La morbi-mortalidad producida por este microorganismo está relacionado en gran parte con la resistencia antimicrobiana y la capacidad de formar biopelículas. En este estudio se analizaron las variaciones fenotípicas de susceptibilidad de 50 cepas de K. pneumoniae (26, productoras de bLEE y 24, no productoras de bLEE) aisladas de pacientes pediátricos con septicemia nosocomial, recluidos en la Unidad de Alto Riesgo Neonatal del Instituto Autónomo Hospital Universitario de Los Andes, Mérida, Venezuela y su asociación con la formación de biopelículas. Las cepas fueron identificadas según los métodos microbiológicos estandarizados, los patrones de susceptibilidad se determinaron por concentración inhibitoria mínima, y la formación de biopelícula por técnicas espectrofotométricas. La mayoría de las cepas productoras de bLEE se asociaron con frecuencia a otros marcadores de resistencia, tales como: tobramicina, kanamicina, tetraciclina, cloranfenicol (38%, cada uno), estreptomicina (36%), y netilmicina (34%). El fenotipo multirresistente predominante fue el constituido por bLEE+-AMG-TET-CLF (30%). El 18% de las cepas de K. pneumoniae fue sensible a los antibióticos probados. La formación de biopelículas se observó en el 77,8% de las cepas sensibles y en el 96,2% de las resistentes. No hubo relación estadística entre las variaciones fenotípicas de susceptibilidad y la formación de biopelículas (p= >0,05). Sin embargo, ambas características en K. pneumoniae deben ser consideradas en el momento de decidir una conducta terapéutica para el tratamiento de pacientes con infección nosocomial.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Klebsiella pneumoniae is the principal opportunist pathogen associated with nosocomial infections. The morbidity and mortality produced by this microorganism is related mostly to antimicrobial resistance and its capacity to form biofilms. In this study, the phenotypic variations of susceptibility in 50 Klebsiella pneumoniae strains (26 bLEE-producer and 24 no bLEE-producer) isolated from pediatric patients with nosocomial septicemia at the High-risk Neonatal Unit of the Instituto Autónomo Hospital Universitario de Los Andes, Mérida, Venezuela and their associations with biofilm formation were analyzed. These strains were biochemically identified by standard microbiology methods. Antimicrobial susceptibility was determined by minimal inhibitory concentration and biofilm formation was detected by spectrophotometric techniques. Most of bLEE strains were frequently associated with resistance markers such as tobramycin, kanamycin, tetracycline, chloramphenicol (38% each), streptomycin (36%) and netilmicin (34%). The predominant multiresistent phenotype was composed of bLEE+-AMG-TET-CLF (30%). 18% of K. pneumoniae strains were sensible to the antibiotics tested. Formation of biofilm was observed in 77.8% and 96.2% of sensible and resistent strains, respectively. There was no statistic relation between phenotipic variations of susceptibility and biofilm formation (p= >0.05). Aceptado: 20-11-2008.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>INTRODUCCI&#211;N</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <I>K. pneumoniae</I> es uno de los principales pat&#243;genos nosocomiales que con  frecuencia produce infecciones graves, especialmente en la poblaci&#243;n pedi&#225;trica  recluida en las unidades de cuidado intensivo neonatal (1, 2). Varios brotes  o epidemias nosocomiales han estado asociados con cepas de <I>K. pneumoniae</I>  resistentes a cefalosporinas de tercera generaci&#243;n, debido a la producci&#243;n  de b-lactamasas de espectro expandido (</FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">bLEE). Los genes TEM y SHV que codifican  para las bLEE est&#225;n mediados por pl&#225;smidos conjugativos que, a menudo,  portan otros determinantes de resistencia, como el de la resistencia a  los aminogluc&#243;sidos y quinolonas (3).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <I>K. pneumoniae</I> incrementa su virulencia no solo por presentar diversos marcadores  de resistencia, sino por expresar en su superficie diversos factores de  adherencia (fimbrias, pili, exopolisac&#225;ridos, entre otros) que le permiten  adem&#225;s de colonizar los tejidos del hospedero, adherirse a superficies  abi&#243;ticas donde se organiza en comunidades bacterianas llamadas biopel&#237;culas  (4, 5).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La biopel&#237;cula es un dep&#243;sito no estructurado de c&#233;lulas y de glucoc&#225;liz  acumulado, compuesto por exopolisac&#225;ridos y una compleja comunidad de bacterias,  donde se observa un ensamblado de microorganismos dentro de una matriz  con la capacidad de adherirse a biomateriales o tejidos da&#241;ados del hospedero  (6).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La cronicidad o persistencia de algunos procesos infecciosos causados por  <I>K. pneumoniae</I> est&#225;n asociados a la formaci&#243;n de biopel&#237;culas en dispositivos  o implantes m&#233;dicos (sondas, cat&#233;teres, pr&#243;tesis, etc.) (4). Las infecciones  donde las biopel&#237;culas est&#225;n involucradas son dif&#237;ciles de tratar con antibi&#243;ticos,  debido a que estos no logran penetrar en su estructura (7). La naturaleza  ani&#243;nica del exopolisac&#225;rido retarda la difusi&#243;n de los antibi&#243;ticos cati&#243;nicos  como los aminogluc&#243;sidos. Este fen&#243;meno qu&#237;mico retarda la muerte celular  al conferirle mayor tiempo a la bacteria para que implemente una respuesta  adaptativa. En el caso, de microorganismos productores de b-lactamasas  como <I>K. pneumoniae</I>, aumentan la producci&#243;n enzim&#225;tica en respuesta a la  exposici&#243;n del antibi&#243;tico  </FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">b-lact&#225;mico; las b-lactamasas se acumulan en  el exopolisac&#225;rido como resultado de su secreci&#243;n o por lisis celular,  producci&#233;ndo de esta manera la inactivaci&#243;n del b-lact&#225;mico en las capas  superficiales de la biopel&#237;cula y evitando que el antibi&#243;tico se difunda  hacia el interior de la misma (6, 8, 9).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Por otra parte, en la biopel&#237;cula las bacterias cohabitan en microcolonias,  que favorecen la transferencia de pl&#225;smidos por conjugaci&#243;n y promueven  de esta manera la acumulaci&#243;n de determinantes de resistencia (7).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El objetivo de este estudio fue determinar las variaciones fenot&#237;picas  de susceptibilidad en cepas de <I>K. pneumoniae</I> de origen nosocomial y su  asociaci&#243;n con la formaci&#243;n de biopel&#237;culas en placas de policloruro de  vinilo.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>MATERIALES Y M&#201;TODOS&nbsp;</B></FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se aisl&#243; un total de 50 cepas de <I>K. pneumoniae </I>en hemocultivos de pacientes  pedi&#225;tricos con septicemia nosocomial recluidos en la Unidad de Alto Riesgo  Neonatal (UARN) del Instituto Aut&#243;nomo Hospital Universitario de Los Andes  (IAHULA), M&#233;rida, Venezuela, durante el per&#237;odo comprendido desde el a&#241;o  1997 hasta el 2004. En un estudio previo (3), a 26 de estas cepas se les  hab&#237;a detectado por m&#233;todos moleculares, la producci&#243;n de b-lactamasas  de espectro extendido (</FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">bLEE) tipo SHV-5.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El aislamiento y la identificaci&#243;n bioqu&#237;mica de <I>K. pneumoniae</I> se realiz&#243;  de acuerdo a la metodolog&#237;a cl&#225;sica descrita por Koneman y col. (10).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los fenotipos de susceptibilidad fueron determinados por concentraci&#243;n  inhibitoria m&#237;nima (CIM) siguiendo el m&#233;todo de diluci&#243;n en agar de acuerdo  a la t&#233;cnica estandarizada por Woods y Washington (11). Los resultados  de la CIM fueron interpretados de acuerdo a lo descrito por el Clinical  and Laboratory Standards Institute (12). Los antimicrobianos utilizados  en estos ensayos fueron: piperacilina-tazobactam (Wyeth-Lederle), meropenem  (AstraZeneca), tobramicina (Valmorca), netilmicina (Schering-Plough), kanamicina  (Bristol-Myers), estreptomicina (Sigma), gentamicina (Farmachem), amikacina  (Renochem-AG), tetraciclina (Sigma), cloranfenicol (Sigma) y ciprofloxacina  (Proula) Se consider&#243; como fenotipo multirresistente toda cepa con producci&#243;n  de bLEE asociada a otros marcadores de resistencia. Se utiliz&#243; <I>Escherichia  coli </I>ATCC 25922 como cepa control de estos ensayos.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Para la determinaci&#243;n de la formaci&#243;n de biopel&#237;culas en las cepas de <I>K.  pneumoniae,</I> fue necesario establecer previamente el tiempo de ocurrencia  de la fase estacionaria del crecimiento en dos cepas del grupo de estudio  seleccionadas al azar y las cepas control <I>Pseudomonas aeruginosa</I> ATCC 27853,  <I>K. pneumoniae</I> MZ2098 y <I>K. pneumoniae</I> ATCC 23357. Se realiz&#243; una curva de  crecimiento mediante el cultivo de las cepas en caldo Luria Bertani (LB)  en aerobiosis a 37<SUP> 0</SUP>C, y se verific&#243; cada hora la concentraci&#243;n de c&#233;lulas  por densidad &#243;ptica (DO). Los resultados determinaron que la fase estacionaria  ocurri&#243; durante el lapso de 10 a 18 horas. Teniendo en cuenta este resultado  y lo reportado en la literatura (13, 14), se establecieron 18 horas de  incubaci&#243;n como el tiempo suficiente para que las cepas bacterianas en  fase estacionaria formaran biopel&#237;culas. Posteriormente, se estandariz&#243;  el in&#243;culo bacteriano para los ensayos de determinaci&#243;n de la biopel&#237;cula,  a partir de cultivos frescos de 18 horas en caldo BHI suplementado con  MOPS 2 mM (3-[N-morfolino] &#225;cido propanosulf&#243;nico (Sigma), se tomaron 100  &#181;L de cultivo de cada cepa y se colocaron en los pozos de una placa de  policloruro de vinilo (PVC) (BD, Falcon), se ley&#243; en un espectrofot&#243;metro  (Genios Tecan) a una longitud de onda de 595 nm. Las densidades &#243;pticas  alcanzadas en las 50 cepas de <I>K. pneumoniae</I> oscilaron entre 0,19 y 0,52.  Con base en estos resultados, todas las cepas fueron ajustadas a un in&#243;culo  correspondiente a una DO de 0,19 en caldo BHI-MOPS.</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La formaci&#243;n de biopel&#237;cula se realiz&#243; de acuerdo al procedimiento descrito  por O&#146;Toole y col. (14) modificado por Suesc&#250;n y col. (15). Se colocaron  150 &#181;L del in&#243;culo estandarizado de cada cepa en un pozo de una placa de  PVC, a 37&#176;C por 18 horas. Se verific&#243; el crecimiento bacteriano en un espectrofot&#243;metro  a una longitud de onda de 595 nm.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Para visualizar la formaci&#243;n de la biopel&#237;cula se removieron las c&#233;lulas  planct&#243;nicas (bacterias libres), y se descart&#243; el medio de cultivo por  inmersi&#243;n de la placa de PVC en un bandeja con agua destilada y se dej&#243;  secar a temperatura ambiente por 20 minutos. Posteriormente, las c&#233;lulas  s&#233;siles (adheridas a las paredes del pozo) fueron te&#241;idas con 200 &#181;L del  colorante cristal violeta al 1%, por 20 minutos a temperatura ambiente.  El exceso de colorante fue eliminado lavando los pozos 3 veces con agua  destilada. La formaci&#243;n firme de un anillo violeta adherido a la superficie  interna del pozo acompa&#241;ada de una DO&gt;0,23 se consider&#243; como una prueba  positiva para la producci&#243;n de biopel&#237;cula.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Para el procesamiento estad&#237;stico de los datos se utilizaron el Statistical  Product and Service Solutions (SPSS) versi&#243;n 11.0, el Statistical Analysis  System (SAS) versi&#243;n 8.02 y el Origin 5.0. Los resultados fueron analizados  mediante la aplicaci&#243;n de la prueba Chi cuadrado y el test exacto de Fisher.  Una p<I> </I>&lt; 0,05 se consider&#243; con significancia estad&#237;stica.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>RESULTADOS&nbsp;</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En la <a href="#tab1">Tabla I</a>, se muestra que las cepas de <I>K. pneumoniae</I> bLEE+ se asociaron  con mayor frecuencia a la presencia de otros marcadores de resistencia,  con porcentajes superiores al 30% para la resistencia a tobramicina, kanamicina,  tetraciclina, cloranfenicol, estreptomicina y netilmicina, 20% para gentamicina  y 18% para piperacilina-tazobactam y amikacina. Aunque en el grupo de cepas </FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">bLEE- el porcentaje de resistencia a los antibi&#243;ticos no super&#243; el 22%,  los marcadores de resistencia m&#225;s frecuentes fueron: en primer lugar, estreptomicina  (22%), seguida por tobramicina y netilmicina (14%, respectivamente). Todas  las cepas fueron sensibles al meropenem y ciprofloxacina, independientemente  de la producci&#243;n o no de bLEE. De este grupo de cepas, el 75,6% (34/45) produjo  biopel&#237;culas a una DO&gt;0,8. S&#243;lo 5 cepas (0,1%) registraron DO &#163; 0,23, lo  cual se correspondi&#243; con la no formaci&#243;n de biopel&#237;cula (<a href="#fig1">Fig. 1</a>). Los resultados  de la asociaci&#243;n de los patrones de susceptibilidad a los antibi&#243;ticos  con la formaci&#243;n de biopel&#237;culas se muestran en la <a href="#tab2">Tabla II</a>. El 18% de  las cepas de <I>K. pneumoniae</I> no presentaron marcadores de resistencia. De  este grupo, el 77,8% form&#243; biopel&#237;cula, mientras que m&#225;s del 90% de las  cepas resistentes produjo biopel&#237;culas. El fenotipo multirresistente m&#225;s  frecuente (30%) fue el constituido por </FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">bLEE<SUP>+</SUP>-aminogluc&#243;sidos-tetraciclina-  cloranfenicol (bLEE<SUP>+</SUP>-AMG-TET-CLF). Casi todas las cepas que conformaron  este fenotipo formaron biopel&#237;culas (93,3%). Pip/Taz: piperacilina-tazobactam.</FONT></P>     <P ALIGN="LEFT"><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">* Corresponde a cepas con DO  ± 0,23 (ver <a href="#fig1">Fig. 1</a>).</FONT></P> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>DISCUSI&#211;N</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <I>K. pneumoniae </I>ha sido reconocida como una de las especies en las que se  han detectado con mayor frecuencia diversos mecanismos de resistencia,  especialmente la producci&#243;n de b-lactamasas y de enzimas modificadoras  de aminogluc&#243;sidos (1-3).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En este estudio, se demostr&#243; que m&#225;s del 30% de las cepas de<I> K. pneumoniae</I>  bLEE+ se asoci&#243; con frecuencia a otros marcadores de resistencia tales  como: tobramicina (38%), kanamicina (38%), tetraciclina (38%), cloranfenicol  (38%), estreptomicina (36%), netilmicina (34%) y piperacilina-tazobactam  (18%). Estos resultados avalan las afirmaciones de Kizirgil y col. (16),  quienes sustentan que cepas de <I>K. pneumoniae</I> productoras de  </FONT><FONT COLOR="#1f1a17" FACE="Verdana" SIZE="2">bLEE aisladas  en Unidades de Cuidado Intensivo, son frecuentemente m&#225;s resistentes a  los aminogluc&#243;sidos que aquellas que no producen esta enzima. De hecho,  los patrones bLEE<SUP>+</SUP>- AMG-TET-CLF y bLEE<SUP>+</SUP>-AMG-TET-CLF- Pip/ Taz fueron los  fenotipos multirresistentes predominantes (30 y 18% respectivamente).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Las cepas analizadas en este estudio fueron aisladas de pacientes pedi&#225;tricos  con diagn&#243;stico de septicemia nosocomial. Por su condici&#243;n cl&#237;nica grave,  estos pacientes fueron sometidos a procedimientos invasivos que facilitaron  muy probablemente la colonizaci&#243;n de los mismos por <I>K. pneumoniae.</I> Numerosos  estudios se&#241;alan que los dispositivos m&#233;dicos invasivos (cat&#233;teres, sondas,  pr&#243;tesis, etc.), as&#237; como el tejido desvitalizado o lesionado por cirug&#237;as  o traumatismos, pueden ser colonizados por <I>K. pneumoniae</I> (4, 5, 17, 18).  Bajo estas circunstancias <I>K. pneumoniae</I> puede desarrollar la formaci&#243;n  de biopel&#237;culas, que favorecen la instauraci&#243;n de procesos cr&#243;nicos o persistentes  refractarios a la terapia antimicrobiana (4, 7, 9). En este estudio, se  demostr&#243; que el 90% de las cepas analizadas form&#243; biopel&#237;culas; esta propiedad  fue independiente de cualquiera de las variaciones fenot&#237;picas de susceptibilidad  presentes en las cepas estudiadas. Es probable que la formaci&#243;n de biopel&#237;cula  en <I>K. pneumoniae</I> sea una condici&#243;n inherente del microorganismo y que la  misma, est&#233; regulada por factores propios del microambiente bacteriano  (pH, temperatura, ox&#237;geno, nutrientes, etc), as&#237; como por la secreci&#243;n  o estimulaci&#243;n de sustancias que promueven la comunicaci&#243;n bacteriana o  la percepci&#243;n de qu&#243;rum (del ingl&#233;s <I>quorum sensing)</I> para la expresi&#243;n particular  de esta caracter&#237;stica fenot&#237;pica (19, 20). Los resultados de este estudio  han puesto en evidencia que en la UARN del IAHULA reside una poblaci&#243;n  bacteriana con adaptaciones importantes al ambiente y circunstancias donde  convive. La resistencia a los agentes antimicrobianos y en especial la  formaci&#243;n de biopel&#237;culas fueron caracter&#237;sticas que le otorgaron a las  cepas estudiadas de <I>K. pneumoniae, </I>ventajas para su persistencia y diseminaci&#243;n  en el ambiente intrahospitalario.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Donlan y Costerton (6) se&#241;alaron que las bacterias que forman biopel&#237;culas  disminuyen considerablemente su susceptibilidad a los agentes antimicrobianos.  Esto es debido, por una parte, a que el transporte o difusi&#243;n de los antibi&#243;ticos  a trav&#233;s de la matriz de una biopel&#237;cula es lento o limitado y requiere  concentraciones de antibi&#243;ticos cien o mil veces m&#225;s elevadas que las bacterias  en fase planct&#243;nica (c&#233;lulas libres) y, por la otra, la acci&#243;n final de  los antibi&#243;ticos est&#225; impedida al encontrarse bacterias que en condici&#243;n  de biopel&#237;cula que reducen su tasa de crecimiento y disminuyen su  actividad metabólica (22).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Otro de los fen&#243;menos que la formaci&#243;n de biopel&#237;culas favorece en la poblaci&#243;n  bacteriana, es el del intercambio de informaci&#243;n gen&#233;tica (4, 22). Ehlers  y Bouwer (23), demostraron la transferencia conjugativa de un pl&#225;smido  entre diferentes bacterias gramnegativas que formaban una biopel&#237;cula.  La tasa de transferencia horizontal del pl&#225;smido fue superior en varios  &#243;rdenes de magnitud en la biopel&#237;cula a la de los cultivos l&#237;quidos de  estas mismas bacterias. En un estudio gen&#233;tico previo, a 26 de las cepas  estudiadas en este trabajo se les detect&#243; un pl&#225;smido de 87 kb que mediaba  adem&#225;s, de la producci&#243;n bLEE tipo SHV-5, otros determinantes de resistencia  (3). Es posible que estas cepas bajo la condici&#243;n de biopel&#237;cula <I>in vivo</I>  hayan facilitado la transferencia de marcadores de resistencia entre ellas,  permitiendo de esta manera demostrar la presencia de una gran variedad  de patrones de susceptibilidad que se extendieron desde bacterias totalmente  sensibles a cepas con fenotipos multirresistentes complejos.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los resultados de este estudio permiten afirmar que la resistencia antimicrobiana  y la capacidad para formar biopel&#237;culas, fueron caracter&#237;sticas independientes  en las cepas estudiadas, pero que constituyeron las bases para el establecimiento  de <I>K. pneumoniae</I> como un pat&#243;geno nosocomial.
