Su espectro evolutivo es variable, con pronóstico habitualmente muy grave en los primeros cinco años de vida. El objetivo de este trabajo es describir el espectro del fenotipo clínico y evolución de este trastorno en cuatro pacientes venezolanos. Todas las pacientes evaluadas cumplieron los criterios diagnósticos mayores, mostraron retraso psicomotor grave y convulsiones de inicio muy temprano. En dos pacientes se observó microftalmía. En tres (75%) de ellas se observó coloboma del nervio óptico, quistes interhemisféricos y heterotopías periventriculares. La primera paciente, con mayor tiempo de evolución, tiene actualmente 22 años. Todos los casos exhibieron un patrón asimétrico de hipsarritmia alternante en el electroencefalograma. Estos casos ilustran la expresión clínica y gravedad variables del síndrome de Aicardi. Su diagnóstico debe considerarse en niñas con retardo del desarrollo psicomotor y crisis convulsivas recurrentes iniciadas en la infancia temprana.]]></p></abstract>

The evolution of this disorder is variable, with a severe outcome over the first five years of age. The purpose of this report was to demonstrate the spectrum of the clinical phenotype and the course of this disorder in four Venezuelan patients. All patients met the major criteria, had severe psychomotor impairment and early onset seizures. There were microphtalmia in two of the patients. Three patients (75%) showed coloboma, interhemispheric cyst and periventricular heterotopias. The first patient, with longer follow-up, is currently aged 22. They all exhibited a typical asymmetric pattern on the electroencephalogram. These cases illustrate the variable clinical expression and severity of the Aicardi syndrome. A diagnosis of this disorder should be considered in girls with developmental delay, particularly, when there are accompanying recurrent seizures occurring in early childhood.]]></p></abstract>

Aceptado: 26-03-2010.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>INTRODUCCI&#211;N&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En 1965, Aicardi, Lefebvre y Lerique-Koechlin describieron un s&#237;ndrome  caracterizado por agenesia del cuerpo calloso, espasmos infantiles y anomal&#237;as  retinianas (1). Despu&#233;s de su publicaci&#243;n original, Aicardi y colaboradores  publicaron otro trabajo en 1969, en el que incluyeron otros hallazgos habitualmente  asociados con la presentaci&#243;n cl&#237;nica, tales como las anomal&#237;as esquel&#233;ticas  y el retraso psicomotor (2). El s&#237;ndrome de Aicardi (SA) representa del  1 al 4% de los casos de s&#237;ndrome de West, probablemente de transmisi&#243;n  dominante ligada al X, casi exclusivo en mujeres. Se presume que los fetos  varones afectados con este trastorno no lleguen a ser viables (3). No obstante,  se han publicado dos casos en varones con esta entidad, ambos asociados  a cariotipo 47 XXY, con edades de 10 y 12 a&#241;os (4, 5).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El SA es una causa frecuente de encefalopat&#237;a epil&#233;ptica que suele mostrar  un pron&#243;stico poco favorable con respecto al control de las crisis convulsivas,  el desarrollo psicomotor y la presencia de complicaciones. Si bien el espectro  evolutivo es variable, su pron&#243;stico es habitualmente muy grave. Menos  del 25% de las pacientes logran la marcha espont&#225;nea o asistida, y unas  pocas desarrollan alguna forma de lenguaje (6). En dos estudios sobre la  historia natural de este s&#237;ndrome se obtuvieron tasas variables de supervivencia  que a los 27 a&#241;os era cercana al 62% (7, 8). La ocurrencia de espasmos  o crisis parciales graves algunas veces intratables, se ha relacionado  con el pron&#243;stico, aunque en casos excepcionales, las mismas pueden ser  controladas con antiepil&#233;pticos (9). As&#237; mismo, se ha detectado un deterioro  cl&#237;nico en los primeros 5 a&#241;os de vida, pero se desconoce si &#233;ste se relaciona  con las complicaciones m&#233;dicas o representa una consecuencia de las crisis  convulsivas refractarias. Sin embargo, en los &#250;ltimos a&#241;os, se han descrito  casos con afectaci&#243;n menos grave (10).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Adem&#225;s de las manifestaciones cl&#237;nico-radiol&#243;gicas inicialmente descritas,  se ha establecido que otras anomal&#237;as tambi&#233;n pueden contribuir a la variaci&#243;n  en la expresi&#243;n cl&#237;nica del trastorno, tales como las heterotop&#237;as periventriculares  y subcorticales, los quistes del plexo coroideo, colobomas, asimetr&#237;as  hemisf&#233;ricas, displasias corticales, tumores intra o extracraneales, as&#237;  como otras anomal&#237;as craneofaciales (11).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El objetivo de este trabajo fue describir cuatro casos de pacientes venezolanos  quienes actualmente cuentan con edades de 22 a&#241;os, 2, 1 y 16 meses, que  presentan este s&#237;ndrome tan poco frecuente, y mostrar tanto el espectro  cl&#237;nico-fenot&#237;pico, como su evoluci&#243;n.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>PRESENTACI&#211;N DE LOS CASOS&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Caso 1</B>&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Paciente de sexo femenino quien desde el 2<SUP>do</SUP> mes de vida, present&#243; episodios  bruscos de espasmos con flexi&#243;n generalizados en extremidades. Producto  de madre de 20 a&#241;os y padre de 25. Novena gestaci&#243;n, embarazo simple y  no controlado; la madre ingiri&#243; misoprostol en el primer trimestre, terminando  el embarazo a t&#233;rmino en un parto vaginal dist&#243;cico con asfixia perinatal  grave. La exploraci&#243;n f&#237;sica a los 3 meses fue: circunferencia cef&#225;lica  40 cm, frente amplia y occipucio aplanado, microftalm&#237;a izquierda; la fondoscopia  revel&#243; lesi&#243;n coriorretiniana peripapilar bilateral (<a href="#fig1">Fig. 1</a>). Hipoton&#237;a  generalizada, pobre control cef&#225;lico a la tracci&#243;n, apoyo plantar y marcha  refleja lenta para la edad, hiperreflexia osteotendinosa, Babinski bilateral.  Exploraciones complementarias mostraron cariotipo 46 XX, hemograma, bioqu&#237;mica,  amino&#225;cidos en sangre y orina, normales. Serolog&#237;a TORCH negativa. La resonancia  magn&#233;tica cerebral (RM) practicada en el mes siguiente revel&#243; agenesia  total del cuerpo calloso, hemiatrofia cerebral izquierda, displasia cortical,  heterotop&#237;as periventriculares, y coloboma del nervio &#243;ptico (<a href="#fig2">Fig. 2</a>).  En el electroencefalograma (EEG) se observ&#243; hipsarritmia alternante. La  paciente actualmente a los 22 a&#241;os de edad presenta retraso mental grave,  lenguaje que consiste en bis&#237;labos y palabras aisladas, crisis epil&#233;pticas  parcialmente controladas, marcha espont&#225;nea hemipar&#233;tica y manipulaci&#243;n  de objetos conservada.</FONT></P>     <P ALIGN="center"><a name="fig1"> <img border="0" src="/img/fbpe/ic/v51n3/art11fig1.gif" width="373" height="407"></a></P>

<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Caso 2</B>&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Lactante de sexo femenino quien a los 2 meses de edad fue evaluada por  presentar crisis miocl&#243;nicas. Producto de madre de 26 y padre de 30 a&#241;os  de edad, no consangu&#237;neos, de primer embarazo simple a t&#233;rmino, controlado,  y complicado con infecci&#243;n urinaria y vulvovaginitis tratadas. Naci&#243; por  ces&#225;rea y se hospitaliz&#243; por neumon&#237;a intrauterina en los primeros d&#237;as  de vida. A la exploraci&#243;n se observ&#243; microcefalia, postura extensora de  las extremidades, hipoton&#237;a axial e hiperton&#237;a generalizada, reflejos osteotendinosos  aumentados, Babinski bilateral, ausencia de apoyo plantar y marcha refleja.  La fondoscopia revel&#243; lesiones coriorretinianas bilaterales. Exploraciones  complementarias: cariotipo 46XX, hemograma y bioqu&#237;mica normal. Serolog&#237;a  para TORCH negativa. La resonancia magn&#233;tica (RM) cerebral mostr&#243; agenesia  del cuerpo calloso, heterotop&#237;a periventricular y quiste intracraneal interhemisf&#233;rico.  El registro electroencefalogr&#225;fico (EEG) demostr&#243; hipsarritmia alternante  (<a href="#fig3">Fig. 3</a>). Serie &#243;sea y Rx de t&#243;rax normales. A los 14 meses de edad, la  paciente presenta retraso psicomotor grave.</FONT></P>     <P ALIGN="center"><a name="fig3"> <img border="0" src="/img/fbpe/ic/v51n3/art11fig3.gif" width="579" height="491"></a></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     
<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Caso 3</B>&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Lactante de sexo femenino de 1 mes de edad con episodios de espasmos en  flexi&#243;n generalizados en extremidades. Producto de madre de 30 y padre  de 42 a&#241;os de edad, no consangu&#237;neos, de primera gestaci&#243;n de 42 semanas,  con polihidramnios e infecci&#243;n urinaria, obtenida por ces&#225;rea. La exploraci&#243;n  cl&#237;nica mostr&#243; normocefalia, asimetr&#237;a facial durante el llanto por agenesia  unilateral del triangular del labio, hipoton&#237;a axial y generalizada, Babinski  bilateral. La exploraci&#243;n del fondo de ojo revel&#243; lesi&#243;n coriorretiniana  bilateral. Entre las exploraciones paracl&#237;nicas destacan: cariotipo 46  XX, hemograma y bioqu&#237;mica normal. Serolog&#237;a para TORCH negativa. En el  examen de resonancia magn&#233;tica (RM) cerebral demostr&#243; agenesia del cuerpo  calloso, coloboma del nervio &#243;ptico, malformaciones corticales, quiste  aracnoideo y papiloma de plexo coroideo (<a href="#fig4">Fig. 4</a>). En el EEG demostr&#243; hipsarritmia  alternante. Serie &#243;sea y Rx de t&#243;rax normales. La paciente exhibe actualmente  importante afectaci&#243;n del desarrollo psicomotor.</FONT></P>     <P ALIGN="center"><a name="fig4"> <img border="0" src="/img/fbpe/ic/v51n3/art11fig4.gif" width="578" height="290"></a></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     
<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Caso 4</B>&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Neonato de sexo femenino quien a los 15 d&#237;as de edad present&#243; episodios  de espasmos en flexi&#243;n en miembros superiores. Producto de padres no consangu&#237;neos,  madre de 28 y padre de 30 a&#241;os de edad, de segunda gestaci&#243;n de evoluci&#243;n  normal, parto normal. Ultrasonido prenatal a las 32 semanas registr&#243; la  existencia de hidrocefalia. Este diagn&#243;stico fue descartado al revisar  el estudio. La exploraci&#243;n f&#237;sica y neurol&#243;gica revel&#243; normocefalia, microftalm&#237;a  izquierda, hipoton&#237;a generalizada y lesiones retinianas bilaterales pigmentadas  a la fondoscopia. El hemograma y la bioqu&#237;mica resultaron normales. La  serolog&#237;a para TORCH fue negativa. En la resonancia magn&#233;tica cerebral  (RM) se detect&#243; agenesia del cuerpo calloso, quiste del plexo coroideo,  coloboma del nervio &#243;ptico y un quiste interhemisf&#233;rico. El trazado electroencefalogr&#225;fico  (EEG) demostr&#243; hipsarritmia alternante. La serie &#243;sea y el Rx de t&#243;rax  fueron normales. A los 16 meses de edad la paciente presenta retraso psicomotor  grave sin habilidades de comunicaci&#243;n.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>DISCUSI&#211;N&nbsp;</B> </FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En una publicaci&#243;n reciente Aicardi propus&#243; una lista de criterios para  el diagn&#243;stico del SA (<a href="#TABLA_I">Tabla I</a>). La microftalm&#237;a o la aparici&#243;n de las crisis convulsivas  facilitan la sospecha diagn&#243;stica temprana. Aunque las anormalidades cerebrales  pudieran ser objetivadas mediante el ultrasonido prenatal, no es lo que  ocurre en la mayor&#237;a de los pacientes. En el cuarto caso descrito, la paciente  fue err&#243;neamente diagnosticada en el estudio ultrasonogr&#225;fico craneal como  portadora de una hidrocefalia y referida al servicio de neuropediatr&#237;a  a los 14 d&#237;as de vida por presentar microftalm&#237;a y crisis convulsivas.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los espasmos infantiles suelen presentarse a edad temprana y se pueden  asociar diversos tipos de crisis: cl&#243;nicas, parciales complejas, miocl&#243;nicas,  t&#243;nico-cl&#243;nicas generalizadas, crisis t&#243;nicas o combinaciones de ellas  (12). El electroencefalograma (EEG) muestra habitualmente una desorganizaci&#243;n  de la actividad basal e hipsarritmia alternante, con un patr&#243;n &#147;paroxismos-supresi&#243;n&#148;  independiente entre ambos hemisferios (<I>split brain</I>) (13). Las anomal&#237;as  oculares son de importancia diagn&#243;stica considerable. La coriorretinitis  lacunar, antes considerada patognom&#243;nica del SA y hoy d&#237;a tambi&#233;n descrita  en otras condiciones (14), es de variable extensi&#243;n y generalmente bilateral,  como se observ&#243; en las cuatro pacientes que se describen en este estudio  (<a href="#fig1">Fig. 1</a>). El coloboma del nervio &#243;ptico, es otra alteraci&#243;n bastante frecuente,  com&#250;nmente unilateral y puede acompa&#241;ar la coriorretinitis lacunar. En  las im&#225;genes de resonancia magn&#233;tica cerebral se muestra como una dilataci&#243;n  qu&#237;stica retrobulbar del nervio (3). Algunos autores mencionan la asociaci&#243;n  con diversas malformaciones faciales, como el labio y paladar hendido (12,  15); en nuestra serie s&#243;lo la paciente 3 present&#243; hipoplasia del triangular  del labio izquierdo. A pesar de que las anomal&#237;as costo-vertebrales son  consideradas uno de los criterios menores mas comunes, ninguna de las pacientes  descritas mostr&#243; tales hallazgos esquel&#233;ticos.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Como se ha indicado, la neuroimagen ha sido esencial en la descripci&#243;n  de este s&#237;ndrome y, con el desarrollo de t&#233;cnicas modernas, se ha demostrado  que la agenesia parcial o total del cuerpo calloso es s&#243;lo parte de un  complejo de malformaciones del sistema nervioso central. Entre estas anomal&#237;as  se incluyen la displasia cortical (microgiria), heterotop&#237;as, formaciones  qu&#237;sticas, anomal&#237;as vermianas, papilomas del plexo coroideo y, otras menos  frecuentes como los tumores embrionarios, la holoprosencefalia y la esquisencefalia  (16, 17). Las heterotop&#237;as periventriculares son verdaderamente comunes,  probablemente constantes. Estas pueden escapar del reconocimiento si la  calidad de la neuroimagen no es &#243;ptima; algunas son grandes y numerosas  como las detectadas en la paciente 1 (<a href="#fig2">Fig. 2</a>), y otras peque&#241;as que son  relativamente m&#225;s frecuentes (6). Algunos estudios se&#241;alan el rol de las  displasias corticales en la determinaci&#243;n de los signos neurol&#243;gicos, control  de las convulsiones y retraso mental (18). Es dif&#237;cil precisar si el deterioro  de los pacientes con SA pueda estar asociado con las anomal&#237;as que presentan  o son la consecuencia de crisis convulsivas no bien controladas. El engrosamiento  asim&#233;trico de un hemisferio es com&#250;n y contribuye a la peculiar hemiplej&#237;a  esp&#225;stica observada en estos pacientes (17). Resulta de inter&#233;s destacar  dos hechos que han sido comentados por varios autores respecto al diagn&#243;stico  y, secundariamente, al pron&#243;stico de este trastorno. Se ha se&#241;alado que  los pacientes que presentan muy pronto los espasmos infantiles, crisis  epil&#233;pticas de dif&#237;cil control y el viraje a otro patr&#243;n convulsivo, exhiben  un peor pron&#243;stico (7, 19). Lo mismo ocurre para la observaci&#243;n de lesiones  oculares graves, tales como lagunas retinianas de gran tama&#241;o que comprometen  la m&#225;cula y la retina central, o la agenesia completa del cuerpo calloso.  El primer caso descrito en el presente trabajo y con mayor tiempo de seguimiento,  mostr&#243; algunos elementos de mal pron&#243;stico, tales como el inicio precoz  de los espasmos a los 2 meses, el control tard&#237;o de las crisis, y la agenesia  total del cuerpo calloso. No obstante, su evoluci&#243;n confirma la heterogeneidad  de la gravedad cl&#237;nica, con una incapacidad menor y un pron&#243;stico vital  menos grave.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En el presente estudio, el examen gen&#233;tico y especialmente el cariotipo  realizado en 3 de los pacientes, resultaron normales. En ninguno de los  pacientes se realiz&#243; estudios de alta resoluci&#243;n que pudieran demostrar  alteraciones estructurales cromos&#243;micas que permitieron el diagn&#243;stico  diferencial con otros trastornos.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La b&#250;squeda de genes candidatos para este trastorno ha sido infructuosa,  a pesar de los esfuerzos dedicados con diversas t&#233;cnicas, incluyendo la  hibridaci&#243;n gen&#243;mica comparativa. Un estudio reciente que examin&#243; el cromosoma  X en 18 pacientes con SA no encontr&#243; deleciones o duplicaciones asociadas  con el trastorno (20). Sobre la base de estas limitaciones, el diagn&#243;stico  de SA sigue dependiendo en gran medida de los criterios cl&#237;nicos. Para  facilitar una mejor caracterizaci&#243;n, la presencia de convulsiones de inicio  temprano en una paciente con retraso del desarrollo y compromiso de su  funcionamiento global, debe incluirse el SA en el diagn&#243;stico diferencial.  El SA es un trastorno heterog&#233;neo en su presentaci&#243;n cl&#237;nica y evoluci&#243;n.  Diferentes fenotipos podr&#237;an estar asociados con la variabilidad en el  pron&#243;stico y evoluci&#243;n de este s&#237;ndrome.
