Los resultados se compararon con los mismos datos registrados en el grupo entre marzo y diciembre de 1999 cuando no recibieron el programa educativo. El programa educativo suministró información sobre el proceso de la enfermedad, las opciones de tratamiento y atención médica, principios nutricionales, vigilancia de glicemia capilar, incorporación del ejercicio al estilo de vida, prevención de complicaciones agudas y crónicas y ajuste psicosocial en su vida cotidiana. El cumplimiento del registro semanal del peso, la tensión arterial y la dieta fue autoevaluado de acuerdo a una escala. Dos tercios del grupo incrementó el puntaje de autoevaluación del cumplimiento de la dieta y el ejercicio, pero el cambio no alcanzó significancia estadística. Los cambios en pérdida de peso, descenso de la glucosa en ayunas, reducción de la hemoglobina glicosilada y de los triglicéridos fueron estadísticamente significativos. Los cambios registrados durante 1999 no fueron estadísticamente significantes. Los cambios observados en los indicadores evaluados muestran un efecto positivo del programa educativo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The aim of the study was to assess the changes in fasting blood sugar, glycosilated hemoglobin, lipids profile and body weight in 22 noninsulin dependent petroleum industry workers, who participated voluntarily in an educational and health care program from March to December 2000. The results were compared with the same data registered in the group from March to December 1999, when they did not received the educational program. In the educational program information was given on diabetes disease process, treatment options and medical care, nutritional principles, monitoring blood glucosa, incorporating physical activity into lifestyle, preventing and control of acute and chronic complications and psychosocial adjustment of dayly life. Weekly body weight recording, blood pressure and diet compliance was registered in agreement with a scale of self assessment. Two thirds of the group increased their score in the self assessment of the dietetic and excercise program compliance, but the change did not reach statistical significance. Changes in weight loss, lowered fasting sugar, reduced glycosilated hemoglobin and triglycerides were statistically significant. Changes registered during 1999 did not reach statistical significance. The observed changes of the indicators measured shows a positive effect of the educational program.]]></p></abstract>

Lifestyle changes.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">INTRODUCCI&Oacute;N</font></B></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La diabetes mellitus es una enfermedad cr&oacute;nica que constituye un verdadero problema de salud p&uacute;blica por su magnitud y sus implicaciones a nivel individual y colectivo (1,2) ya que la hiperglicemia cr&oacute;nica de la diabetes mellitus se asocia con disfunci&oacute;n y da&ntilde;o a largo plazo a &oacute;rganos vitales como retina, ri&ntilde;ones, nervios, coraz&oacute;n y vasos sangu&iacute;neos, aumentando la morbimortalidad general con una disminuci&oacute;n de la expectativa de vida y de la productividad, incrementado los costos de atenci&oacute;n m&eacute;dica (3,4).</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Como toda enfermedad cr&oacute;nica requiere cuidado m&eacute;dico continuo y educaci&oacute;n para prevenir complicaciones agudas y reducir el riesgo de complicaciones cr&oacute;nicas (5).  Los resultados publicados en 1993, del estudio prospectivo &quot;<I>Diabetes Control and Complications Trial</I>&quot; mostraron una reducci&oacute;n del 50-75 % del riesgo de desarrollo de complicaciones cr&oacute;nicas (retinopat&iacute;a, nefropat&iacute;a y neuropat&iacute;a) en un grupo de pacientes diab&eacute;ticos tipo 1 tratados con reg&iacute;menes intensivos y metas de valores de hemoglobina glicosilada (HbA1c) inferiores a 7 %, en relaci&oacute;n con un grupo de pacientes tratados en forma convencional (HbA1c&lt; 9 %) (6).  Un estudio posterior, el UK <I>Prospective Diabetes Study Group</I> realizado con pacientes diab&eacute;ticos tipo 2, tambi&eacute;n demostr&oacute; una disminuci&oacute;n del 25 % del riesgo de desarrollo de nefropat&iacute;a y retinopat&iacute;a en el grupo con estricto control metab&oacute;lico (HbA1c&lt; 7 %) respecto al grupo con terapia convencional.  Adem&aacute;s por el descenso de 1 punto en los niveles de HbA1c se observ&oacute; una reducci&oacute;n del 35 % en el riesgo de complicaciones microvasculares (7-9).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Numerosos estudios (10-14) han reportado los beneficios de los programas de educaci&oacute;n diabetol&oacute;gica en lograr el cumplimiento del plan terap&eacute;utico y alcanzar un control metab&oacute;lico adecuado para prevenir complicaciones agudas y cr&oacute;nicas.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para el logro de estos objetivos es b&aacute;sico el establecimiento de un programa de cuidado m&eacute;dico y de entrenamiento en destrezas de autocontrol que tome en cuenta la interacci&oacute;n de los diversos factores de &iacute;ndole biol&oacute;gico, psicol&oacute;gico y social involucrados, lo cual implica el concurso de diferentes especialistas relacionados con este problema de salud: m&eacute;dicos, enfermeras, nutricionistas, psic&oacute;logos, soci&oacute;logos y otros expertos (2).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Es por ello que la organizaci&oacute;n de Salud de PDVSA, dentro del protocolo para la prevenci&oacute;n, detecci&oacute;n, evaluaci&oacute;n y tratamiento de la diabetes mellitus dise&ntilde;&oacute; un programa sistem&aacute;tico de cuidado m&eacute;dico y actividades educativas destinadas a entrenar a los participantes en el desarrollo de destrezas de autocontrol y en el mantenimiento de un estilo de vida saludable.  Este programa se estructur&oacute; siguiendo un enfoque multidisciplinario (m&eacute;dico, nutricional, psicosocial y de reacondicionamiento f&iacute;sico) y en este trabajo se describen los cambios experimentados por 22 participantes en captaci&oacute;n educativa, cumplimiento de programas de intervenci&oacute;n nutricional y ejercicio, grado de control metab&oacute;lico, peso corporal y l&iacute;pidos sangu&iacute;neos durante 10 meses de entrenamiento.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">MATERIAL Y M&Eacute;TODOS</font></B></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Se inscribieron voluntariamente en el programa que se inici&oacute; el 1º de marzo de 2000, 22 empleados de PDVSA, &Aacute;rea Metroplitana, los cuales cumpl&iacute;an con los siguientes criterios de inclusi&oacute;n:</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">1. Persona con diagn&oacute;stico conocido de diabetes mellitus tipo 2.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">2. Persona con los criterios diagn&oacute;sticos definidos por el Comit&eacute; experto en diagn&oacute;stico y clasificaci&oacute;n de la <I>American Diabetes Association</I> en 1995: a. glicemia en ayunas &gt; 126 mg/dL; b. s&iacute;ntomas cl&aacute;sicos m&aacute;s determinaci&oacute;n casual de glicemia &gt; 200 mg/dL; c. glicemia &gt; 200 mg/dL 2 horas despu&eacute;s de una carga oral de 75 g de glucosa (3).  No se incluyeron en el programa embarazadas o personas con diabetes secundaria a otros procesos t&oacute;xicos, infecciosos, hormonales o como parte de s&iacute;ndromes gen&eacute;ticos.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">A todos los participantes se les realiz&oacute; una evaluaci&oacute;n inicial que comprend&iacute;a: historia m&eacute;dica de ingreso, historia nutricional, evaluaci&oacute;n psicol&oacute;gica, podol&oacute;gica, oftalmol&oacute;gica (retiniana), estudio sociocultural y entrevista con enfermera educadora en diabetes; adem&aacute;s cada uno era explorado para cl&iacute;nicamente con los siguientes ex&aacute;menes: HbA1c, l&iacute;pidos s&eacute;ricos (colesterol total, triglic&eacute;ridos, HDL colesterol, LDL colesterol), glicemia basal, &aacute;cido &uacute;rico, microalbuminuria, sedimento urinario, depuraci&oacute;n de creatinina, proteinuria de 24 horas (si hay albuminuria cl&iacute;nica), prueba de esfuerzo, Doppler arterial y electromiograf&iacute;a de miembros inferiores y <I>mapeo</I> de sensibilidad en pies.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los participantes asistieron a un programa informativo y de entrenamiento en destrezas de autocontrol durante 30 sesiones, 1 sesi&oacute;n semanal de 2 horas de duraci&oacute;n.  En cada reuni&oacute;n se verificaban cambios conductuales (cada participante llevaba su registro de autocontrol) y situaciones vivenciales, se dictaba una charla o una sesi&oacute;n pr&aacute;ctica sobre aspectos m&eacute;dicos, psicosociales, diet&eacute;ticos y de ejercicio f&iacute;sico relacionados con la diabetes.  Adem&aacute;s recibieron 8 horas de adiestramiento culinario y un taller (4 horas) dirigido a sus familiares.  Cada participante recib&iacute;a adicionalmente entrenamiento individualizado para su autocontrol por la enfermera-educadora y un programa de entrenamiento f&iacute;sico dirigido por un profesor de educaci&oacute;n f&iacute;sica en un gimnasio de la corporaci&oacute;n llamado Laboratorio de la Salud, al que asist&iacute;a 2 a 3 veces por semana.  Al inicio y al final del programa se le suministr&oacute; a cada integrante del grupo un cuestionario para medir nivel de conocimientos sobre la enfermedad, con 10 items cada uno (pretest y postest).</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Luego de la exploraci&oacute;n inicial, cada participante era evaluado mensualmente por la enfermera educadora, el m&eacute;dico internista, la nutricionista y el psic&oacute;logo, con ex&aacute;menes de laboratorio trimestrales: HbA1c, glicemia basal, l&iacute;pidos y microalbuminuria (de resultar positiva al inicio) adem&aacute;s de un registro diario de autocontrol llevado por el mismo participante sobre mediciones de glicemia capilar y cumplimiento de h&aacute;bitos de alimentaci&oacute;n y ejercicio.  Para la autocalificaci&oacute;n de estos &uacute;ltimos se ide&oacute; una escala arbitraria del 0 al 3, donde cero representaba ning&uacute;n cumplimiento y 3 cumplimiento estricto.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Adicionalmente de la historia m&eacute;dica de cada participante fueron obtenidos sus registros correspondientes a marzo y diciembre de 1999, del peso corporal y niveles de l&iacute;pidos sangu&iacute;neos, glicemia basal y glicohemoglobina.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los datos generados por las diferentes evaluaciones fueron recogidos en el Sistema Integral de Inform&aacute;tica M&eacute;dica de PDVSA y fueron procesados con el programa estad&iacute;stico StatGraphics version 6.1 aplicando el test de Wilcoxon para datos pareados.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">RESULTADOS</font></B></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">No hubo ninguna deserci&oacute;n en el grupo de 22 participantes duante los diez meses de seguimiento.  De ellos 19 s&oacute;lo tomaban hipoglicemiantes orales, 2 insulina, adem&aacute;s de hipoglicemiantes orales, y 1 se controlaba con dieta y ejercicios.  Los participantes, 19 hombres y 3 mujeres, ten&iacute;an una edad promedio de 50 a&ntilde;os, con rango entre 31 y 59 a&ntilde;os.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La dislipidemia fue la patolog&iacute;a m&aacute;s frecuentemente encontrada (63 %), seguida de obesidad (46 %) e hipertensi&oacute;n (36 %).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En cuanto a las complicaciones cr&oacute;nicas, la neuropat&iacute;a fue la m&aacute;s frecuente (36 %), seguida de la nefropat&iacute;a (23 %), cardiopat&iacute;a isqu&eacute;mica (9 %) y retinopat&iacute;a (5 %).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">De la aplicaci&oacute;n del cuestionario para evaluar el nivel de conocimientos sobre la enfermedad, el rango promedio obtenido por el grupo en el pretest fue de 6, y ascendi&oacute; a 8 en el postest (P= 0,0001).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El an&aacute;lisis de la puntuaci&oacute;n mensual de la autoevaluaci&oacute;n del cumplimiento de la dieta y el ejercicio, mostr&oacute; un comportamiento ascendente, pero en ambos casos el incremento no fue estad&iacute;sticamente significativo (P= 0,064 para la dieta y P= 0,084 para el ejercicio), al comparar las puntuaciones correspondientes a marzo y diciembre de 2000 (<a href="#cuadr1">Cuadro 1)</a>.</font></P>     <P ALIGN="CENTER"><font face="Times New Roman" size="3"><b><a name="cuadr1"></a>Cuadro 1</b></font></P>     ]]></body>

<body><![CDATA[<P ALIGN="CENTER"><font face="Times New Roman" size="3">31</font></TD> <TD WIDTH="10%" VALIGN="TOP">     <P ALIGN="CENTER"><font face="Times New Roman" size="3">29</font></TD> <TD WIDTH="10%" VALIGN="TOP">     <P ALIGN="CENTER"><font face="Times New Roman" size="3">33</font></TD> <TD WIDTH="11%" VALIGN="TOP">     <P ALIGN="CENTER"><font face="Times New Roman" size="3">0,055</font></TD> </TR> </TABLE>    </center> </div>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">P= variaci&oacute;n entre mes inicial y final.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En cuanto a la variaci&oacute;n de los l&iacute;pidos s&eacute;ricos durante 1999, hubo aumento estad&iacute;sticamente significativo en los niveles de colesterol total y LDL colesterol, mientras que las variaciones del HDL colesterol y triglic&eacute;ridos no alcanzaron significancia estad&iacute;stica (<a href="#cuadr2">Cuadro 2</a>).</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">DISCUSI&Oacute;N</font></B></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El presente estudio eval&uacute;a el comportamiento de una serie de variables cognitivas, conductuales, cl&iacute;nicas y bioanal&iacute;ticas en 22 participantes de un programa educativo, bajo la modalidad de adiestramiento individual y reforzamiento grupal, con un enfoque multidisciplinario, siguiendo las recomendaciones de la <I>American Diabetes Association</I> sobre &quot;<I>National Standards for Diabetes Self-Management Education </I>(2).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los hallazgos encontrados en el grupo de participantes en cuanto a edad promedio, tipo de diabetes, prevalencia de complicaciones cr&oacute;nicas y patolog&iacute;as asociadas muestran concordancia con lo descrito en la literatura (1,7-10).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Durante el per&iacute;odo de entrenamiento (marzo-diciembre 2000), los indicadores para medir nivel de conocimientos (puntuaci&oacute;n pretest, postest) tuvieron un incremento estad&iacute;sticamente significativo, mientras que las puntuaciones por autoevaluaci&oacute;n del cumplimiento de dieta y ejercicio mostraron una tendencia positiva al incrementarse sus valores pero que no lleg&oacute; a tener significaci&oacute;n estad&iacute;stica.  Es posible que en ambos casos la escala arbitraria aplicada de solo 4 opciones no resulte lo suficientemente sensible para medir los cambios conductuales.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El comportamiento favorable de algunos indicadores de control metab&oacute;lico como glicemia basal, HBA1c y l&iacute;pidos s&eacute;ricos, durante el per&iacute;odo de entrenamiento, al ser comparado con el comportamiento de estas mismas variables en el mismo grupo de pacientes durante un per&iacute;odo similar antes de entrar en el programa educativo, sugiere que los participantes aplicaron los conocimientos adquiridos y mejoraron su control metab&oacute;lico.  Anteriores estudios publicados sobre evaluaci&oacute;n de programas multidisciplinarios de educaci&oacute;n y cuidado a pacientes diab&eacute;ticos reportan efectos similares al intervenir con educaci&oacute;n y modificaci&oacute;n del estilo de vida (10-14).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El tama&ntilde;o de la muestra estudiada y el estrecho rango de la escala utilizada para medir cambios conductuales (cumplimiento de dieta y ejercicio) nos limita para extrapolar estos resultados a otros grupos poblacionales, as&iacute; como para emitir conclusiones que se constituyan en evidencia epidemiol&oacute;gica firme, pero s&iacute; nos permite vislumbrar los posibles beneficios de un enfoque educativo integral para cumplir con el objetivo de lograr un control metab&oacute;lico adecuado que permita prevenir complicaciones agudas y el desarrollo o progresi&oacute;n de complicaciones cr&oacute;nicas.  Tambi&eacute;n nos permite justificar la necesidad de realizar en esta misma l&iacute;nea, estudios prospectivos adicionales con una muestra mayor y grupos controles.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">Conclusiones</font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">• En el grupo de sujetos estudiados la patolog&iacute;a asociada m&aacute;s frecuente fueron las dislipidemias y la complicaci&oacute;n cr&oacute;nica m&aacute;s encontrada la neuropat&iacute;a perif&eacute;rica.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">• Durante el per&iacute;odo de entrenamiento el indicador usado para medir nivel de conocimientos mostr&oacute; un incremento estad&iacute;sticamente significativo al igual que los valores de HDL colesterol, mientras que el peso corporal y las variables del grado de control metab&oacute;lico (glicemia basal, HbA1c, triglic&eacute;ridos) tuvieron un descenso de sus valores que alcanz&oacute; un nivel de significancia estad&iacute;stica.  En el per&iacute;odo sin entrenamiento estas mismas variables no experimentaron cambios significativos.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">• Los niveles de colesterol total, LDL colesterol y la puntuaci&oacute;n de autoevaluaci&oacute;n de cumplimiento de dieta y ejercicio no tuvieron variaciones estad&iacute;sticamente significativas durante el per&iacute;odo de entrenamiento, pero el LDL colesterol y colesterol total tuvieron un incremento estad&iacute;sticamente significativo durante el a&ntilde;o 1999.</font></P>     <P ALIGN="JUSTIFY"><B><font face="Times New Roman" size="3">REFERENCIAS</font></P>     <!-- ref --><P ALIGN="left"></B><FONT face="Times New Roman" size=3>1.
