El objetivo del presente estudio fue determinar de forma prospectiva la prevalencia de Diabetes Mellitus(DM) e intolerancia a la Glucosa en ayunas (IGA) en pacientes con hepatitis C crónica no tratados o naive, en comparación con la población general y con pacientes con enfermedades hepáticas de diferente etiología. Se incluyo una muestra de 13 pacientes que acudieron a la consulta de la Fundación Zuliana del Hígado desde Enero del año 2008 hasta Diciembre del año 2009 en el estudio. La prevalencia de DM e IGA fue de 38% comparado con 8% de la Población general (Centro Venezolano de información y estadística) y 25% del grupo control de 16 pacientes con enfermedades hepáticas de otras etiologías. De los 13 pacientes, cinco fueron Genotipo 1b, uno genotipo 1a y siete Genotipo 2a. De los pacientes con DM o IGA, dos fueron Genotipo 1b, uno 1a y dos 2a. De los cinco pacientes con DM o IGA cuatro tenían antecedente familiares de Diabetes. En conclusión, pacientes con HCV crónica tienen una mayor prevalencia de DM e IGA en comparación con la población general y con pacientes afectados por enfermedades hepáticas de otra etiología. El Genotipo no tuvo relación en este estudio con la DM o la IGA; los marcadores antropométricos de Obesidad estuvieron asociados en tres de los cinco pacientes lo cual sumado a la historia familiar de Diabetes siguiere una relación multifactorial en la patogénesis de la DM en los pacientes con HCV.]]></p></abstract>

Of 13 patients five were Genotype 1b, one genotype 1a and seven Genotype 2a. Of the patients with DM or IGA, two were Genotype 1b, one 1a and two 2a. Of the five patients with DM or IGA four had family history of Diabetes. In conclusion, patients with chronic HCV have a greater prevalence of DM and IGA in comparison with the general population and patients affected by different hepatics diseases. <body><![CDATA[<p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>INTRODUCCIÓN </span></b></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Se ha establecido una relación epidemiológica entre Hepatitis crónica C (HCV) y Diabetes <span class=SpellE>Mellitus</span> (DM). Esta asociación está basada en estudios realizados en todo el mundo incluyendo los Estados <span class=GramE>Unidos(</span>1,2,3,4). La mayoría de estos estudios describen esta asociación basándose en análisis retrospectivos, los cuales tienen el problema de que existen factores que aparecen en el transcurso o evolución de la enfermedad y que pueden tener influencia en su desenlace. Por ejemplo el estadio histológico de la enfermedad o el tratamiento con <span class=SpellE>Interferon</span> el cual es sabido puede inducir <span class=SpellE>insulino</span> resistencia en el área <span class=SpellE>esplácnica</span> y en los tejidos <span class=SpellE>perféricos</span> además de producir anticuerpos contra las células de los islotes de <span class=SpellE><span class=GramE>Langerhans</span></span><span class=GramE>(</span>5 6, 7). El objetivo del presente estudio es: (i<span class=GramE>)establecer</span> la prevalencia de Diabetes <span class=SpellE>Mellitus</span> Tipo 2 (DM) y/o intolerancia a <st1:PersonName ProductID="la Glucosa" w:st="on">la Glucosa</st1:PersonName> en ayunas (IGA) en pacientes con Hepatitis C crónica no tratados con Interferón o <span class=SpellE>Naive</span> y compararlo con la prevalencia en la población general y en pacientes con enfermedades hepáticas de otras etiologías, (<span class=SpellE>ii</span>) definir el papel de los factores del huésped y factores virales como <span class=SpellE>predictores</span> de DM y/o IGA en pacientes con hepatitis C crónica. </span></p>      <p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>MATERIALES Y MÉTODOS </span></b></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Estudio prospectivo de corte transversal, en el cual se incluyó una muestra de 13 pacientes de la consulta de <st1:PersonName ProductID="la Fundación Zuliana" w:st="on">la  Fundación Zuliana</st1:PersonName> del hígado desde Enero del 2008 hasta Diciembre del 2009, de ambos sexos y mayores de 18 años y con diagnóstico de Hepatitis C establecido por <span class=SpellE>inmunoensayo</span> de tercera generación y corroborado por PCR. Los pacientes previamente tratados con Interferón fueron <span class=SpellE>excluídos</span> independientemente del tipo y duración de la terapia toda vez que el interferón ha mostrado inducir <span class=SpellE>insulino</span> resistencia en los tejidos <span class=SpellE>esplácnicos</span> y anexos, además existe una controversia sobre si el Interferón produce anticuerpos contra los islotes de <span class=SpellE>Langerhans</span>. Igualmente fueron <span class=SpellE>excluídos</span> aquellos pacientes que tomen o hayan tomado corticosteroides los cuales pudieran alterar el metabolismo de <st1:PersonName ProductID="la Glucosa. También" w:st="on">la  Glucosa. También</st1:PersonName> fueron excluidos pacientes con cirrosis hepática descompensada. A todos los pacientes se les realizó una historia clínica completa con sus datos incluyendo antecedentes familiares de Diabetes y abuso de alcohol (más de 140 <span class=SpellE>grs</span>/semana en las mujeres y más de 210 <span class=SpellE>grs</span>/semana en los hombres). Medidas Antropométricas. Se midieron el peso y la talla para medir el índice de masa corporal o superficie corporal (IMC). </span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>La clasificación de <span class=SpellE>Indice</span> de masa corporal se realizó tomando 25-29,9 como sobrepeso y mayor de 30 obeso. A todos los pacientes se les tomó una muestra de Glicemia en ayunas y de acuerdo a <st1:PersonName ProductID="la Asociación Americana" w:st="on">la Asociación Americana</st1:PersonName> de Diabetes se consideraron diabéticos aquellos pacientes con Glicemias iguales o mayores a 126mgs<span class=GramE>./</span><span class=SpellE>dl.</span> <span class=GramE>en</span> ayunas en al menos dos ocasiones y niveles alterados de Glucosa en ayunas o Intolerancia a <st1:PersonName ProductID="la Glucosa" w:st="on">la Glucosa</st1:PersonName> en ayunas aquellos con cifras entre 112- 125 <span class=SpellE>mgs</span>./<span class=SpellE>dl</span>(8). El factor viral investigado fue el genotipo. El Grupo Control estuvo conformado por un grupo de pacientes con enfermedades hepáticas diferente a la hepatitis C <span class=SpellE>diagnósticados</span> y evaluados en la misma Institución cuyos datos e información fueron obtenidos de las historias clínicas y de la base de datos de <st1:PersonName ProductID="la Fundación.Los" w:st="on">la <span  class=SpellE>Fundación. Los</span></st1:PersonName> resultados se presentan en cifras absolutas y porcentajes. Para el análisis se usó el paquete <span class=SpellE>epinfo</span> 3.5.1 </span></p>      <p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>RESULTADOS </span></b></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Un total de 13 pacientes positivos para hepatitis C diagnosticada por <span class=SpellE>inmunoensayo</span> de tercera generación y corroborado por PCR constituyeron la población estudiada. De los trece pacientes cinco tuvieron DM ó IGA de acuerdo a los criterios de <st1:PersonName ProductID="la Asociación Americana" w:st="on">la  Asociación Americana</st1:PersonName> de Diabetes ADA lo cual representó una prevalencia de 38%. De los cinco pacientes uno solo fue del sexo masculino los cuatro restantes fueron mujeres, todos fueron mayores de 60 años tres en el grupo <span class=SpellE>etario</span> entre 60-70 años y dos en el grupo <span class=SpellE>etario</span> entre 70 y 80 años. El <a href="#cua1">cuadro 1</a> presenta la distribución de los casos de hepatitis C y Diabetes <span class=SpellE>Mellitus</span> de acuerdo al sexo. La diabetes está presente en el 38,46% de los casos de hepatitis C. De ellos 30,76% corresponde al sexo femenino, representando un 80% del total de casos de Diabetes. En el sexo masculino la frecuencia de Diabetes fue del 7,70%. Las diferencias observadas no son significativas (p&gt;0,05).<span style='color:#231F20'>Dos pacientes tenían genotipo 1b, uno genotipo 1a y dos  genotipo 2a. tal como se observa en el <a href="#cua2">cuadro 2</a>.</span></span></p>      <p align=center><span style='font-size:10.0pt; font-family:Verdana'> <a name="cua1"> <img id="_x0000_i1025" src=/img/fbpe/gen/v65n1/art10fig1.gif border=0></a></span></p>

<body><![CDATA[<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>De acuerdo al Índice de masa corporal tres de los pacientes eran obesos del sexo femenino, los dos restantes un hombre y una mujer estaban dentro del rango de <span class=SpellE>Indice</span> de masa corporal normal.</span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>En relación al índice de masa corporal, las cifras del <a href="#cua3">Cuadro 3</a> revelan que el sobrepeso y la obesidad representan el 76,92 % del total de casos de Hepatitis C. En los Diabéticos, el exceso de masa corporal está presente en el 60,01 % del total de casos, mientras que el exceso de masa corporal está presente en el 87,48 % de los casos sin Diabetes. Las diferencias entre diabéticos y no diabéticos no tienen significación estadística (p&gt;0,05). En cuatro de los cinco pacientes con DM o IGA había antecedentes familiares de Diabetes.</span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El  <a href="#cua4">Cuadro 4</a> presenta información sobre la relación del antecedente familiar de Diabetes <span class=SpellE>Mellitus</span>. Se observa que el antecedente familiar de Diabetes <span class=SpellE>Mellitus</span> está presente en 4 de los 5 casos de Hepatitis C y Diabetes <span class=SpellE>Mellitus</span>, es decir, en el 80 % de los mismos; en los casos de Hepatitis C sin Diabetes, el antecedente familiar está presente en 1 de 8 casos, es decir, en el 12,5 %. Las diferencias observadas son <span class=SpellE>significativas</span> y revelan que el antecedente familiar aumenta la probabilidad de Diabetes en los casos de Hepatitis C. Para determinar la posible relación entre <st1:PersonName ProductID="la Hepatitis C" w:st="on">la Hepatitis C</st1:PersonName> y Diabetes <span class=SpellE>Mellitus</span>, así como la <span class=SpellE>influencia</span> de otras enfermedades hepáticas en el riesgo de Diabetes, se compararon las cifras obtenidas en el estudio con la prevalencia de Diabetes en la población general (8% según Centro venezolano de Información y <span class=SpellE>estdística</span>). <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Al comparar las Enfermedades <span class=GramE>Hepáticas ,</span> con la población general como factor de riesgo de Diabetes <span class=SpellE>Mellitus</span>, se aprecia que las personas con Enfermedades hepáticas en general (VHC y de otra etiología)) comportan un riesgo de Diabetes <span class=SpellE>Mellitus</span> 5,18 veces mayor que la población general. Para Hepatitis C el riesgo es 7,19, con p &lt; 0,05 y test exacto de Fisher = 0,007. Para otras Enfermedades <span class=GramE>hepáticas(</span> de otra etiología: autoinmune, VHB, coinfección, alcohol)) el riesgo es 3,83 veces mayor que en la población general. Estos resultados traducen una gran asociación entre Hepatitis C y Diabetes <span class=SpellE>Mellitus</span>.</span></p>      <p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>DISCUSIÓN </span></b></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Este estudio analizó <span class=SpellE>prospectivamente</span> 13 pacientes y confirmó una asociación entre <st1:PersonName ProductID="la HCV" w:st="on">la HCV</st1:PersonName> y DM. Una prevalencia significativamente elevada (38,5%) fue demostrada en pacientes con HCV comparada con la población general (8%) y comparada con el grupo control <span class=SpellE>constituído</span> por pacientes con otras enfermedades hepáticas (25%) aunque entre estos últimos no hubo diferencias estadísticamente <span class=SpellE>significativas</span>. Estudios retrospectivos han descrito una alta prevalencia de Diabetes en pacientes con HCV, y algunos pocos estudios de corte transversal han arrojado resultados divergentes de prevalencia de DM y <span class=GramE>HCV(</span>9). En el presente estudio evaluamos el comportamiento en nuestro medio relacionándolo con factores como genotipo viral, índice de masa corporal, antecedente familiar de diabetes. Es interesante observar que uno de los factores bien reconocidos como de riesgo para DM/IGA en la población general(10,11,12), como es la obesidad definida por los marcadores antropométricos, peso y talla no estuvo independientemente asociado con DM/IGA en los pacientes con HCV; lo cual coincide con otros estudios. </span></p>      <p align=center><span style='font-size:10.0pt; font-family:Verdana'> <a name="graf1"> <img id="_x0000_i1030" src=/img/fbpe/gen/v65n1/art10fig6.gif border=0></a></span></p>

<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>De todas las variables asociadas estudiadas la historia familiar o antecedente de DM/IGA fue un <span class=SpellE>predictor</span> independiente <span class=SpellE>significativo</span> lo cual conduce a concluir que la predisposición genética juega un papel importante y determinante en los pacientes con Hepatitis C como factor de riesgo para desarrollar DM. No hubo relación entre el genotipo viral y <st1:PersonName ProductID="la DM" w:st="on">la DM</st1:PersonName>/IGA en este estudio. Estas observaciones sugieren que la patogénesis de DM en <st1:PersonName ProductID="la HCV" w:st="on">la HCV</st1:PersonName> es única, multifactorial, y difiere de la patogénesis usual de DM en pacientes sin HCV. Parece ser que factores asociados con HCV y factores genéticos <span class=SpellE>predisponentes</span> para DM son los responsables en la alteración del metabolismo de <st1:PersonName ProductID="la Glucosa. Estudios" w:st="on">la Glucosa. Estudios</st1:PersonName> posteriores para dilucidad la naturaleza y los mecanismos de esta asociación serán de suprema importancia. En resumen, este estudio demuestra una alta prevalencia de DM en pacientes con HCV en comparación con <st1:PersonName ProductID="la Población" w:st="on">la Población</st1:PersonName> general. </span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>La relación con el grupo control <span class=SpellE>constituído</span> por pacientes con otras enfermedades hepáticas fue mayor pero sin diferencia estadísticamente significativa. La historia familiar fue el único factor predictivo como variable independiente asociado con DM/IGA en nuestra cohorte de pacientes con HCV. La presencia de DM impacta la sobrevida de los pacientes con Cirrosis cuando se compara con los pacientes sin <span class=GramE>DM(</span>13). Además la diabetes <span class=SpellE>prexistente</span> en pacientes que van a ser trasplantados disminuye la sobrevida después del trasplante y aumenta la incidencia de rechazo, infecciones y complicaciones <span class=SpellE><span class=GramE>multisistémicas</span></span><span class=GramE>(</span>14).
