Las complicaciones vasculares son principalmente secundarias a calcificaciones y ateroesclerosis. En los últimos años se ha renovado el interés por la asociación entre niveles de ácido úrico y riesgo cardiovascular. El objetivo de esta investigación fue relacionar la presencia de calcificaciones vasculares (CV) y aterosclerosis, evaluadas mediante ecografía carotídea, con niveles de ácido úrico en pacientes con ERC estadio 5 en diálisis. Se observaron CV en 56% de los pacientes; 46% tuvo criterios ecográficos para aterosclerosis con un promedio general de 0,89 mm (DE: ± 0,28), siendo mayor en los pacientes con hipertensión arterial y diabetes mellitus; este grupo también mostró mayor predisposición para CV (p= 0,01). Los niveles de urea (141,3 mg/dL) (p= 0,01) y ácido úrico (6,9 mg/dL) (p= 0,04) mostraron asociación estadísticamente significativa con la presencia de CV. Los eventos cardiovasculares adversos predominaron en los pacientes con aterosclerosis y CV (p= 0,01). Esta investigación demostró que un incremento en los niveles de ácido úrico por encima de 6 mg/L está relacionado con mayor riesgo de presentar calcificaciones y eventos cardiovasculares adversos en pacientes con ERC.]]></p></abstract>

<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Recibido: 26-04-2010 Aceptado: 22-09-2011</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>INTRODUCCI&#211;N&nbsp;</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La incidencia de enfermedad renal cr&#243;nica (ERC) y, paralelamente, el n&#250;mero  de pacientes en tratamiento sustitutivo renal est&#225; aumentando. El &#225;cido  &#250;rico (AU) producto final del catabolismo de las purinas, est&#225; asociado  con la enfermedad renal y riesgo cardiovascular; se encuentra elevado en  personas con hipertensi&#243;n arterial y s&#237;ndrome metab&#243;lico; tambi&#233;n est&#225;  relacionado con otras alteraciones metab&#243;licas tales como dislipidemia,  resistencia a la insulina y otras (1).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se han sugerido diversos mecanismos por los que el AU podr&#237;a jugar un papel  patog&#233;nico directo sobre la enfermedad cardiovascular; sin embargo, es  todav&#237;a incierto si la hiperuricemia es directamente patog&#233;nica o es simplemente  un marcador de otros factores de riesgo (2).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En animales de experimentaci&#243;n se ha observado que el AU, estimula la adhesi&#243;n  plaquetaria y puede ser uno de los mecanismos claves para la activaci&#243;n  del sistema renina-angiotensina y la ciclooxigenasa-2. Estos mecanismos  act&#250;an en la progresi&#243;n de la enfermedad renal y podr&#237;an estar mediados  por un efecto regulador del incremento en receptores de la angiotensina-1  en c&#233;lulas endoteliales y producir lesi&#243;n vascular (3, 4).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Por su parte, las calcificaciones vasculares (CV) se han observado en pacientes  con ERC desde hace d&#233;cadas. Existen dos formas diferentes, pero relacionadas,  de CV; estas son en la media e &#237;ntima de los vasos y se producen por mecanismos  separados (5). Las calcificaciones de la media, est&#225;n relacionadas con  los niveles de calcio y f&#243;sforo, da&#241;o en la distensibilidad de los vasos  y la formaci&#243;n de ves&#237;culas con matriz con microcristales. Las calcificaciones  de la &#237;ntima se muestran m&#225;s relacionadas con procesos ateroscler&#243;ticos  avanzados, como se observa en situaciones inflamatorias cr&#243;nicas; as&#237; mismo,  est&#225;n ligadas con dep&#243;sitos de colesterol.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La enfermedad inflamatoria puede contribuir a la formaci&#243;n de CV y aterosclerosis  en la ERC. Se ha estudiado un amplio n&#250;mero de citoquinas proinflamatorias  y antiinflamatorias como posibles factores contributorios (osteopontina,  osteoprotegerina, fetuina, y otras). Algunos elementos del metabolismo  mineral, particularmente calcio, f&#243;sforo y hormona paratiroidea, est&#225;n  alterados en la enfermedad renal; no es de extra&#241;ar que estos mismos elementos  est&#233;n implicados en la g&#233;nesis de las CV y aterosclerosis en enfermos renales  cr&#243;nicos (6, 7).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Finalmente, y en vista de la carencia de estudios que relacionen los niveles  de acido &#250;rico, aterosclerosis y calcificaciones vasculares en pacientes  con ERC, se plantea la realizaci&#243;n de esta investigaci&#243;n a fin de demostrar  si estos elementos se encuentran asociados a individuos sometidos a tratamiento  sustitutivo de la funci&#243;n renal.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>PACIENTES Y M&#201;TODOS&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Es un estudio observacional, prospectivo y de corte transversal. Los pacientes  cumpl&#237;an con los criterios de inclusi&#243;n; esto es, ser portadores de ERC  estadio 5 en tratamiento sustitutivo de la funci&#243;n renal (hemodi&#225;lisis  o di&#225;lisis peritoneal). Se descartaron pacientes con cambio reciente de  modalidad dial&#237;tica, con injuria renal aguda y otros con ERC agudizada  que requirieron apoyo dial&#237;tico.&nbsp; </FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se evaluaron 82 pacientes que acudieron a recibir tratamiento sustitutivo  renal al servicio de Nefrolog&#237;a del Hospital Universitario Ru&#237;z y P&#225;ez  de Ciudad Bol&#237;var, Venezuela; de ellos, 50 cumpl&#237;an con los criterios de  inclusi&#243;n y manifestaron su consentimiento y aprobaci&#243;n para participar  en el estudio. La informaci&#243;n y datos tales como, identificaci&#243;n, edad,  g&#233;nero, causa de enfermedad renal, antecedentes cardiovasculares previos,  tiempo y tipo de di&#225;lisis, ex&#225;menes de laboratorio y resultados ecogr&#225;ficos,  se recogieron en una hoja de protocolo dise&#241;ada para tal fin.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> A cada paciente, previo a un descanso de 15 minutos, se le tom&#243; una muestra  sangu&#237;nea para hematolog&#237;a y qu&#237;mica; las cuales fueron almacenadas en  fr&#237;o y, posteriormente, centrifugadas durante 15 minutos. Todas las muestras  fueron recolectadas a cada paciente en el d&#237;a interdial&#237;tico y procesadas  con un analizador autom&#225;tico de hematolog&#237;a y qu&#237;mica sangu&#237;nea marca Express  Plus.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Todos los pacientes fueron sometidos a un Ecosonograma carot&#237;deo para evaluar  CV y/o ateroesclerosis. Para ello se utiliz&#243; un equipo de ecograf&#237;a digital  marca Mindray<SUP>&#174;</SUP>, modelo M5, con un transductor lineal de 7,5 MHz, utilizando  el modo B y se procesaron los datos a trav&#233;s del software THI<SUP>&#174;</SUP> 2,0 incluido  en el equipo. Los estudios fueron realizados por un experto ecosonografista  en ambas arterias car&#243;tidas. Luego de un per&#237;odo de 15 minutos de descanso,  se coloc&#243; la cabeza del paciente con una inclinaci&#243;n hacia atr&#225;s, girando  30 grados a ambos lados para cada procedimiento. Se visualiz&#243; todo el trayecto  de los vasos a ambos lados, en orientaci&#243;n transversal y longitudinal;  se tomaron medidas del espesor &#237;ntima-media a 0,5, 1 y 2 cm por debajo  del bulbo carot&#237;deo en el plano longitudinal con el transductor perpendicular  al vaso. Las im&#225;genes obtenidas se almacenaron en la memoria interna del  equipo y se tom&#243; un m&#237;nimo de 3 medidas por cada arteria. Se denomin&#243; CV  a las im&#225;genes hiperecog&#233;nicas con sombra ac&#250;stica posterior en la pared  del vaso y como criterio para aterosclerosis, un espesor &#237;ntima-media </FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Symbol"> &#179;</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana">  0,9 mm.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El an&#225;lisis estad&#237;stico de este estudio se realiz&#243; con el programa SPSS<SUP>&#174;</SUP>  17.0. Se estimaron frecuencias absolutas, porcentajes, promedios y desviaciones  est&#225;ndar. Cuando interesaba cruzar una variable categ&#243;rica con otra cuantitativa,  se construyeron gr&#225;ficos de cajas y tablas de contingencia que permitieron  calcular el riesgo relativo asociado a cada exposici&#243;n contenida en la  variable categ&#243;rica. En caso de hallar alguna relaci&#243;n entre la exposici&#243;n  y la enfermedad se realiz&#243; el Test Exacto de Fisher para comprobar si dicha  diferencia era significativa o no, lo cual se decide en base al p-valor  obtenido en la prueba.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>RESULTADOS</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El rango de edad se ubic&#243; entre 13 a 73 a&#241;os, con un promedio de 47,9 a&#241;os  (desviaci&#243;n est&#225;ndar (DE): &#177;16,2); el grupo et&#225;reo con mayor n&#250;mero de  pacientes fue el comprendido entre 45 y 60 a&#241;os (n= 18). La distribuci&#243;n  de acuerdo al g&#233;nero mostr&#243; 56% (n= 28) del sexo femenino y 44% (n= 22)  del sexo masculino. En modalidad dial&#237;tica predomin&#243; la hemodi&#225;lisis con  52% (n= 26) y el tiempo de di&#225;lisis promedio fue 34,96 meses (DE: &#177; 35,48). <body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los antecedentes cardiovasculares m&#225;s frecuentes fueron, enfermedad vascular  cerebral isqu&#233;mica (24%) y el infarto agudo al miocardio (12%), seguidos  de trombosis venosa profunda (10%) y otros (14%). El 40% de los pacientes  no tuvo antecedente de enfermedad cardiovascular.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se observaron calcificaciones carot&#237;deas en 56% de los pacientes y criterios  para aterosclerosis en 46%. El espesor &#237;ntima-media mostr&#243; relaci&#243;n significativa  con las CV (p=0,0006) (<a href="#fig1">Fig. 1</a>). La mayor&#237;a de los pacientes diab&#233;ticos  (13/18) e hipertensos (11/17) mostraron esta asociaci&#243;n. El 60,8% (14/23)  de los pacientes con aterosclerosis y el 60,7% (17/28) de los pacientes  con CV tuvieron al menos un evento cardiovascular como antecedente (p=0,01).</FONT></P>     <P ALIGN="center"><a name="fig1"> <img border="0" src="/img/fbpe/ic/v53n1/art06fig1.gif" width="418" height="420"></a></P>     
<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Al comparar los promedios de los ex&#225;menes de laboratorio entre los pacientes  con y sin CV se evidenci&#243; que los niveles s&#233;ricos de &#250;rea (158,35 mg/dL  vs 118,92 mg/dL; p=0,01) y &#225;cido &#250;rico (7,82 mg/dL vs 5,91 mg/dL; p=0,04)  fueron significativos en comparaci&#243;n con los otros par&#225;metros estudiados  (<a href="#TABLA_II">Tabla II</a> y <a href="#fig2">Fig. 2</a>). <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>DISCUSI&#211;N</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Esta investigaci&#243;n revel&#243; que los pacientes con ERC en tratamiento dial&#237;tico  presentan mayor incidencia de calcificaciones carot&#237;deas y eventos cardiovasculares.  Como se ha comentado anteriormente, los eventos cardiovasculares son la  causa m&#225;s com&#250;n de muerte en los pacientes con ERC. La calcificaci&#243;n de  los tejidos blandos y las paredes de los vasos sangu&#237;neos ocurre m&#225;s frecuentemente  en los pacientes dializados en comparaci&#243;n con la poblaci&#243;n no ur&#233;mica  (8).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Estudios cl&#237;nicos recientes, sugieren una estrecha relaci&#243;n entre la hiperuricemia  y las enfermedades renales, no s&#243;lo como un marcador de disminuci&#243;n en  la funci&#243;n renal, sino tambi&#233;n como un factor de riesgo para el desarrollo  y progresi&#243;n de las enfermedades renales (9). La alteraci&#243;n de la excreci&#243;n  renal de AU es el principal mecanismo patog&#233;nico que conduce a una elevaci&#243;n  de su concentraci&#243;n en sangre. No obstante, pacientes con grado variable  de enfermedad renal, muestran un amplio rango de concentraciones s&#233;ricas  de AU, indicando que otros factores, probablemente relacionados con la  generaci&#243;n de AU, tambi&#233;n est&#225;n involucrados en el grado de severidad de  la hiperuricemia y sus consecuencias (2). En investigaciones realizadas  en animales de experimentaci&#243;n, el &#225;cido Ox&#243;nico inducido por la hiperuricemia  produce hipertensi&#243;n arterial, hipertrofia glomerular, esclerosis arteriolar  aferente e infiltraci&#243;n de macr&#243;fagos en c&#233;lulas del tejido renal. La hiperuricemia  tambi&#233;n induce arteriolopat&#237;a de vasos preglomerulares, lo que altera la  respuesta autorreguladora de las arteriolas aferentes, dando lugar a la  hipertensi&#243;n glomerular y obliteraci&#243;n de la luz inducida por el engrosamiento  de la pared vascular, lo que conduce a hipoperfusi&#243;n renal grave. Finalmente,  ocurre entrada directa de AU a las c&#233;lulas endoteliales y del m&#250;sculo liso  vascular ocasionando inhibici&#243;n local de los niveles de &#243;xido n&#237;trico endotelial,  la estimulaci&#243;n de la proliferaci&#243;n de c&#233;lulas vasculares del m&#250;sculo liso,  y la estimulaci&#243;n de mediadores inflamatorios y vasoactivos (10). Quedar&#237;a  por determinar el rol del AU en la fisiopatolog&#237;a de las CV en la ERC.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En la ERC, las CV involucran no s&#243;lo el dep&#243;sito pasivo de calcio y f&#243;sforo  en los vasos ateroscler&#243;ticos, sino tambi&#233;n la &#147;osificaci&#243;n&#148; activa de  las estructuras vasculares. La hiperfosfatemia y el producto Ca &#215; P aumentado  son contribuyentes importantes en las CV en los pacientes ur&#233;micos, y parecen  tambi&#233;n estar asociados a mayor mortalidad. En particular, las concentraciones  elevadas de f&#243;sforo se asocian con calcificaciones ect&#243;picas y mayor riesgo  de calcifilaxis. Desafortunadamente, todav&#237;a no se conocen con certeza  los mecanismos patog&#233;nicos de la hiperfosfatemia, el aumento del producto  Ca &#215; P, el hiperparatiroidismo secundario o la enfermedad renal en s&#237; misma  y el aumento de la CV en la ERC (11). En este estudio no se encontr&#243; asociaci&#243;n  significativa cuando se compararon los niveles de calcio y f&#243;sforo o producto  Ca &#215; P; sin embargo, el promedio general se mostr&#243; elevado para el f&#243;sforo  (6,5 mg/dL), bajo para el calcio (8,6 mg/dL) y en el l&#237;mite superior para  el producto Ca x P (55,4 (VN: &lt; 55).&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El presente estudio evidenci&#243; que el grupo de mayor riesgo para CV y aterosclerosis  estuvo representado por los pacientes con HTA y diabetes; resultados similares  obtuvieron Rosvall y col., Murakami y col. y Lorenz y col. utilizando la  ecograf&#237;a y otras t&#233;cnicas de imagen para mostrar esta asociaci&#243;n (12-14).  Por otro lado, otras investigaciones no revelaron datos significativos  cuando compararon pacientes ur&#233;micos con diab&#233;ticos sin alteraci&#243;n renal  (15). En promedio, el grupo de pacientes con calcificaciones mostr&#243; niveles  de urea m&#225;s elevados que el grupo libre de calcificaciones; as&#237; mismo,  el espesor &#237;ntima-media fue superior en este grupo (1,0 mm vs 0,7 mm);  evidenciando la estrecha relaci&#243;n entre el estado ur&#233;mico y el da&#241;o vascular.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Acorde con los resultados de otras investigaciones, los hallazgos del presente  estudio muestran correlaci&#243;n entre la presencia de CV y el espesor &#237;ntima-media  en enfermos renales bajo un r&#233;gimen de tratamiento dial&#237;tico. Otros autores  como Blacher y col. (16) observaron un aumento en la frecuencia de aterosclerosis  en pacientes con ERC5 cuando los compararon con la poblaci&#243;n no ur&#233;mica.  Evidencia reciente sugiere que el aumento del espesor &#237;ntima-media por  encima de 0,9 mm medido por ecograf&#237;a, representa un &#237;ndice confiable para  el diagn&#243;stico de aterosclerosis (16, 17). Contradictoriamente a otros  estudios, no se observ&#243; asociaci&#243;n entre la concentraci&#243;n de AU y aterosclerosis;  sin embargo, algunos autores han reportado resultados similares (18, 19).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Finalmente, esta investigaci&#243;n demostr&#243; que un incremento en los niveles  de &#225;cido &#250;rico por encima de 6 mg/dL est&#225; relacionado con mayor riesgo  de presentar calcificaciones y eventos cardiovasculares adversos en pacientes  con ERC.
