soviética. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La OPS reporta al cáncer como la segunda causa de muerte en América Latina y el Caribe. La mortalidad por cáncer en la región es de 107 por 100.000 habitantes; las más altas están en Uruguay, Barbados, Perú, Argentina y Chile</font><Sup><font size="2">(3)</font></Sup><font size="2">. Frecuente en mayores de 65 años; predomiando en hombres</font><Sup><font size="2">(4,5)</font></Sup><font size="2">.  </font></SPAN ></P>     ]]></body>
<body><![CDATA[<P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">El descenso reciente en la incidencia del cáncer gástrico,  debería atribuirse al uso generalizado de la endoscopia y al tamizaje, y no a la antibioticoterapia</font><Sup><font size="2">(6)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">En Venezuela (1985) murieron más hombres por cáncer gástrico que por el prostático y pulmonar. En las mujeres lo superaron, el cáncer cervicouterino y del seno</font><Sup><font size="2">(7)</font></Sup><font size="2">.</font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Para el último quinquenio, entre los tumores digestivos, el cáncer gástrico representa la primera causa de muerte (37 %),  siguiéndole hepatobiliar (33,5 %) y colónico (23 %)</font><Sup><font size="2">(8)</font></Sup><font size="2">.</font></SPAN ></P>     <P style="text-align:justify; margin-bottom:30px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Lo expuesto confirma que este tumor constituye para nosotros un serio problema de salud pública. </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-size:10pt; font-weight:700; color:#211E1E; font-style:normal" >FACTORES DE RIESGO</SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Howson et al</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> en 1980 reportaron una disminución en la incidencia del cáncer gástrico en el mundo, especialmente en naciones desarrolladas, calificándolo como “un triunfo epidemiológico inexplicado”</font><Sup><font size="2">(9)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Existe consenso para señalar a la alimentación como principal factor causante del cáncer gástrico. Epidemiológica y experimentalmente son reconocidos los siguientes factores:  </font> </SPAN ></P>     <p align="justify"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:bold; color:#211E1E" ><font size="2">Alimentarios </font> </SPAN ></p>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">En emigrantes, especialmente japoneses a Australia, a Hawai y a USA, se demostró la importancia de la exposición inicial ocurrida en la niñez. 10 Existe un largo período de incubación -de 20 a 30 años-, que corrobora la importancia de dicha exposición.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Carnes y pescados curados y conservados (salados y/o ahumados)</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> su consumo aumenta el riesgo de cáncer gástrico, por ser fuente de nitritos, nitrosamidas y nitrosaminas exógenas<sup>(11,12,13,14)</sup>. </font></SPAN ></P>     ]]></body>
<body><![CDATA[<P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" > <font size="2">La sal en exceso actúa inicialmente, produciendo gastritis  atrófica. Las nitrosaminas obran tardíamente, transformando la metaplasia  intestinal y la displasia en neoplasia. Usar neveras para preservar los  alimentos, sin salarlos, ha minimizado ese riesgo. Asar en brasas o freír la  carne, la pirolisis de las proteínas por la cocción, origina derivados de  hidrocarburos policíclicos aromáticos y aminas heterocíclicas del grupo de las  piridinas e imidazol, con efectos carcinogénicos<sup>(15)</sup>.</font></SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Frutas y verduras. </font></SPAN > <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" > <font size="2">El bajo consumo de vegetales aumenta el riesgo de cáncer gástrico<sup>(12,13,16,17,18,19,20,21,22,23)</sup>. Las hortalizas verdes son protectoras, también las cocidas, a pesar que sus vitaminas termolábiles (vitamina C, E y carotenos) se destruyen hasta un 50% por la cocción. Contrariamente, vegetales fermentados (vinagreta), de gran consumo en Asia y en islas caribeñas, encaran gran riesgo, por su alto contenido en nitrosaminas, sal y bajo en antioxidantes. Es reconocido el efecto anticarcinogénico del ajo y la cebolla; contienen compuestos órgano sulfurados, que inhiben la promoción tumoral. Las frutas son ricas en vitamina C, betacarotenos y otros carotenoides, por lo que su consumo es básico. Además, proporcionan otros compuestos indispensables: Fibras, folatos, ácido fólico, fenoles, indoles, isocianatos, tiocianatos,  flavonoides, cumarinas, compuestos sulfurados e inhibidores de proteasas. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Vitaminas.</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> La vitamina E, y especialmente la C, son antioxidantes -capturan radicales libres- y neutralizan las nitrosaminas y nitrosamidas exógenas; por ello reducen el riesgo de cáncer gástrico</font><Sup><font size="2">(15)</font></Sup><font size="2">. La Vitamina C se concentra más en el jugo gástrico que en el plasma. El  </font></SPAN > <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Helicobacter pylori</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">, disminuye dicha concentración en la luz gástrica, y se normaliza una vez erradicado.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Nitratos, nitritos y nitrosaminas</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">. Las verduras contienen abundantes nitratos que se convierten en nitritos por las bacterias de la saliva.15 Los nitritos transformados en nitrosamidas y nitrosaminas en el estómago, dañan solo la mucosa inflamada, no la sana. La vitamina C neutraliza las nitrosaminas. Consumir simultáneamente frutas y verduras con alimentos curados, tiene un efecto protector similar</font><Sup><font size="2">(11, 17)</font></Sup><font size="2">.  </font></SPAN ></P>     <p align="justify"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:bold; color:#211E1E" ><font size="2">Ambiemtal </font> </SPAN ></p>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Hábito de Fumar.</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> El humo del tabaco llega al estómago inhalado o con la mucosidad bronquial deglutida. Contiene N-nitrosaminas y óxidos del nitrógeno, ambos cancerígenos gástricos, porque forman radicales libres con elevado potencial oxidante</font><Sup><font size="2">(24,25)</font></Sup><font size="2">. El fumar, produce hiporexia, descenso de vitamina C y de betacarotenos en el suero</font><Sup><font size="2">(25,26)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Consumo de Alcohol.</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> Este no parece carcinogénico gástrico. Su efecto gastrolesivo puede sensibilizar a la mucosa ante agentes cancerígenos</font><Sup><font size="2">(27)</font></Sup><font size="2">. Dicho efecto es atribuido a nitrosaminas contenidas en algunas bebidas, como: Cerveza, whisky, sidra y menor proporción el vino.</font></SPAN ></P>     <p> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:bold; color:#211E1E" ><font size="2">Condiciones Precursoras </font> </SPAN ></p>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Anemia Perniciosa</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> cursa con hipoclorhidria e hipergastrinemia ambas promotoras de  cáncer gástrico. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-style: normal; font-weight: normal" > <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Gastrectomía Parcial</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> Benigna practicada en jóvenes causa gastritis crónica, metaplasia intestinal y displasia del muñón gástrico, por aclorhidria y acción del reflujo alcalino bilio-pancreático. El cáncer del muñón aparece 15 a 20 años después del Billroth II. Estos pacientes deben monitorizarse endoscópicamente cada 1 - 2 años a partir del décimo año de operados. </font> </SPAN ></SPAN ></P>     ]]></body>
<body><![CDATA[<P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Enfermedad de Menetrier</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> (gastropatía hipertrófica), morbo raro que puede degenararse en cáncer gástrico. </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Pólipos Hiperplásicos</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> considerados no premalignos. Sin embargo, reportes de varios casos en el mundo demuestran lo contrario</font><Sup><font size="2">(28,29)</font></Sup><font size="2">. También en el Centro de Control del Cáncer Gastrointestinal de San Cristóbal, se comprobó su transformación maligna</font><Sup><font size="2">(30)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Pólipos Adenomatosos son premalignos. Su malignización oscila del 5 - 15% para los tubulares y del 15 - 75% para los adenomas vellosos y guarda relación con el tamaño del pólipo, edad del paciente y grado de displasia. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Esófago de Barrett</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> definido como un reemplazo del epitelio normal esofágico por epitelio metaplasico. El aumento del cáncer proximal en países desarrollados parece relacionado con la incidencia aumentada del Barrett. </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La transformación del epitelio de Barrett en cáncer, se comprueba por la presencia de displasia en la biopsia, considerada ésta un marcador de malignidad. Aunque es poco sensible. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Se ha buscado otra alternativa útil como marcador precoz de malignidad que debe ser sensible, específica, objetiva, y económica. El p53 gen -supresor que interviene en controlar el crecimiento celular- al mutar incrementa la vida media de la proteína p53 y se acumula en el núcleo celular. La proteína mutada se detecta fácilmente empleando anticuerpos específicos con inmunohistoquímica. Es un buen marcador alternativo, porque su coloración inmunohistoquímica resulta económica, está presente en la mucosa de Barrett asiento de displasia, es frecuente en pacientes con Barrett y cáncer, y la discordancia entre expertos es menor que en la displasia</font><Sup><font size="2">(28)</font></Sup><font size="2">.  Además, la inmunoexpresión p53 existe antes del desarrollo de displasia y carcinoma</font><Sup><font size="2">(29,30,28)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Yasin G et al estudiaron 50 pacientes con esófago de Barrett. Las biopsias fueron teñidas para p53 mediante inmunohistoquímica. En 40(80%) de ellos p53 fue negativo. En los 10 restantes (20%) se detectó p53, de los cuales 2 pacientes (20%) desarrollaron displasia de alto grado/cáncer. Concluyendo que la expresión p53 precedió en varios años al desarrollo de displasia o adenocarcinoma</font><Sup><font size="2">(28)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Úlcera Gástrica Benigna</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">, todavía se discute su transformación maligna. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Factores Genéticos </font> </SPAN > <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">se incluyen, historia familiar de cáncer gástrico y grupo sanguíneo.  </font> </SPAN ></P>     <p> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:bold; color:#211E1E" ><font size="2">Factores Ambientales </font> </SPAN ></p>     ]]></body>
<body><![CDATA[<P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Riesgo laboral </font> </SPAN > <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">observado en trabajadores de diversas industrias: caucho, minas, ceramistas, cementeras, canteras, sastres y actividades agroforestales, expuestos a sustancias como sílice, plomo y asbesto, que irritan la mucosa gástrica y actúan como cancerígenos</font><Sup><font size="2">(35,36,37)</font></Sup><font size="2">.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Condiciones Medioambientales:</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2"> El bajo nivel socioeconómico de muchos habitantes del planeta facilita tanto la transmisión y perpetuación del  </font> </SPAN > <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Helicobacter pylori</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">, como la ingesta de sustancias carcinogénicas. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#211E1E" ><font size="2">Infección por Helicobacter pylori</font></SPAN ><SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">: Al colonizar el estómago origina una respuesta inflamatoria que dura décadas;  considerándose causa principal de gastritis crónica. Inicialmente surge la gastritis crónica que evoluciona a atrofia gástrica con zonas de metaplasia entérica. Se produce un estallido oxidativo que lesiona el ADN e induce mutaciones en las células germinales mucosas, que de no corregirse culminará con la aparición del cáncer gástrico. La actividad inflamatoria altera el ADN de genes supresores de oncogenes, inactivándolos y favoreciendo el desarrollo neoplásico.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Otro mecanismo de carcinogénesis observado en esta infección es el aumento del grado de proliferación celular condicionado por un incremento del factor de crecimiento epidérmico estimulado por factores liberados por el germen y por hipergastrinemia secundaria, que remite tras la erradicación bacteriana. Determinarlos tiene ventajas, porque es sencillo y económico, por ello se está utilizando como tamizaje para esas patologías<sup>(60,61,62,63)</sup>.  </font></SPAN ></P>     <p align="justify"> <SPAN style="font-family:'Verdana'; font-weight:bold; color:#211E1E; font-style:normal" ><font size="2">BASES MOLECULARES DEL CÁNCER GÁSTRICO </font> </SPAN ></p>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">El cáncer gástrico tiene un pobre pronóstico. <body><![CDATA[<P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La activación del complejo Factor de Crecimiento Epidermico (FCE) / receptor del Factor de Crecimiento Epidermico (rFCE) tiene repercusiones importantes en la actividad proliferativa de las células basales del estrato foveolar. La presencia de mutaciones que activen en forma permanente dicho complejo, significa un constante estímulo proliferativo, independiente de los mecanismos reguladores. Esto lleva a las células basales gástricas a un estado de híper proliferación, característico del cáncer intestinal y también de sus lesiones precursoras</font><Sup><font size="2">(73,84,85)</font></Sup><font size="2">. En este sentido, varias mutaciones del FCE y/o rFCE han sido identificadas en el cáncer gástrico, algunas activan simultáneamente factor y receptor, favoreciendo una vía autocrina de proliferación celular (ejemplo Factor de Crecimiento Tumoral  &#945; y su receptor) o favoreciendo vías paracrinas como factor de crecimiento de hepatocitos y su receptor c-met. La vía paracrina ocurre entre células estromales (factor de crecimiento de hepatocitos) y células epiteliales tumorales (c-met), produciendo un sinergismo estroma - parénquima neoplásico dentro de la misma mucosa gástrica</font><Sup><font size="2">(73)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La formación del complejo FCE/rFCE provoca la expresión de genes relacionados con el ciclo celular, algunos de estos de respuesta inmediata como c-myc y otros tardía como las ciclinas y la proteína-kinasa-ciclina-dependientes</font><Sup><font size="2">(88)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Los productos proteicos de ambos genes forman un complejo ciclina/proteína kinasa ciclina dependiente con una dinámica de asociación y disociación a través del ciclo celular. La formación del complejo ciclina/proteína kinasa ciclina dependiente permitirá la entrada a la fase M (mitosis) del ciclo celular por una parte y a la de transición G1 (crecimiento) – S (síntesis), por otra. Son distintas ciclinas las que participan en ambas fases y por ello se habla de ciclina mitótica y ciclina G1-S. La ciclina mitótica se degrada al  finalizar la mitosis, liberando proteína kinasa ciclina dependiente, la cual interactua con la ciclina G1-S e inicia así los eventos necesarios para la transición de G1 – S. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Es evidente el papel protagónico que en la patogénesis del cáncer gástrico desempeñan tanto la proliferación celular como el ciclo celular, lo que ha permitido comprobar la existencia de mecanismos inhibitorios de estos procesos. El más importante de estos mecanismos es el bloqueo de la transición G1- S. Este se basa en la revisión del genoma para detectar posibles mutaciones genéticas, previo a la entrada en la transición G1 – S; así, si se detectan mutaciones, se produce una detención de esta transición hasta lograr la corrección del daño. Si esto no es posible, se activan los mecanismos de apoptosis. El gen supresor de tumores p53 cumple un papel central en este mecanismo. Se ha demostrado que la proteína p53 se sobre expresa posterior a la inducción de mutaciones del ADN</font><Sup><font size="2">(88)</font></Sup><font size="2">.  </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La expresión de p53 denota detención de la transición G1 – S para reparar los daños producidos, pero si en esta etapa se incrementa la proliferación por estímulos externos antes de completar la corrección, p53 induce apoptosis impidiendo así la replicación de células mutantes. Si las mutaciones persisten y logran inactivar la función de p53, se permitirá a las células basales mutantes replicarse sin control dando origen a la expansión clonal que a la postre se manifestará como una neoplasia. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Los conceptos expuestos sirven en la actualidad para comprender el origen multifactorial del cáncer gástrico. Por ello al investigar la génesis de este cáncer, con base a una sola explicación, nos conducirá inevitablemente a un camino sin salida quedando muchas preguntas sin una respuesta convincente.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:30px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">Es imperativo orientar los esfuerzos en investigar no solo una causa determinada, ya sea ambiental o hereditaria, sino también a encontrar la correlación entre los múltiples factores que interactúan para originar este tumor maligno que tantos estragos ocasiona a la humanidad.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-size:10pt; font-weight:700; color:#211E1E; font-style:normal" >CONCLUSIONES</SPAN ><SPAN style="font-family:'Verdana'; font-size:10pt; font-weight:normal; color:#211E1E; font-style:normal" > </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">El cáncer gástrico resulta de una agresión crónica a la mucosa gástrica donde la atrófica gástrica y la metaplasia intestinal incompleta III encaran el mayor riesgo de carcinogénicidad. </font></SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">El diagnóstico histológico de Metaplasia Intestinal debe informar cuál Tipo es y qué Tipo de mucinas predomina en sus células. </font> </SPAN ></P>     ]]></body>
<body><![CDATA[<P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">La alimentación es causa principal de cáncer gástrico: El exceso de sal, abusar de carnes a la brasa o fritas, dietas carentes de vitaminas antioxidantes y de  fibras no digeribles. </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:15px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">No hay dudas que el tabaco causa cáncer en general y gástrico en particular.  </font> </SPAN ></P>     <P style="text-align:justify; margin-bottom:0px"> <SPAN style="font-family:'Verdana'; font-weight:normal; color:#211E1E; font-style:normal" ><font size="2">El alcohol no califica como cancerígeno.
