Sin embargo, se han reportado casos relacionados con la enfermedad hepática causados por virus no hepatótropos como el Citomegalovirus (CMV) y el Virus Epstein Barr (VEB). El objetivo del presente estudio fue establecer el diagnóstico diferencial de virus hepatótropos y no hepatótropos en pacientes con síntomas y signos sugestivo de infección hepática, de la ciudad de Maracaibo-Venezuela. Año 2007. Se recolectaron 268 muestras de sangre de pacientes procedente de diferentes centros de salud de la ciudad de Maracaibo, que acudieron al Laboratorio regional de Referencias Virológica. La presencia de anticuerpos de tipo IgM, para el virus de hepatitis A (VHA), B (VHB), C (VHC), CMV y VEB fue detectada mediante la prueba de ELISA. Los resultados obtenidos indican una alta incidencia del VHA 118/268(44,03%), seguido del VHB 5/268(1,86%) y ausencia del VHC. Para virus no hepatótropos, el 4,48% (12/268) fueron positivos para el VEB y el 9,7% (26/268) para CMV. El 39,92% (107/268) son considerados negativos para los virus antes mencionados. En este estudio se señala que los virus CMV y VEB suelen estar comprometidos con daño hepático y deben ser consideradas como posibles agentes etiológicos ante cualquier caso de hepatitis.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Viral hepatitis is a public health problem worldwide, caused by different etiologic agents, such as hepatitis A, B, C, D, E, F or G. However, cases have been reported related to liver disease caused by hepatotropic viruses, such as cytomegalovirus (CMV) and Epstein Barr Virus (EBV). The aim of this study was to establish the differential diagnosis of hepatotropic and non-hepatotropic viruses in patients with symptoms and signs suggestive of liver infection in the city of Maracaibo, Venezuela, 2007. 268 blood samples were collected from patients at different health centers in the city of Maracaibo, who recurred to the Regional Viral Reference Laboratory. The presence of IgM antibodies to hepatitis A virus (HAV), B (HBV), C (HCV), CMV and EBV was detected by ELISA. This study suggests that CMV and EBV  viruses are often involved with liver damage and should be considered as  possible etiologic agents for any case of hepatitis. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Key words:</B> Cytomegalovirus, Epstein Barr Virus, hepatitis.</FONT></P>     <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Recibido: 24-09-09 /  Aceptado: 08-03-10</FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Introducción</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">La hepatitis viral constituye uno de los problemas de salud pública, que  con mayor frecuencia afecta a la población a nivel mundial, siendo una  de las principales causas de morbi-mortalidad &nbsp;en la población humana (1-2).  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">La hepatitis se traduce en la inflamación del tejido hepático que puede  evolucionar a cirrosis y carcinoma hepatocelular producto de una respuesta  inmunitaria principalmente de tipo celular dirigida contra antígenos expresados  en estas células (1-2).  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Los síntomas más frecuentes de esta infección son &nbsp;astenia, cefalea, fiebre,  náuseas, diarrea, dolores musculares y abdominales (3). &nbsp;De forma típica  &nbsp;los pacientes suelen desarrollar malestar y debilidad, &nbsp;seguido por inapetencia,  náuseas y vomito. Dentro de los signos tenemos, ictericia, &nbsp;hepatomegalia,  anorexia, molestias gástricas, trastorno de las funciones hepáticas y acolia  (4-5). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Entre &nbsp;los agentes virales involucrados en esta infección tenemos, &nbsp;el virus  de la hepatitis A (VHA), &nbsp;que pertenece a la familia <I>Picornaviridae</I>, se  caracteriza por causar una enfermedad autolimitada que da lugar a inmunidad  duradera. Su vía de transmisión más frecuente es la fecal-oral (6-7). El  &nbsp;virus de la hepatitis E (VHE) &nbsp;por su parte es un hepevirus, comparte con  el VHA su mecanismo de transmisión. Entre los virus de transmisión parenteral  sexual tenemos el virus de la hepatitis B (VHB), el cual pertenece a la  familia hepadnaviridae, se transmite por contacto con sangre, semen, u  otro líquido biológico de una persona infectada &nbsp;a una susceptible. El &nbsp;virus  de la hepatitis C (VHC) pertenece a la familia Flaviviridae, la principal  vía de transmisión es la exposición directa o indirecta con sangre contaminada,  por ultimo tendremos &nbsp;el virus de la hepatitis D (VHD) perteneciente a la  familia Deltaviridae, es un virus satélite, necesitando la presencia del  VHB para su replicación (7-8). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> La infección hepática es causada en la mayoría de los casos por el VHA,  afectando principalmente a &nbsp;personas menores de 20 años (8). En países industrializados,  la prevalencia de la infección por VHA es baja y su incidencia y prevalencia  están asociadas al grado de desarrollo socioeconómico e higiénico sanitario  de la región, siendo endémico en los países en vías de desarrollo. El VHE  suele ser un agente exótico ya que solo se registran como caso importado  en inmigrantes y viajeros, sin embargo, suele causar brotes epidémicos  en algunos países donde ha alcanzado una mortalidad de hasta el 20% en  mujeres embarazadas (1-5). &nbsp;Por su parte la prevalencia de la infección  por VHB varía de una región geográfica a otra y aún dentro de una misma  región, pudiendo variar notablemente en los distintos grupos poblacionales. </FONT></P>      ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Otros agentes causantes de la hepatitis &nbsp;son agentes no hepatótropos como  el virus de Epstein Barr (VEB) y Citomegalovirus (CMV) y en menor grado  los virus Herpes Simplex (VHS), Varicela Zoster (VVZ), Rubéola, Adenovirus,  Dengue, Parvovirus B19, el virus de la Fiebre Amarilla, el virus de la  Parotiditis y Sarampión, los cuales pueden llegar a causar inflamación  así como disfunción en el hígado y producir una hepatitis aguda (4,8). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">El VEB infecta alrededor del 90% de la población mundial y persiste durante  toda la vida en el huésped, en los linfocitos B (células de larga vida,  responsables de la persistencia de la infección) y en las células epiteliales  de la cavidad oral, (probablemente responsables de su propagación). La  primo infección por este virus, se produce en los primeros años de vida,  casi siempre asintomática; en la adolescencia se presenta como un cuadro  de mononucleosis. En un pequeño porcentaje de los pacientes actúa como  un agente carcinógeno y está asociado con el desarrollo de enfermedades  malignas como el Linfoma de Burkitt y el carcinoma nasofaríngeo (9-10).  El VEB &nbsp;también puede estar involucrar con inflamación en el hígado, evidenciándose  a través de algunos signos como son los &nbsp;niveles elevados de transaminasas  (11-12).  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">El CMV humano no ha sido considerado un virus patógeno, teniendo en cuenta  que las formas clínicas sintomáticas de la infección por CMV en el huésped  normal son usualmente autolimitadas y de buen pronóstico, los pocos reportes  de casos clínicos severos han sido en infecciones congénitas y perinatales  en neonatos. Sin embargo, su importancia como agente patógeno ha ido en  aumento en las últimas dos décadas, debido a que su infección ha sido relacionada  con un alto porcentaje de rechazo de los órganos, luego de un transplante.  En la actualidad, del 70 al 100% de la población adulta presentan anticuerpos  contra el virus (13-14). La infección primaria de CMV puede seguir un largo  estado de latencia, en algún momento incluso una reactivación (13-15).  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> VEB y CMV, pertenecen &nbsp;a la familia Herpesviridae, y como el resto de los  virus de esta familia presentan la propiedad biológica de permanecer en  forma latente después de la primoinfección, aunque la respuesta inmune  que inducen conduce a su aparente eliminación (6). La primoinfección por  estos virus ocurre en diferentes edades de la vida, dependiendo aparentemente  del grado de desarrollo socio-económico de cada país (13).<SUP> </SUP></FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">El objetivo del presente estudio fue establecer el diagnóstico diferencial  de virus hepatótropos y no hepatótropos en pacientes con síntomas y signos  sugestivo de infección &nbsp;hepática, &nbsp;de la ciudad de Maracaibo – Venezuela.  Año 2007. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Material y Métodos</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Población</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Durante el período de Enero a Diciembre del año 2007, se recolectó 268  muestras de suero de una población de &nbsp;pacientes en edades comprendida entre  1 año y &nbsp;64 años (43±12), de ambos sexos, procedentes de diferentes organismos  dispensadores de salud de la ciudad de Maracaibo, &nbsp;que acudieron al Laboratorio  de Referencias Virológicas de la Escuela de Bioanálisis de la Facultad  de Medicina &nbsp;de la Universidad de Zulia, Venezuela, con sintomatología sugestiva  de infección hepática. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Toma de muestra</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">A cada paciente en estudio se le tomo 8 ml de sangre venosa, sin anticoagulante,  para la obtención del suero, el cual fue guardado a -20°C hasta el momento  de su procesamiento. </FONT></P>      ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> <B>Criterio de Inclusión</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Todos pacientes que presentase síntomas (astenia, cefalea, fiebre, náuseas,  diarrea, dolores musculares y abdominales) &nbsp;y signos (ictericia, acolia),  cuyo diagnóstico clínico y de laboratorio sugestivo a una hepatitis. Niveles  de transaminasa elevados al menos cinco veces por encima de los niveles  normales. (Niveles normales en sangre de transaminasa glutamicooxalacética  (GOT: 5-40 U/ml.) y &nbsp;transaminasa glutamicopirúvica (GPT: 5-30 U/ml.). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Criterio de exclusión</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Paciente inmunosuprimido, con insuficiencia renal, pacientes VIH positivo,  paciente oncológico. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Limitación del estudio</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Por limitaciones financieras no fue posible la determinación de: Hepatitis  E, Parvovirus B19, Rubéola, Dengue, &nbsp;Herpesvirus 6 y 7 y Varicela Zoster. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Consideraciones Éticas</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">El presente estudio fue aprobado por el Comité de Ética de la Facultad  de Medicina de La Universidad del Zulia. A todos pacientes y en el caso  de menores, a los representantes de los pacientes, les fue solicitado un  consentimiento previo por escrito para su inclusión en el estudio, siguiendo  los lineamientos de la Declaración de Helsinski del año 2003 para estudios  en humanos. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Método</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">La detección de la IgM de Hepatitis A (IgM-VHA), &nbsp;antígeno de superficie  contra Hepatitis B (HBsAg), antígeno “e” del virus hepatitis B (HBeAg)  y anticuerpos de tipo IgM e IgG del core (IgM e IgG  anti-HBc) de la hepatitis  B, se realizó mediante la técnica de ELISA, &nbsp;(Meia, IMx System Abbott Laboratoy  Diagnostic. Abbott Park, USA). </FONT></P>      ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Para la detección de anticuerpos contra el virus de hepatitis C se utilizó  el kit Anti-VHC, Innotest®, Bélgica.  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> La determinación de anticuerpos de tipo IgM contra CMV y &nbsp;IgM-VCA contra  VEB (Vironostica <SUP>®</SUP>, USA) se realizó por la técnica de inmunoensayo ELISA  de cuarta generación. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2"><B>Análisis Estadísticos</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Se presentaron los datos de las diferentes variables en forma de tablas  de frecuencia. Se analizaron los datos a través del análisis de Chi cuadrado  y el exacto de Fisher según corresponda. Se estudió la relación entre las  variables a través del análisis de coeficiente de correlación de Spearman. Se determinó la mayor incidencia en los  grupos de 10 a 14 años (90,9%) y de 1 a 4 años (80%). La incidencia del  VHB demostrada por la presencia del AgsHB e IgM-HBc se observó en el grupo  de 30 a 34 años (1,86%). <body><![CDATA[<P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> <B>Discusión</B> </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">El conocimiento en relación a las hepatitis virales ha experimentado un  notable incremento en los últimos años, al ser &nbsp;reconocidas como un serio  problema en salud pública a nivel mundial que requiere atención prioritaria.  En general, los virus que suelen causar enfermedad hepática lo hacen en  el contexto de una infección diseminada, en el caso de los llamados virus  no hepatótropos (CMV y VEB), o de forma preferente o exclusiva en el caso  de los virus conocidos como agentes hepatótropos (2). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">De forma general puede decirse que los agentes etiológicos de los cuadros  clínicos de las hepatitis virales, a pesar de pertenecer a diferentes familias  ocasionan un síndrome clínico similar, siendo necesario el diagnóstico  del laboratorio a través de los diferentes marcadores serológicos, en las  diferentes fases clínicas de la infección. Generalmente los virus no hepatótropos  no son tomados en cuenta para su estudio y de acuerdo a los resultados  obtenidos en esta investigación, se hace necesario el diagnóstico diferencial  a fin de determinar que virus se encuentra involucrado en el cuadro clínico. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">En el presente estudio se observó una mayor incidencia del VHA en los grupos  en edades de 10 a 14 años (90,9%), resultados similares a los obtenidos  &nbsp;en otros ámbitos geográficos (16-19). &nbsp;Esta incidencia &nbsp;suele estar relacionada  con el factor edad y el mecanismo de transmisión (fecal-oral). Se ha indicado  que la incidencia de la infección está directamente relacionada con los  factores de riesgos presente en los diferentes grupos poblacionales y que  la infección afecta directamente a los grupos de bajos recursos socio-económicos.  Los resultados obtenidos en el presente estudio confirmas que también en  Venezuela, la incidencia de la infección esta directamente relacionada  con la calidad de vida.  </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Por su parte la prevalencia de la infección varía en los diferentes países;  siendo baja en países desarrollados (alrededor de 20%) y alta en países  en vías de desarrollo (aproximadamente 90%), esta prevalencia es mayor  en estratos socioeconómicos bajos (1, 4-5). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> La incidencia obtenida en relación a la infección por el VHB fue de 1,86%,  porcentaje este que se mantiene teniendo en cuenta que Venezuela es un  país de mediana endemicidad (2,1-3,2%). (21-22). &nbsp;En Latinoamérica, se estima  sobre una población de 400 millones de habitantes, &nbsp;una incidencia de 140.000  a 400.000 casos al año, de los cuales dos tercios ocurren en Sudamérica  (20). Se considera en la presente investigación que los factores de riesgos  involucrados en la transmisión de la infección como es la vía sexual y  parenteral permanecen latentes en la región. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Estudios realizados en Chile, Argentina y Brasil, indican que la seroprevalencia  por este virus aumenta progresivamente con la edad (22), concordando estos  datos con los porcentajes obtenidos, donde la mayor prevalencia se obtuvo  en el grupo de edad de 30 a 34 años, lo anterior confirma lo relacionado  con las vías de transmisión de este agente. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">En relación al diagnóstico realizado en la detección de CMV y VEB se encontró  una prevalencia o incidencia de 9,70% y 4,48% respectivamente, lo que indica  la importancia del rol que estos virus juegan en la infección hepática.  &nbsp;En &nbsp;investigaciones realizadas recientemente (9-10, 21-22), relacionan al  VEB como causante de compromiso hepático, a pesar de que no es su principal  órgano blanco, al igual que en este estudio su detección se baso en la  determinación de anti-IgM, la aparición de esta inmunoglobulina indica  infección primaria o primera exposición al antígeno viral (23). Estudios  realizados en Turquía, señalan la presencia de síntomas y signos de una  &nbsp;hepatitis típica (esplenomegalia, dolor abdominal, vómitos, niveles altos  de transaminasas, entre otros) en un paciente, siendo negativo para anticuerpos  contra el VHA, VHB y VHC pero positivo para el VEB (24). </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" SIZE="2" FACE="Verdana"> Igualmente se ha demostrado &nbsp;la correlación entre CVM y otros virus humanos  con la presencia de cuadros hepáticos a través de métodos diagnósticos  como PCR en tiempo real, encontrándose ADN de CMV hasta en un 15,4%, demostrando  que CMV puede ser uno de los mayores patógenos de la hepatitis infantil  y adulta &nbsp;(3, 9). La presencia del CMV, coincide con los resultados de otras  investigaciones, lo cual demuestra que la presencia de CMV está claramente  relacionada con la disfunción hepática. </FONT></P>      <P ALIGN="justify"><FONT COLOR="000000" FACE="Verdana" SIZE="2">Por otra parte, un aspecto importante a considerar es que, el 39,92% de  los pacientes sintomáticos pero seronegativos &nbsp;contra los virus VHA, VHB,  VHC, CMV y VEB, &nbsp;podrían ser portadores de otros agentes virales como, &nbsp;Herpesvirus  6 y 7, Parvovirus Humano B19, Virus TT (VTT), Varicela Zoster, Rubéola  y Dengue; a los que también se les ha atribuido una relación con &nbsp;la producción  de daño hepático (4, 25-31). </FONT></P>      ]]></body>
