La salud mental de los individuos con hemofilia y su entorno familiar, forma parte importante de su tratamiento integral y el conocimiento de las creencias y la actitud que perciben hacia su enfermedad permitiría influir positivamente en su mejoría clínica, su respuesta al tratamiento médico y su calidad de vida. Con fundamento en la "Teoría de la Acción Razonada" (Azjen y Fishbein, 1980), se aplicó en 43 individuos con hemofilia un Instrumento Generador de Creencias Salientes, para obtener información sobre lo que piensan acerca de su enfermedad. Las creencias obtenidas permitieron elaborar y aplicar el instrumento final para obtener la actitud denominado Modelo de Actitud en Pacientes con Hemofilia (MAPACHE). El 72% de los hemofílicos asignó la mayor importancia a los aspectos clínicos de la enfermedad, el 40% conoce aspectos generales de la misma, sólo el 18% le da importancia a la deficiencia del factor VIII o IX y a sus formas de tratamiento y los aspectos psicosociales son abordados también en baja proporción (20%). Al aplicar el MAPACHE se obtuvo una actitud global ligeramente positiva (4,44 ± 1,12 EE) hacia su enfermedad en la mayoría de los hemofílicos (74,5%). No se encontraron diferencias significativas entre la actitud y el grado de severidad, tipo de tratamiento, frecuencia del tratamiento e incapacidad física. En base a los resultados de las creencias y la actitud de los hemofílicos hacia su enfermedad, se recomienda que se haga mayor énfasis en la atención a su salud mental y la de su entorno familiar; además es importante la formación de un equipo multidisciplinario de atención integral que oriente sus esfuerzos hacia medidas preventivas para evitar secuelas propias de la enfermedad y mejorar la calidad de vida de los individuos con hemofilia.]]></p></abstract>

The knowledge of the beliefs and attitudes perceived by the patients toward their disease will make possible a positive influence in their clinical improvement, their response to the treatment, as well as their quality of life. On the basis of the Azjen and Fishbein’s Theory of Reasoned Action, a questionnaire was applied to 43 adult hemophilics to determine the salient beliefs about their disease. These beliefs permitted to elaborate a main structured questionnaire named Attitude Model in Patients with Hemophilia (Modelo de Actitud en Pacientes con Hemofilia, MAPACHE, in spanish), which was administered to the individuals and thus, the attitude toward their disease was obtained. Seventy two percent (72%) gave a major importance to the clinical aspects of the disease (hemorrhage, joint discomfort and trauma), 40% knew the general concepts of hemophilia (heredity, care and seriousness of the disease), 20% mentioned the implications of the psychosocial factors and only 18% had knowledge concerning the coagulation factors deficiency and the appropriate treatment. The MAPACHE showed a slightly positive score attitude (4,44 ± 1, 12 SEM) towards the disease in the majority of the groups (74.5%); with 26% of the hemophilics with a negative attitude. There were no significant differences between attitude and clinical parameters. It is recommended that a multidisciplinary team of caregivers should focus their efforts toward education and preventive measures in order to avoid the complications and consequences of the disease, to make possible a better quality of life in individuals with hemophilia.]]></p></abstract>

Aceptado: 15-04-2004&nbsp;</font></P>     <P ALIGN="justify"> <B><font color="#1f1a17">INTRODUCCI&#211;N&nbsp;</font></B> </P>     <P ALIGN="justify"><font color="#1f1a17"> La hemofilia es una enfermedad hemorr&#225;gica hereditaria que sin el adecuado  tratamiento puede ocasionar en la edad adulta temprana, incapacidad f&#237;sica  cr&#243;nica dependiente de la gravedad de la enfermedad y sus consecuencias  van m&#225;s all&#225; de las manifestaciones cl&#237;nicas del paciente porque afecta  su actitud mental y todo lo relacionado con su entorno psicosocial.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> Rittermann (1) define tres componentes que pueden afectar la condici&#243;n  del hemof&#237;lico: En primer lugar, la actitud del individuo frente a su enfermedad,  su sentido de impotencia frente a sus sangrados y su percepci&#243;n de que  no es una persona normal. En segundo lugar, el contexto familiar donde  su enfermedad forma parte clave en la interacci&#243;n con sus padres y hermanos;  los costos del tratamiento para transfusiones y hospitalizaciones son altos  y la vida familiar normal es interrumpida por las frecuentes emergencias.  Por otra parte, el complejo de culpa de la madre emerge como un factor  disociante, con sobreprotecci&#243;n del hijo afectado y exclusi&#243;n de los dem&#225;s  miembros de la familia. El padre generalmente se mantiene a distancia del  problema y en muchos casos se separa de su esposa, creando m&#225;s inseguridad  en el n&#250;cleo familiar. En tercer lugar, est&#225; la participaci&#243;n de la sociedad  (escuela, los amigos, clubes, el trabajo, etc.), que tiende a ser negativa  en la adaptaci&#243;n del hemof&#237;lico a su ambiente social.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> De acuerdo con la Federaci&#243;n Mundial de Hemofilia, el desaf&#237;o es ayudar  a prestar un cuidado integral a todas las familias con hemofilia (2). La  meta central del tratamiento del hemof&#237;lico es, adem&#225;s de aplicar el tratamiento  sustitutivo en forma eficaz y adecuada para evitar secuelas incapacitantes,  restaurar su sensaci&#243;n de bienestar, es decir, el derecho a percibirse  como persona, parte de una familia, de un grupo de amigos, con talento  y capacidad para el desarrollo, a&#250;n cuando presente una enfermedad que  le impone limitaciones y cuidados.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> Uno de los factores que influye en forma determinante en el comportamiento  de las personas con hemofilia es la actitud, entendi&#233;ndose &#233;sta como el  juicio evaluativo (positivo o negativo) que poseen los hemof&#237;licos hacia  su enfermedad, consider&#225;ndose adem&#225;s, que ella es un factor predictivo  de la conducta que posteriormente adopta en relaci&#243;n con la sintomatolog&#237;a  que presenta y con las secuelas incapacitantes que pudieran instalarse  durante el proceso evolutivo de la hemofilia. En consecuencia, se plantea  a trav&#233;s del an&#225;lisis de la actitud de las personas adultas con hemofilia,  establecer un diagn&#243;stico de una parte importante dentro de la salud mental,  lo que permitir&#237;a abordar de una manera integral la hemofilia, al enfocar  simult&#225;neamente su sintomatolog&#237;a cl&#237;nica con el aspecto psicosocial para  mejorar la salud personal y familiar de los hemof&#237;licos.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> En consideraci&#243;n de que en el tratamiento integral del hemof&#237;lico, juega  papel importante, el cuidado de su salud mental, se realiz&#243; el presente  trabajo fundamentado en la Teor&#237;a de la Acci&#243;n Razonada de Azjen y Fishbein  (3). Esta teor&#237;a es conocida desde hace dos d&#233;cadas y se fundamenta en  que las creencias de un sujeto hacia una determinada situaci&#243;n, generan  una intenci&#243;n que posteriormente se traduce en una actitud. La actitud  est&#225; relacionada con procesos cognitivos y afectivos que luego van a definir  una conducta (4), la cual a su vez, puede llevar a un cambio definitivo  sobre la situaci&#243;n inicial planteada. Esta teor&#237;a se ha empleado en diversas  situaciones donde se le asigna un valor preventivo o predictivo para establecer  conductas apropiadas, como es el caso del uso de drogas y alcohol (5),  el h&#225;bito de fumar (6, 7) y en enfermedades sexuales trasmisibles, especialmente  HIV-SIDA (8, 9).&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> Al adoptar esta teor&#237;a, se crea un modelo metodol&#243;gico para la identificaci&#243;n  y descripci&#243;n de las relaciones funcionales entre las creencias, actitudes  y la conducta, el cual permite la expresi&#243;n matem&#225;tica y en consecuencia  la posibilidad de estudiar el problema desde el punto de vista cualitativo  y cuantitativo.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> El prop&#243;sito del presente trabajo fue conocer las creencias y determinar  la actitud de los individuos con hemofilia hacia su enfermedad.&nbsp;</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"> <B><font color="#1f1a17">MATERIAL Y M&#201;TODOS&nbsp;</font></B> </P>     <P ALIGN="justify"><font color="#1f1a17"> Se realiz&#243; un trabajo de tipo descriptivo-prospectivo, aplicando la teor&#237;a  de la Acci&#243;n Razonada (3). En una primera fase se aplic&#243; una encuesta abierta,  denominada <I><B>Instrumento Generador de Creencias Salientes en Hemofilia</B></I> a  43 hemof&#237;licos (edad promedio 29,35 &#177; 8,5 a&#241;os de edad), 39 con hemofilia  A y 4 hemofilia B, que asisten al Banco de Sangre del Estado Zulia, en  Maracaibo, Venezuela. Veinticinco de ellos estaban clasificados como grado  leve (&gt;5% de la Actividad Residual del Factor), trece como moderado (de  1 a 5% de la ARF) y cinco severos (&lt;1% de la ARF). El 95% de los individuos  hab&#237;a recibido crioprecipitado como tratamiento sustitutivo, el 58% liofilizado  del factor deficitario y el 43 % plasma fresco congelado. El 33% hab&#237;a  recibido las tres modalidades de tratamiento, el 30% dos y el 35% s&#243;lo  una forma de tratamiento.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> La encuesta comprendi&#243; preguntas sobre su conocimiento acerca de la hemofilia  y sobre los problemas que ella ocasiona y fue respondida por el 100% de  los individuos. Las creencias de los participantes agrupadas y clasificadas,  tomando en cuenta el contexto general de la enfermedad, en cuatro categor&#237;as:  1) Conocimientos generales de su enfermedad, 2) aspectos cl&#237;nicos, 3) Tratamiento  y 4) Aspectos psicosociales. Los resultados se muestran en forma de frecuencia  de las respuestas emitidas.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> Una vez obtenidas las creencias, se procedi&#243; a elaborar y aplicar, el instrumento  <I><B>Modelo de actitud en pacientes con hemofilia </B></I>(MAPACHE) (<a href="#ANEXO 1">Anexo I</a>) para determinar  la actitud, conformado por catorce preguntas, formuladas para explorar  el componente cognitivo (fuerza de las creencias) y el afectivo (contenido  de las creencias). Estos dos componentes fueron valorados por los encuestados  en funci&#243;n de una escala bipolar (positiva a negativa), construida en base  a cinco categor&#237;as (dos positivas, una neutra y dos negativas) en t&#233;rminos  de muy bueno a muy malo (afectividad) y muy cierto a muy falso (cognici&#243;n).  Una vez obtenidas las respuestas, se procedi&#243; a calcular una estimaci&#243;n  de la actitud, para ello se multiplic&#243; lo obtenido en la evaluaci&#243;n de  las afirmaciones del componente cognitivo por su hom&#243;loga en el componente  afectivo. En cada uno de los componentes el valor arbitrario estuvo comprendido  entre 2 y &#150;2, por lo tanto el resultado de multiplicar las dos escalas  estar&#237;a entre +4 y &#150;4. La actitud de cada individuo ser&#237;a la sumatoria  total del valor de las 14 creencias y el signo establecer&#237;a si la actitud  es positiva o negativa. La actitud experimentar&#237;a entonces variaciones  entre +56 y &#150;56, nunca superior a estos l&#237;mites [14 x (&#177;4) = &#177;56], lo cual  significa que la actitud de los sujetos con puntuaci&#243;n comprendida entre  +1 y +56 es considerada como actitud positiva e igualmente aquellos individuos  con puntuaci&#243;n comprendida entre &#150;1 y &#150;56 son calificados como de actitud  negativa. Se consider&#243; actitud neutra si su valor num&#233;rico era igual a  cero.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> El MAPACHE fue previamente revisado y validado por siete expertos en psicolog&#237;a  y psiquiatr&#237;a y la confiabilidad de mismo fue estimada de acuerdo con el  escalograma de Guttman (10) en 10 encuestas escogidas al azar, el cual  report&#243; un valor de 0,92; valor &#233;ste que indica una reproducibilidad de  los resultados adecuada para establecer las conclusiones (<a href="#fig1">Fig. 1</a>).&nbsp;</font></P>     <P ALIGN="center"><a name="fig1"><img border="0" src="/img/fbpe/ic/v45n3/art06img01.jpg" width="497" height="478"></a></P>

<body><![CDATA[<P ALIGN="justify"><font color="#1f1a17"> La <a href="#fig2"> Fig. 2 </a> muestra las frecuencias en las respuestas en relaci&#243;n con las  diferentes categor&#237;as. La categor&#237;a de Aspectos Cl&#237;nicos result&#243; la de  mayor importancia, mencionando la afectaci&#243;n de las articulaciones y sus  complicaciones o secuelas (83%), en segundo lugar las hemorragias (75%)  y en tercer lugar el temor a golpes o traumatismos (55%). El conocimiento  general de la enfermedad estuvo relacionado con la trasmisi&#243;n hereditaria  (55%), secundariamente mencionaron que es una enfermedad de cuidado y de  gran severidad. Es importante destacar que los sujetos mencionaron en menor  proporci&#243;n, aspectos sobre el factor de la coagulaci&#243;n deficiente (28%)  y su forma de tratamiento (8%). En relaci&#243;n con los aspectos psicosociales,  estos son conocidos en una baja proporci&#243;n, se&#241;alando principalmente la  incapacidad f&#237;sica para el deporte y el trabajo (38%), problemas psicol&#243;gicos  (25%) y el desconocimiento de la enfermedad (8%).&nbsp;</font></P>     <P ALIGN="center"><a name="fig2"><img border="0" src="/img/fbpe/ic/v45n3/art06img02.jpg" width="526" height="433"></a></P>

En la  etapa preescolar, el hemof&#237;lico no tiene conciencia de su enfermedad porque  es la madre quien soporta el peso y la sensaci&#243;n de culpabilidad por la  enfermedad; en la etapa escolar, ve como es relegado de actividades que  realizan sus compa&#241;eros y comienzan las ausencias, deserciones y bajo rendimiento  acad&#233;mico (11), en la adolescencia, el hemof&#237;lico ya internaliza que no  es una persona &#147;normal&#148; y entra en situaciones de ansiedad y depresi&#243;n  (12, 13), en la etapa adulta, su vida social se afecta por el desempleo,  situaci&#243;n &#233;sta agravada si presenta incapacidad f&#237;sica. La sexualidad y  relaci&#243;n de pareja se ven afectadas por el temor de contraer enfermedades  transmisibles por derivados sangu&#237;neos y si es seropositivo para SIDA,  se crea un ambiente de inestabilidad emocional, dif&#237;cil de manejar, que  influye notablemente en el n&#250;cleo familiar (14-16).&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> La obtenci&#243;n de las creencias de los hemof&#237;licos, fundamentada en la Teor&#237;a  de la Acci&#243;n Razonada de Azjen y Fishbein, constituy&#243; la primera fase para  el conocimiento de la actitud de ellos hacia su enfermedad, la cual fue  posteriormente determinada con la aplicaci&#243;n del MAPACHE, el valor de sus  componentes cognoscitivo y afectivo permiti&#243; la estimaci&#243;n de la actitud  de los hemof&#237;licos y en consecuencia, esto permitir&#237;a modificar o mejorar  conductas no apropiadas en ellos y su entorno familiar. El presente estudio  demuestra que las personas con hemofilia encuestadas no conocen a fondo  su enfermedad, su cuidado adecuado y sus formas de tratamiento. La importancia  que asignan a los aspectos cl&#237;nicos evidencia que hasta el momento, la  atenci&#243;n m&#233;dica s&#243;lo existe cuando aparecen la hemartrosis u otras formas  de sangrado, asociadas generalmente a traumatismos.&nbsp;</font></P>     <P ALIGN="justify"><font color="#1f1a17"> En el aspecto de su salud mental, el hemof&#237;lico desconoce mayoritariamente  la ayuda psicosocial que se le puede prestar a &#233;l y su familia. La actitud  ligeramente positiva evidenciada en el presente estudio, se afianza fundamentalmente  en las creencias sobre el mejor conocimiento general de la enfermedad,  la atenci&#243;n m&#233;dica de emergencia, el cuidado de la misma y la posibilidad  de mejor&#237;a econ&#243;mica, para llevar una mejor calidad de vida. Por otra parte,  las creencias sobre enfermedades transmisibles a trav&#233;s de transfusiones,  la afectaci&#243;n articular con secuelas incapacitantes, la herencia y la deficiencia  del factor causante de la hemorragia, contribuyen a generar una actitud  negativa. En este sentido, algunos autores demuestran una asociaci&#243;n entre  los par&#225;metros cl&#237;nicos y aspectos psicosociol&#243;gicos en pacientes con enfermedad  cr&#243;nica (17). Es importante destacar que los factores econ&#243;micos pueden  contribuir a la inestabilidad social del hemof&#237;lico, especialmente el desempleo  o la econom&#237;a informal. La autopercepci&#243;n de individuos con hemofilia muestra  adem&#225;s de autoestigmatizaci&#243;n, depresi&#243;n, ansiedad e inseguridad, recomendando  en estas situaciones un programa basado en la participaci&#243;n de la comunidad  para mejorar la adaptaci&#243;n social de los individuos con hemofilia (18).&nbsp;</font></P>     ]]></body>
