Método: Se practicaron 614 cordocentesis en 490 pacientes con técnica de manos libres y guiadas por ultrasonido, con fines diagnósticos y/o terapéuticos. Las muestras fueron tomadas en embarazos entre 18 y 38 semanas de gestación. Ambiente: Unidad de Alto Riesgo Obstétrico de la consulta de Prenatal de la Maternidad "Concepción Palacios" y Centro Nacional de Genética Humana y Experimental de la Universidad Central de Venezuela. Resultados: No hubo medicación materna previa o posterior al procedimiento excepto las transfusiones intravasculares; no se aplicó anestesia local. La complicación más frecuente fue la bradicardia fetal transitoria en 44 casos (7,17%). La mortalidad fetal imputable al procedimiento fue de 0,48%. <body><![CDATA[<P ALIGN="JUSTIFY">El acceso a la circulaci&oacute;n fetal y la extracci&oacute;n de sangre es un procedimiento cada vez m&aacute;s sencillo y seguro, debido a los avances en las t&eacute;cnicas obst&eacute;tricas y de ultrasonograf&iacute;a (1).  Debido a que existen embarazos donde es necesario obtener sangre fetal para conocer o descartar la presencia de una patolog&iacute;a, es como surge la idea de desarrollar una t&eacute;cnica segura y con bajos riesgos para el binomio madre-feto, donde se logre conseguir la informaci&oacute;n deseada.  El primer acceso a la sangre fetal se logr&oacute; por medio de la placentocentesis pero ocurr&iacute;a con frecuencia la contaminaci&oacute;n con sangre materna.  La fetoscopia asegur&oacute; la pureza de la muestra fetal pero se acompa&ntilde;&oacute; de un alto &iacute;ndice de complicaciones, adem&aacute;s de la complejidad y el costo de la t&eacute;cnica (2).  Daffos y col. en 1983 (3,4) comunicaron la primera experiencia en la obtenci&oacute;n de sangre fetal del cord&oacute;n umbilical con el uso de una aguja por v&iacute;a percut&aacute;nea, guiada por ultrasonograf&iacute;a y que denominaron cordocentesis o fun&iacute;culocentesis.  Esta  t&eacute;cnica es sencilla, puede realizarse desde el 2º trimestre de embarazo hasta el t&eacute;rmino, garantiza la pureza de la muestra y presenta una baja tasa de p&eacute;rdidas fetales, convirti&eacute;ndose as&iacute; en el m&eacute;todo ideal para la obtenci&oacute;n de sangre fetal (1,3-10).  En el pa&iacute;s el procedimiento se viene realizando en la Maternidad &quot;Concepci&oacute;n Palacios&quot; desde el a&ntilde;o 1993 por diferentes indicaciones de evaluaci&oacute;n fetal (11) y luego de conocida y desarrollada ampliamente esta t&eacute;cnica presentamos nuestra experiencia a la poblaci&oacute;n m&eacute;dica venezolana.</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">MATERIAL Y M&Eacute;TODOS</P>     <P ALIGN="JUSTIFY">&nbsp;</P> </B>    <P ALIGN="JUSTIFY">Desde abril de 1993 hasta abril de 1998, en 490 pacientes con 492 fetos (dos gemelares), provenientes de la Consulta de Alto Riesgo del Servicio de Prenatal de la Maternidad &quot;Concepci&oacute;n Palacios&quot; (297) y del Centro Nacional de Gen&eacute;tica (193), se tomaron 614 muestras de sangre fetal para la realizaci&oacute;n de diversos ex&aacute;menes diagn&oacute;sticos y procedimientos terap&eacute;uticos intrauterinos.  </P>     <P ALIGN="JUSTIFY">En la preparaci&oacute;n de la paciente no se establecen condiciones de ayuno y se solicita vaciamiento vesical espont&aacute;neo.  Previo consentimiento informado, por escrito, donde se establece la indicaci&oacute;n para el examen, y al conocer el grupo sangu&iacute;neo materno, se realiza un estudio ulrasonogr&aacute;fico detallado (ATL <I>Ultramark</I> 1 con transductor lineal de 3,5 MHz, <I>Medison Sonoace</I> 4800 con transductor<I> convex </I>de 3,5 MHz, <I>General Electric Logic</I> 200 con transductor <I>convex</I> de 3,5 MHz) para establecer bienestar fetal y edad de la gestaci&oacute;n, localizaci&oacute;n placentaria y del cord&oacute;n, cantidad de l&iacute;quido amni&oacute;tico, posici&oacute;n fetal y caracter&iacute;sticas anat&oacute;micas, para descartar posibles malformaciones.</P>     <P ALIGN="JUSTIFY">Dos operadores, se colocan guantes est&eacute;riles y proceden a practicar antisepsia del abdomen de la gestante, desde la s&iacute;nfisis del pubis hasta los bordes costales y ambas crestas il&iacute;acas, se colocan campos est&eacute;riles en los extremos inferior y superior y se introduce el transductor, con gel aplicado, dentro de un guante est&eacute;ril.  Se preparan inyectadoras desechables de 3 ml con heparina o sin ella, seg&uacute;n la prueba a realizar (muestra anticoagulada o no).  Usamos aguja desechable de punci&oacute;n lumbar 22 G x 3 1/2&quot; para la toma de la muestra fetal.</P>     <P ALIGN="JUSTIFY">Mediante la colocaci&oacute;n del transductor sin guiador (t&eacute;cnica de manos libres) (1,4,10) de manera transversal con respecto a la paciente, sin anestesia local y advertida la paciente, se realiza punci&oacute;n percut&aacute;nea, bajo visualizaci&oacute;n ecogr&aacute;fica continua de la aguja, hasta alcanzar el vaso previamente identificado y asegurada la ubicaci&oacute;n de la aguja dentro del mismo, lo cual se constata como un punto ecorrefringente, se procede a realizar una aspiraci&oacute;n suave del &eacute;mbolo.  La escogencia del asa de cord&oacute;n a punzar obedece a la accesibilidad que tenga &eacute;ste en ese momento, lo cual puede ser desde una asa libre, la inserci&oacute;n placentaria o en la inserci&oacute;n del cord&oacute;n en el feto.  Primero se extrae 1 ml de sangre que es descartado para evitar posible contaminaci&oacute;n con l&iacute;quido amni&oacute;tico y c&eacute;lulas maternas, luego se extraen de 2 a 5 ml m&aacute;s, de los cuales 0,5 ml anticoagulado se destina para la realizaci&oacute;n del volumen corpuscular medio (VCM) en un equipo automatizado de hematolog&iacute;a como prueba confirmatoria de la procedencia fetal de la muestra y el resto para el estudio a realizar.</P>     <P ALIGN="JUSTIFY">Extra&iacute;da la aguja, se busca la presencia de complicaciones como hemorragia en el sitio de punci&oacute;n, alteraciones de la frecuencia card&iacute;aca fetal y contracciones uterinas.  En ausencia de ellas la paciente se retira a su casa con reposo de 24 horas.  Cuando hay complicaciones se controla por ultrasonograf&iacute;a su evoluci&oacute;n inmediata por una hora  y a las 24 horas, y si la edad de la gestaci&oacute;n lo permite, se realiza control electr&oacute;nico de la frecuencia card&iacute;aca fetal.</P>     <P>Caracter&iacute;sticas de la poblaci&oacute;n estudiada: la edad promedio de las pacientes fue 30,4 y la menor de ellas de 13 a&ntilde;os, el mayor n&uacute;mero de gestantes se ubic&oacute; en el grupo de 36  a 40 a&ntilde;os (Cuadro 1).</P>     ]]></body>

Hubo pacientes con m&aacute;s de una indicaci&oacute;n y varias punciones como parte de la vigilancia de su patolog&iacute;a como es el caso de pacientes Rh (-) sensibilizadas (Cuadro 3).  La edad materna mayor de 35 a&ntilde;os fue la principal indicaci&oacute;n para determinar el cariotipo fetal en 244 pacientes (48,52%), seguida del estudio de feto malformado actual en 88 pacientes (17,5%) (Cuadro 4). De los 74 casos complicados, la bradicardia fetal transitoria se present&oacute; en 44 fetos (7,17%).   Murieron 12 de los 492 fetos estudiados y de estos 6 eran de pacientes Rh (-) sensibilizadas que estaban severamente afectados y fueron sometidos a transfusi&oacute;n intrauterina, 3 ten&iacute;an retardo de crecimiento intrauterino y oligoamnios severos y fueron sometidos a otros procedimientos diagn&oacute;sticos-terap&eacute;uticos como amnioinfusi&oacute;n.  Estas muertes ocurrieron posteriormente a 24 horas del procedimiento y antes de una semana.  S&oacute;lo en tres fetos no hubo enfermedad materna ni fetal concurrentes que explicaran la muerte (Cuadro 5), por lo que denominamos a esta mortalidad como debida directamente al procedimiento y representan el 0,48% de todos los procedimientos realizados.  En ellas la muerte ocurri&oacute; posterior a bradicardia severa irrecuperable del feto en el transcurso de la primera hora y se reportaron exclusivamente durante el primer a&ntilde;o de iniciada la t&eacute;cnica.</P> </FONT><B><FONT SIZE=4>    <P ALIGN="JUSTIFY"></P> </FONT><FONT FACE="Times,Times New Roman">    <P ALIGN="JUSTIFY">DISCUSI&Oacute;N</P> </B></FONT>    <P ALIGN="JUSTIFY">&nbsp;</P> <FONT FACE="Times,Times New Roman">    <P ALIGN="JUSTIFY">A partir del segundo trimestre de embarazo se puede obtener sangre fetal a trav&eacute;s de una punci&oacute;n percut&aacute;nea del cord&oacute;n umbilical, bajo visualizaci&oacute;n ultrasonogr&aacute;fica como lo describieron Daffos y col. (3) por primera vez en 1983.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">El hecho de que la cordocentesis debe ofrecerse a la paciente en algunos casos para estudio de su feto y estimulados por las informaciones continuamente publicadas en la literatura, es que nos vimos motivados a iniciar esta t&eacute;cnica en estos dos centros de trabajo e investigaci&oacute;n, para as&iacute; llenar un vac&iacute;o que exist&iacute;a en la exploraci&oacute;n invasiva de la perinatolog&iacute;a nacional.</P>     <P ALIGN="JUSTIFY">En nuestro trabajo el principal grupo de estudio fue el de pacientes entre 36 y 40 a&ntilde;os de edad, ya que la principal indicaci&oacute;n para realizar la cordocentesis fue la determinaci&oacute;n del cariotipo fetal por edad materna.  En nuestros centros de trabajo el diagn&oacute;stico de cariotipo fetal se lleva a cabo principalmente a trav&eacute;s de la amniocentesis, pero en aquellos casos donde la paciente presenta m&aacute;s de 20 semanas se le ofrece la alternativa de la cordocentesis como m&eacute;todo invasivo de diagn&oacute;stico prenatal.  Esto nos trae a la reflexi&oacute;n para insistir en que los estudios invasivos para diagn&oacute;stico prenatal deben indicarse lo antes posible durante el embarazo y que la cordocentesis en ning&uacute;n momento debe sustituir a la amniocentesis para conocer el cariotipo fetal, sino para aquellos casos donde no se pudo realizar antes de las 20 semanas y se desee conocer r&aacute;pidamente este resultado y para aquellos casos donde obtenci&oacute;n de sangre fetal sea la regla.</P>     <P ALIGN="JUSTIFY">As&iacute; como lo describen Daffos y col. (3,4), Sonek y col. (1) y D’Alton (5) cl&aacute;sicamente la punci&oacute;n se realiza en la base del cord&oacute;n, a 1-3 cm de la placenta.  Nosotros ubicamos con el transductor una zona del cord&oacute;n que sea f&aacute;cilmente accesible, la mayor incidencia de punciones es en la salida placentaria de &eacute;ste.  Tenemos experiencia con punciones en asas libres del cord&oacute;n, al igual que Berrebi y col. (12), quienes manifiestan que de esta forma se pudiera evitar la punci&oacute;n transplacentaria que en algunas ocasiones produce una mayor tasa de transfusi&oacute;n feto materna lo cual es importante en casos de pacientes Rh negativo (7,13-15).  Por el mayor riesgo de sensibilizaci&oacute;n por Rh con este procedimiento es que se recomienda la aplicaci&oacute;n de inmunoglobulina anti D en toda paciente Rh (-) no sensibilizada (14,15).</P>     <P ALIGN="JUSTIFY">El sitio de punci&oacute;n utilizado por nosotros en todo momento tiene la intenci&oacute;n obtener una muestra fetal lo m&aacute;s pura posible y evitar la obtenci&oacute;n de sangre materna o la mezcla de &eacute;sta con la fetal, lo cual es importante cuando tratamos de determinar cariotipo (16-20), o en la determinaci&oacute;n del grupo sangu&iacute;neo, valores hematol&oacute;gicos y para la realizaci&oacute;n de transfusiones intrauterinas descrito por Shah y col. (21) y otros (22-29) esta &uacute;ltima fue la indicaci&oacute;n en 53 casos.</P>     <P ALIGN="JUSTIFY">Tanto el n&uacute;mero de punciones como el tiempo del procedimiento fueron mayores en las pacientes sometidas a transfusi&oacute;n intrauterina intravascular, donde ocurr&iacute;a deslizamiento del cord&oacute;n por el movimiento fetal, hasta que se comenz&oacute; a usar la administraci&oacute;n de pancuronio al feto para inmovilizarlo durante el procedimiento (13,30).  Tambi&eacute;n el numero de punciones y el tiempo disminuy&oacute; considerablemente luego del primer a&ntilde;o de iniciadas las cordocentesis, actualmente la muestra se obtiene con s&oacute;lo una punci&oacute;n.</P>     <P ALIGN="JUSTIFY">Las limitaciones que encontramos en la realizaci&oacute;n del procedimiento son la posici&oacute;n de la placenta y el cord&oacute;n, la posici&oacute;n y los movimientos del feto, la cantidad de l&iacute;quido amni&oacute;tico, la presencia de miomas y la obesidad.  Estos problemas son superables con medidas tales como cambios de posici&oacute;n de la paciente, instilaci&oacute;n de soluciones en la cavidad amni&oacute;tica (amnioinfusi&oacute;n) y mucha paciencia.  Todos los procedimientos fueron bien tolerados por las pacientes.</P>     <P ALIGN="JUSTIFY">La confirmaci&oacute;n fetal de las muestras se llev&oacute; a cabo con la determinaci&oacute;n del VCM a trav&eacute;s de un equipo de hematolog&iacute;a automatizado; si el resultado es &gt; 100 fentolitros (fl) se confirma como sangre fetal y si es &lt; 100 fl se considera sangre maternal la cual se descarta.  Esta determinaci&oacute;n es muy confiable, ya que nos est&aacute; confirmando que la muestra sangu&iacute;nea contiene gl&oacute;bulos rojos de mayor tama&ntilde;o dado por la presencia de eritroblastos, (3,5,12,13).  S&oacute;lo en una muestra se report&oacute; VCM &lt;100 fl pero se envi&oacute; al laboratorio de citogen&eacute;tica por la certeza que se ten&iacute;a cuando se tom&oacute; la muestra por la observaci&oacute;n de la aguja dentro del cord&oacute;n y el cariotipo result&oacute; 46 XY, sexo que fue corroborado por ultrasonido.</P>     <P ALIGN="JUSTIFY">En nuestra serie no se observ&oacute; complicaci&oacute;n en el 87,95% de los procedimientos, la bradicardia fetal se present&oacute; en 17,17%, en estos casos se coloc&oacute; a las pacientes en dec&uacute;bito lateral izquierdo y se observ&oacute; a los fetos por ultrasonido durante 10 minutos hasta la completa recuperaci&oacute;n de la frecuencia card&iacute;aca en todos los casos; fueron reevaluados a la hora y a las 24 horas del procedimiento y, cuando la edad de la gestaci&oacute;n era mayor de 32 semanas, por control electr&oacute;nico de la frecuencia card&iacute;aca.  Hay que hacer notar que esta complicaci&oacute;n fue autolimitada y no dur&oacute; m&aacute;s de 1 minuto en promedio, comparable con lo reportado por Daffos (30) y Weiner y col. (31); su posible causa es la punci&oacute;n arterial y consecuente liberaci&oacute;n de sustancias vaso activas como endotelinas y prostaglandinas (32).  En ning&uacute;n caso se us&oacute; estimulaci&oacute;n farmacol&oacute;gica y/o vibroac&uacute;stica.</P>     <P ALIGN="JUSTIFY">El riesgo de muerte fetal debida al procedimiento fue 0,48%.  Descrita como muerte fetal de causa inexplicada es similar a algunas series y menor en otra (1-6,8,9,13,17,19,20,28,30).  Hay que hacer notar que las muertes reportadas, sin aparente causa materno fetal, ocurrieron solamente durante el primer a&ntilde;o de iniciada la t&eacute;cnica, el resto de las muertes fetales fueron relacionadas con afectaci&oacute;n fetal importante por isoinmunizaci&oacute;n Rh, retardo intrauterino severo y otras.  No se practic&oacute; ces&aacute;rea de emergencia ya que el promedio de edad fetal fue de 26,6 semanas.  Los riesgos de hemorragia y hematoma en el sitio de la punci&oacute;n y la placenta, de contracciones uterinas y de lipotimias maternas no son importantes en nuestro estudio.</P>     <P ALIGN="JUSTIFY">Los resultados muestran la factibilidad, con un alto margen de seguridad materno y fetal, de lograr al acceso a los vasos umbilicales del feto con la finalidad de obtener una muestra sangu&iacute;nea, a la cual se le puede practicar cualquier examen propio de una muestra sangu&iacute;nea, as&iacute; como la introducci&oacute;n de sustancias (sangre, f&aacute;rmacos, etc.) con fines terap&eacute;uticos, todo lo cual significa avances en el manejo del embarazo de alto riesgo obst&eacute;trico.</P> </FONT>    ]]></body>
