Las principales células que participan en la morfogénesis de las lesiones ateroescleróticas son los macrófagos y las células musculares lisas. Se evaluaron 39 aortas de fetos, recién nacidos y lactantes menores para corroborar la existencia de lesiones ateroescleróticas tempranas y relacionar dichas lesiones con la densidad de células inflamatorias presentes en las mismas. Se realizó estudio macroscópico con tinción de sudán IV para identificar lesiones sudanofílicas y microscopia óptica con coloraciones especiales. En 20 casos se realizó inmunohistoquímica para identificar linfocitos T (CD45-Ro), linfocitos B (CD20) y macrófagos (CD68) en la pared arterial. Ningún caso presentó lesiones sudanofílicas macroscópicas. Histológicamente, en el 39 % de los casos se observó algún grado de engrosamiento intimal que tiende a aumentar con la edad. La localización de estas lesiones intimales fue preferencial en las áreas de bifurcación del vaso. En todos los engrosamientos intimales y en la adventicia arterial, hubo mayor cantidad de macrófagos derivados de los monocitos (114 células CD68) que de linfocitos T (69 células CD45RO). No se comprobaron linfocitos B (CD20). Con estas observaciones, teorizamos que la penetración de los monocitos al espacio subendotelial, marca el inicio de las lesiones ateroescleróticas, la cual comienza en la vida fetal. Con nuestros resultados y con la revisión bibliográfica sobre este problema, evaluamos algunos aspectos sobre la naturaleza de los engrosamientos intimales en arterias fetales y neonatales y la secuencia de los eventos que originan la atracción, adhesión y penetración de monocitos en la subíntima así como la influencia de las lipoproteínas de baja densidad en este proceso. Concluimos que estos mecanismos in vivo, siguen siendo controversiales.]]></p></abstract>

No case presented macroscopic sudanophilic lesions. By histology, 39 % of the cases showed some degree of intimal thickening wich tends to increase with age. Monocytes.</P> <B>    <P>INTRODUCCI&Oacute;N</P> </B>     <P ALIGN="JUSTIFY">La ateroesclerosis es un problema de salud p&uacute;blica mundial y en Venezuela; la enfermedad y sus complicaciones se encuentran entre las tres primeras causas de mortalidad, junto con los accidentes de tr&aacute;nsito y las neoplasias (1-3).  Numerosos estudios han evidenciado la presencia de lesiones sudanof&iacute;licas en aortas de sujetos j&oacute;venes y lactantes (4-10).  Se ha insistido sobre el papel que los niveles altos del colesterol, desempe&ntilde;an en la g&eacute;nesis de la ateroesclerosis y recientemente se ha involucrado al sistema inmune en la patog&eacute;nesis de estas lesiones (11-17).  Sin embargo, los factores predictivos m&aacute;s fiables para esta enfermedad a&uacute;n no han sido identificados (18).  Las principales c&eacute;lulas involucradas en el proceso aterog&eacute;nico son los macr&oacute;fagos y las c&eacute;lulas musculares lisas (19-21).  Gerrity sugiri&oacute; que los monocitos circulantes invaden la &iacute;ntima, fagocitan las grasas y se convierte en una c&eacute;lula espumosa (16).  Otros autores opinan que las c&eacute;lulas espumosas derivan tanto de las c&eacute;lulas musculares lisas de la capa media, como de los mononucleares procedentes de la sangre (22).  Ha sido demostrado que la hipercolesterolemia inducida por la dieta produce da&ntilde;o endotelial (estructural y bioqu&iacute;mico), promueve la adhesi&oacute;n de los monocitos al endotelio vascular y su invasi&oacute;n a la &iacute;ntima, seguido por la formaci&oacute;n de estr&iacute;as lip&iacute;dicas (23).  Los principales procesos que inician las lesiones ateroescler&oacute;ticas como la adhesi&oacute;n de los leucocitos y plaquetas a las c&eacute;lulas endoteliales, la acumulaci&oacute;n de l&iacute;pidos en la pared a&oacute;rtica y la infiltraci&oacute;n de linfocitos T y monocitos en el subendotelio son semejantes a la respuesta inflamatoria cr&oacute;nica (24,25).</P>     <P ALIGN="JUSTIFY">Desde hace varios a&ntilde;os, se propuso un origen fetal para la enfermedad ateroescler&oacute;tica coronaria.  Morfol&oacute;gicamente, Stary describi&oacute; la presencia de c&eacute;lulas espumosas en vasos arteriales de reci&eacute;n nacidos y se&ntilde;al&oacute; que la disfunci&oacute;n endotelial es un evento temprano importante en la aterog&eacute;nesis.  En la actualidad, la disfunci&oacute;n endotelial ha sido reconocida y evidenciada en ni&ntilde;os y adultos con factores de riesgo para la ateroesclerosis (26,27).</P>     <P ALIGN="JUSTIFY">Desde el punto de vista morfol&oacute;gico, las arterias presentan engrosamientos adaptativos en localizaciones definidas desde el nacimiento, como una respuesta autolimitada de la &iacute;ntima a fuerzas hemodin&aacute;micas, las cuales son las primeras &aacute;reas que evolucionan hacia lesiones m&aacute;s avanzadas (28,29).  En estas &aacute;reas, ha sido detectada la presencia de c&eacute;lulas inflamatorias, interleucina-1-, interleucina-2 y el factor de necrosis tumoral <font face="Symbol"> g</font> (<font face="Symbol">g</font>TNF) en el citoplasma de las c&eacute;lulas infiltrantes, lo que sugiere un mecanismo de hipersensibilidad mediado por c&eacute;lulas, como un evento importante para el desarrollo de la ateroesclerosis as&iacute; como alteraciones en la expresi&oacute;n de los ant&iacute;genos del complejo mayor de histocompatibilidad clase II en las c&eacute;lulas musculares y en las c&eacute;lulas inflamatorias, elementos que podr&iacute;an contribuir a la perpetuaci&oacute;n de la reacci&oacute;n autoinmune aterogen&eacute;tica (30-32).  </P>     <P ALIGN="JUSTIFY">Para comprobar la existencia de lesiones ateroscler&oacute;ticas tempranas en las aortas de una poblaci&oacute;n de ni&ntilde;os en edad fetal y neonatal y relacionar la presencia de lesiones ateroscler&oacute;ticas tempranas macro y microsc&oacute;picamente, con la densidad de c&eacute;lulas inflamatorias y macr&oacute;fagos presentes en las mismas, realizamos una investigaci&oacute;n de tipo transversal en 39 fetos, neonatos y lactantes menores.</P> <B>    <P>MATERIALES Y M&Eacute;TODOS</P> </B>     <P ALIGN="JUSTIFY">La muestra est&aacute; integrada por 39 fetos mayores de 20 semanas de edad gestacional y lactantes menores de hasta 3 meses de edad, autopsiados durante el per&iacute;odo de un a&ntilde;o en el Instituto Anatomopatol&oacute;gico &quot;Dr. Jos&eacute; A. O Daly&quot; (IAP-UCV) y procedentes de los servicios de Obstetricia, Neonatolog&iacute;a y Pediatr&iacute;a del Hospital Universitario de Caracas (HUC).  Los casos de un g&eacute;nero u otro se dividieron en grupos de edades seg&uacute;n la clasificaci&oacute;n de pediatr&iacute;a y obstetricia en tres grupos: Grupo 1: mortinatos o neonatos pret&eacute;rminos (14-36 semanas de gestaci&oacute;n); Grupo 2: mortinatos o neonatos a t&eacute;rmino (37-41 semanas de gestaci&oacute;n) y Grupo 3: lactantes menores (ni&ntilde;os mayores de 28 d&iacute;as de edad y menores de 1 a&ntilde;o de edad).  Esta clasificaci&oacute;n se aplic&oacute; tanto a los nacidos vivos como a los fetos nacidos sin signos vitales.  Los datos de los diagn&oacute;sticos cl&iacute;nicos maternos y de los diagn&oacute;sticos anatomopatol&oacute;gicos definitivos de cada uno de los casos fueron obtenidos de las historias cl&iacute;nicas y de los protocolos de autopsia consignados.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Procedimientos.  Se obtuvieron las aortas procedentes de esta poblaci&oacute;n seg&uacute;n el m&eacute;todo utilizado por el PIA (Proyecto Internacional de Ateroesclerosis), fijando en soluci&oacute;n de formalina al 10 % durante 48 horas o m&aacute;s, sobre un cart&oacute;n, toda la aorta desde su porci&oacute;n ascendente hasta la bifurcaci&oacute;n de las il&iacute;acas, completamente abierta longitudinalmente por su porci&oacute;n posterior.  Las aortas se ti&ntilde;eron &quot;in toto&quot; con una soluci&oacute;n de Sud&aacute;n IV seg&uacute;n el m&eacute;todo de Guzm&aacute;n modificado por la secci&oacute;n de Patolog&iacute;a Cardiovascular del IAP-UCV (33).  Una vez te&ntilde;idas, se observ&oacute; la &iacute;ntima de cada aorta con el microscopio estereosc&oacute;pico para identificar lesiones sudanof&iacute;licas iniciales o tempranas (puntos y estr&iacute;as lipoideas).  Posteriormente, se tomaron los fragmentos representativos para el estudio histol&oacute;gico de los sitios donde se observaron lesiones sudanof&iacute;licas y de las no sudanof&iacute;licas, en la porci&oacute;n dorsal cercana a los orificios de salida de las ramas intercostales y de otras colaterales, as&iacute; como de la porci&oacute;n ventral de la aorta.  Las muestras fueron procesadas seg&uacute;n las t&eacute;cnicas habituales para el estudio histopatol&oacute;gico con microscopio de luz: inclusi&oacute;n en parafina y coloraciones de los cortes con Hematoxilina y Eosina, Tricr&oacute;mico de Masson, M&eacute;todo de Veroheff para fibras el&aacute;sticas, y Pas azul alciano para identificar proteoglucanos.  En todos los casos se obtuvieron muestras fijadas a las cuales se les practic&oacute; corte por congelaci&oacute;n y tinci&oacute;n con Sud&aacute;n IV y Sud&aacute;n negro.</P>     <P ALIGN="JUSTIFY">Se obtuvieron 468 cortes histol&oacute;gicos de 20 aortas seleccionadas, los cuales fueron utilizados para el estudio inmunohistoqu&iacute;mico con el m&eacute;todo de inmunoperoxidasa, para identificar: macr&oacute;fagos (CD68), linfocitos T (CD45-Ro) y linfocitos B (CD20).  Estos procedimientos fueron realizados en la Secci&oacute;n de Inmunohistoqu&iacute;mica del IAP.  (34).  La selecci&oacute;n de los 20 casos a los que se les practic&oacute; inmunohistoqu&iacute;mica, se bas&oacute; en los siguientes criterios histol&oacute;gicos: 1. Presencia de alteraciones en la capa &iacute;ntima: engrosamientos y presencia de cojinetes.  2. Aumento de la celularidad de las capas &iacute;ntima, media o adventicia y 3. Alteraciones de la capa media: desorganizaci&oacute;n de fibras el&aacute;sticas y aumento del tama&ntilde;o de los n&uacute;cleos.</P>     <P ALIGN="JUSTIFY">En cada uno de los casos se examin&oacute; cada corte histol&oacute;gico y se seleccionaron las &aacute;reas m&aacute;s representativas, en las cuales se cuantific&oacute; el n&uacute;mero de c&eacute;lulas inmunomarcadas en 20 y m&aacute;s campos de 400x.</P> <B>    <P>RESULTADOS</P> </B>     <P ALIGN="JUSTIFY">Del total de 39 aortas procedentes de fetos, neonatos y lactantes menores de 3 meses de edad, distribuidos por grupos de edades y g&eacute;nero, el 61,5 % correspondi&oacute; al g&eacute;nero masculino y el 38,5 %, al femenino. (<a href="#Cuadro 1">Cuadro 1</a>).  El promedio de edad gestacional para el primer grupo fue de 29,4 semanas y para el segundo, de 38,4 semanas. Obs&eacute;rvese la ausencia de lesiones sudanof&iacute;licas en estas dos muestras, en las que se aprecia la grasa peria&oacute;rtica te&ntilde;ida de rojo (control).</P> <B>    <P>Hallazgos microsc&oacute;picos (Microscopia de luz convencional)</P> </B>    <P ALIGN="JUSTIFY">En las aortas de 15 casos, se observ&oacute; alg&uacute;n grado de engrosamiento de la capa &iacute;ntima, localizados predominantemente en los sitios de ramificaci&oacute;n de la arteria a manera de &quot;cojinetes adaptativos&quot; y en otras &aacute;reas no relacionados a la turbulencia.  De acuerdo a los grupos de edad (ya definidos), 9 corresponden al grupo 1,5 al grupo 2 y 1 al grupo 3.  En todos estos casos, la capa &iacute;ntima estaba focalmente engrosada, con presencia de proteoglicanos &aacute;cidos distribuidos en forma laxa y difusa, en escasa cantidad; fibras col&aacute;genas delgadas y c&eacute;lulas mononucleadas aisladas.  En uno de ellos, se observ&oacute; mayor cantidad de matriz extracelular con macr&oacute;fagos dispersos y mayor densidad de c&eacute;lulas mononucleares (Autopsia N° 032-00).  En todos los cortes observados, la coloraci&oacute;n de Veroheff demostr&oacute; ausencia de fibras el&aacute;sticas en las &aacute;reas engrosadas. Subyacente a &eacute;sta, resalt&oacute; una l&aacute;mina refringente, continua y gruesa correspondiente a la l&aacute;mina el&aacute;stica interna.  La capa media estaba formada por laminillas de fibras el&aacute;sticas paralelas con fragmentaci&oacute;n artefactual ocasional.  Se observ&oacute; adem&aacute;s, n&uacute;cleos de c&eacute;lulas mesenquim&aacute;ticas redondeados u ondulados, hipercrom&aacute;ticos, que variaron de densidad por &aacute;rea en los diferentes cortes.  La capa adventicia present&oacute; algunos fibroblastos, linfocitos en variable cantidad y aisladas c&eacute;lulas mononucleares (<a href="#fig3">Figura 3</a>).</P>     ]]></body>

D: &quot;cojinete adaptativo&quot; en la bifurcaci&oacute;n (tricr&oacute;mico l00x).</P> <B>    <P>Relaci&oacute;n entre los engrosamientos intimales y los factores de riesgo maternos</P> </B>    <P ALIGN="JUSTIFY">Los datos maternos obtenidos a trav&eacute;s de las historias cl&iacute;nicas fueron insuficientes en la mayor&iacute;a de los casos.  Ninguna de las historias cl&iacute;nicas revisadas present&oacute; datos concernientes al lipidograma o niveles de colesterolemia de las madres.  Predomin&oacute; la insuficiencia de informaci&oacute;n cl&iacute;nica de las madres cuyos hijos presentaron engrosamiento intimal (7 casos), seguido por los productos de embarazo mal controlado (4 casos) y los embarazos gemelares (2 casos).  Dos ni&ntilde;os fueron productos de madre con preeclampsia, de los cuales, uno, present&oacute; engrosamiento intimal.  S&oacute;lo una de las historias cl&iacute;nicas mencion&oacute; los h&aacute;bitos tab&aacute;quicos de la madre, cuyo hijo present&oacute; engrosamiento intimal.  </P> <B>    <P>Hallazgos inmunohistoqu&iacute;micos</P> </B>    <P ALIGN="JUSTIFY">En el <a href="#Cuadro 2"> Cuadro 2</a> se expone el contaje de linfocitos y macr&oacute;fagos observados en el total de los casos de los tres grupos.  En ning&uacute;n caso se observ&oacute; la presencia de linfocitos B (CD20) en las capas de la aorta.  Los linfocitos T (CD45Ro) fueron m&aacute;s frecuentes en la adventicia, alrededor de los vasos sangu&iacute;neos, de los ganglios linf&aacute;ticos y dispersos en el tejido conjuntivo. <body><![CDATA[<P ALIGN="JUSTIFY">Los macr&oacute;fagos (CD68) se comprobaron en los engrosamientos intimales, distribuidos de forma difusa y en escasa cantidad.  En el grupo 3, se observ&oacute; mayor cantidad de macr&oacute;fagos en la &iacute;ntima que en los otros 2 grupos.  Hacemos la observaci&oacute;n que, el grupo 3 est&aacute; constituido solamente por 2 infantes, uno de los cuales (autopsia Nº 32-00), present&oacute; pr&aacute;cticamente la misma cantidad de macr&oacute;fagos que la totalidad cuantificada en los otros 2 grupos (<a href="#fig5">Figura 5</a>).</P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="CENTER"><a name="fig5"><IMG SRC="/img/fbpe/GMC/v110n4/art06img05.jpg" WIDTH=379 HEIGHT=123></a></P>

C: Macr&oacute;fagos inmunomarcados con CD68 en la adventicia arterial (250x).  1: &iacute;ntima, M: media, A: adventicia.</P>     <P ALIGN="JUSTIFY">Las tinciones de Sud&aacute;n realizadas en tejidos fijados y cortados por congelado, resultaron negativas en todas las aortas evaluadas.</P> <B>    <P>DISCUSI&Oacute;N</P> </B>     <P ALIGN="JUSTIFY">La aterog&eacute;nesis es un proceso extraordinariamente complejo que involucra fen&oacute;menos inflamatorios, degenerativos, gen&eacute;ticos e inmunol&oacute;gicos y las lesiones iniciales en los fetos est&aacute;n relacionados con la hipercolesterolemia maternas (35,36).</P>     <P ALIGN="JUSTIFY">Durante el nacimiento ocurren profundos cambios fisiol&oacute;gicos y circulatorios, que comienzan con una descarga de catecolamina <i> in &uacute;tero</i> lo que contribuye al incremento de la presi&oacute;n arterial sist&eacute;mica durante el per&iacute;odo prenatal y posteriormente durante la transici&oacute;n del cambio de patr&oacute;n de la circulaci&oacute;n fetal una vez ocurrido el nacimiento (37,38).  El incremento de la presi&oacute;n arterial producir&iacute;a una exageraci&oacute;n de las lesiones coronarias cong&eacute;nitas descritas en estudios experimentales (39).  Las c&eacute;lulas involucradas en los inicios de la aterog&eacute;nesis son: los macr&oacute;fagos y las c&eacute;lulas musculares lisas cuya aparici&oacute;n en la secuencia de los fen&oacute;menos inflamatorios durante la formaci&oacute;n de las lesiones ateroescler&oacute;ticas tempranas, es controvertida: Se imponen varias preguntas al respecto:  ¿Es simult&aacute;nea la presencia de los macr&oacute;fagos y de las lipoprote&iacute;nas en las lesiones ateroescler&oacute;ticas tempranas?  ¿Es normal o disfuncionante el endotelio?  ¿Qui&eacute;n penetra en primer lugar al subendotelio: el monocito o la lipoprote&iacute;na? (40).</P>     <P ALIGN="JUSTIFY">En 1992, Asada postul&oacute; un mecanismo etiopatog&eacute;nico en el cual, la hipercolesterolemia produce da&ntilde;o endotelial, se liberan factores quimiot&aacute;cticos quienes atraen los monocitos y permiten su adhesi&oacute;n al endotelio; posteriormente estos invaden la &iacute;ntima arterial y se inicia de este modo la formaci&oacute;n de la estr&iacute;a lip&iacute;dica (23).  Sin embargo, otros autores sostienen que las lipoprote&iacute;nas de baja densidad (LDL) es transportado a trav&eacute;s de un endotelio intacto para acumularse en la regi&oacute;n subintimal de las arterias aparentemente sanas donde interviene en el reclutamiento de monocitos a trav&eacute;s de la activaci&oacute;n de genes para la s&iacute;ntesis de citoquinas y mol&eacute;culas reguladoras de crecimiento (41-43).  Ha sido demostrado que la activaci&oacute;n de los genes, depende de la modificaci&oacute;n de las LDL nativas (bien sea por oxidaci&oacute;n o por acetilaci&oacute;n en el espacio subintimal) para que se induzca este proceso (44).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Por otra parte, en aortas morfol&oacute;gicamente normales existen engrosamientos intimales que se acompa&ntilde;an de ausencia de la l&aacute;mina el&aacute;stica interna y de dep&oacute;sitos de LDL (45).  Aproximadamente la mitad de los infantes en los primeros 6 meses de edad, presentan peque&ntilde;as colecciones de macr&oacute;fagos y c&eacute;lulas espumosas en segmentos &quot;susceptibles&quot; o &quot;pronos&quot; de las arterias coronarias, los cuales han sido llamados &quot;preateromas&quot; (46).  En nuestro estudio observamos que los macr&oacute;fagos estuvieron presentes s&oacute;lo en las arterias que presentaron engrosamiento intimal.  </P>     <P ALIGN="JUSTIFY">Recientemente, Napoli y col., sostienen que el proceso aterog&eacute;nico comienza en los fetos humanos demostrando que la oxidaci&oacute;n de las LDL ocurre durante la vida fetal y est&aacute; muy relacionada con la hipercolesterolemia materna, siendo la llegada de los macr&oacute;fagos un evento subsiguiente (47,48).</P>     <P ALIGN="JUSTIFY">En nuestra investigaci&oacute;n, no observamos lesiones ateroescler&oacute;ticas macrosc&oacute;picas en las aortas te&ntilde;idas con Sud&aacute;n.  En los trabajos de Napoli anteriormente citados, los estudios morfom&eacute;tricos con an&aacute;lisis de im&aacute;genes, demostraron la presencia de l&iacute;pidos en el 60 % de todas las secciones de aorta evaluadas comprobando que estas &aacute;reas fueron m&aacute;s extensas en la aorta abdominal, seguidas del arco a&oacute;rtico (35).  </P>     <P ALIGN="JUSTIFY">Microsc&oacute;picamente comprobamos en nuestros casos que, la mayor&iacute;a de las aortas presentaban una capa &iacute;ntima delgada, con un endotelio poco visible y con una capa fina de glucoc&aacute;lix.  En 15 casos se observ&oacute; alg&uacute;n grado de engrosamiento de la capa &iacute;ntima, predominantemente ubicadas en las &aacute;reas de bifurcaci&oacute;n de la arteria.  En todos estos casos la tinci&oacute;n para demostrar grasas en tejido fijado sin procesar y en tejido procesado, resultaron negativas.  En 14 de los 15 casos, la poblaci&oacute;n de linfocitos y macr&oacute;fagos result&oacute; escasa, como habr&iacute;a de esperarse si consideramos de que se trata de lesiones adaptativas, es decir, lesiones de tipo I de la clasificaci&oacute;n de Stary (49).  En un solo caso (Nº 32-00), observamos una lesi&oacute;n ateroescler&oacute;tica en la aorta perteneciente a un lactante de 2 meses de edad con antecedente de colocaci&oacute;n de marcapaso por bloqueo A-V completo cong&eacute;nito y portador de meningitis linfocitaria.  En este caso, se observ&oacute; engrosamiento de la &iacute;ntima con aumento de la sustancia fundamental y de la celularidad, predominantemente dada por la presencia de c&eacute;lulas mononucleadas CD68 positivas.  Este hallazgo coincide con las observaciones del grupo de Napoli en las que s&oacute;lo evidenciaron la presencia de macr&oacute;fagos en las lesiones establecidas (35).  Lamentablemente, en este caso as&iacute; como en los otros, los datos relacionados con la colesterolemia, h&aacute;bitos tab&aacute;quicos, h&aacute;bitos alimenticios y estado de la madre, as&iacute; como la colesterolemia del ni&ntilde;o, no se mencionaron en la historia cl&iacute;nica.  Por otro lado, m&aacute;s de un tercio de la muestra estudiada present&oacute; engrosamientos intimales, con tendencia no significativa a aumentar con la edad.  Estos resultados no concuerdan con los hallazgos de Napoli y col. quienes observaron que las concentraciones de colesterolemia fetal eran mayores en edades tempranas del desarrollo fetal y que &eacute;stas disminu&iacute;an a medida que transcurr&iacute;a el embarazo, sugiriendo que las lesiones observadas en los fetos podr&iacute;an regresar cuando los niveles de colesterolemia declinan (36).  </P>     <P ALIGN="JUSTIFY">En 1995, Berliner hipotetiz&oacute; que una vez que las lipoprote&iacute;nas de baja densidad han penetrado al subendotelio, son oxidadas por varias v&iacute;as las cuales activan un factor de transcripci&oacute;n parecido al factor NFkB que induce la expresi&oacute;n de genes cuyo producto proteico inicia la respuesta inflamatoria y el consiguiente desarrollo de la estr&iacute;a lip&iacute;dica (50).  El NF-kB es un factor nuclear que controla la transcripci&oacute;n de la Prote&iacute;na-1 quimioatrayente de monocitos (MCP-1), principal factor quimiot&aacute;ctico involucrado en la migraci&oacute;n de los monocitos, la cual se expresa en respuesta a diferentes est&iacute;mulos como lo son las LDLs, IL-1, TNF-alfa, el interferon gamma y el factor estimulante de colonias de macr&oacute;fagos (51).  </P>     <P ALIGN="JUSTIFY">La adhesi&oacute;n y transmigraci&oacute;n de los leucocitos a trav&eacute;s del endotelio est&aacute;n asociadas con cambios en la expresi&oacute;n de sus mol&eacute;culas de adhesi&oacute;n celular (CAMs) presentes en la superficie endotelial y por integrinas B-2 como la LFA-1, Mac-1 y la integrina B-1 VLA-4 expresadas sobre la superficie celular del leucocito (52-54).  Las cadherinas son mol&eacute;culas que se encuentran predominantemente en las zonas adherentes intercelulares, unidas a la actina del citoesqueleto por la alfa catenina, de tal manera que, cuando se aproximan los monocitos a la c&eacute;lula endotelial, se realiza un cambio de distribuci&oacute;n temporal de las mol&eacute;culas de cadherinas del endotelio y de la integrina LFA-1 de la superficie leucocitaria, promoviendo la interacci&oacute;n entre los dos tipos celulares (55,56).  Estos mecanismos moleculares observados <i> in vitro</i>, no han sido totalmente demostrados en estudios experimentales <i> in vivo</i>.</P>     <P ALIGN="JUSTIFY">Debido a todas estas evidencias, actualmente, se considera que la prevenci&oacute;n de la ateroesclerosis a trav&eacute;s del control de los factores de riesgo debe comenzar en la adolescencia y m&aacute;s a&uacute;n desde la ni&ntilde;ez (57).</P> <B>    <P>Conclusiones</P> </B>    <P ALIGN="JUSTIFY">Existen engrosamientos intimales predominantemente ubicados en sitios de bifurcaci&oacute;n de las colaterales a&oacute;rticas, los cuales son observados solo histol&oacute;gicamente.  El n&uacute;mero de aortas fetales con engrosamientos intimales tiende a ser directamente proporcional a la edad gestacional del feto, lo cual probablemente est&aacute; relacionado con una mayor susceptibilidad del tejido a&oacute;rtico a los niveles del colesterol en sangre materna.  </P>     <P ALIGN="JUSTIFY">Los engrosamientos intimales observados corresponden a lesiones adaptativas, en los que existe incremento leve de la matriz extracelular, fibras col&aacute;genas delgadas y aisladas c&eacute;lulas mononucleadas (macr&oacute;fagos y linfocitos). </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">En la &iacute;ntima de las aortas que presentan engrosamientos, existen linfocitos T y macr&oacute;fagos distribuidos de forma dispersa y en escasa cantidad.  Los linfocitos B est&aacute;n ausentes en los engrosamientos intimales.  En la capa media, los linfocitos T y macr&oacute;fagos est&aacute;n pr&aacute;cticamente ausentes.  En la adventicia, los linfocitos T y los macr&oacute;fagos se distribuyen de forma dispersa, predominantemente alrededor de los vasos sangu&iacute;neos.  Los linfocitos B est&aacute;n ausentes en cualquiera de las capas de la pared a&oacute;rtica.  </P>     <P ALIGN="JUSTIFY">La naturaleza y significado de los engrosamientos intimales a&uacute;n son tema de controversia.  Algunos autores opinan que estos engrosamientos adaptativos deben ser considerados ateroescler&oacute;ticos ya que se encuentran en los mismos lugares en los que posteriormente asientan las lesiones establecidas t&iacute;picas y  porque adem&aacute;s, son lesiones circunscritas y exc&eacute;ntricas (en los casos de las arterias coronarias) que se proyectan hacia el lumen del vaso.  Otros autores se&ntilde;alan que aunque los engrosamientos adaptativos no constituyen una lesi&oacute;n por s&iacute; misma, son un prerequisito para la retenci&oacute;n y acumulaci&oacute;n de l&iacute;pidos y por tanto para la formaci&oacute;n de la lesi&oacute;n.  Ninguna de estas concepciones son completamente ciertas, dado que, cuando las concentraciones de lipoprote&iacute;nas son extremadamente altas, los macr&oacute;fagos y los l&iacute;pidos se acumulan a&uacute;n en ausencia de lesiones adaptativas.  Por otra parte, las lesiones avanzadas tambi&eacute;n se desarrollan en localizaciones que no presentan previamente engrosamientos.  La migraci&oacute;n de los monocitos hacia la pared arterial es un evento cr&iacute;tico en el desarrollo de las lesiones ateroescler&oacute;ticas, y su reclutamiento en la &iacute;ntima arterial caracteriza la iniciaci&oacute;n de la lesi&oacute;n ateroscler&oacute;tica.  La penetraci&oacute;n de estas c&eacute;lulas al espacio subendotelial est&aacute; mediada por diversos elementos y procesos moleculares.  Las lipoprote&iacute;nas ingresan al subendotelio, donde son modificadas promoviendo la expresi&oacute;n de mol&eacute;culas de adhesi&oacute;n y citoquinas implicadas en la aterog&eacute;nesis temprana.</P>     <P ALIGN="JUSTIFY">La secuencia de los siguientes eventos: penetraci&oacute;n de LDL al espacio subendotelial, modificaci&oacute;n de las LDL por oxidaci&oacute;n o acetilaci&oacute;n, reclutamiento de monocitos y conversi&oacute;n de estos en c&eacute;lulas espumosas a&uacute;n no ha sido completamente dilucidada <i> in vivo</i>.</P>     <P ALIGN="JUSTIFY">La ateroesclerosis se inicia en etapas fetales de la vida, por lo tanto, la identificaci&oacute;n de los factores de riesgo y su control deben implementarse desde los primeros meses del embarazo.  </P>     <P ALIGN="JUSTIFY">Recomendamos incorporar el lipidograma materno dentro de los ex&aacute;menes rutinarios del control prenatal y de admisi&oacute;n hospitalaria de mujeres en trabajo de parto, tal como se cumple con las enfermedades infecciosas en las que se indica, de forma obligatoria, la realizaci&oacute;n de la serolog&iacute;a para VIH y VDRL.  </P> <B>    <P>REFERENCIAS</P> </B>    <!-- ref --><P ALIGN="JUSTIFY">1.
